Current trends of liver cirrhosis in Mexico: Similitudes and differences with other world regions

AIM To investigate the main current etiologies of cirrhosis in Mexico.METHODS We performed a cross-sectional retrospective multicenter study that included eight hospitals in different areas of Mexico. These hospitals provide health care to people of diverse social classes. The inclusion criteria were a histological, clinical, biochemical, endoscopic, or imaging diagnosis of liver cirrhosis. Data were obtained during a 5-year period(January 2012-December 2017). RESULTS A total of 1210 patients were included. The mean age was 62.5 years(SD = 12.1), and the percentages of men and women were similar(52.0% vs 48.0%). The most frequent causes of liver cirrhosis were hepatitis C virus(HCV)(36.2%), alcoholic liver disease(ALD)(31.2%), and nonalcoholic steatohepatitis(23.2%), and the least frequent were hepatitis B virus(1.1%), autoimmune disorders(7.3%), and other conditions(1.0%).CONCLUSION HCV and ALD are the most frequent causes of cirrhosis in Mexico. However, we note that non-alcoholic fatty liver disease(NAFLD) as an etiology of cirrhosis increased by 100% compared with the rate noted previously. We conclude that NAFLD will soon become one of the most frequent etiologies of liver cirrhosis in Mexico.

Identification and Quantification of Monosaccharides in Aloe vera Gel by Acid and Enzymatic Hydrolysis and Wet Heat Treatment

Aloe vera (Aloe barbadensis Miller) possesses curative and therapeutic properties attributed to the polysaccharides found in its tissue. This work aimed to study hydrolyzed (chemical and enzymatical) Aloe vera gel extracts using ultrafiltration membranes with a pore size of 0.22 micron (μm). Nine chemical treatments were achieved with H2SO4 and three wet heat treatments were carried out at a pressure of 1.2 lbf/in2 for 15 minutes;in both cases, three different concentrations of Aloe vera gel juice (AGJ) were used: 1.5%, 2.5% and 3.5% w/w. The concentrations of H2SO4 were 0.10%, 0.25% and 0.40% w/w. Chemical experiments are performed over a factorial 32 design and results were analyzed using SPSS software (version 17, SPSS Inc.), finding the one labeled T7 (15 ml of AGJ 3.5% with 15 ml of H2SO4 0.10% added) the best of them, as it leaded to 0.0446 mg/ml of liberated glucose. Among the three wet heat treatments, the one labeled TC3 (15 ml of AGJ 3.5% with 15 ml of H2O added) was the best-performing one, as it leaded to 0.292 mg/ml of liberated glucose. Furthermore, an enzymatic hydrolysis was carried out using Novozymes’ Pectinex? AR and Viscozyme?. Hydrolisis with both enzymes yield to better results than acid hydrolysis: in the treatment with Pectinex? AR, 3.282 mg/ml of liberated glucose were obtained and 3.302 mg/ml in the treatment with Viscozyme?. The hydrolyzed substances obtained by acid and enzymatic hydrolysis, as well as by wet heat treatment, were subsequently analyzed by thin layer chromatography (TLC), using glucose, galactose and arabinose 1000 ppm solutions as reference patterns. Among the treatments by H2SO4, the one labeled T4 obtained an Rf value of 50, the same as on the galactose reference pattern.

Purification and Partial Characterization of Polyphenol Oxidase from Sapodilla Plum (Achras sapota)

The browning of fruits can be considered as an enzymatic oxidation which is believed to be one of the main causes of quality loss during post-harvest handling. The enzymes responsible for this are the oxidoreductases;the polyphenol oxidase (PPO) (monophenol, o-diphenol, oxygen oxidoreductase;EC 1.***.18.1) belongs to this group. This enzyme, which is found in the sapodilla plum (Achras sapota), was purified using a phenylsepharose and a SephacrylS-200 columns. The molecular weight of the purified enzyme was estimated to be about 66 kDa by gel filtration and 29 kDa by SDS-PAGE. A single protein band was found using the latter system (SDS-PAGE), which shows that the PPO of the pulp of the sapodilla plum may be composed of two protein subunits with similar molecular weight. The optimum pH was 7.0 and the optimum temperature 60℃. The most effective inhibitors tested were ascorbic acid, sodium metabisulfite and acetic acid.

Extraction and Stability of Anthocyanins Present in the Skin of the Dragon Fruit (Hylocereus undatus)

The extraction of anthocyanins present in the skin of the dragon fruit was performed using trifluoroacetic acid (TFA) plus a mixture of methanol, acetic acid and water;the anthocyanins were then purified with a LC-18 cartridge, using methanol acidified with TFA as eluent, reaching concentrations of 44.3865 ± 1.3125 mg/100g of sample. The extracts were put through stability tests under different storage conditions, modifying the pH of the extracts (pH of 1, 4 and 6), the temperature (4℃, 25℃ and 68℃) and the absence and presence of light for a time period of 4 days;the tests indiated that anthocyanins remain more stable at a temperature of 4℃?with a pH of 4 in the absence of light, retaining up to 80% of the pigment. Three anthocyanins were partially identified in the extracts by high performance liquid chromatography (HPLC);they were: cyanidin 3-O-glucoside, cyanidin 3,5 O-glucoside and pelargonidin 3,5 O-glucoside.

