Laparoscopic management of remnant gall bladder with stones: Lessons from a tertiary care centre's experience

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone *** its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this *** objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant ***:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in South *** patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were *** following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative ***:In total,36 patients were included and *** majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 *** most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open *** fistula to the colon was observed in one *** was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients ***:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and *** who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain *** study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder

Study of damage behavior and repair effectiveness of patch repaired carbon fiber laminate under quasi-static indentation loading

Damage caused due to low-velocity impacts in composites leads to substantial deterioration in their residual strength and eventually provokes structural *** work presents an experimental investigation on the effects of different patch and parent laminate stacking sequences on the enhancement of impact strength of carbon Fiber Reinforced Polymers(cFRP)composites by utilising the adhesively bonded external patch repair *** evolution study is also performed with the aid of Acoustic Emission(AE).Two different quasi-isotropic configurations were selected for the parent laminate,viz.,[45°/45°/0°/0°]s and[45°/0°/45°/0°]*** Static Indentation(QSI)test was performed on both the pristine laminates,and damage areas were detected by using the c-scan inspection *** laminates were repaired by using a single-sided patch of two different configurations,viz.,[45°/45°/45°/45°]and[45°/0°/0°/45°],and employing a circular plug to fill the damaged *** different combinations of repaired laminates with two configurations of each parent and patch laminate were produced,which were further subjected to the QSI *** results reveal the effectiveness of the repair method,as all the repaired laminates show higher impact resistance compared to the respective pristine *** of[45°/0°/0°/45°]configuration when repaired by taking[45°/45°/0°/0°]s and[45°/0°/45°/0°]s as parents exhibited 68%and 73%higher peak loads,respectively,than the respective pristine ***,parent and patch of configuration[45°/0°/45°/0°]s and[45°/0°/0°/45°],respectively,attain the highest peak load,whereas[45°/45°/0°/0°]s and[45°/45°/45°/45°]combinations possess the most gradual decrease in the load.

Open Air Interface(OAI)alliance recently introduced a new disaggregated Open Radio Access Networks(O-RAN)framework for next generation telecommunications and *** disaggregated architecture is open,automated,software defined,virtual,and supports the latest advanced technologies like Artificial Intelligence(AI)Machine Learning(AI/ML).This novel intelligent architecture enables programmers to design and customize automated applications according to the business needs and to improve quality of service in fifth generation(5g)and Beyond 5g(B5g).Its disaggregated and multivendor nature gives the opportunity to new startups and small vendors to participate and provide cheap hardware software solutions to keep the market *** paper presents the disaggregated and programmable O-RAN architecture focused on automation,AI/ML services,and applications with Flexible Radio access network Intelligent controller(FRIc).We schematically demonstrate the reinforcement learning,external applications(xApps),and automation steps to implement this disaggregated O-RAN *** idea of this research paper is to implement an AI/ML enabled automation system for software defined disaggregated O-RAN,which monitors,manages,and performs AI/ML-related services,including the model deployment,optimization,inference,and training.

crystal structure and luminescence dynamics of highly pure LiM(PO_(3))_(3):Eu^(3+)(M=Sr,ca) red phosphors for white light emitting diodes

Highly pure red phosphors LiM(PO_(3))_(3):Eu^(3+)(M=Sr,ca) doped with Eu^(3+)(1 mol%) were synthesized via solution combustion method and their crystal structure and luminescence dynamics were studied to explore its suitability in white light emitting *** Rietveld refinement analysis of the powder Xray diffraction patterns reveals that the phosphors belong to the pure triclinic phase of LiSr(PO_(3))_(3) and Lica(PO_(3)) with space group P-1(2).The scanning electron microscopy images showed the agglomerated *** photoluminescence emission spectra under 393 nm show an orange band at 594 nm and a red band at 613 nm ascribed to ^(5)D_(0)→^(7)F_(1).^(5)D_(0)→^(7)F_(2) transitions of Eu^(3+)ion in both the ***,the spectroscopic properties such as luminescence behaviour,and Stark splitting were used to examine the symmetry of Eu^(3+)ions in LiM(PO_(3))_(3):Es^(3+)(M=Sr,ca) phosphors in terms of distortion induced upon *** Stark splitting shows that the actual site symmetry for Eu^(3+)ion was estimated to be D_(2) type for both *** photometric properties of Lica(PO_(3))_(3):Eu^(3+)such as commission International de l'Eclairage coordinates(x=0.64,y=0.36) near to the standard one(red),high color purity(95%) and higher brightness reveal that the phosphor has the capability of acting as a red component in n-UV white light emitting diodes.

