Monocyte chemoattractant protein-1, transforming growth factor-β1, nerve growth factor, resistin and hyaluronic acid as serum markers：comparison between recurrent acute and chronic pancreatitis

BAcKGROUND： Diagnostic parameters that can predict the presence of chronic pancreatitis（cP） in patients with recurrent pain due to pancreatitis would help to direct appropriate therapy. This study aimed to compare the serum levels of monocyte chemoattractant protein-1（McP-1）, transforming growth factor-β1（TGF-β1）, nerve growth factor（NGF）, resistin and hyaluronic acid（HA） in patients with recurrent acute pancreatitis（RAP） and cP to assess their ability to differentiate the two ***： Levels of serum markers assessed by enzymelinked immunosorbent assay（ELISA） were prospectively compared in consecutive patients with RAP, cP and in controls and stepwise discriminant analysis was performed to identify the markers differentiating RAP from ***： One hundred and thirteen consecutive patients（RAP=32, cP=81） and 78 healthy controls were prospectively enrolled. The mean（SD） age of the patients was 32.0（14.0）years; 89（78.8%） were male. All markers were significantly higher in cP patients than in the controls（P〈0.001）; McP-1NGF and HA were significantly higher in RAP patients than in the controls（P〈0.001）. Stepwise discriminant analysis showed significant difference（P=0.002） between RAP and cP for resistin with an accuracy of 61.9%, discriminant scores of ≤-0.479 and ≥0.189 indicating RAP and cP, respectively. The other markers had no differential value between RAP and ***： Serum resistin is a promising marker to differentiate between RAP and cP and needs validation in future studies, especially in those with early cP.

comparing acid steatocrit and faecal elastase estimations for use in M-ANNHEIM staging for pancreatitis

AIM To compare two tests for exocrine pancreatic function(EPF) for use in M-ANNHEIM staging for pancreatitis. METHODS One hundred and ninety four consecutive patients with acute pancreatitis(AP; n = 13), recurrent acute pancreatitis(RAP; n = 65) and chronic pancreatitis(cP; n = 116) were enrolled. EPF was assessed by faecal elastase-1(FE-1) estimation and stool fat excretion by the acid steatocrit method. Patients were classified as per M-ANNHEIM stages separately based on the results of the two tests for comparison. Independent Student's t-test, χ~2 test, Kruskal-Wallis test, Mann-Whitney U test and Mc Nemar's test were used as appropriate. RESULTS Sixty-one(52.5%) patients with cP had steatorrhoea when assessed by the acid steatocrit method; 79 (68.1%) with cP had exocrine insufficiency by the FE-1 test(χ~2 test, P cid steatocrit and FE-1 showed a significant negative correlation(Spearman's rho =-0.376, P cally significant difference was seen between the M-ANNHEIM stages as classified separately by acid steatocrit and the FE-1. Thirteen(6.7%), 87(44.8%), 89(45.8%) and 5(2.5%) patients were placed in M-ANNHEIM stages 0,?Ⅰ, Ⅱ, and Ⅲ respectively, with the use of acid steatocrit as against 13(6.7%), 85(43.8%), 75(38.6%), and 21(10.8%) respectively by FE-1 in stages 0,?Ⅰ, Ⅱ, and Ⅲ thereby altering the stage in 28(14.4%) patients(P c Nemar's test). cONcLUSION FE-1 estimation performed better than the acid steatocrit test for use in the staging of pancreatitis by the M-ANNHEIM classification since it diagnosed a higher proportion of patients with exocrine insufficiency.

Defining acute-on-chronic liver failure: East,West or Middle ground?

Acute-on-chronic liver failure(AcLF),a newly recognized clinical entity seen in hospitalized patients with chronic liver disease including cirrhosis,is associated with high short- and medium term morbidity and *** the definitions of AcLF proposed so far have been universally accepted,the two most commonly used being those proposed by the Asia-Pacific Association for the Study of Liver(APASL) and the European Association for the Study of Liver- chronic Liver Failure(EASL-cLIF) *** paper both definitions and diagnostic criteria appear to be different from each other,reflecting the differences in cut-off values for individual parameters used in diagnosis,the acute insult or precipitating event and the underlying chronic liver *** directly comparing these two criteria are limited,and available studies reveal different outcomes when the two are applied to the same set of *** a review of the literature suggests that both definitions do not seem to identify the same set of *** definition given by the APASL consortium is easier to apply in day-to-day practice but the EASLcLIF criteria appear to better predict mortality in *** World Gastroenterology Organization working party have proposed a working definition of AcLF which will identify patients from whom relevant data can be collected so that the similarities and the differences between the two regions,if any,can be clearly defined.

