Impact of conditioning regimen on peripheral blood hematopoietic cell transplant

AIM To investigate infused hematopoietic cell doses and their interaction with conditioning regimen intensity +/-total body irradiation(TBI) on outcomes after peripheral blood hematopoietic cell transplant(PBHcT).METHODS Our retrospective cohort included 247 patients receiving a first, T-replete, human leukocyte antigen-matched allogeneic PBHcT and treated between 2001 and2012. correlations were calculated using the Pearson product-moment correlation coefficient. Overall survival and progression free survival curves were generated using the Kaplan-Meier method and compared using the log-rank *** Neutrophil engraftment was significantly faster after reduced intensity TBI based conditioning [reduced intensity conditioning(RIc) + TBI] and > 4 × 10~6 cD34+cells/kg infused. A higher total nucleated cell dose led to a higher incidence of grade II-IV acute graft-versus-host disease in the myeloablative + TBI regimen group(P = 0.03), but no significant difference in grade III-IV graft-versus-host disease. A higher total nucleated cell dose was also associated with increased incidence of moderate/severe chronic graft-versus-host disease, regardless ofconditioning regimen. Overall and progression-free survival were significantly better in patients with a RIc + TBI regimen and total nucleated cell dose > 8 ×10~8/kg(3 years, overall survival: 70% vs 38%, P = 0.02, 3 years, progression free survival: 64% vs 38%, P = 0.02).cONcLUSION TBI and conditioning intensity may alter the relationship between infused cell doses and outcomes after PBHcT. Immune cell subsets may predict improved survival after unmanipulated PBHcT.

cardiovascular disease remains a leading cause of morbidity and mortality,a fact that is commonly associated with co-morbidities such as clinical *** phase II cardiac rehabilitation is an established intervention for those with cardiovascular disease,its effect on patients who also suffer from depression are under ***:To quantify Pre-and Post-cardiac rehabilitation questionnaire scores collected from a large patient data *** this investigation,27670 patients completed Patient Health Questionnaire-9 questionnaires both Pre-and Post-rehabilitation(averaging[28.08.7]phase II sessions).Findings reveal that questionnaire scores decreased by 40%–48%across all groups,a finding that was independent of assigned sex,race,and ***,when data were stratified for questionnaire scores that may indicate major and minor depressive disorder,phase II cardiac rehabilitation outcomes were lower by 61%and 49%*** all groups exhibited lower questionnaire scores following cardiac rehabilitation participation,numerical differences at Preand Post-rehabilitation time points indicate that males and White patients have more favorable *** latter observation,while not confirmed currently,appears to be linked to referral rates to phase II cardiac rehabilitation,which remain poor for females,racial and ethnic minorities.

Understanding the forces acting in self-assembly and the implications for constructing three-dimensional （3D） supercrystals

The assembly of nanocrystals into ordered structures called supercrystals or superstructures has become a pivotal frontier owing to numerous useful applications such as correlating the arrangements of atoms in macroscopic crystals and tuning the collective properties to meet the demands of various applications. In this article, recent progress in the preparation of three-dimensional superlattices of nanocrystals is outlined, with a particular emphasis on the driving forces and evolutionary routes beyond orderly assembly. First, the leading or repulsive forces that internally and externally govern the formation of three-dimensional supercrystals are systematically identified and discussed with respect to their origins and functions in three-dimensional self-organization. Then a synoptic introduction of commonly applied means of nanocrystal self-assembly based on growth scenarios such as droplet evaporation and a liquid/liquid interface is presented with specific cases and detailed analyses. Finally, the existing challenges and prospects for this field are briefly highlighted.

The translocon on the outer membrane of mitochondria （TOM） facilitates the import of nuclear-encoded proteins. The principal machinery of mitochondrial protein transport seems conserved in eukaryotes; however, divergence in the composition and structure of TOM components has been observed between mammals, yeast, and plants. TOM9, the plant homolog of yeast Tom22, is significantly smaller due to a truncation in the cytosolic receptor domain, and its precise function is not understood. Here we provide evidence showing that TOM9.2 from Arabidopsis thaliana is involved in the formation of mature TOM com- plex, most likely by influencing the assembly of the pore-forming subunit TOM40. Dexamethasone- induced RNAi gene silencing of TOM9.2 results in a severe reduction in the mature TOM complex, and the assembly of newly imported TOM40 into the complex is impaired. Nevertheless, mutant plants are fully viable and no obvious downstream effects of the loss of TOM complex, i.e., on mitochondrial import ca- pacity, were observed. Furthermore, we found that TOM9.2 can bind calmodulin （cAM） in vitro and that caM impairs the assembly of TOM complex in the isolated wild-type mitochondria, suggesting a regula- tory role of TOM9.2 and a possible integration of TOM assembly into the cellular calcium signaling network.

Endoscopic ultrasound staging for early esophageal cancer: Are we denying patients neoadjuvant chemo-radiation?

