Serum ferritin and the risk of early-onset colorectal cancer

baCKGROUND The incidence of early-onset colorectal cancer(EO-CRC)is rising in the United States,and is often diagnosed at advanced *** serum ferritin is often incidentally discovered in young adults,however,the indication for endoscopy in EO-CRC is *** To compare serum ferritin between patients with EO-CRC and healthy controls(HCs),and examine the association of serum ferritin in EO-CRC with patient-and disease-specific *** A retrospective study of patients<50 years with newly-diagnosed EO-CRC was conducted from 1/2013-12/*** were included if serum ferritin was measured within 2 years prior to 1 year following CRC histologic *** supplement the analysis,a cohort of HCs meeting similar inclusion and exclusion criteria were identified for comparison.A sensitivity analysis including only patients with serum ferritin obtained at or before diagnosis was separately performed to minimize risk of *** Among 85 patients identified with EO-CRC(48 females),the median serum ferritin level was 26 ng/mL(range<1-2759 ng/mL).Compared to HCs(n=80211),there were a higher proportion of individuals with EO-CRC with serum ferritin<20 ng/mL(female 65%,male 40%)versus HCs(female 32.1%,male 7.2%)age 29-39 years(P=0.002 and P<0.00001,respectively).Stage IV disease was associated with significantly higher serum ferritin compared to less advanced stages(P<0.001).Serum ferritin obtained before or at the time of diagnosis was lower than levels obtained after *** findings were confirmed in the sensitivity *** Severe iron deficiency may indicate an increased risk of EO-CRC,particularly at earlier *** studies defining the optimal serum ferritin threshold and routine incorporation of serum ferritin in screening algorithms is essential to develop more effective screening strategies for EO-CRC.

背景本研究旨在评价全麻诱导期间小儿麻醉主治医师、住院医师及患儿母亲三者预测2．16岁儿童（n=125）手术前焦虑的能力。方法麻醉医师及患儿母亲运用视觉模拟评分对患儿的手术前焦虑提供预测，患儿的实际焦虑状况则使用有效行为观测工具——改良耶鲁手术前焦虑量表进行评估。所有患儿母亲在全麻诱导期间均在场，所有患儿均不接受手术前镇静用药。采集数据后进行相关分析。结果125例2—16岁的患儿、患儿母亲及实施麻醉的主治医师和住院医师共同参与了此项研究。相关分析显示全麻诱导期间麻醉主治医师对患儿手术前焦虑的预测与患儿实际焦虑状况之间存在显著相关性（rs=0.38，P〈0.001）。而诱导期间麻醉住院医师和患儿母亲的预测与患儿客观焦虑状况之间无显著相关（rs分别为0.01及0.001）。就预测的准确性而言，在麻醉主治医师所提供的预测中，47．2％与患儿的客观焦虑状况在一个标准差误差范围内，70．4％在2个标准差误差范围内。结论相比患儿母亲，参与实施儿科麻醉的主治医师在全麻诱导期间更能准确地预测儿童患者的手术前焦虑。虽然这一结论具有重要的临床意义，但是否可将其推广至其他较少从事儿科麻醉的麻醉主治医师尚未知。

CD8+T cell metabolic flexibility elicited by CD28-ARS2 axisdriven alternative splicing of PKM supports antitumor immunity

