Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the ***: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November ***: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among ***: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

Association of sleep quality with sociodemographic factors and lifestyle in adolescents from southern Brazil

Background Good sleep quality is an essential component for growth, development, and physical and mental health of the individual, in addition to being a positive indicator of health and well-being. The objective of this study was to estimate the prevalence of poor sleep quality and to identify the association of poor sleep quality with sociodemographic factors, lifestyle, weight status, and associated aerobic fitness in adolescents. Methods This study included 1110 students aged 14–19 years from city of S?o José/SC, Brazil. Sleep quality of the subjects was assessed using a questionnaire and classified as adequate and low. Independent variables were gender, age, monthly household income, maternal education, school shift, global physical activity, proper diet, screen time, sleeping hours of full week, sleeping hours of weekdays, sleeping hours of weekends, weight status, and aerobic fitness. Binary logistic regression was used to estimate odds ratios and confidence intervals of 95%. Results Prevalence of poor sleep quality was 60.4%. More likely to have poor sleep quality were identified in female students (OR 1.68, 95% CI 1.11–2.55), whose mothers had higher levels of education (OR 1.44, 95% CI 1.20–1.73) and low aerobic fitness (OR 1.80, 95% CI 1.02–3.19). Conclusion Adolescents who were most likely to report poor sleep quality were female, those whose mothers had a higher educational level and had low aerobic fitness. Students with sedentary risk behavior were less likely of reporting poor sleep quality.

A 74-year-old man with terminal chronic kidney disease, under hemodialysis and with residual diuresis, was admitted due to myalgia, arthralgia, fever and pyuria in the previous 10 days. The patient had a recent diagnosis of high-grade non-invasive bladder cancer and was doing weekly BCG intravesical administrations. The symptoms started three days before the fifth administration. He had done cefixime as an outpatient and started piperacillin-tazobactam on hospital admission, but the fever persisted, and there was no bacterial isolation in urine or blood culture. On the tenth and seventeenth day after the last BCG intravesical administration Mycobacterium bovis was still isolated in the urine culture. The diagnosis of BCGitis was made and treatment was started, with a good response. Forty days after the last administration and under treatment, the culture remained positive for Mycobacterium bovis in the urine. We raise the question about the safety of BCG administration in patients with residual diuresis.

AIM To evaluate the sex-specific effects of a hydroalcoholic extract from Eugenia punicifolia(HEEP) leaves on gastric ulcer *** In this rat study involving males, intact(cycling) females, and ovariectomized females, gastric ulcers were induced using acetic acid. A vehicle, lansoprazole, or HEEP was administered for 14 d after ulcer induction. Body weight was monitored throughout the treatment period. At the end of treatment, the rats were euthanized and the following in vivo and in vitro investigations were performed: macroscopic examination of the lesion area and organ weights, biochemical analysis, zymography, and evaluation of protein expression levels. Additionally, the concentration-dependent effect of HEEP was evaluated in terms of subacute toxicity and *** Compared to the vehicle, HEEP demonstrated a great healing capacity by substantially reducing the ulcerative lesion area in males(52.44%), intact females(85.22%), and ovariectomized females(65.47%), confirming that HEEP accelerates the healing of acetic acidinduced gastric lesions and suggesting that this effect is modulated by female sex hormones. The antiulcer effect of HEEP was mediated by prostaglandin E2 only in male rats. Overall, the beneficial effect of HEEP was the highest in intact females. Notably, HEEP promoted the expression of vascular endothelial growth factor(intact vs ovariectomized females) and decreased the expression of Caspase-8 and Bcl-2(intact female vs male or ovariectomized female). Additionally, HEEP enhanced fibroblast proliferation and migration into a wounded area in vitro, confirming its healing effect. Assessment of the Anaerobic Speed Reserve during Specific High-Intensity Exercise in Judo Athletes

Purpose This study aimed to describe self-selected intensity and its relationship to performance during a high-intensity all-out judo protocol,and to verify whether athletes with different anaerobic speed reserve(ASR)perform differently during a high-intensity all-out interval uchi-komi(judo-specific skill).Methods We analyzed the performance of 28 judo athletes as a whole group and divided into groups,according to their *** performed several judo-specific protocols using a specific skill:maximal speed sprint(MSS)and maximal aerobic speed(MAS),and high-intensity intermittent uchi-komi protocol(12×20 s all-out:10 s passive recovery).Results The main results demonstrated a decrease in the number of repetitions and an increase in heart rate across the *** with greater ASR performed a higher number of repetitions,compared to the low ASR group,and they were able to exercise at a higher percentage of the MAS(low ASR=126%;SD=13%;high ASR=152%,SD=l9%)and at a lower percentage of MSS compared to the low ASR group(low ASR=84%,SD=7%;high ASR=76%,SD=11%).However,both groups exercised at a similar percentage of their own ASR during the protocol(low ASR=50%,SD=20%;high ASR=53%,SD=19%).Conclusions Athletes with different ASR seem to set their pace at around 50%of their ASR within an all-out uchi-komi ***,the ASR is useful for predicting performance during high-intensity judo-specific interval exercise intervals.
