Monocyte chemoattractant protein-1, transforming growth factor-β1, nerve growth factor, resistin and hyaluronic acid as serum markers：comparison between recurrent acute and chronic pancreatitis

BAcKGROUND： Diagnostic parameters that can predict the presence of chronic pancreatitis（cP） in patients with recurrent pain due to pancreatitis would help to direct appropriate therapy. This study aimed to compare the serum levels of monocyte chemoattractant protein-1（McP-1）, transforming growth factor-β1（TGF-β1）, nerve growth factor（NGF）, resistin and hyaluronic acid（HA） in patients with recurrent acute pancreatitis（RAP） and cP to assess their ability to differentiate the two ***： Levels of serum markers assessed by enzymelinked immunosorbent assay（ELISA） were prospectively compared in consecutive patients with RAP, cP and in controls and stepwise discriminant analysis was performed to identify the markers differentiating RAP from ***： One hundred and thirteen consecutive patients（RAP=32, cP=81） and 78 healthy controls were prospectively enrolled. The mean（SD） age of the patients was 32.0（14.0）years; 89（78.8%） were male. All markers were significantly higher in cP patients than in the controls（P〈0.001）; McP-1NGF and HA were significantly higher in RAP patients than in the controls（P〈0.001）. Stepwise discriminant analysis showed significant difference（P=0.002） between RAP and cP for resistin with an accuracy of 61.9%, discriminant scores of ≤-0.479 and ≥0.189 indicating RAP and cP, respectively. The other markers had no differential value between RAP and ***： Serum resistin is a promising marker to differentiate between RAP and cP and needs validation in future studies, especially in those with early cP.

comparing acid steatocrit and faecal elastase estimations for use in M-ANNHEIM staging for pancreatitis

AIM To compare two tests for exocrine pancreatic function(EPF) for use in M-ANNHEIM staging for pancreatitis. METHODS One hundred and ninety four consecutive patients with acute pancreatitis(AP; n = 13), recurrent acute pancreatitis(RAP; n = 65) and chronic pancreatitis(cP; n = 116) were enrolled. EPF was assessed by faecal elastase-1(FE-1) estimation and stool fat excretion by the acid steatocrit method. Patients were classified as per M-ANNHEIM stages separately based on the results of the two tests for comparison. Independent Student's t-test, χ~2 test, Kruskal-Wallis test, Mann-Whitney U test and Mc Nemar's test were used as appropriate. RESULTS Sixty-one(52.5%) patients with cP had steatorrhoea when assessed by the acid steatocrit method; 79 (68.1%) with cP had exocrine insufficiency by the FE-1 test(χ~2 test, P cid steatocrit and FE-1 showed a significant negative correlation(Spearman's rho =-0.376, P cally significant difference was seen between the M-ANNHEIM stages as classified separately by acid steatocrit and the FE-1. Thirteen(6.7%), 87(44.8%), 89(45.8%) and 5(2.5%) patients were placed in M-ANNHEIM stages 0,?Ⅰ, Ⅱ, and Ⅲ respectively, with the use of acid steatocrit as against 13(6.7%), 85(43.8%), 75(38.6%), and 21(10.8%) respectively by FE-1 in stages 0,?Ⅰ, Ⅱ, and Ⅲ thereby altering the stage in 28(14.4%) patients(P c Nemar's test). cONcLUSION FE-1 estimation performed better than the acid steatocrit test for use in the staging of pancreatitis by the M-ANNHEIM classification since it diagnosed a higher proportion of patients with exocrine insufficiency.

Defining acute-on-chronic liver failure: East,West or Middle ground?

Acute-on-chronic liver failure(AcLF),a newly recognized clinical entity seen in hospitalized patients with chronic liver disease including cirrhosis,is associated with high short- and medium term morbidity and *** the definitions of AcLF proposed so far have been universally accepted,the two most commonly used being those proposed by the Asia-Pacific Association for the Study of Liver(APASL) and the European Association for the Study of Liver- chronic Liver Failure(EASL-cLIF) *** paper both definitions and diagnostic criteria appear to be different from each other,reflecting the differences in cut-off values for individual parameters used in diagnosis,the acute insult or precipitating event and the underlying chronic liver *** directly comparing these two criteria are limited,and available studies reveal different outcomes when the two are applied to the same set of *** a review of the literature suggests that both definitions do not seem to identify the same set of *** definition given by the APASL consortium is easier to apply in day-to-day practice but the EASLcLIF criteria appear to better predict mortality in *** World Gastroenterology Organization working party have proposed a working definition of AcLF which will identify patients from whom relevant data can be collected so that the similarities and the differences between the two regions,if any,can be clearly defined.

A framework for building energy management system with residence mounted photovoltaic

Efficient utilization of a residential photovoltaic (PV) array with grid connection is difficult due to power fluctuation and geographical dispersion. Reliable energy management and control system are required for overcoming these obstacles. This study provides a new residential energy management system (REMS) based on the convolution neural network (cNN) including PV array environment. The cNN is used in the estimation of the nonlinear relationship between the residence PV array power and meteorological datasets. REMS has three main stages for the energy management such as forecasting, scheduling, and real functioning. A short term forecasting strategy has been performed in the forecasting stage based on the PV power and the residential load. A coordinated scheduling has been utilized for minimizing the functioning cost. A real-time predictive strategy has been used in the actual functioning stage to minimize the difference between the actual and scheduled power consumption of the building. The proposed approach has been evaluated based on real-time power and meteorological data sets.

