The local scour around a new pile-group foundation of offshore wind turbine subjected to a bi-directional current was physically modeled with a bi-directional flow flume. In a series of experiments, the flow velocity and topography of the seabed were measured based on a system composed of plane positioning equipment and an ADV.Experimental results indicate that the development of the scour hole was fast at the beginning, but then the scour rate decreased until reaching equilibrium. Erosion would occur around each pile of the foundation. In most cases, the scour pits were connected in pairs and the outside widths of the scour holes were larger than the inner widths. The maximum scour depth occurred at the side pile of the foundation for each test. In addition, a preliminary investigation shows that the larger the flow velocity, the larger the scour hole dimensions but the shorter equilibrium time. The field maximum scour depth around the foundation was obtained based on the physical experiments with the geometric length scales of 1:27.0, 1:42.5 and 1:68.0, and it agrees with the scour depth estimated by the HEC-18 equation.

Market participants can only bid with lagged information disclosure under the existing market mechanism,which can lead to information asymmetry and irrational market behavior,thus influencing market efficiency.To promote rational bidding behavior of market participants and improve market efficiency,a novel electricity market mechanism based on cloudedge collaboration is proposed in this paper.Critical market information,called residual demand curve,is published to market participants in real-time on the cloud side,while participants on the edge side are allowed to adjust their bids according to the information disclosure prior to closure gate.The proposed mechanism can encourage rational bids in an incentive-compatible way through the process of dynamic equilibrium while protecting participants’privacy.This paper further formulates the mathematical model of market equilibrium to simulate the process of each market participant’s strategic bidding behavior towards equilibrium.A case study based on the IEEE 30-bus system shows the proposed market mechanism can effectively guide bidding behavior of market participants,while condensing exchanged information and protecting privacy of participants.

As voltage measuring devices are widely used in the high voltage power system with a primary-secondary-fusion structure,electronic voltage transformers are directly connected to the primary conductor and would face much more serious electromagnetic environments than general secondary equipment.As a result,they would still experience failures even when general protection measures are adopted to suppress overvoltages'amplitudes.To address this issue,an innovative method is proposed after analyses are conducted on the resonance between a transformer and an overvoltage.Within this method,an air-cored coil is installed in a transformer to artificially shift its natural frequency and to dodge the high-frequency dominant component of a transient overvoltage,avoiding the aforementioned resonance and suppressing the secondary overvoltage.In addition,simulations and laboratory tests are conducted to prove this method's validity.

The accumulating evidence revealed that gut microbiota plays an important role in pathological process of disease including obesity,type 2 diabetes mellitus,heart failure,and non-alcoholic fatty liver disease.Polysaccharides extracted from Chinese medicine(CM)can not only alleviate pathological status but also promote health by anti-inflammatory,regulating immunity,lowering blood glucose and lipids,anti-cancer,and anti-oxidation.The alterations of gut microbiota composition and metabolism pathways are the potential mechanisms of CM polysaccharides treatment.In addition,they exert functions through gut-organ axis or play an indirect role by synergistic actions with other drugs or components mediated by gut microbiota.This review summarizes the molecular mechanisms of CM polysaccharides interacted with intestinal microbial inhabitants as potential prebiotics forpromoting health.

