Hepatocellular Carcinoma: Known and Emerging Risk Factors

Hepatocellular carcinoma (HCC) is the most frequent primary liver cancer with a high mortality rate. While chronic hepatitis B virus (HBV) and hepatitis C virus (HCV) infections represent the leading risk factors worldwide, the spreading of metabolic disorders, such as diabetes, obesity and non-alcoholic fatty liver disease (NAFLD) justifies the increasing attention on their oncogenic mechanisms. This review discusses about the main pathogenic mechanisms implicated in occurrence of HCC in presence of viral and metabolic diseases. Additionally, it points to the importance of clinical surveillance for those patients considered at risk of HCC and highlights the strategical role of serum markers, such as alfa-fetoprotein (αFP) and Protein Induced by Vitamin K Absence or Antagonist II (PIVKA-II), which, in association to a strictly instrumental follow-up, contribute to the early detection of hepatic nodules with a better prognosis for affected patients.

Early Non-Invasive Angiographic Results of the “Button Inside” Technique for the Reattachment of Coronary Arteries in the Aortic Root Replacement

Background and Aim: The Button technique was the operative technique generally used for the reimplantation of coro- nary arteries in the aortic root replacement because of reduced anastomotic complications respect to the original wrapped operation described by Bentall and De Bono. The “Button Inside” technique is a modified Button technique recently performed in all consecutive series of patients operated on aortic root replacement at our Institution. In this study we report early postoperative angiographic results of patients operated on aortic root replacement using the “But- ton Inside” technique. Methods: Magnetic Resonance Imaging angiography was achieved in all patients operated on aortic root replacement. Morphological characteristics of the site of coronary buttons reattachment were accurately evaluated. Results: No postoperative anastomotic leakages were detected at the site of coronary buttons anastomosis. Moreover, in this first series of postoperative control aneurisms of coronary buttons were not detected. Conclusions: Early postoperative non-invasive angiographic results of the “Button Inside” technique are encouraging. This surgical approach for the reattachment of coronary arteries is suggested in aortic root replacement when is possible to mobilize coronary arteries in order to reduce coronary artery anastomotic complications.

Sand Dune Species Distribution and Size Variations in Two Areas Inside a Natural Protected Area Subjected to Different Human Disturbance

Sand dune species were analyzed across two areas included in the “Roman Coastal State Nature Reserve” (Italy): Ostia (O) and Marina di Palidoro (P). Significant variations in species distribution, dry mass and size over the gradient from the water-edge toward the inland between O and P were observed. Species colonize O, on an average, at 79 m from the water-edge extended along the gradient toward the inland over a length of 26 m. PD is 40.8 ± 9.9 plants.m-2. Ononis variegata and Elymus farctus have the highest PD (15.4 ± 1.3 plants.m-2), followed by Silene colorata and Sporobulus pungens (4.3 ± 3.9 plants.m-2), by Cakile maritima, Echinophora spinosa, Eryngium maritimum, Ammophila arenaria, Anthemis maritima, Cyperus capitatus, Medicago marina and Calystegia soldanella (.m-2), with C. soldanella having the lowest PD (0.02 plants.m-2). On an average, in P, the species colonize the dune at 17 m from the water-edge for a length of 46 m toward the inland, PD is on an average 21.5 ± 16.1 plants.m-2. Endoscopic ultrasound-guided drainage of pancreatic fluid collections

Pancreatic fluid collections (PFCs) develop secondary to either fluid leakage or liquefaction of pancreatic necrosis following acute pancreatitis, chronic pancreatitis, surgery or abdominal trauma. Pancreatic fluid collections include acute fluid collections, acute and chronic pancreatic pseudocysts, pancreatic abscesses and pancreatic necrosis. Before the introduction of linear endoscopic ultrasound (EUS) in the 1990s and the subsequent development of endoscopic ultrasound-guided drainage (EUS-GD) procedures, the available options for drainage in symptomatic PFCs included surgical drainage, percutaneous drainage using radiological guidance and conventional endoscopic transmural drainage. In recent years, it has gradually been recog-nized that, due to its lower morbidity rate compared to the surgical and percutaneous approaches, endoscopic treatment may be the preferred first-line approach for managing symptomatic PFCs. Endoscopic ultrasound-guided drainage has the following advantages, when compared to other alternatives such as surgical, per-cutaneous and non-EUS-guided endoscopic ***-GD is less invasive than surgery and therefore does not require general anesthesia. The morbidity rate is lower, recovery is faster and the costs are lower. EUS-GD can avoid local complications related to per-cutaneous drainage. Because the endoscope is placed adjacent to the fluid collection, it can have direct ac-cess to the fluid cavity, unlike percutaneous drainage which traverses the abdominal wall. Complications such as bleeding, inadvertent puncture of adjacent viscera, secondary infection and prolonged periods of drainage with resultant pancreatico-cutaneous fistulae may be avoided. The only difference between EUS and non-EUS drainage is the initial step, namely, gaining access to the pancreatic fluid collection. Design and Evaluation of SIMpliLife, an NFC Platform to Ease Day-to-Day Actions of People＇s Life

In this paper the authors describe the design, prototyping and evaluation of SIMpliLife, a framework for mobile phones with the aim of making people＇s life easier, providing day-to-day services such as payment, ticketing and information retrieval by means of NFC （Near Field Communication） technology. It was developed via the collaboration between two research labs of Sapienza University of Rome： the RFID （Radio Frequency Identification） Lab and the Usability and Accessibility Lab （LUA）, both belonging to CATTID （Centre for Applications of Teleservices and of Technologies for Innovation in Digital world） research centre. Technically, SIMpliLife is a SIM-based platform for NFC mobile phones that interfaces several applications able to manage the abovementioned services. The high-level User Interface has been developed by means of Smart Card Web Server （SCWS） technology, thus providing enhanced user experience if compared to traditional SIM-based implementations based on SIM Toolkit.

Physical exercise intensity prescription to improve health and fitness in overweight and obese subjects: A review of the literature

Obesity is one of the greatest public health challenges of the 21st century. Overweight and obesity drastically increase a person’s risk of developing chronic non-communicable diseases (NCDs), including cardiovascular disease, cancer and diabetes. Furthermore, obesity is already responsible for 2% -8% of health costs and 10% -13% of deaths in several industrialized countries. Lifestyle modifications involving changes in exercise, diet and psychological support are effective in reducing the incidence of overweight. Moreover, positive effects of physical activity (PA) for weight loss and prevention of weight regain are well documented. It was recognized that health benefits regarding both psychological and physiological aspects, such as improving cardiorespiratory and muscular fitness and/or decreasing depression symptoms, can be obtained from numerous activities. Public health institutions (American College of Sports Medicine, World Health Organization) provide recommendations for PA (volume, frequency, intensity and type of exercise) to achieve positive effects, at all ages and for many diseases and disorders situations. Although exercise under guidelines can be safely performed by obese subjects, several questions still need to be fully answered. In facts, the exercise program should be tailored according to an individual’s habitual physical activity, physical function, health status, exercise responses, and stated goals. Thus, this review analyzes the intensity of PA parameters. In the last years, research has been focused on the individualization of the right intensity in which different types of subjects’ condition must undergo to achieve the health goals. Aerobic exercise has been commonly used to reach weight loss goal.
