Impact of payment source, referral site, and place of residence on outcomes after allogeneic transplantation in Mexico

BAcKGROUnD The impact of social determinants of health in allogeneic transplant recipients in low-and middle-income countries is poorly described.This observational study analyzes the impact of place of residence,referring institution,and transplant cost coverage(out-of-pocket vs government-funded vs private insurance)on outcomes after allogeneic hematopoietic stem cell transplantation(alloHScT)in two of Mexico's largest public and private institutions.AIM To evaluate the impact of social determinants of health and their relationship with outcomes among allogeneic transplant recipients in Mexico.METHODS In this retrospective cohort study,we included adolescents and adults≥16 years who received a matched sibling or haploidentical transplant from 2015-2022.Participants were selected without regard to their diagnosis and were sourced from both a private clinic and a public University Hospital in Mexico.Three payment groups were compared:Out-of-pocket(OOP),private insurance,and a federal Universal healthcare program“Seguro Popular”.Outcomes were compared between referred and institution-diagnosed patients,and between residents of nuevo Leon and out-of-state.Primary outcomes included overall survival(OS),categorized by residence,referral,and payment source.Secondary outcomes encompassed early mortality,event-free-survival,graft-versus-host-relapse-free survival,and non-relapse-mortality(nRM).Statistical analyses employed appropriate tests,Kaplan-Meier method,and cox proportional hazard regression modeling.Statistical software included SPSS and R with tidycmprsk library.RESULTS Our primary outcome was overall survival.We included 287 patients,n=164 who lived out of state(57.1%),and n=129 referred from another institution(44.9%).The most frequent payment source was OOP(n=139,48.4%),followed by private insurance(n=75,26.1%)and universal coverage(n=73,25.4%).no differences in OS,event-free-survival,nRM,or graft-versus-host-relapse-free survival were observed for patients diagnosed locally vs in another institution,nor pat

With deployment of measurement units,fitting static equivalent models of distribution networks(Dns)by linear regression has been recognized as an effective method in power flow analysis of a transmission network.Increasing volatility of measurements caused by variable distributed renewable energy sources makes it more difficult to accurately fit such equivalent models.To tackle this challenge,this letter proposes a novel data-driven method to improve equivalency accuracy of Dns with distributed energy resources.This letter provides a new perspective that an equivalent model can be regarded as a mapping from internal conditions and border voltages to border power injections.Such mapping can be established through 1)Koopman operator theory,and 2)physical features of power flow equations at the root node of a Dn.Performance of the proposed method is demonstrated on the IEEE 33-bus and IEEE 136-bus test systems connected to a 661-bus utility system.

Organic-inorganic hybrid perovskites have gained great attention as promising photocatalysts for hydrogen generation.However,due to their poor stability in water,the use of aqueous hydrohalic acid solutions is specifically required for an efficient hydrogen evolution.Herein,three novel photoactive lead-free hybrid perovskites based on bismuth and triazolium cations(denoted as IEF-15,IEF-16,and IEF-17(IEF stands for IMDEA energy frameworks))were synthesized and fully characterized(structural,compositional,optical,etc.).Further,these solids were proposed as photocatalysts for the challenging gas phase overall water splitting(OWS)reaction.Accordingly,IEF-16 thin films exhibited a remarkable photocatalytic activity in both H_(2) and O_(2) evolution,as a consequence of its appropriate bandgap and energy-band alignment,achieving hydrogen evolution rates ofμmol·g_(H_(2))^(-)1846 and 360 after 24 h under ultraviolet-visible(UV-vis)irradiation or simulated solar irradiation,respectively.This study additionally highlights the remarkable structural and photochemical stability of IEF-16 under different operational conditions(i.***.water volume,irradiation and temperature),paving the way for green hydrogen production from OWS using perovskite-based photocatalysts.

