Effects of left ventricular assist device on pulmonary functions and pulmonary hemodynamics:A meta-analysis

Hydrodynamic instability experiments with three-dimensional modulations at the National Ignition Facility

The first hydrodynamic instability growth measurements with three-dimensional(3D) surface-roughness modulations were performed on CH shell spherical implosions at the National Ignition Facility(NIF) [G. H. Miller, E. I. Moses, and C. R. Wuest, Opt. Eng. 43, 2841(2004)]. The initial capsule outer-surface amplitudes were increased approximately four times, compared with the standard specifications, to increase the signal-to-noise ratio, helping to qualify a technique for measuring small 3D modulations. The instability growth measurements were performed using x-ray through-foil radiography based on time-resolved pinhole imaging. Averaging over 15 similar images significantly increased the signal-to-noise ratio, making possible a comparison with 3D simulations. At a convergence ratio of～2.4, the measured modulation levels were～3 times larger than those simulated based on the growth of the known imposed initial surface modulations. Several hypotheses are discussed, including increased instability growth due to modulations of the oxygen content in the bulk of the capsule. Future experiments will be focused on measurements with standard 3D ‘nativeroughness' capsules as well as with deliberately imposed oxygen modulations.

Comparing dietary determinants of serum vitamin D status among African-Americans and Caucasians

African-Americans (AAs) maintain a high prevalence of vitamin D insufficiency compared to Caucasians. The relationship between dietary sources of vitamin D and serum 25-hydroxyvitamin D (25(OH)D) among AAs remains poorly understood. This research examines the dietary determinants of 25(OH)D levels in AAs and Caucasians, controlling for demographic characteristics. 121 Caucasians and 119 AAs participated in a biomarker based study, comparing the correlation of serum 25(OH) D with dietary sources of vitamin D. Diet was assessed by multiple 24-hour (24-hr) recall and Food Frequency Questionnaire (FFQ). Parallel multiple linear and logistic regression models stratified by race were then used to regress 25(OH)D level and sufficiency (25(OH)D > 20 ng/ml) on major dietary sources of vitamin D—multivitamins (MV), milk, and fish—controlling for age, gender, and BMI. Results show that AAs are sensitive to the method of dietary assessment, with multiple 24 hr recall demonstrating highest correlation (0.4) with 25 (OH)D levels. MV’s association with 25(OH)D levels was highest in both groups, 25% higher in AAs vs. Caucasians (8.6 vs. 6.5 ng/ml per 400 IU). Milk’s impact on serum was similar in both groups but fish was only significant among Caucasians. MV consumption significantly increased odds of vitamin D sufficiency in both groups. Given the high prevalence of vitamin D insufficiency and the strong contribution as compared to milk or fish, MV supplementation is recommended. Additionally, future studies aimed at measuring vitamin D intake should consider the use of multiple 24 hr recall instead of a single measure of FFQ or 24 hr recall since the multiple 24 hr recalls have a stronger correlation to serum 25(OH)D.

Atypical presentation of a hepatic artery pseudoaneurysm:A case report and review of the literature

Classically, hepatic artery pseudoaneurysms(HAPs) arise secondary to trauma or iatrogenic causes. With an increasing prevalence of laparoscopic procedures of the hepatobiliary system the risk of inadvertent injury to arterial vessels is increased. Pseudoaneurysm formation post injury can lead to serious consequences of rupture and subsequent hemorrhage, therefore intervention in all identified visceral pseudoaneurysms has been advocated. A variety of interventional methods have been proposed, with surgical management becoming the last step intervention when minimally invasive therapies have failed. The authors present a case of a HAP in a 56-year-old female presenting with jaundice and pruritis suggestive of a Klatskin's tumor. This presentation of HAP in a patient without any significant past medical or surgical intervention is atypical when considering that the majority of HAP cases present secondary to iatrogenic causes or trauma. Multiple minimally invasive approaches were employed in an attempt to alleviate the symptomology which included jaundice and associated inflammatory changes. Ultimately, a right hepatic trisegmentectomy was required to adequately relieve the mass effect on biliary outflow obstruction and definitively address the HAP. The presentation of a HAP masquerading as a malignancy with jaundice and pruritis, rather than the classic symptoms of abdominalpain, anemia, and melena, is unique. This presentation is only further complicated by the absent history of either trauma or instrumentation. It is important to be aware of HAPs as a potential cause of jaundice in addition to the more commonly thought of etiologies. Furthermore, given the morbidity and mortality associated with pseudoaneurysm rupture, intervention in identifiable cases, either by minimally invasive or surgical interventions, is recommended.

Nanotechnology-based approach for safer enrichment of semen with best spermatozoa

Background: Advances in nanotechnology have permitted molecular-based targeting of cells through safe and biocompatible magnetic nanoparticles(MNP). Their use to detect and remove damaged spermatozoa from semen doses could be of great interest. Here, MNP were synthesized and tested for their ability to target apoptotic(annexin V) and acrosome-reacted(lectin) boar spermatozoa, for high-throughout retrieval in a magnetic field(nanoselection). The potential impacts of nanoselection on sperm functions and performance of offspring sired by sperm subjected to nanoselection were determined. Fresh harvested and extended boar semen was mixed with various amounts(0, 87.5, and 175 μg) of MNP-conjugates(Annexin V-MNP or lectin-MNP) and incubated(10 to15 min) for 37 °C in Exp. 1. In Exp. 2, extended semen was mixed with optimal concentrations of MNP-conjugates and incubated(0, 30, 90, or 120 min). In Exp. 3, the synergistic effects of both MNP-conjugates(87.5 μg– 30 min)on spermatozoa was evaluated, followed by sperm fertility assessments through pregnancy of inseminated gilts and performance of neonatal offspring. Sperm motion, viability, and morphology characteristics were evaluated in all ***: Transmission electron microscopy, atomic force microscopy, and hyperspectral imaging techniques were used to confirm attachment of MNP-conjugates to damaged spermatozoa. The motility of nanoselected spermatozoa was improved(P lity of boar sperm, as assessed by the abundance of reactive oxygen species and the integrity of the acrosome, plasma membrane, and mitochondrial membrane was not different between nanoselected and control spermatozoa.
