目的由于胶质母细胞瘤(GBM)的MR所见表现与基因改变、基因表达以及病人生存率有关,应用公认的MRI术语,对GBM的肿瘤大小、成分进行全面的影像评估分析。材料与方法因为所有参与本研究的病人均已被癌症基因组计划匿名化,而且应用的是不包括病人标符、HIPAA所允许的数据集,所以不再需要机构审查委员会的批准。应用标准化特征集,3位神经放射学医师对75例基因信息已录入Tc.A门户网站的GBM病人根据MR影像中肿瘤的大小、位置及形态进行分级。应用Krippendorff α统计以及组内相关系数对观察者间一致性进行分析。应用多变量c.x回归模型确定生存率、肿瘤大小与肿瘤形态之间的相关性;应用Fisher确切检验研究影像特征与基因组学之间的相关性。结果就肿瘤强化区、无强化区、坏死、水肿及肿瘤大小等变量而言,观察者间具有显著一致性。肿瘤强化体积及最大长径与较差的生存率高度相关(风险率分别为:8.84,P=0.0253;1.02,P=0.00973),甚至在卡氏评分校正后亦如此(P=0.0208)。原神经型GBM的强化水平明显低于其他亚型(P=0.02),而间质型GBM无强化肿瘤的水平较低(P<0.01)。结论分析显示,此方法能够可重复地描述脑肿瘤的影像特征;本研究显示,对影像特征的影像学肉眼评估可与基因改变、基因表达亚型结合,以更深入地观察GBM亚型潜在的生物学属性。

牛地方性血尿症的确切病因仍不清楚。欧洲蕨(Pteridium aquilinum)的介入似乎是肯定的,因为在蕨丛生的农场和地方性血尿症之间有着十分密切的联系。蕨的一些毒素已被鉴定,但其主要的致癌成分仍需明确证实。新近,在地方性血尿症的病因学中又涉及到牛的乳头瘤病毒。本文也讨论了它与蕨致癌原的可能相互作用。地方流行性血尿症是牛的一种以广泛的地理分布和地方流行为特征的疾病。发生于包括英国在内的许多欧洲国家、北美洲、南美洲、非洲、亚洲和澳大利亚。通常见于某些局部的高地,且常与贫瘠的草场和未开垦地联系在一起。可见的主要临床症状是间歇性血尿,血尿休间期可为数周至数月且与多尿及排尿困难有关。病牛均为成年牛,一般4～6岁时第一次出现症状。病程常持续数年最后以死亡告终。无性别倾向。病牛的红细胞计数、血细胞压积、血红蛋白浓度和白细胞计数降低。尿中含有红细胞、蛋白质(特別是白蛋白)及白细胞。膀胱的组织病理学变化包括上皮的增生和脱落。可见移行上皮索侵入固有膜形成的所谓布鲁氏巢(Brunn′snests)。固有膜內可见单核细胞浸润。膀胱血管显著扩张,具有血管瘤性的腔隙(cooper和Johnstone 1984)。已叙述了上皮、间叶和混合源性的多种肿瘤,包括移行细胞癌、鳞状细胞癌、腺癌、血管瘤及纤维瘤等(世界卫生组织,1974)。

背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的PONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生PONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（P〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，P〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（P〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，P〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少PONV，主要是通过减少不良反应而获得。

Hypobaric.hypoxia (HH) exposure c.n c.use serious brain injury as well as life-threatening c.rebral edema in severe c.ses. Previous studies on the mec.anisms of HH-induc.d brain injury have been c.nduc.ed primarily using non-primate animal models that are genetic.lly distant to humans, thus hindering the development of disease treatment. Here, we report that c.nomolgus monkeys (Mac.c. fasc.c.laris) exposed to ac.te HH developed human-like HH syndrome involving severe brain injury and abnormal behavior. Transc.iptome profiling of white blood c.lls and brain tissue from monkeys exposed to inc.easing altitude revealed the c.ntral role of the HIF-1 and other novel signaling pathways, suc. as the vitamin D rec.ptor (VDR) signaling pathway, in c.-regulating HH-induc.d inflammation proc.sses. We also observed profound transc.iptomic.alterations in brains after exposure to ac.te HH, inc.uding the ac.ivation of angiogenesis and impairment of aerobic.respiration and protein folding proc.sses, whic. likely underlie the pathologic.l effec.s of HH-induc.d brain injury. Administration of progesterone (PROG) and steroid neuroprotec.ant 5α-androst-3β,5,6β-triol (TRIOL) signific.ntly attenuated brain injuries and resc.ed the transc.iptomic.c.anges induc.d by ac.te HH. Func.ional investigation of the affec.ed genes suggested that these two neuroprotec.ants protec. the brain by targeting different pathways, with PROG enhanc.ng erythropoiesis and TRIOL suppressing glutamate-induc.d exc.totoxic.ty. Thus, this study advanc.s our understanding of the pathology induc.d by ac.te HH and provides potential c.mpounds for the development of neuroprotec.ant drugs for therapeutic.treatment.
