Our goal as medical educators is for our students to learn what they need to know and competently perform ophthalmic *** addition,the students need to have good communication skills and behave professionally and ethically(1,2).Most medical educators are not taught how to teach but rather learn by role-modeling their *** can be good or bad depending on the mentor!In addition,the amount of knowledge and numbers of surgical procedures is ever increasing and yet,in most countries,the amount of time to train and become competent is not ***,our goal of creating competent ophthalmologists is ever more challenging.

Patients with diabetes mellitus(DM) need psychological support throughout their life span from the time of diagnosis. The psychological make-up of the patients with DM play a central role in self-management behaviors. Without patient's adherence to the effective therapies, there would be persistent sub-optimal contro of diseases, increase diabetes-related complications,causing deterioration in quality of life, resulting in increased healthcare utilization and burden on healthcare systems. however, provision of psychosocial support is generally inadequate due to its challenging nature of needs and demands on the healthcare systems. This review article examines patient's psychological aspects in general, elaborates in particular about emotion effects on health, and emotion in relation to other psychological domains such as cognition, self-regulation,self-efficacy and behavior. Some descriptions are also provided on willpower, resilience, illness perception and proactive coping in relating execution of new behaviors,coping with future-oriented thinking and influences of illness perception on health-related behaviors. These psychological aspects are further discussed in relationto DM and interventions for patients with DM. Equipped with the understanding of the pertinent nature of psychology in patients with DM; and knowing the links between the psychological disorders, inflammation and cardiovascular outcomes would hopefully encourages healthcare professionals in giving due attention to the psychological needs of patients with DM.

AIM：To assess strabismus control and motor ocular alignment for basic exotropia surgery at 5y ***：The medical records of 80 consecutive patients aged less than 17 years of age,who underwent surgery for basic exotropia by a single surgeon between years 2000 to 2009 and completed a minimum of 5y follow-up post-operatively were ***- and post-operative characteristics were documented at 1wk,6mo,1,3 and 5y *** at 5-year follow-up were assigned to the success group if they had a postoperative angle of deviation within 10 prism diopters of exotropia or within 5 prism diopters of esotropia for distance on prism cover test,and had moderate to good strabismus *** remaining subjects were assigned to the failure ***：Post-operative surgical success at one week was 75%,which decreased to 41% at 5y *** success group was noted to have more patching pre-operatively（P=0.003）.The duration of patching a day（P=0.020） and total duration of patching preoperatively（P=0.030） was higher in the success *** success at 1y（P=0.004） and 3y（P=0.002） were associated with higher surgical success at 5y ***：Post-operative motor alignment and strabismus control for basic exotropia surgery at 1y and beyond is associated with higher exotropia surgery success at 5-year *** is an association between pre-operative patching and 5-year surgical success of basic intermittent exotropia surgery.

目的:针对新加坡的花岗岩地质结构,研究将典型马蹄形隧道挖掘成双拱隧道时岩石支撑的设计问题。利用离散元法分析侧边导坑对隧道主体开口的影响。创新点:1.基于平面应变假设,提出一个针对性的离散元模型以分析侧边导坑的设置对主体隧道支撑的影响;2.此模型适用于解决底下长跨度开口的支撑设计问题,如大众捷运系统隧道和紧贴式双孔隧道的建造,补充了传统经验主义的设计方法。方法:1.采用离散元法对马蹄形隧道开口支撑问题进行非连续分析;2.采用二维平面应变模型简化问题。结论:1.离散元法可用于分析有限岩石覆盖层的失效机制,单隧道支撑要求以及增加侧边导坑后的支撑要求;2.模型计算结果显示,侧边导坑的挖掘使得支柱总的螺栓连接作用力增加了一倍。

