Introduction Obesity affects one-third of the US population and accounts for$200 billion/year in expenditures[1].Diet and exercise form the cornerstone of management,yet are often ineffective[2].mobile health applications(mHealth apps)can augment traditional strategies;Gill and Panda[3]demonstrated that obese patients can modify their eating behavior and lose weight using an mHealth app to track food *** promising,these apps typically require active data input and currently lack passively collected,physiologic input to support sustained behavior change.

Quick response research conducted by social scientists in the aftermath of a disaster can reveal important findings about hazards and their impacts on *** to collect perishable data,or data that will change or be lost over time,immediately following disaster has been supported for decades by two programs in the United States,amassing a collection of quick response studies and an associated research *** culture is currently being challenged to better address power imbalances between researchers and disaster-affected *** recently,Canada has not had a quick response grant *** order to survey the state of knowledge and draw from it in helping to shape the new program in Canada,this article systematically analyzes the body of research created by the two US *** results reveal a wide-ranging literature:the studies are theoretically,conceptually,topically,and methodologically quite unique to one *** diversity might appropriately reflect the nature of disasters,but the finding that many studies are not building on previous quick response research and other in sights indicate opportunities for how a new grant program in Canada can contribute to growing a robust subdiscipline of disaster research.

Androgen receptor(Ar)is a major transcription factor that plays a role in inflammatory response including interleukin-6(IL6)signaling.1 While Ar regulation through paracrine loop signaling in prostate tissue is well-studied,its impact through an IL6 autocrine loop in the lung has not been wellstudied despite the organ's response to respiratory viral *** inhibition and rNA knockdown of Ar identified a bZIP transcription factor mAF to be a common target of inflammation using these perturbations in lung cells.

AIm To evaluate the association between virulence factor status and antibiotic resistance in Helicobacter pylori(H. pylori)-infected patients in Ireland. mETHODS DNA was extracted from antral and corpus biopsies obtained from 165 H. pylori-infected patients. Genotyping for clarithromycin and fluoroquinolone-mediating mutations was performed using the Genotype Helico Dr assay. cag A and vac A genotypes were investigated using PCr. rESULTS Primary, secondary and overall resistance rates for clarithromycin were 50.5%(n = 53/105), 78.3%(n = 47/60) and 60.6%(n = 100/165), respectively. Primary, secondary and overall resistance rates for fluoroquinolones were 15.2%(n = 16/105) and 28.3%(n = 17/60) and 20%(n = 33/165), respectively. resistance to both antibiotics was 12.4%(n = 13/105) in treatment-na?ve patients, 25%(n = 15/60) in those previously treated and 17%(n = 28/165) overall. A cag A-positive genotype was detected in 22.4%(n = 37/165) of patient samples. The dominant vac A genotype was S1/m2 at 44.8%(n = 74/165), followed by S2/m2 at 26.7%(n = 44/165), S1/m1 at 23.6%(n = 39/165) and S2/m1 at 4.8%(n = 8/165). Primary clarithromycin resistance was significantly lower in cag A-positive strains than in cag A-negative strains [32%(n = 8/25) vs 56.3%(n = 45/80); P = 0.03]. Similarly, in patients infected with more virulent H. pylori strains bearing the vac A s1 genotype, primary clarithromycin resistance was significantly lower than in those infected with less virulent strains bearing the vac A s2 genotype, [41%(n = 32/78) vs 77.8%(n = 21/27); P = 0.0001]. No statistically significant association was found between primary fluoroquinolone resistance and virulence factor *** Genotypic H. pylori clarithromycin resistance is high and cag A-negative strains are dominant in our population. Less virulent(cag A-negative and vac A S2-containing) strains of H. pylori are associated with primary clarithromycin resistance.