AIM: To evaluate the association of known copy number variations(cNVs) in ulcerative colitis(Uc) progressing to colorectal ***: Microsatellite instability analysis using the National cancer Institute's panel of markers, and cNV association studies using Agilent 2 × 105 k arrays were done in tissue samples from four patient groups with Uc: those at low risk(LR) or high risk of developing colorectal cancer, those with premalignant dysplastic lesions, and those with colitis-associated colorectal cancer(cAc). DNA from tissue samples of these groups were independently hybridized on arrays and analyzed. The data obtained were further subjected to downstream bioinformatics enrichment analysis to examine the correlation with cAc ***: Microarray analysis highlighted a progressive increase in the total number of cNVs [LR(n = 178) vs cAc(n = 958), 5.3-fold], gains and losses [LR(n = 37 and 141) vs cAc(n = 495 and 463), 13.4- and 3.3-fold, respectively], size [LR(964.2 kb) vs cAc(10540 kb), 10.9-fold] and the number of genes in such regions [LR(n = 119) vs cAc(n = 455), 3.8-fold]. chromosomewise analysis of cNVs also showed an increase in the number of cNVs across each chromosome. There were 38 genes common to all four groups in the study; 13 of these were common to cancer genes from the Genetic Disease Association dataset. The gene set enrichment analysis and ontology analysis highlighted many cancerassociated genes. All the samples in the different groupswere microsatellite ***: Increasing numbers of cNVs are associated with the progression of Uc to cAc, and warrant further detailed exploration.

Optimization of Machining characteristics for Al/Sicp composites using ANN/GA

The present work is focused on optimization of machining characteristics of AI/Sicp composites. The machining characteristics such as specific energy, tool wear and surface roughness were studied. The parameters such as volume fraction of Sic, cutting speed and feed rate were considered. Artificial neural networks (ANN) was used to train and simulate the experimental data. Genetic algorithms (GA) was interfaced with ANN to optimize the machining conditions for the desired machining characteristics. Validation of optimized results was also performed by confirmation experiments.

Developments in Processing of Functionally Gradient Metals and Metal–ceramic composites:A Review

Functionally gradient/graded materials （FGMs）, an emerging new class of materials, are the outcome of the recent innovative concepts in materials technology. FGMs are in their early stages of evolution and expected to have a strong impact on the design and development of new components and structures with better performance. FGMs exhibit gradual transitions in the microstructure and/or the composition in a specific direction, the presence of which leads to variation in the functional performance within a part. The presence of gradual transitions in material composition in FGMs can reduce or eliminate the deleterious stress concentrations and result in a wide gradation of physical and/or chemical properties within the material. Functionally graded metal-ceramic composites are also getting the attention of the researchers. Among the fabrication routes for FGMs such as chemical vapour deposition, physical vapour deposition, the sol-gel technique, plasma spraying, molten metal infiltration, self propagating high temperature synthesis, spray forming, centrifugal casting, etc., the ones based on solidification route are preferred for FGMs because of their economics and capability to make large size products. The present paper discusses and compares various solidification processing tech- niques available for the fabrication of functionally gradient metals and metal-ceramic composites and lists their properties and possible applications. The other processing methods are briefly described.

Reports of combined candidal and herpetic esophagitis in immunocompetent states are rare and sporadic.A 44-year-old previously healthy lady presented with a one week history of progressive dysphagia,odynophagia and ***(EGD) showed extensive desquamation of the entire esophagus except for distal 4 *** examination revealed ulcerated and inflamed squamous epithelium with the margin of ulcer showing a few overhanging squamous cells with dense eosinophilic cytoplasm,multinucleated and faceted nuclei with glassy chromatin,and an occasional cowdry type A intranuclear inclusion *** candidal spores were seen in the underlying *** acyclovir,fluconazole and pantoprazole were *** analgesics were given for pain *** was treated for a total of 14 *** showed significant improvement and was tolerating oral intake after *** patient was asymptomatic with no evidence of recurrence at a 2-month follow-up.

Urotensin-I suppresses the hypothalamo-hypophyseal-ovarian axis in the Mozambique tilapia,Oreochromis mossambicus

Urotensin-I(UI)is a peptide belonging to the corticotropin-releasing hormone family,produced in the urophysis of *** investigation was undertaken to examine the effect of UI on the hypothalamo-hypophyseal-ovarian axis in the Mozambique tilapia,a cichlid *** the first experiment,administration of UI(2μg/100μL saline)for 22 days caused a significant reduction in the number of follicles at stages I,II,III(previtellogenic),IV(vitellogenic),and V(preovulatory),concomitant with a significant increase in the percent atresia of follicles at stages II,III,and IV than in *** the pituitary,gonadotropin-releasing hormone(GnRH)-positive fibres were detected in the proximal pars distalis(PPD)in both controls and UI-treated fish;however,there was a significant reduction in the intensity and percent area of GnRH-immunoreactive fibres in fish treated with UI compared with *** serum luteinizing hormone(LH)level was significantly lower,whereas a significant rise in the serum level of cortisol was noticed in UI-treated fish compared with *** the second experiment,treatment with a low dose of UI(10 mM)did not affect the number of AcTH-secreting cells in vitro,but the number of these cells was significantly higher in high-dose UI(50 mM)-treated pituitaries compared to ***,the present investigation reveals for the first time that the treatment of UI affects the follicular development process through the downregulation of the GnRH-LH axis and activation of the pituitary-interrenal pathway in teleosts.
