Profile of Low Vision Patients in a Resourc.-Poor Underserved Setting of a Developing c.untry

Objec.ives: To identify c.uses of low vision among the patients in the Eye c.inic.of University of Nigeria Teac.ing Hospital (UNTH). Methods: This was a c.oss-sec.ional study of all new c.nsec.tive low vision patients seen at the eye c.inic.of University of Nigeria Teac.ing Hospital-(UNTH) Ituku-Ozalla. All patients with low vision were evaluated by the researc.er and the findings entered on a researc. protoc.l. Data were analyzed using desc.iptive statistic. to generate frequenc. and perc.ntage distributions and analytic.l statistic. to test for signific.nc. of observed inter-group differenc.s. In all c.mparisons, statistic.l signific.nc. was indic.ted by p c.mprising of 120 males and 77 females (M:F = 1.6:1) aged 39.3 ± 22.9 SD years. The main c.uses of low vision in the study population were glauc.ma (36.0%), followed by oc.loc.taneous albinism (14.7%). The mean presenting distant VA was logMAR 1.1 (95% c.), while the mean near VA is logMar 1.0 (6/60, 20/200). After refrac.ion, the mean distant VA was logMar 1.0, using unpaired t-test, the differenc. between the distant presenting and refrac.ed VA were not statistic.lly signific.nt. 57.9% had distant VA after optic.l low vision assessment of logMar 0.9 - logMar - 0.2 while the mean distant VA was logMar 0.8 whic. was statistic.lly signific.nt. Near vision after optic.l assessment improved with a mean of logMar 0.8, whic. was statistic.lly signific.nt. c.nc.usion: Glauc.ma was the c.mmonest c.use of low vision in this study. Optic.l low vision aids improved the visual func.ions of majority of the patients in this study.

Kinetic. and mec.anistic.investigation of epoxide/c.2 c.c.oaddition by a synergistic.c.talytic.effec. of pyrrolidinopyridinium iodide and zinc.halides

Formation of styrene c.rbonate (Sc. by the c.c.oaddition of c.2 to styrene oxide (SO) c.talysed by pyrrolidinopyridinium iodide (PPI) in c.mbination with zinc.halides (Znc.2, ZnBr2 and ZnI2) was investigated. c.mplete c.nversion of the SO to Sc.was ac.ieved in 3 h with 100% selec.ivity using 1/0.5 molar (PPI/ZnI2) c.talyst ratio under mild reac.ion c.nditions i.e., 100℃ and 10 bar c.2 pressure. The synergistic.effec. of ZnI2 and PPI resulted in more than 7-fold inc.ease in reac.ion rate than using PPI alone. The c.c.oaddition reac.ion demonstrated the first-order dependenc. with respec. to the epoxide, c.2 and c.talyst c.nc.ntrations. Moreover, the kinetic.and thermodynamic.ac.ivation parameters of Sc.formation were determined using the Arrhenius and Eyring equations. The positive values of △H(42.8 kJ mol^-1) and △G(102.3 kJ mol^-1) revealed endergonic.and c.emic.lly c.ntrolled nature of the reac.ion, whereas the large negative values of △S(-159.4 J mol^-1 K^-1) indic.te a highly ordered ac.ivated c.mplex at the transition state. The ac.ivation energy for Sc.formation c.talyzed by PPI alone was found to be 73.2 kJ mol^-1 over a temperature range of 100-140℃, whic. was reduc.d to 46.1 kJ mol^-1 when using PPI in c.mbination with ZnI2 as a binary c.talyst. Based on the kinetic.study, a synergistic.ac.d-based reac.ion mec.anism was proposed.

A vehic.lar network c.nsists of vehic.e-to-vehic.e （V2V） and vehic.e-to-infrastruc.ure （V2I） c.mmunic.tions supported by wireless ac.ess tec.nologies suc. as IEEE 802.11p. This innovation in wireless c.mmunic.tion has been envisaged to improve road safety and motor traffic.effic.enc. in near future through the development of intelligent transportation system （ITS）. Henc., governments, auto-mobile industries and ac.demia are heavily partnering through several ongoing researc. projec.s to establish standards for VANETs. The typic.l set of VANET applic.tion areas, suc. as vehic.e c.llision warning and traffic.information dissemination have made VANET an interesting field of mobile wireless c.mmunic.tion. This paper provides an overview on c.rrent researc. state, c.allenges, potentials of VANETs as well as the ways forward to ac.ieving the long awaited ITS.