Background:Exercise training promotes brain plasticity and is associated with protection against cognitive impairment and Alzheimer’s disease(AD).These beneficial effects may be partly mediated by blood-borne factors.Here we used an in vitro model of AD to investigate effects of blood plasma from exercise-trained donors on neuronal viability,and an in vivo rat model of AD to test whether such plasma impacts cognitive function,amyloid pathology,and neurogenesis.Methods:Mouse hippocampal neuronal cells were exposed to AD-like stress using amyloid-βand treated with plasma collected from human male donors 3 h after a single bout of high-intensity exercise.For in vivo studies,blood was collected from exercise-trained young male Wistar rats(high-intensity intervals 5 days/week for 6 weeks).Transgenic AD rats(McGill-R-Thyl-APP)were inj ected 5 times/fortnight for 6 weeks at2 months or 5 months of age with either(a)plasma from the exercise-trained rats,(b)plasma from sedentary rats,or(c)saline.cognitive function,amyloid plaque pathology,and neurogenesis were assessed.The plasma used for the treatment was analyzed for 23 cytokines.Results:Plasma from exercised donors enhanced cell viability by 44.1%(p=0.032)and reduced atrophy by 50.0%(pn amyloid-β-treated cells.In vivo exercised plasma treatment did not alter cognitive function or amyloid plaque pathology but did increase hippocampal neurogenesis by~3 fold,regardless of pathological stage,when compared to saline-treated rats.concentrations of 7 cytokines were significantly reduced in exercised plasma compared to sedentary plasma.conclusion:Our proof-of-concept study demonstrates that plasma from exercise-trained donors can protect neuronal cells in culture and promote adult hippocampal neurogenesis in the AD rat brain.This effect may be partly due to reduced pro-inflammatory signaling molecules in exercised plasma.

The levered-dual response(LEDAR)coulomb-damped system attains near resonant vibration isolation by differential preloads/offsets in linear springs.It takes the advantages of both the preloads/offsets in linear springs and the guiderail friction for realizing different levels of vibration isolation.The isolation capacities are investigated on the strategies with both the horizontal and vertical guiderails,with the horizontal rail only,and without guiderails.The compressive preloads generally result in the consumption of most of the initial excitation energy so as to overcome the potential threshold.The isolation onsets at the frequency ratio of 1∓0.095 on the left-hand side(LHS)and the right-hand side(RHS)of the lever are relative to the load plate connector.The observed near resonant isolation thus makes the LEDAR system a candidate for the isolation of the mechanical systems about resonance while opening a path for simultaneous harvesterisolation functions and passive functions at extreme frequencies.

chiral transmission by an open evolution trajectory in a non-Hermitian system

Exceptional points(EPs),at which two or more eigenvalues and eigenstates of a resonant system coalesce,are associated with non-Hermitian Hamiltonians with gain and/or loss elements.Dynamic encircling of EPs has received significant interest in recent years,as it has been shown to lead to highly nontrivial phenomena,such as chiral transmission in which the final state of the system depends on the encircling handedness.Previously,chiral transmission for a pair of eigenmodes has been realized by establishing a closed dynamical trajectory in parity-time-(PT-)or anti-PT-symmetric systems.Although chiral transmission of symmetry-broken modes,more accessible in practical photonic integrated circuits,has been realized by establishing a closed trajectory encircling EPs in anti-PTsymmetric systems,the demonstrated transmission efficiency is very low due to path-dependent losses.Here,we demonstrate chiral dynamics in a coupled waveguide system that does not require a closed trajectory.Specifically,we explore an open trajectory linking two infinite points having the same asymptotic eigenmodes(not modes in PT-and anti-PT-symmetric systems),demonstrating that this platform enables high-efficiency chiral transmission,with each eigenmode localized in a single waveguide.This concept is experimentally implemented in a coupled silicon waveguide system at telecommunication wavelengths.Our work provides a new evolution strategy for chiral dynamics with superior performance,laying the foundation for the development of practical chiral-transmission devices.

