Background:Healthcare pathways of patients with prostate cancer are heterogeneous and complex to apprehend using traditional descriptive statistics.clustering and visualization methods can enhance their characterization.Methods:Patients with prostate cancer in 2014 were identified in the French National Healthcare database(Système National des Données de Santé—SNDS)and their data were extracted with up to 5 years of history and 4 years of follow‐up.Fifty‐one‐specific encounters constitutive of prostate cancer management were synthesized into four macro‐variables using a clustering approach.Their values over patient follow‐ups constituted healthcare pathways.Optimal matching was applied to calculate distances between pathways.Partitioning around medoids was then used to define consistent groups across four exclusive cohorts of incident prostate cancer patients:Hormone‐sensitive(HSPc),metastatic hormone‐sensitive(mHSPc),castration‐resistant(cRPc),and metastatic castration‐resistant(mcRPc).Index plots were used to represent pathways clusters.Results:The repartition of macro‐variables values—surveillance,local treatment,androgenic deprivation,and advanced treatment—appeared to be consistent with prostate cancer status.Two to five clusters of healthcare pathways were observed in each of the different cohorts,corresponding for most of them to relevant clinical patterns,although some heterogeneity remained.For instance,clustering allowed to distinguish patients undergoing active surveillance,or treated according to cancer progression risk in HSPc,and patients receiving treatment for potentially curative or palliative purposes in mHSPc and mcRPc.conclusion:Visualization methods combined with a clustering approach enabled the identification of clinically relevant patterns of prostate cancer management.characterization of these care pathways is an essential element for the comprehension and the robust assessment of healthcare technology effectiveness.

Wave propagation analysis for a second strain gradient rod theory

In this work,an enriched model describing the longitudinal wave propagation is established based on Mindlin’s Second Strain Gradient(SSG)theory,which can describe the heterogeneity caused by the micro-structure interactions in the frame of continuum mechanics.The governing equation and associated boundary conditions are derived based on Hamilton’s principle,then the dispersion relation of non-classical longitudinal wave together with the extra-waves appearing exclusively in SSG theory model are investigated.The investigations are based on the modal density,energy flow,and forced response of the rod.Wave transmission and reflection through planar interfaces based on the proposed model have been calculated.Finally,the results of the enriched model are well interpreted by comparing with the classical theory results,and some useful conclusions are derived on the SSG theory based model in the wave propagation characterization.

Therapeutic strategies for post-transplant recurrence of hepatocellular carcinoma

Despite stringent selection criteria,hepatocellular carcinoma recurrence after liver transplantation(LT)still occurs in up to 20%of cases,mostly within the first 2–3 years.No adjuvant treatments to prevent such an occurrence have been developed so far.However,a balanced use of immunosuppression with minimal dose of calcineurin inhibitors and possible addition of mammalian target of rapamycin inhibitors is strongly advisable.Moreover,several pre-and posttransplant predictors of recurrence have been identified and may help determine the frequency and duration of post-transplant follow-up.When recurrence occurs,the outcomes are poor with a median survival of 12 mo according to most retrospective studies.The factor that most impacts survival after recurrence is timing(within 1–2 years from LT according to different authors).Several therapeutic options may be chosen in case of recurrence,according to timing and disease presentation.Surgical treatment seems to provide a survival benefit,especially in case of late recurrence,while the benefit of locoregional treatments has been suggested only in small retrospective studies.When systemic treatment is indicated,sorafenib has been proved safe and effective,while only few data are available for lenvatinib and regorafenib in second line.The use of immune checkpoint inhibitors is controversial in this setting,given the safety warnings for the risk of acute rejection.

