Coronary heart disease （CHD） is the leading cause of death worldwide and becomes increasingly prevalent among patients aged 65 years and *** patients are at a higher risk for complications and accelerated physical deconditioning after a cardiovascular event,especially compared to their younger *** last few decades were privy to multiple studies that demonstrated the beneficial effects of cardiac rehabilitation （CR） and exercise therapy on mortality,exercise capacity,psychological risk factors,inflammation,and obesity among patients with ***,a significant portion of the available data in this field pertains to younger patients.A viable explanation is that older patients are grossly underrepresented in these programs for multiple reasons starting with the patient and extending to the *** this article,we will review the benefits of CR programs among the elderly,as well as some of the barriers that hinder their participation.

There has been considerable innovation and advancement in the field of exercise and physical activity (PA).In regards to the battle against chronic non-communicable disease,however,we believe the model of PA that would prove most salu-tary is one closely mimicking that of our hunter-gatherer *** the purpose of longevity,the human body and our genetic makeup have been evolutionarily adapted to respond best to moderate amounts of high-intensity interval training,in conjunction with high amounts of low intensity ***,to optimize resiliency and cardiorespiratory fitness,a hunter-gatherer fitness regimen must include cross training targeted at flexibility,balance and *** not com-monly understood,the health benefits from exercise comprise a reverse *** endurance athletes residing on the furthest reaches of the PA spectrum appear to lose a substantial portion of the exercise-related longevity and cardiovascular benefits due to cardiac overuse ***,there is an emerging body of evidence suggesting leisure time exercise done with peers in a natural environment is significantly superior to that training done in *** idea of the "power of play" lends support to the ancestral model of PA whereby humans are evolutionary adapted to be highly social,outdoor creatures capable of a diverse range of PA at varying intensities.

Purpose To summarize current non-exercise prediction models to estimate cardiorespiratory fitness(CRF),cross-validate these models,and apply them to predict health *** PubMed search was up to August 2018 for eligible *** current review was comprised of three *** first step was to search the literature on non-exercise prediction *** key words combined non-exercise,CRF and one among prediction,prediction model,equation,prediction equation and *** second step was to search the literature about cross-validation of non-exercise *** key words included non-exercise,CRF and one among validation,cross-validation and *** last step was to search for application of CRF assessed from non-exercise *** key words were non-exercise,CRF,mortality,all-cause mortality,cardiovascular disease(CVD)mortality and cancer *** Sixty non-exercise equations were ***,gender,percent body fat,body mass index,weight,height and physical activity status were commonly used in the *** researchers cross-validated non-exercise equations and proved their *** addition,non-exercise estimated CRF was significantly associated with all-cause mortality and fatal and nonfatal *** Measurement of CRF from non-exercise models is practical and viable when exercise testing is not *** the limitations of equations,application of CRF from non-exercise methods showed accuracy and predictive ability.

Objective The primary objective was to investigate temporal trends and between-practice variability of paediatric test use in primary *** and analysis This was a descriptive study of population-based data from Clinical Practice Research Datalink Aurum primary care consultation records from 1 january 2007 to 31 December *** aged 0–15 who were registered to one of the eligible 1464 general practices and had a diagnostic test code in their clinical record were *** primary outcome measures were(1)temporal changes in test rates measured by the average annual percent change,stratified by test type,gender,age group and deprivation level and(2)practice variability in test use,measured by the coefficient of *** 14299598 diagnostic tests were requested over 27.8 million child-years of observation for 2542101 *** test use increased by 3.6%/year(95%CI 3.4 to 3.8%)from 399/1000 child-years to 608/1000 child-years,driven by increases in blood tests(8.0%/year,95%CI 7.7 to 8.4),females aged 11–15(4.0%/year,95%CI 3.7 to 4.3),and children from the most socioeconomically deprived group(4.4%/year,95%CI 4.1 to 4.8).Tests subject to the greatest temporal increases were faecal calprotectin,fractional exhaled nitric oxide and vitamin *** classified as high-use and high-practice variability were iron studies,coeliac testing,vitamin B12,folate,and vitamin *** In this first nationwide study of paediatric test use in primary care,we observed significant temporal increases and practice variability in *** reflects inconsistency in practice and diagnosis rates and a scarcity of evidence-based *** test use generates more clinical activity with significant resource implications but conversely may improve clinical *** research should evaluate whether increased test use and variability are warranted by exploring test indications and test results and directly examine how increased test use impacts

In this paper we present a novel method for dividing and clustering large volumetric scalar out-of-core datasets. This work is based on the Ordered Cluster Binary Tree (OCBT) structure created using a top-down or divisive clustering method. The OCBT structure allows fast and efficient sub volume queries to be made in combination with level of detail (LOD) queries of the tree. The initial partitioning of the large out-of-core dataset is done by using non-axis aligned planes calculated using Principal Component Analysis (PCA). A hybrid OCBT structure is also proposed where an in-core cluster binary tree is combined with a large out-of-core file.

Photodynamic therapy in the treatment of intracranial gliomas:A review of current practice and considerations for future clinical directions

Invasive grade III and IV maliguant gliomas remain difficult to treat with a typical survival time post-diagnosis hovering around 16 months with only minor extension thereof seen in the past decade,whereas some improvements have been obtained towards five-year survival rates for which completeness of resection is a *** techniques such as fluorescence guided resection(FGR)and photodynamic therapy(PDT)are promising adjuvant techniques to in-crease the tumor volume reduction *** has been used in combination with surgical resection or alternatively as standalone treat ment strategy with some sucoess in extending the median survival time of patients compared to surgery alone and the current standard of *** document reviews the outcome of past clinical trials and highlights the general shift in PDT therapeutic *** also looks at the current approaches for interstitial PDT and research options into increasing PDT's glioma treatment fficacy through exploiting both physical and biological based approaches to maximize PDT selectivity and therapeutic index,particularly in brain adjacent to tumor(BAT).Potential reasons for failing to demonstrate a significant survival advantage in prior PDT clinical trials will become evident in light of the improved understanding of glioma biology and PDT dosimetry.

Computational gas dynamical simulations using the WENO-LF method are applied to modeling the high Mach number astrophysical jet XZ Tauri, including the effects of radiative cooling. Mach 55 simulations of the pulsed proto-jet are presented and analyzed in terms of interacting nonlinear waves： terminal Mach disks, bow shocks, and Meshkov-Richtmyer instabilities of the leading jet contact boundary.

Contemporary biomechanical theory of traumatic brain injury has its foundation in Holbourn’s thesis on shear strain and Ommaya’s primate experimentation demonstrating the role of rotation in a variety of lesions including subdural hematoma(SDH)and diffuse axonal *** human observations have since confirmed,for the most part,the early *** concerns regarding primate research,however,have prompted in vitro models,which in turn has led to challenges with respect to the correlation between in vitro observations and the clinical *** these challenges,medicolegal proceedings may call upon biomechanical engineers to reconstruct complex injury scenarios and offer opinions on the scientific plausibility of clinical disease states,such as SDH,hemorrhagic retinopathy,and cerebral edema,associated with hypothetical or proffered action sequences during the course of an unwitnessed *** is important to note,however,that in vitro models by their nature are low‑evidence quality studies that attempt to advance hypotheses but do not address cause and *** a whole,biomechanical models,as they pertain specifically to the brain and spine,are mathematically ***,endpoints of limited relevance are relied upon(e.g.,skull fracture thresholds),which predictably overestimate the in vivo risk of significant *** the increasing role of biomechanical engineering in the interpretation of fatal pediatric head trauma,a heightened awareness of the limitations warranted.