AIm To compare(1) demographics in urea breath test(UBT) vs endoscopy patients; and(2) the molecular detection of antibiotic resistance in stool vs biopsy *** Six hundred and sixteen adult patients undergoing endoscopy or a UBT were prospectively recruited to the study. The Geno Type Helico Dr assay was used to detect Helicobacter pylori(H. pylori) and antibiotic resistance using biopsy and/or stool samples from CLOpositive endoscopy patients and stool samples from UBT-positive patients. rESULTS Infection rates were significantly higher in patients referred for a UBT than endoscopy(overall rates: 33% vs 19%; treatment-na?ve patients: 33% vs 14.7%, respectively). H. pylori-infected UBT patients were younger than H. pylori-infected endoscopy patients(41.4 vs 48.4 years, respectively, P r percentage of H. pylori-infected males in the endoscopy-compared to the UBT-cohort(52.6% vs 33.3%, P = 0.03). The Geno Type Helico Dr assay was more accurate at detecting H. pylori infection using biopsy samples than stool samples [98.2%(n = 54/55) vs 80.3%(n =53/66), P mples from CLO-positive endoscopy patients revealed a higher detection rate ofresistance-associated mutations using stool samples compared to biopsies. The concordance rates between stool and biopsy samples for the detection of H. pylori DNA, clarithromycin and fluoroquinolone resistance were just 85%, 53% and 35%, respectively. CONCLUSION Differences between endoscopy and UBT patients provide a rationale for non-invasive detection of H. pylori antibiotic resistance. However, the Geno Type Helico Dr assay is an unsuitable approach.

Portal vein thrombosis is an uncommonly reported complication of percutaneous transhepatic cholangiography (PTC). A thorough review of the available literature shows no reported cases. In this case, a 29 year old female presented on two separate occasions with portal vein thrombosis following PTC without drain placement. This unusual complication of image guided percutaneous biliary access is unreported in the literature and prompted evaluation of the patient's coagulation parameters. A thrombophilia screen demonstrated a mutation in the Prothrombin (Factor Ⅱ) gene. A thorough literature review shows no reported cases of portal vein thrombosis following percutaneous biliary access, is an unusual complication, and should raise suspicion of an underlying pro-coagulant state.

AIm: To determine utility of transplant liver biopsy in evaluating efficacy of percutaneous transluminal angioplasty(PTA) for hepatic venous obstruction(HVOO). mETHODS: Adult liver transplant patients treated with PTA for HVOO(2003-2013) at a single institution were reviewed for pre/post-PTA imaging findings, manometry(gradient with right atrium), presence of HVOO on prePTA and post-PTA early and late biopsy(EB and LB, r > 60 d after PTA), and clinical outcome, defined as good(no clinical issues, non-HVOO-related death) or poor(surgical correction, recurrent HVOO, or HVOOrelated death). rESULTS: Fifteen patients meeting inclusion criteria underwent 21 PTA, 658 ± 1293 d after *** procedures with pre-PTA biopsy(n = 19), no difference was seen between pre-PTA gradient in 13/19 procedures with HVOO on biopsy and 6/19 procedures without HVOO(8 ± 2.4 mm Hg vs 6.8 ± 4.3 mm Hg; P = 0.35). Post-PTA, 10/21 livers had EB(29 ± 21 d) and 9/21 livers had LB(153 ± 81 d). On clinical follow-up(392 ± 773 d), HVOO on LB resulted in poor outcomes and absence of HVOO on LB resulted good outcomes. Patients with HVOO on EB(3/7 good, 4/7 poor) and no HVOO on EB(2/3 good, 1/3 poor) had mixed outcomes. CONCLUSION: Negative liver biopsy greater than 60 d after PTA accurately identifies patients with good clinical outcomes.

In this article, the author discusses her experiences as an artist in residence in the Department of Palliative Care and rehabilitation medicine at the University of Texas m. D. Anderson Cancer Center. Emphasis is placed on applied aesthetics in palliative care and their implications for addressing communication, spiritual, and health care issues for military service members. Drawing on six vivid case studies, the author examines the various ways in which end of life narratives can shed valuable light on key issues concerning individuals’ life experiences in the Navy, the Army, and the Air Force. These cherished images strengthened people’s spirits at the end of life, and each of the men told their stories with pride.