AIM To evaluate the accuracy of endoscopic ultrasound(EUS) in early esophageal cancer(Ec) performed in a highvolume tertiary cancer center. METHODS A retrospective review of patients undergoing esophagectomy was performed and patients with c T1 N0 and c T2 N0 esophageal cancer by EUS were evaluated. Patient demographics, tumor characteristics, and treatment were reviewed. EUS staging was compared to surgical pathology to determine accuracy of EUS. Descriptive statistics was used to describe the cohort. Student's t test and Fisher's exact test or χ~2 test was used to compare variables. Logistic regression analysis was used to determine if clinical variables such as tumor location and tumor histology were associated with EUS *** Between 2000 and 2015, 139 patients with clinical stage Ⅰ or Ⅱ?A esophageal cancer undergoing esophagectomy were identified. There were 25(18%) female and 114(82%) male patients. The tumor location included the middle third of the esophagus in 11(8%) and lower third and gastroesophageal junction in 128(92%) patients. Ninety-three percent of patients had adenocarcinoma. Preoperative EUS matched the final surgical pathology in 73/139 patients for a concordance rate of 53%. Twenty-nine patients(21%) were under-staged by EUS; of those, 19(14%) had unrecognized nodal disease. Positron emission tomography(PET) was used in addition to EUS for clinical staging in 62/139 patients. Occult nodal disease was only found in 4 of 62 patients(6%) in whom both EUS and PET were negative for nodal involvement. cONcLUSION EUS is less accurate in early Ec and endoscopic mucosal resection might be useful in certain settings. The addition of PET to EUS improves staging accuracy.

comparing 11 early warning scores and three shock indices in early sepsis prediction in the emergency department

BAcKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROc]0.75,95%cI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%cI 0.37-0.52)and 0.88(95%cI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

AIM： To evaluate the predictive value of hepatocyte proliferation and hepatic angiogenesis for the occurrence of Hepatocellular carcinoma （Hcc） in hepatitis c virus （HcV） cirrhotic patients.METHODS： One hundred-five patients （69 males, 36 females; age range, 51-90 year; median 66 year） with biopsy proven HcV cirrhosis were prospectively monitored for Hcc occurrence for a median time of 64 too. Angiogenesis was assessed by using microvessel density （MVD）, hepatocyte turnover by MIB1 and PcNA indexes at inclusion in liver biopsies.RESULTS： Forty six patients （43.8%） developed Hcc after a median time of 55 （6-120） mo while 59 （56.2%） did not. Patients were divided into two groups according to the median value of each index. The difference between patients with low （median MVD = 3; range 0-20） and high （median MVD = 7; range 1-24） MVD was statist（caUy s（gn（ficant （χ^2= 22.06; P 〈 0.0001） wh（ch was not the case for MIB1 or PcNA （MIB-I： χ^2 = 1.41; P = 0.2351; PcNA： χ^2 = 1.27; P = 0.2589）. The median MVD was higher in patients who developed Hcc than in those who did not. HOe-free interval was significantly longer in patients with the MVD ≤ 4 （P = 0.0006）. No relationship was found between MIB1 or PcNA and MVD （MIB-1 r^2 = 0.00007116, P = 0.9281; PcNA： P =0.001950; P = 0.6692）. MVD only was able to predict the occurrence of Hcc in these patients. Among other known risk factors for Hcc, only male sex was statistically associated with an increased risk. cONcLUSION： Liver angiogenesis has a role for in HcV- related liver carcinogenesis and for defining patients at higher risk.

Synchronous colorectal liver metastases: a national survey of surgeon opinions on simultaneous resection and multidisciplinary cooperation

Background: The management of patients with synchronous colorectal liver metastases (scRLM) has evolved significantly (improved chemotherapy, hepatic surgery advancements, colonic stenting, consultation synergies). We sought to better understand surgeon viewpoints on optimal referral patterns and the delivery of simultaneous resections. Methods: A 40 question on-line survey was offered to members of the canadian surgical community. Statistical analysis was descriptive. Results: A total of 52 surgeons responded. Most colorectal surgeons (cRS) had access to and a good working relationship with regional hepatobiliary (HPB) surgeons (86%) and medical oncologists (100%). The majority (92%) believed there was a role for simultaneous resection of scRLM, with 69% having first hand experience. Many cRS (62%) discussed all cases of known hepatic metastases with HPB prior to any resection. When a lesion was asymptomatic/minimally symptomatic, most cRS (92%) discussed them with medical oncology/HPB prior to resection (8%). Bilobar metastases (58%), patient comorbidities (35%), portal lymphadenopathy (35%), and patient age (15%) restricted cRS from obtaining HPB consultations. Many cRS (46%) did not believe that resecting hepatic metastases prior to the primary lesion might be beneficial. Most cRS (60%) reported they could not accurately predict hepatic resectability, with only 27%familiarity with evidence-based guidelines. Despite working in smaller hospitals with less access to HPB and less experience with simultaneous resections, non-cR general surgeons more commonly supported a 'liver-first' approach. conclusions: There was general agreement between cRS and general surgeons on numerous topics, but additional education is required with regard to HPB surgical capabilities and to provide truly individualized patient-centered care.
