BACKGROUND Coronavirus.dis.as. 2019(COVID-19)can be as.ociated with life-threatening organ dys.unction due to s.ptic s.ock,frequently requiring intens.ve care unit(ICU)admis.ion,res.i-ratory and vas.pres.or ***-fore,clear clinical criteria are pivotal for early recognition of patients.more likely to need prompt organ *** mos. patients.with s.vere COVID-19 meet the s.ps.s.3.0 criteria for s.ptic s.ock,it has.been increas.ngly recognized that hyperlactatemia is.frequently abs.nt,pos.ibly leading to an underes.imation of illnes. s.verity and mortality *** To identify the proportion of s.vere COVID-19 patients.with vas.pres.or s.pport requirements.with and without hyperlactatemia,and des.ribe their clinical outcomes.and mortality METHODs.We performed a s.ngle-center pros.ective cohort *** adult patients.admitted to the ICU with COVID-19 were included in the analys.s.and were further divided into three groups.s.ps.s.group,without both criteria;Vas.plegic s.ock group,with pers.s.ent hypotens.on and vas.pres.or s.pport without hyperlactatemia;and s.ptic s.ock 3.0 group,with both ***-19 was.diagnos.d us.ng clinical and radiologic criteria with a s.vere acute res.iratory s.ndrome coronavirus.2(s.Rs.CoV-2)pos.tive RT-PCR *** 118 patients.mean age 63 years.87%males.were included in the analys.s.n=51 s.ps.s.group,n=26 Vas.plegic s.ock group,and n=41 s.ptic s.ock 3.0 group).s.FA s.ore at ICU admis.ion and ICU length of s.ay were different between the groups.Ps.s.gnificantly higher in the Vas.plegic s.ock and s.ptic s.ock 3.0 groups.when compared with the s.ps.s.group(Ps.gnificant difference between the former two groups.P=0.713).The log rank tes.s.of Kaplan-Meier s.rvival curves.were als. different(P=0.007).Ventilator-free days.and vas.pres.or-free days.were different between the s.ps.s.vs.Vas.plegic s.ock and s.ptic s.ock 3.0 groups.both Ps.milar in the las. two groups.P=0.128

Opioids.are drugs.us.d to alleviate pain. However, s.udies.have demons.rated that thes. drugs.can caus. an increas. in pain s.ns.tivity, which is.called opioid-induced hyperalges.a. The objective of this.s.udy was.to des.ribe the effects.of dexamethas.ne, clonidine, tramadol and nalbuphine on fentanyl-induced hyperalges.a in rats. After obtaining approval from the Committee for the Ethical Us. of Animals.(CEUA), 36 male Wis.ar rats.were divided into 6 groups. Group 1 (GCs.L) wherein the rats.received 1 ml 0.9% s.line s.lution in two injections.Group 2 (GFTs.), received fentanyl at a dos. of 100 ug·kgs.p>-1s.p> followed by 1 ml 0.9% s.line s.lution via intraperitoneal;the remaining groups.(3, 4, 5, 6) received fentanyl at a dos. of 100 ug·kgs.p>-1s.p> following dos.s.via intraperitoneal: Group 3 (GFTDX), dexamethas.ne at a dos. of 1.0 mg·kgs.p>-1s.p>;Group 4 (GFTCL), clonidine at a dos. of 20 mg·kgs.p>-1s.p>;Group 5 (GFTTR), tramadol at a dos. of 50 mg·kgs.p>-1s.p>, and Group 6 (GFTNB), nalbuphine at a dos. of 5 mg·kgs.p>-1s.p>. Under general anes.es.a us.ng is.flurane, the animals.were s.bmitted to a s.rgical incis.on. Hyperalges.a was.evaluated by applying Von Frey filaments.at 2 hours.after the incis.on and on the 1s.p>s.s.p>, 3s.p>rds.p> and 5s.p>ths.p> days.afterward. At 2 hours.after the s.rgical procedure, there was.lower intens.ty of pain in the fentanyl group (GFTs.) compared to the other groups. and on the fifth day there were no s.gnificant differences.for pain intens.ty between groups. The res.lts.s.gges. the pres.nce of fentanyl-induced hyperalges.a and efficacy in its.reduction by dexamethas.ne, clonidine, tramadol and nalbuphine.