Oil palm biomass(OPB)represents major portion of the lignocellulosic waste in Malaysia that can be converted into *** review aims to provide important insights in OPB-derived bio-oil production by first discussing the chemical compositions of different OPB and their effects to the bio-oil yield and quality obtained from pyrolysis process,followed by discussing the addition of plastics and catalysts into the pyrolysis for bio-oil upgrading,and lastly summarizing the existing technoeconomic and environmental studies and the potential use of process integration and intensification in this ***(PP),low density polyethylene(LDPE),and high density polyethylene(HDPE)have been commonly used in co-pyrolysis of OPB,which can effectively increase the heating value of bio-oil up to 80%that of ***,acidic,basic,and neutral catalysts have been applied to increase the amount of hydrocarbon and phenol in the bio-oil,further improving the heating value to be comparable to *** bio-oil production from OPB is currently still limited to demonstration scale despite the favorable environmental compatibility and technoeconomic feasibility shown by studies focused on empty fruit bunch(EFB).Several promising advanced pyrolysis processes that integrate other processes such as anaerobic digestion,hydrogen production process,and heat and power generation units as well as the advanced reactor designs are also overviewed here as future innovation of the bio-oil production from OPB,which may play more significant role as the technology matures.

The reduction of carbon dioxide to methane by hydrogen("CO2 methanation")using renewable energy is a promising process for recycling *** catalysts and better reactors are both required for the practical application of CO2 *** study examines how the operating parameters affect CO2 methanation in a highly efficient fluidized-bed *** first measured the kinetics of the CO2 methanation reaction using an NiMgW catalyst,which has been reported to exhibit superior catalytic *** then developed a fluidized-bed reactor model based on an earlier model for CO2 *** fluidized bed model indicated that the NiMgW was indeed superior to two other previously studied catalysts in terms of faster conversion of reactants and higher concentrations of product CH4 throughout the *** overall rate of production of CH4 increased with temperature and H2/CO2 ratio and decreased as the inlet reactant flow rate,catalyst particle diameter,and catalyst particle sphericity increased.

Many orogenic belts in the world exhibit accretionary and collisional orogenic phases to varying *** accretion evolves into collision of the Central Asian Orogenic Belt(CAOB),the largest Phanerozoic accretionary orogenic belt,is an intriguing *** this paper,we present new U-Pb age,geochemical and isotopic data for Permian-Triassic granitoids from middle Inner Mongolia,Northern China in the southeastern CAOB,and delineate the magmatic transition from subduction to(soft)*** magmatic record of soft collision is identified and characterized by thickened lower crust-derived high Sr/Y granitoids with a sub-linear distribution along the Solonker suture *** from Early Permian to Late Permian became more enriched in whole-rock Nd and zircon Hf isotopic compositions(εNd(t)values from 2.4 to-19.5,εHf(t)values from 11.6 to-33.7),indicating increasing incorporation of old *** change in peak timing of magmatism from west(ca.264 Ma)to east(ca.251 Ma)along the Solonker suture zone implies"scissor-like"closure of the Paleo-Asian *** with previous studies,a three-stage tectonic model from the Permian to Triassic by accretion leading to collision on the south-eastern margin of CAOB is proposed.(1)Early Permian(>ca.285 Ma):Juvenile magmatism on an active continental margin with double-sided subduction of the Paleo-Asian Ocean;(2)Middle Permian to Middle Triassic(ca.285–235 Ma):Magma source transition from juvenile to old crust induced by a tectonic switch from arc to"scissor-like"closure and subsequent intracontinental orogenic contraction;(3)Late Triassic(

目的　多药耐药(multidrugresistance,MDR)是目前临床肿瘤治疗的主要障碍。本文研制了新型阿霉素抗耐药性隐形脂质体(DARSLs),并对其体外细胞毒和体内毒性进行评价。方法　采用硫酸铵梯度法将阿霉素(DOX)和维拉帕米(VER)药物同时包载到隐形脂质体内,制备成DARSLs;采用耐药性鼠前列腺肿瘤细胞株MLLB2和人子宫肉瘤细胞株MES SA/DX5进行体外细胞毒性评价;采用SD大鼠对阿霉素抗耐药性隐形脂质体进行体内毒性评价。结果　在药脂比(DOX/VER/Lipid,w/w/w)为1∶0 11∶10时,阿霉素包封率大于90%,维拉帕米包封率约为70%。平均粒径为(118 .1±22. 3)nm。体外细胞毒性实验证实该脂质体能够在体外有效地逆转肿瘤细胞耐药性,并导致耐药肿瘤细胞生长抑制。体内系统毒性及心脏毒性实验结果显示,该脂质体能够明显改善游离阿霉素单独使用或与维拉帕米联合使用时产生的全身毒性,尤其是心脏毒性。结论　DARSLs具有相对较低的毒性,且能有效抑制耐药肿瘤的生长。