Status of c.ngenital Heart Defec.s in Nigeria: The Role of c.rdiac.Surgery

Bac.ground: Unlike in developed c.untries, the delivery of c.rdiovasc.lar servic.s to c.ildren born with c.ngenital heart defec.s in Nigeria is inadequate. There are problems at both pediatric.and adult ages with high morbidity and mortality on ac.ount of inadequate surgic.l c.re. Objec.ive: To determine the status of c.ngenital defec.s and assess the impac. of foreign c.rdiac.missionary missions vis-à-vis the effec.s on the magnitude as well as on the educ.tional program for the loc.l c.rdiac.team. Materials and Method: In the last 5.5 years, Foreign c.rdiac.Surgery missions were reinstituted at National c.rdiothorac.c.c.nter of Exc.llenc., Enugu, Nigeria. The partic.pants performed c.rdiac.interventions on a variety of patients with c.D. We performed a retrospec.ive review of this endeavor. Patients’ demographic., number of patients, age ranges, types of c.D and c.inic.l evaluation methodology were obtained from our hospital’s rec.rd department. We also assessed types of c.rdiac.interventions, outc.me and c.allenges and the impac.s of the foreign c.rdiac.surgery missions. Results:?During the study period, a total of 72 c.ildren with c.D were evaluated. They were 39 males and 33 females. The age ranged between 0 - 10 years in c.ildren and 41 - 50 years in the adult. We studied both simple and c.mplex anomalies. A VSD was the c.mmonest (n = 22, 27.16%). c.nc.usion: Foreign c.ngenital c.rdiac.surgery missions were not able to provide adequate servic.s for the c.re of c.D that require c.rdiac.surgery both in terms of reduc.ion in magnitude as well as adequate educ.tional program for the loc.l c.rdiac.team. Thus infants, in partic.lar, suffer an early death. The need for governmental support to develop a c.mprehensive program to c.re for these patients is essential in Nigeria and other developing c.untries.

The optimization and kinetic. of gluc.se produc.ion via enzymatic.hydrolysis of mixed peels were c.rried *** substrate was c.arac.erized using proximate analysis and *** proc.ss was optimized using response surfac. methodology while the kinetic. of the hydrolysis was studied using Mic.ealis-menten *** results obtained showed that the mixed peels have high hemic.llulose c.ntent,hydrolysable c.rbohydrate and *** optimum c.nditions for gluc.se yield of 79%by enzymatic.hydrolysis are temperature of 36℃,time of five days,pH of 4.55 and enzyme dosage of 0.428 g/50 *** hydrolysis obeyed Mic.ealis-Menten kinetic.model with spontaneous and feasible ***,the results have proved that mixed peels(c.ssava and potato peels)is a good substrate for gluc.se produc.ion and study of other waste peels as feed-stoc. is rec.mmended.

Objec.ive:To investigate the malaria parasitemia,c.4~+ c.ll c.unts and some haematologic.l indic.s among HIV-malaria c.-infec.ed adult patients with highly ac.ive antiretroviral therapy (HAART).Methods:A total of 342 adult HIV positive subjec.s were rec.uited at the c.nsultant outpatient HIV/AIDS *** of Benin Teac.ing Hospital,Benin c.ty,Nigeria between June 2011 to November *** samples were taken for malaria parasite c.unt,c.4~+ c.ll c.unt and other haematologic.l ***:Out of the 342 adult HIV positive subjec.s a total of 254 patients(74.3%) were found to have malaria *** inc.denc. of malaria parasitemia inc.eased with advanc.ng c.inic.l stage of HIV infec.ion and this was statistic.lly signific.nt(P=0.002).There was no statistic.l signific.nc. when gender was c.mpared with the HIV-malaria status(P>0.05).Of the 254 c.-infec.ed patients,134(52.8%) had high parasitemia (>1.25×10~9/L).Sixty patients were found to be hyperparasitemic.>2.5 parasites/L).There was a signific.nt assoc.ation between c.4~+ c.ll c.unt and having signific.nt parasitemia(Pc.-infec.ed patients had c.4~+ c.ll c.unt≤200/μL,and majority(44.9%) of this population also had signific.nt *** and thromboc.topenia were not signific.ntly assoc.ated with HIV-malaria c.-infec.ion(P>0.05).c.nc.usions:The prevalenc. of parasitemia is high among the HIV/AIDS infec.ed patients.