The use of prefabricated vertical drains(PVD)in liquefiable deposits is gaining attention due to enhanced ***,investigations on PVD in mitigating re-liquefaction during repeated shaking events are not *** study performed a series of shaking table experiments on untreated and PVD-treated specimens prepared with 40%and 60%relative *** sinusoidal loading was applied with an incremental peak acceleration of 0.1g,0.2g,0.3g,and 0.4g,at 5 Hz shaking frequency with 40 s *** performance of treated ground was evaluated based on the generation and dissipation of excess pore water pressure(EPWP),induced sand densification,subsidence,and cyclic stress *** addition,the strain accumulated in fresh and exhumed PVD was investigated using geotextile tensile testing apparatus aided with digital image *** evidence of pore pressure was reported up to 0.2g peak acceleration for 40%and 60%relative density *** continuous occurrence of soil densification and drainage medium restrained and delayed the generation of EPWP and expedited the dissipation *** study demonstrates PVD can mitigate re-liquefaction,without suffering from deterioration,when subjected to medium to high intense repeated shaking events.

Thermodynamic properties and phase diagram of quark matter within non-extensive Polyakov chiral SU(3)quark mean field model

In the present study,we applied Tsallis non-extensive statistics to investigate the thermodynamic properties and phase diagram of quark matter in the Polyakov chiral SU(3)quark mean field *** this model,the properties of the quark matter were modified through the scalar fieldsσ,ζ,δ,χ,vector fieldsω,ρ,ϕ,and Polyakov fieldsΦandΦ¯at finite temperature and chemical ***-extensive effects were introduced through a dimensionless parameter q,and the results were compared to those of the extensive case(q→1).In the non-extensive case,the exponential in the Fermi-Dirac(FD)function was modified to a q-exponential *** influence of the q parameter on the thermodynamic properties,pressure,energy,and entropy density,as well as trace anomaly,was *** speed of sound and specific heat with non-extensive effects were also ***,the effect of non-extensivity on the deconfinement phase transition as well as the chiral phase transition of u,d,and s quarks was *** found that the critical end point(cEP),which defines the point in the(T−μ)phase diagram where the order of the phase transition changes,shifts to a lower value of temperature,TcEP,and a higher value of chemical potential,μcEP,as the non-extensivity is increased,that is,q>1.

Role of doxycycline in the treatment of dengue infection: An open-label, randomized, controlled, pilot trial

Objective:To measure the effect of doxycycline on inflammatory marker[IL-6,TNF-α,ferritin and c reactive protein(cRP)]levels in patients with dengue ***:A single-centre,open-label,parallel-group randomized controlled trial was done in PgIMER chandigarh from June 2021 to October *** were randomized using a simple randomization process into two groups:the doxycycline treatment group(n=35)and the control group(n=34).Patients in the treatment group were given oral doxycycline 100 mg twice daily for five days along with standard treatment,whereas patients in the control group received only standard *** objective was to measure the effect of doxycycline on inflammatory markers in dengue ***:On comparative analysis at day 5,there was a statistically significant reduction in the median values of ferritin and cRP in cases compared to the control group(ferritin:P=0.006 and cRP:P=0.006).No significant reduction was noted in the levels of IL-6 and TNF-α.conclusions:Doxycycline treatment led to a reduction of inflammatory markers in dengue infection.