Optimisation of Thermal comfort of Building in a Hot and Dry Tropical climate: A comparative Approach between compressed Earth/concrete Block Envelopes

compressed earth blocks (cEB) are an alternative to cement blocks in the construction of wall masonry. However, the optimal architectural construction methods for adequate thermal comfort for occupants in hot and arid environments are not mastered. This article evaluates the influence of architectural and constructive modes of buildings made of cEB walls and concrete block walls, to optimize and compare their thermal comfort in the hot and dry tropical climate of Ouagadougou, Burkina Faso. Two identical pilot buildings whose envelopes are made of cEB and concrete blocks were monitored for this study. The thermal models of the pilot buildings were implemented in the SketchUp software using an extension of EnergyPlus. The models were empirically validated after calibration against measured thermal data from the buildings. The models were used to do a parametric analysis for optimization of the thermal performances by simulating plaster coatings on the exterior of walls, airtight openings and natural ventilation depending on external weather conditions. The results show that the cEB building displays 7016 hours of discomfort, equivalent to 80.1% of the time, and the concrete building displays 6948 hours of discomfort, equivalent to 79.3% of the time. The optimization by modifications reduced the discomfort to 2918 and 3125 hours respectively;i.e. equivalent to only 33.3% for the cEB building and 35.7% for the concrete building. More study should evaluate thermal optimizations in buildings in real time of usage such as residential buildings commonly used by the local middle class. The use of cEB as a construction material and passive means of improving thermal comfort is a suitable ecological and economical option to replace cementitious material.

A core-satellite self-assembled SERS aptasensor containing a“biological-silent region”Raman tag for the accurate and ultrasensitive detection of histamine

Herein,a novel interference-free surface-enhanced Raman spectroscopy(SERS)strategy based on magnetic nanoparticles(MnPs)and aptamer-driven assemblies was proposed for the ultrasensitive detection of histamine.A core-satellite SERS aptasensor was constructed by combining aptamer-decorated Fe_(3)O_(4)@Au MnPs(as the recognize probe for histamine)and complementary DnA-modified silver nanoparticles carrying 4-mercaptobenzonitrile(4-MBn)(Ag@4-MBn@Ag-c-DnA)as the SERS signal probe for the indirect detection of histamine.Under an applied magnetic field in the absence of histamine,the assembly gave an intense Raman signal at“Raman biological-silent”region due to 4-MBn.In the presence of histamine,the Ag@4-MBn@Ag-c-DnA SERS-tag was released from the Fe_(3)O_(4)@Au MnPs,thus decreasing the SERS signal.Under optimal conditions,an ultra-low limit of detection of 0.65×10^(-3)ng/mL and a linear range 10^(-2)-10^5 ng/mL on the SERS aptasensor were obtained.The histamine content in four food samples were analyzed using the SERS aptasensor,with the results consistent with those determined by high performance liquid chromatography.The present work highlights the merits of indirect strategies for the ultrasensitive and highly selective SERS detection of small biological molecules in complex matrices.

The effects of a magnetic dipole on a nonlinear thermally radiative ferromagnetic liquidﬂowing over a stretched surface in the presence of Brownian motion and thermophoresis are investigated.By means of a similarity transformation,ordinary differential equations are derived and solved afterwards using a numerical(the BVP4c)method.The impact of various parameters,namely the velocity,temperature,concentration,is presented graphically.It is shown that the nanoparticles properties,in conjunction with the magnetic dipole effect,can increase the thermal conductivity of the engineered nanoﬂuid and,consequently,the heat transfer.comparison with earlier studies indicates high accuracy and effectiveness of the numerical approach.An increase in the Brow-nian motion parameter and thermophoresis parameter enhances the concentration and the related boundary layer.The skin-friction rises when the viscosity parameter is increased.A larger value of the ferromagnetic para-meter results in a higher skin-friction and,vice versa,in a smaller nusselt number.