BACKGROUND According to practice guidelines,endoscopic band ligation(EBL)and endoscopic tissue adhesive injection(TAI)are recommended for treating bleeding from esophagogastric varices.However,EBL and TAI are known to cause serious complications,such as hemorrhage from dislodged ligature rings caused by EBL and hemorrhage from operation-related ulcers resulting from TAI.However,the optimal therapy for mild to moderate type 1 gastric variceal hemorrhage(GOV1)has not been determined.Therefore,the aim of this study was to discover an individualized treatment for mild to moderate GOV1.AIM To compare the efficacy,safety and costs of EBL and TAI for the treatment of mild and moderate GOV1.METHODS A clinical analysis of the data retrieved from patients with mild or moderate GOV1 gastric varices who were treated under endoscopy was also conducted.Patients were allocated to an EBL group or an endoscopic TAI group.The differences in the incidence of varicose relief,operative time,operation success rate,mortality rate within 6 wk,rebleeding rate,6-wk operation-related ulcer healing rate,complication rate and average operation cost were compared between the two groups of patients.RESULTS The total effective rate of the two treatments was similar,but the efficacy of EBL(66.7%)was markedly better than that of TAI(39.2%)(P<0.05).The operation success rate in both groups was 100%,and the 6-wk mortality rate in both groups was 0%.The average operative time(26 min)in the EBL group was significantly shorter than that in the TAI group(46 min)(P<0.01).The rate of delayed postoperative rebleeding in the EBL group was significantly lower than that in the TAI group(11.8%vs 45.1%)(P<0.01).At 6 wk after the operation,the healing rate of operation-related ulcers in the EBL group was 80.4%,which was significantly greater than that in the TAI group(35.3%)(P<0.01).The incidence of postoperative complications in the two groups was similar.The average cost and other related economic factors were greater for the EBL than for the TAI(P<0.01).CONC

BACKGROUND Breast cancer brain metastasis(BCBM)is an advanced breast disease that is difficult to treat and is associated with a high risk of death.Patient prognosis is usually poor,with reduced quality of life.In this context,we report the case of a patient with HER-2-positive BCBM treated with a macromolecular mAb(ine-tetamab)combined with a small molecule tyrosine kinase inhibitor(TKI).CASE SUMMARY The patient was a 58-year-old woman with a 12-year history of type 2 diabetes.She was compliant with regular insulin treatment and had good blood glucose control.The patient was diagnosed with invasive carcinoma of the right breast(T3N1M0 stage IIIa,HER2-positive type)through aspiration biopsy of the ipsilateral breast due to the discovery of a breast tumor in February 2019.Immunohistochemistry showed ER(-),PR(-),HER-2(3+),and Ki-67(55-60%+).Preoperative neoadjuvant chemotherapy,i.e.,the AC-TH regimen(epirubicin,cyclophosphamide,docetaxel-paclitaxel,and trastuzumab),was administered for 8 cycles.She underwent modified radical mastectomy of the right breast in November 2019 and received tocilizumab targeted therapy for 1 year.Brain metastasis was found 9 mo after surgery.She underwent brain metastasectomy in August 2020.Immunohistochemistry showed ER(-)and PR.(-),HER-2(3+),and Ki-67(10-20%+).In November 2020,the patient experienced headache symptoms.After an examination,tumor recurrence in the original surgical region of the brain was observed,and the patient was treated with inetetamab,pyrotinib,and capecitabine.Whole-brain radiotherapy was recommended.The patient and her family refused radiotherapy for personal reasons.In September 2021,a routine examination revealed that the brain tumor was considerably larger.The original systemic treatment was continued and combined with intensity-modulated radiation therapy for brain metastases,followed by regular hospitalization and routine examinations.The patient’s condition is generally stable,and she has a relatively high quality of life.This case report demonstrates that

Prevalence and clinical impact of sarcopenia in liver transplant recipients:A meta-analysis

BACKGROUND The prevalence of sarcopenia in patients undergoing liver transplantation(LT)remains to be determined partly because of different diagnostic criteria.Sarcopenia has recently been recognized as a new prognostic factor for predicting outcomes in LT candidates.AIM To estimate the prevalence of sarcopenia and evaluate its clinical effect on LT candidates.METHODS This systematic search was conducted in PubMed,Web of Science,Embase,and Cochrane Library for original English-language articles that investigated the prevalence and influence of sarcopenia in patients undergoing LT from database inception to November 30,2022.Cohort studies of the definition of sarcopenia that estimate sarcopenia prevalence and evaluate its effect on clinical outcomes and the risk of mortality were included.RESULTS Twenty-five studies involving 7760 patients undergoing LT were included.The pooled prevalence of sarcopenia in patients undergoing LT was 40.7%[95%confidence intervals(95%CI):32.1-49.6].The 1-,3-,and 5-year cumulative probab-ilities of post-LT survival in patients with preoperative sarcopenia were all lower than those without sarcopenia(P<0.05).Sarcopenia was associated with an increased risk of post-LT mortality in patients undergoing LT(adjusted hazard ratio:1.58;95%CI:1.21-2.07).Patients with preoperative sarcopenia had a longer intensive care unit stay,a high risk ratio of sepsis,and serious post-LT complications than those without sarcopenia.CONCLUSION Sarcopenia is prevalent in a substantial proportion of patients undergoing LT and is strongly and independently associated with higher a risk of mortality risk.