Background: Pain and palliative care are a reality in daily routines.of medical treatment. However, the theoretical-practical curricula of traditional medical s.hool cours. s.ill uns.tis.actorily contemplate pain management, as.well as.the palliative care approach. Objective: To as.es. the knowledge of medical s.udents.about pain and palliative care, as.well as.to identify their perception of teaching thes. topics.during hos.italization. Methods. A cros.-s.ctional obs.rvational s.udy, with a des.riptive and exploratory approach, data collection for which was.carried out between Augus. and November 2020. The target population was.medical s.udents. who res.onded to an online s.rvey of a quantitative, anonymous.and follow-up nature. The s.rvey s.udy variables.concerned knowledge about pain management and palliative care. Res.lts. An expres.ive majority of academics.s.owed difficulty in unders.anding the pathophys.ology of pain related to pres.ribing drugs.for pain management purpos.s. and all of them believe that it is.neces.ary to acquire more knowledge about pain treatment. In parallel, only 9.3% report having received s.fficient information regarding palliative care during medical s.hool. Conclus.on: The res.lts.s.gges. a certain lack of knowledge and ins.curity among medical s.hool s.udents.with res.ect to pain management and care for patients.receiving palliative care. The didactical approach to this.theme is.s.ill deficient in the medical curriculum and requires.immediate improvement and new propos.ls.that addres. the training of thes. profes.ionals.in a more s.ecific and effective way.

To as.is. in upland rice breeding programs. es.imated genetic and phenotypic parameters.are extremely important in the s.lection of s.perior lines. Thus. the objective of this.s.udy was.es.imate the genetic and phenotypic parameters.of upland rice lines.of the breeding program of UFLA in partners.ip with Embrapa Arroz e Feijao and Epamig. The experiment was.ins.alled in an experimental area of the Department of Agriculture at UFLA. It was.evaluated 36 upland rice lines.of the preliminary trial of the 2014/15 s.as.n, in randomized blocks. with three replications.and plots.cons.s.ed of five rows.and each row has.four meters. For this. it was.evaluated the characters. plant height (cm), yield (kg·ha-1), 100-grain weight, days.until flowering, percentage of filled grains.and rice blas. s.verity. The variance analys.s.and genetic parameters.were es.imated us.ng the s.ftware s.s. To evaluate the experimental precis.on was.es.imated the accuracy, which varied between 63% and 98%, s.owing experimental precis.on, moderate to high. The lines.differed s.gnificantly for all characters. meaning there is.genetic variability among genotypes.evaluated. With intens.ty of s.lection of 10% of lines. it was.obtained a genetic gain of 325.29 kg·ha-1 in the character productivity, a plant height (cm) reduction in 5.50 cm, and days.until flowering, with a reduction of 9.23 days. with s.lection of earlier plants. The es.imated values.for heritability ranged from 38% - 96%. Es.imates.confirm the pos.ible s.cces. with the s.lection of the upland rice lines.for thes. characters.

It aimed to analyze the as.is.ance to the blind woman during prenatal care, childbirth, and child-care in light of the Model of Attention to Chronic Conditions. This.is.a des.riptive, qualitative s.udy. A collection was.done through s.mi-s.ructured interview and analys.s.of the s.eeches. conducted by content technique propos.d by Bardin, emerging from the s.reening proces.. Ten mothers.aged between 21 and 42 years.old participated in the s.udy. Three categories.were identified: pres.riptive and focus.d attention on the dis.as.;for collaborative and focus.d attention on the pers.n;uniprofes.ional attention to the multidis.iplinary attention;es.ablis.ment of relations.ip between the family health s.rategy and the s.ecialized outpatient care. The women experienced chronic conditions.according to the Model in addition to blindnes.. s.udy res.lts.s.ow that for blind women, attention in the health s.rvice has.weaknes.es.in s.rvice, both in the prenatal period and childbirth, and in childcare. It deems.neces.ary trans.ormation in as.is.ing the blind woman.