background Triple-negative breast cancer(TNBC) is the most aggressive breast cancer subtype and leads to the poorest patient outcomes despite surgery and chemotherapy treatment. Exploring new molecular mechanisms of TNBC that could lead to the development of novel molecular targets are critically important for improving therapeutic options for treating *** We sought to identify novel therapeutic targets in TNBC by combining genomic and functional studies with lipidomic analysis, which included mechanistic studies to elucidate the pathways that tie lipid profile to critical cancer cell properties. Our studies were performed in a large panel of human breast cancer cell lines and patient *** Comprehensive lipid profiling revealed that phospholipid metabolism is reprogrammed in TNBC cells. We discovered that patatin-like phospholipase domain-containing lipase 8(PNPLA8)is overexpressed in TNBC cell lines and tissues from breast cancer patients. Silencing of PNPLA8disrupted phospholipid metabolic reprogramming in TNBC, particularly affecting the levels of phosphatidylglycerol(PG), phosphatidylcholine(PC), lysophosphatidylcholine(LPC) and glycerophosphocholine(GPC). We showed that PNPLA8 is essential in regulating cell viability,migration and antioxidation in TNBC cells and promoted arachidonic acid and eicosanoid production, which in turn activated PI3K/Akt/Gsk3β and MAPK *** Our study highlights PNPLA8 as key regulator of phospholipid metabolic reprogramming and malignant phenotypes in TNBC, which could be further developed as a novel molecular treatment target.

Metabolic flexibility has emerged as a critical determinant of CD8+T-cell antitumor activity,yet the mechanisms driving the metabolic flexibility of T cells have not been *** this study,we investigated the influence of the nuclear cap-binding complex(CBC)adaptor protein ARS2 on mature T *** doing so,we discovered a novel signaling axis that endows activated CD8+T cells with flexibility of glucose ***2 upregulation driven by CD28 signaling reinforced splicing factor recruitment to pre-mRNAs and affected approximately one-third of T-cell activation-induced alternative splicing *** these effects,the CD28-ARS2 axis suppressed the expression of the M1 isoform of pyruvate kinase in favor of PKM2,a key determinant of CD8+T-cell glucose utilization,interferon gamma production,and antitumor effector ***,PKM alternative splicing occurred independently of CD28-driven PI3K pathway activation,revealing a novel means by which costimulation reprograms glucose metabolism in CD8+T cells.

Traumatic brain injury(TBI) is a leading cause of death and disability worldwide,with estimates indicating that -50% of the world's population will acquire a head injury at some point in their lifetime(Maas et al.,2017).Mild TBIs account for -80% of all reported cases,with up to 43% of all TBI patients reporting symptoms beyond 2 weeks *** of 2016,only 7% of preclinical TBI experiments included both male and female sexes,and fewer studies analyzed the data using sex as a biological variable(Spani et al.,2018).

As China works to move away from coal in its energy system as a critical strategy to achieve carbon neutrality before 2060,there are several challenges which will affect the coal *** challenge that has not been fully explored is the impact on coal *** are millions of people employed in various sectors along the coal supply chain,including mining,processing,transport,and power *** this study,we developed a framework to understand the magnitude and characteristics of the employment challenges associated with the coal transition across ***,the magnitude and characteristics of the challenges are evaluated from multiple dimensions,including existing coal sector employment,coal production,coal consumption,GDP contribution from the coal industry,and coal mining productivity,to explore how the transition of the coal industry will likely affect different *** results showed that provinces with large total coal employment today are often those with high coal production and consumption,high GDP contribution from the coal industry,and low productivity of coal *** found that employment impacts of a coal transition will not be equally distributed in China,as both the magnitude and characteristics of the employment challenges vary substantially across all *** is therefore important to develop a tailored strategy to tackle specific needs for each province.

Clinically effective immune checkpoint inhibitors(ICI)have transformed cancer treatment over the last *** progress was built on a foundation of understanding tumor immunology,immune checkpoint biology,and biomarkers for sensitivity to ICI(1).Monoclonal antibodies disrupting the PD-L1/PD-1 axis or CTLA-4 are highly effective in tumors that are primed for an anti-tumor immune response but suppressed by immune checkpoints(1).This is the case for cancer types with abundant neoantigens,such as melanoma and lung cancer,and in tumors regardless of histology that have elevated tumor mutation burden(TMB)or microsatellite instability(MSI)due to mismatch repair deficiency(dMMR).Elevated expression of PD-L1 is a biomarker of immunosuppression and predicts sensitivity to PD-(L)1 blockade in several tumor types,including lung and gastroesophageal *** cholangiocarcinoma(CCA),the immune landscape and factors predicting response to immunotherapy are less well understood(2).