Esophageal cancer ranks among the most prevalent malignant tumors globally,primarily due to its highly aggressive nature and poor survival rates.According to the 2020 global cancer statistics,there were approximately 604000 new cases of esophageal cancer,resulting in 544000 deaths.The 5-year survival rate hovers around a mere 15%-25%.Notably,distinct variations exist in the risk factors associated with the two primary histological types,influencing their worldwide incidence and distribution.Squamous cell carcinoma displays a high incidence in specific regions,such as certain areas in China,where it meets the cost-effect-iveness criteria for widespread endoscopy-based early diagnosis within the local population.Conversely,adenocarcinoma(EAC)represents the most common histological subtype of esophageal cancer in Europe and the United States.The role of early diagnosis in cases of EAC originating from Barrett's esophagus(BE)remains a subject of controversy.The effectiveness of early detection for EAC,particularly those arising from BE,continues to be a debated topic.The variations in how early-stage esophageal carcinoma is treated in different regions are largely due to the differing rates of early-stage cancer diagnoses.In areas with higher incidences,such as China and Japan,early diagnosis is more common,which has led to the advancement of endoscopic methods as definitive treatments.These techniques have demonstrated remarkable efficacy with minimal complications while preserving esophageal functionality.Early screening,prompt diagnosis,and timely treatment are key strategies that can significantly lower both the occurrence and death rates associated with esophageal cancer.

The Xingluokeng deposit is the largest gran-ite-related tungsten deposit within the Wuyi metallogenic belt in South China.The Xingluokeng intrusion primarily consists of porphyritic biotite granite,biotite granite,andfine-grained granite.The deposit is represented by veinlet-disseminated mineralization with K-feldspathization and biotitization,alongside quartz-vein mineralization with gre-isenization and sericitization.This study investigates in-situ analyses of quartz compositions from both the intrusion and hydrothermal veinlets and veins.Trace element correlations indicate that trivalent Al^(3+)and Fe^(3+)replace Si^(4+)within the quartz lattice,with monovalent cations(such as Li^(+),Na^(+),and K^(+))primarily serving as charge compensators.Low Ge/Al ratios(<0.013)of quartz from granites suggest a mag-matic origin.The low Al/Ti and Ge/Ti ratios,accompanied by high Ti contents in quartz,suggest that the porphyritic biotite granite and biotite granite are characterized by rela-tively low levels of differentiation and high crystallization temperatures.In contrast,thefine-grained granite exhibits a higher degree of fractionation,lower crystallization tem-peratures,and a closer association with tungsten miner-alization.Ti contents in quartz from quartz veins indicate Qz-Ⅰformed at temperatures above 400°C,while Qz-Ⅱto Qz-Ⅴformed at temperatures below 350°C.Variations in different generations of quartz,as indicated by Al content and(Al+Fe)/(Li+Na+K)ratio,suggest that Qz-Ⅰprecipi-tated from a less acidicfluid with a stable pH,whereas Qz-Ⅱto Qz-Ⅴoriginated from a more acidicfluid with notable pH variations.Consequently,alkaline alteration and acidic alteration supplied the essential Ca and Fe for the precipita-tion of scheelite and wolframite,respectively,highlighting a critical mechanism in tungsten mineralization at the Xin-gluokeng deposit.