目的研究多囊素的细胞定位，了解正常肾脏与肾组织多囊素的表达及分布差别。方法应用多囊素合成肽单抗抗ｐｅｐＰＢＰ１和抗ｐｅｐＰＢＰ３以及融合蛋白抗体抗ｆｐＡＨ４，对３例正常肾组织和８例多囊肾组织的标本做免疫组织化学染色定位。结果正常肾组织的多囊素组织化学定位主要分布在远端小管和集合管，髓质部更明显，近端小管弱染或阴性，肾小球上皮细胞也可着染。８例多囊肾组织的染色细胞在２８％～６０％，大多数为５０％，囊肿上皮染色比正常肾组织强，囊肿上皮染色强弱不同，少数囊肿上皮染色阴性。结论正常肾组织和多囊肾组织均可表达多囊素，多囊素染色以远端小管和集合管为主，细胞染色有差异性；多囊肾的囊上皮染色存在异质性。

准确及高稳定度的宇航级原子钟是精密卫星导航系统中的关键设备,现有的美国全球定位系统和俄罗斯全球导航卫星系统,以及即将到来的中国北斗卫星导航系统、欧洲伽利略卫星导航系统、印度区域性卫星导航系统和日本准天顶卫星系统都装载着不同类型的原子钟。本文首先探讨了星载原子钟在不同导航系统中的应用,以及导航系统对星载钟的环境和关键性能的要求。SpectraTime（SpT）公司为多个导航系统（欧洲、中国和印度）及其它空间项目提供空间铷钟和被动型氢钟。伽利略在轨验证试验卫星（GIOVE）于2005年12月和2008年4月的两次发射,以及北斗卫星的相继发射,使这两种原子钟技术拥有了若干年的飞行经历。迄今为止SpT公司已生产交付了60多台铷钟和15台被动型氢钟的飞行件,并进行了批量钟的特性鉴定。本文报告了空间铷钟和被动型氢钟的地面批量和寿命试验测试结果,以及在卫星在轨试验所达到的最新性能结果。基于这些星载钟的试验结果,本文对卫星导航系统的地面时间站中原子钟及其相关算法作了简要描述,最后还介绍了一种新颖的在轨技术,即从星载原子钟组（ONcLE）直接产生高度稳健的时间信号。

Retrospective study of a cohort of adult patients with hematological malignancies in a tropical area

AIM: To review the characteristics of hematological malignancies in tropical areas, and to focus on the specific difficulties regarding their management. METHODS: This is a retrospective narrative review of cases of patients with hematological malignancies. All medical files of patients with malignant disease whose treatment was coordinated by the HematoOncology service of the cayenne Hospital in French Guiana between the 1st of January 2010 and the 31 st of December 2012 were reviewed. clinical data were extracted from the medical files and included: Demographic data, comorbidities, serological status for human immunodeficiency virus, human T-lymphotropic virus 1(HTLV1), hepatitis B virus and hepatitis c virusinfections, cytology and pathology diagnoses, disease extension, treatment, organization of disease management, and follow-up. The subgroup of patients with hematological malignancies and virus-related malignancies were reviewed. cases involving patients with Kaposi sarcoma, and information on solid tumor occurrence in virus-infected patients in the whole patient population were included. Since the data were rendered anonymous, no informed consent was obtained from the patients for this retrospective analysis. Data were compiled using EXcEL® software, and the data presentation is descriptive only. The references search was guided by the nature of the data and discussion. RESULTS: In total, the clinical files of 594 patients(pts) were reviewed. Hematological malignancies were observed in 87 patients, and Kaposi sarcoma in 2 patients. In total, 70 patients had a viral infection, and 34 of these also had hematological malignancies. The hematological diagnoses were: Multiple myeloma in 27 pts, lymphoma(L) in 43 pts, myeloproliferative disorders in 17 pts and Kaposi sarcoma in two patients. The DArk Matter Particle Explorer(DAMPE)is a satellite-borne detector for high-energy cosmic rays and y-rays.To fully understand the detector performance and obtain reliable physical results,extensive simulations of the detector are necessary.The simulations are particularly important for the data analysis of cosmic ray nuclei,which relies closely on the hadronic and nuclear interactions of particles in the detector material.Widely adopted simulation softwares include the GEANT4 and FLUKA,both of which have been implemented for the DAMPE simulation tool.Here we describe the simulation tool of DAMPE and compare the results of proton shower properties in the calorimeter from the two simulation softwares.Such a comparison gives an estimate of the most significant uncertainties of our proton spectral analysis.