Main Polysaccharides Isolated and Quantified of Aloe vera Gel in Different Seasons of the Year

We developed and implemented a methodology that allowed extracting and evaluating high molecular weight polysaccharides present in the gel of Aloe barbadensis Miller. One of the fractions evaluated revealed the presence of high molecular weight carbohydrates (200 kDa) with a behavior similar to that of acemannan and another fraction with compounds of molecular weights between 17 and 47 kDa. We quantified the concentration of acemannan for two different growing periods. The concentration of acemannan in the high molecular weight fraction was 99.97 ppm in the rainy season and 106.03 ppm in the dry season. The concentration of acemannan in the fraction of low molecular weight was 9.364 ppm during the season of greatest rainfall and 26.939 ppm in the dry season.

Mesenchymal stem cell secretome and nanotechnology:Combining therapeutic strategies

Mesenchymal stem cells(MSC)have pushed the field of stem cell-based therapies by inducing tissue regeneration,immunosuppression,and angiogenesis mainly through vesicles and soluble factors release(paracrine signaling).MSC-extracellular vesicles(MSC-EV)adaptable secretome and homing to injured sites allowed researchers to unlock a new era of cell-free based therapy.In parallel,nanoparticles(NP)have been explored in contributing to transport and drug delivery systems,giving drugs desired physical-chemical properties to exploit cell behavior.However,NPs can be quickly recognized by immune cells and cleared from circulation.In this viewpoint,we explore how combining both therapeutic strategies can improve efficacy and circumvent limitations of both therapies.MSCEV benefit from the potent MSC membrane composition,guiding chemotaxis to tumor sites,a very restricted microenvironment.MSC-EV has low immunogenicity,high stability,long half-life and can explore tissue targeting ligands as a precise drug carry,even across biological barriers.Those properties promote enhanced targeted drug delivery that can be combined with NP,exploring biological membrane production through:1.direct cell therapy with NP-infused MSC;2.NP-containing MSC-EV generated by NP-infused MSC;3.by coating NP in MSC membrane(“MSC NanoGhosts”),allowing precise cargo definition without losing targeting.Therefore,nanotechnology combined with cell-based therapeutic resources can greatly improve targeted drug delivery,improving efficacy and opening a new venue of therapeutic possibilities.

Comparisons of the Properties of Whitemouth Croaker (Micropogonias furnieri) Surimi and Mechanically Deboned Chicken Meat Surimi-Like Material

The aim of this work was to evaluate and compare the color, texture and sensory characteristics of whitemouth croaker (Micropogonias furnieri) surimi and mechanically separated chicken meat (MDCM) surimi-like material (SLM). The whiteness of the MDCM-SLM was higher (P > 0.05) than that from the surimi. Higher breaking force (P 0.05). The MDCM-SLM was more accepted by the panelists (P < 0.05). The mean scores of overall desirability for MDCM-SLM was similar to that observed for walleye Pollack surimi that is the parameter of quality. The differences in the acceptability of poultry and fish food products are probably due to peoples’ eating habits.

Psychotropic drugs and liver disease:A critical review of pharmacokinetics and liver toxicity

The liver is the organ by which the majority of sub-stances are metabolized, including psychotropic drugs. There are several pharmacokinetic changes in end-stage liver disease that can interfere with the metabolization of psychotropic drugs. This fact is particularly true in drugs with extensive first--pass metabolism, highly protein bound drugs and drugs depending on phase I hepatic metabolic reactions. Psychopharmacological agents are also associated with a risk of hepatotoxicity. The evidence is insufficient for definite conclusions regarding the prevalence and severity of psychiatric drug-induced liver injury. High-risk psychotropics are not advised when there is pre-existing liver disease, and after starting a psychotropic agent in a patient with hepatic impairment, frequent liver function/lesion monitoring is advised. The authors carefully review the pharmacokinetic disturbances induced by end-stage liver disease and the potential of psychopharmacological agents for liver toxicity.

Evaluation of Physicochemical and Functional Properties of Protein Recovered Obtaining from Whitemouth Croaker (Micropogonias furnieri) Byproducts

Proteins recovered were obtained from Whitemouth Croaker (Micropogonias furnieri) byproducts and their physico-chemical and functional properties were evaluated. The proximate composition, presented 8.64% ± 0.10% of moisture, 85.33% ± 0.12% of protein, 2.69% ± 0.09% of ashes and 2.16% ± 0.12% of lipids, in dry basis. The highest solubility was obtained in pH 11 (93.24%), the maximum water holding capacity was presented at pH 11 (25.71 mL H2O/gprotein), the oil holding capacity was 13.71 mL/gprotein and the digestibility in vitro was 91.32% ± 0.15%. The electro-phoretic profile was observed typical of the myofibrillar proteins, with the appearance of the heavy chain of myosin (220 kDa) and actin (50 kDa). The results show that the products of low commercial value of fish that are usually used for the production of animal feed or simply discarded, contributing to environmental pollution, may be used to produce products with a greater added value.

Ilha Grande Bay is one of the biggest producers of bivalves of Rio de Janeiro State. Statistics reports of foodborne diseases are quite low in Brazil, however, this fact is a matter of Public Health. In their majority concerning consumption of bivalves meat, the availability of safe products requires the use of technology as food irradiation. The objective of this paper was to evaluate the presence of bacteria resulting from the environmental contamination and epidemiological importance, Salmonella spp., total and faecal coliforms of mussel (Perna perna) from that region and the use of irradiation on the product in natura. Fifteen indicative samples of mussel were collected from five gr owing points in Ilha Grande Bay. A sample of each point was irradiated with doses of 1.0 and 1.5 kGy. The bacteriological analysis followed the instructions of the Brazilian legislation. The samples presented irregularities in relation to Salmonella spp. and faecal coliforms, the latter for the control group. The control group was noticed as not appropriate for consumption. The dose of 1.0 kGy was effective for the reduction of faecal coliforms, but ineffective for the extinction of Salmonella spp.