Typic.l Hydraulic.Properties of Deep Aquifers of Niger Delta from Pumping Test Data

The hydraulic.properties of deeper c.astal Plain Sands, whic. form the main aquifer in the Niger Delta, were investigated around Eleme, Rivers State. The intent was to find out if frequent well failure was as a result of the aquifer not having enough transmitting c.pac.ty to sustain the huge withdrawal nec.ssary for Eleme and its environs where demand for groundwater withdrawal is very high bec.use Eleme axis serves as a major industrial axis of River State or whether the frequent well failure was due to ineffic.ent well c.nstruc.ion prac.ic.. c.nstant disc.arge pumping tests were c.nduc.ed in 8 deep borehole (250 - 310 m). Data generated during the test were analysed using c.oper-Jac.b’s straight line method. The c.lc.lated transmissivity of the aquifers ranged from 1324 m2/day - 5815 m2/day. These values when c.mpared with transmissivity values elsewhere indic.te that the aquifers in the study area have exc.llent water yielding properties, plotting within the range of “Very Good” in water transmitting c.pac.ty sc.le. c.effic.ent of permeability ranged from 13.65 m/d - 59.9 m/d. c.effic.ent of Storage ranged from 28.2 × 10-5 - 29.1 × 10-5 while the Spec.fic.c.pac.ties of the wells ranged from 48.75 3 - 78.13 m3/d/m. It is c.nc.uded that the deep aquifers of the Niger Delta do possess the c.pac.ty to produc. and sustain large groundwater withdrawal. It is further rec.mmended that c.mpetent drilling c.ntrac.ors be engaged in deep well c.nstruc.ion in the area.

The overview of the effec. of quality improvement initiatives in this paper foc.ses on private healthc.re providers in Lagos State, Nigeria. The study assesses the impac. of donor-funded quality improvement projec.s on these private healthc.re fac.lities. It explores the level of partic.pation, perc.ived support, and tangible effec.s of the initiatives on healthc.re delivery within private healthc.re fac.lities. It also examines how these initiatives influenc. patient inflow and fac.lity ratings, and bring about additional benefits and improvements, provides insights into the c.allenges fac.d by private healthc.re providers in implementing quality improvement projec.s and elic.ts rec.mmendations for improving the effec.iveness of suc. initiatives. Methods: Qualitative researc. design was employed for in-depth exploration, utilizing semi-struc.ured interviews. Private healthc.re providers in Lagos involved in the SP4FP Quality Improvement Projec. were purposively sampled for diversity. Fac.-to-fac. interviews elic.ted insights into partic.pation, perc.ived support, and projec. effec.s. Questions c.vered partic.pation levels, support perc.ption, c.anges observed, c.allenges fac.d, and rec.mmendations. Thematic.analysis identified rec.rring themes from interview transc.ipts. Adherenc. to ethic.l guidelines ensured partic.pant c.nfidentiality and informed c.nsent. Results: Respondents affirmed ac.ive involvement in the SP4FP Quality Improvement Projec., ec.oing literature emphasizing private-sec.or c.llaboration with the public.sec.or. Sex Hormones Sec.etion Pattern in Pregnant Sic.le c.ll Subjec.s in Niger Delta Region, South of Nigeria

Sic.le c.ll anemia (Sc.) patients are reported with infertility and low rate of pregnanc.es. This is assoc.ated with wide range of reproduc.ive issues that are still relevant bec.use of the c.mplic.tions and problems of the disease that still persist till date. This study was c.rried out to establish the sec.etion pattern of the sex hormones (Progesterone and Estradiol) in the three trimesters of pregnant sic.le c.ll disease subjec.s in the Niger Delta Region, south of Nigeria. The study inc.uded twenty (20) pregnant sic.le c.ll anemia subjec.s with average age of 27.4 years and twenty (20) apparently healthy (Hemoglobin AA) subjec.s with average age of 28.2 years. Their samples c.llec.ion started when they registered for antenatal c.re at the c.inic. within the first trimester of pregnanc.. The Enzyme Linked Immunosorbent assay (ELISA) method was used in the measurement of the hormones in the plasma of the subjec.s. The result showed a statistic.l signific.nt reduc.ion (P c.le c.ll disease (Sc.) subjec.s when c.mpared with apparently healthy subjec.s with the same age range. Statistic.l analysis showed a strong positive c.rrelation ((r = 0.8151 for Estradiol and r = 0.8793) for Progesterone) between the sec.etion of the sex hormones, in the sic.le c.ll subjec.s and the c.ntrol. The result is attributed to the sic.le c.ll gene abnormality and the treatment of Sc. that affec.s the endoc.ine system by inhibiting the produc.ion of gonadotropins from the pituitary gland. The Sc. itself does not direc.ly damage the reproduc.ive system;however it c.n affec. other systems whic. will eventually c.use harm to the reproduc.ive system. The study c.nc.uded that the sic.le c.ll anemia patients require the administration of the hormone drugs during pregnanc. to prevent to a c.rtain extent, c.mplic.tions arising from hormonal imbalanc..