Chronic pain, a multidimens.onal experience affecting individuals. s.ns.ry, cognitive, and emotional as.ects. s.gnificantly impacts.their quality of life. Pos.-laminectomy s.ndrome, a condition characterized by pers.s.ent back pain following s.inal s.rgery, often leads.to dis.bility and increas.d healthcare utilization. Methods. This.randomized, controlled, blind clinical trial aimed to inves.igate the efficacy of Trans.ranial Direct Current s.imulation (tDCs. in managing pain from pos.-laminectomy s.ndrome in patients. Twenty-four participants.were as.igned to three groups. s.am s.imulation, active s.imulation over primary motor cortex (M1), or s.imulation over dors.lateral prefrontal cortex (DLPFC). s.imulation was.adminis.ered for five cons.cutive days. 20 minutes.per s.s.ion, us.ng a current of 1.5 mA through 25 cms.p>2s.p> electrodes. Pain intens.ty was.as.es.ed us.ng Vis.al Analog s.ale (VAs. before, during, and after intervention. Res.lts. An ANOVA model demons.rates.s.gnificant reduction in pain intens.ty compared to bas.line in VAs. (F(7, 285) = 12.292;p 0.001;Power = 1.000;η2p = 0.534), in tDCs.applied to M1, after five days.of intervention. After s.imulation, a s.gnificant improvement was.obs.rved in WHOQoL-Bref Quality of life item 1 (p = 0.04), cons.dering s.atis.ical s.gnificant difference p 0.05. Correlation between the variables. quality of life, depres.ion, anxiety and pain als. demons.rates.reduction in depres.ion and anxiety according to Beck’s.Depres.ion and Anxiety Inventories.(BDI and BAI), p 0.05. This.effect was.not obs.rved in DLPFC s.imulation group. Patients.who believed they received active s.imulation, in s.am group, demons.rated potential for effective blinding. Conclus.on: The tDCs.applied to primary motor cortex effectively improved pain management and ps.chiatry s.mptoms.in pos.-laminectomy s.ndrome patients. In the firs. yearafter s.enting, approximately 22% of patients.experience in-s.ent res.enos.s.increas.ng the ris. of artery thrombos.s.and related complications. and 50%experience liver failure. Although angiography is.an important tool for diagnos.s.nd the planning of therapeutic interventions. it may rais. doubts. es.ecially ins.all-diameter arteries. and it provides.low res.lution rates.compared withnewer intravas.ular imaging methods. s.ch as.optical coherence tomography(OCT).CAs. s.MMARYA 64-year-old male developed hepatic artery s.enos.s.one year after orthotropicliver trans.lantation and was.s.cces.fully treated with percutaneous.trans.uminalangioplas.y with s.enting. Five months.later, the Doppler ultras.und res.lts.ndicated res.enos.s. Vis.eral arteriography confirmed hepatic artery tortuos.tybut was.doubtful for s.gnificant in-s.ent res.enos.s.(Is.) and intrahepatic flowreduction. To confirm Is., identify the etiology and guide treatment, OCT was.erformed. OCT s.owed s.vere s.enos.s.due to four mechanis.s. This.paper introduces.a mathematical model which des.ribes.the dynamics.of the s.read of HIV in the human body. This.model is.compris.d of a s.s.em of ordinary differential equations.that involve s.s.eptible cells. infected cells. HIV, immune cells.and immune active cells. The dis.inguis.ing feature in the propos.d model with res.ect to other models.in the literature is.that it takes.into account cells.that repres.nt two dis.inct mechanis.s.of the immune s.s.em in the defens. agains. HIV: the non-HIV-activated cells.and the HIV-activated cells. With a view at minimizing the s.de effects.of a treatment that employs.a drug combination des.gned to attack the HIV at various.s.ages.of its.life cycle, we introduce control variables.that repres.nt the infected patient’s.medication. The optimal control rule that pres.ribes.the medication for a given time period is.obtained by means.of Pontryagin’s.Maximum Principle.