cerebral intracranial aneurysms are serious problems that can lead to stroke,coma,and even *** effect of blood flow on cerebral aneurysms and their relationship with rupture are *** addition,postural changes and their relevance to haemodynamics of blood flow are difficult to measure in vivo using clinical imaging *** simulations investigating the detailed haemodynamics in cerebral aneurysms have been developed in recent times not only to understand the progression and rupture but also for clinical evaluation and *** the present study,the haemodynamics of a patient-specific case of a large aneurysm on the left side internal carotid bifurcation(LIcA)and no aneurysm on the right side internal carotid bifurcation(RIcA)was *** simulation of these patient-specific models using fluid–structure interaction provides a valuable comparison of flow behavior between normal and aneurysm *** influences of postural changes were investigated during standing,sleeping,and head-down(HD)*** changes in flow were observed during the HD position and quit high arterial blood pressure in the internal carotid artery(IcA)aneurysm model was established when compared to the normal IcA *** velocity increased abruptly during the HD position by more than four times(LIcA and RIcA)and wall shear stress by four times(LIcA)to ten times(RIcA).The complex spiral flow and higher pressures prevailing within the dome increase the risk of aneurysm rupture.

The influence of spark plasma sintering temperatures on the microstructure,hardness,and elastic modulus of the nanocrystalline Al-xV alloys produced by high-energy ball milling

Al-xV alloys(x=2 at.%,5 at.%,10 at.%)with nanocrystalline structure and high solid solubility of V were produced in powder form by high-energy ball milling(HEBM).The alloy powders were consolidated by spark plasma sintering(SPS)employing a wide range of temperatures ranging from 200 to 400°*** microstructure and solid solubility of V in Al were investigated using X-ray diffraction analysis,scanning electron microscope and transmission electron *** microstructure was influenced by the SPS temperature and V content of the *** alloys exhibited high solid solubility of V–six orders of magnitude higher than that in equilibrium state and grain sizec was detected at 400℃.Formation of a V-lean phase and bimodal grain size was observed during SPS,which increased with the increase in SPS *** hardness and elastic modulus,measured using nanoindentation,were significantly higher than commercial *** example,Al-V alloy produced by SPS at 200℃ exhibited a hardness of 5.21 GPa along with elastic modulus of 96.21 *** evolution of the microstructure and hardness with SPS temperatures has been discussed.

To explore the cumulative deformation effect of the dynamic response of a tunnel crossing the hauling sliding surface under earthquakes,the shaking table test was conducted in this *** with the numerical calculations,this study proposed magniﬁcation of the Arias intensity(MIa)to characterize the overall local deformation damage of the tunnel lining in terms of the deformation characteristics,frequency domain,and *** the time‐domain analysis method,the plastic effect coefﬁcient(PEc)was proposed to characterize the degree of plastic deformation,and the applicability of the seismic cumulative failure effect(ScFE)was *** results show that the low‐frequency component(f1 and f2≤10 Hz)and the high‐frequency component(f3 and f4>10 Hz)acceleration mainly cause global and local deformation of the tunnel *** local deformation caused by the high‐frequency wave has an important effect on the seismic damage of the *** physical meaning of PEc is more clearly deﬁned than that of the residual strain,and the ScFE of the tunnel lining can also be deﬁ*** ScFE of the tunnel lining includes the elastic deformation effect stage(c–plastic deformation effect stage(0.15g–0.30g),and the plastic deformation effect stage(0.30g–0.40g).This study can provide valuable theoretical and technical support for the construction of trafﬁc tunnels in high‐intensity earthquake areas.

A framework for building energy management system with residence mounted photovoltaic

Efficient utilization of a residential photovoltaic (PV) array with grid connection is difficult due to power fluctuation and geographical dispersion. Reliable energy management and control system are required for overcoming these obstacles. This study provides a new residential energy management system (REMS) based on the convolution neural network (cNN) including PV array environment. The cNN is used in the estimation of the nonlinear relationship between the residence PV array power and meteorological datasets. REMS has three main stages for the energy management such as forecasting, scheduling, and real functioning. A short term forecasting strategy has been performed in the forecasting stage based on the PV power and the residential load. A coordinated scheduling has been utilized for minimizing the functioning cost. A real-time predictive strategy has been used in the actual functioning stage to minimize the difference between the actual and scheduled power consumption of the building. The proposed approach has been evaluated based on real-time power and meteorological data sets.

针对撞击安全性试验中橇车的高速短距离阻拦需求,提出一种基于非密封型阻尼缓冲水缸与高强度柔性阻拦索的新型高速阻拦装置。从该装置的构成与工作原理出发,考虑阻拦索的弹性变形与活塞的惯性效应,建立阻拦过程的动力学模型,获得橇车加速度、水缸压力、阻拦行程等关键动力学参数的变化特征,并与试验数据进行了对比验证,证明了所建立的动力学模型能够较为准确地反映阻拦装置的动力学特征。基于该动力学模型,分析阻拦索刚度、阻尼孔分布形式、阻尼孔面积、活塞质量等参数对阻拦性能的影响规律,结果表明:宜选用轻质高强的柔性绳索作为阻拦绳;选用大斜率双曲线型的节流孔分布方式,水缸的阻尼效果较好;节流孔面积、活塞面积在设计时应进行综合评估;在保证活塞结构强度的前提下,应尽量降低活塞的质量。
