BACKGRoUND Anti-programmed death-1/programmed death-ligand 1(PD-1/PD-L1)immuno-therapy has demonstrated promising results on gastric ca.cer(GC).However,PD-L1 ca. express differently between metastatic sites and primary tumors(PT).AIM To compare PD-L1 status in PT and matched lymph node metastases(LNM)of GC patients and to determine the correlation between the PD-L1 status and clinicopathologica. *** We retrospectively reviewed 284 GC patients who underwent ***-L1 was evaluated by immunohistochemistry(clone SP142)using the com-bined positive *** PD-L1+PT staged as pN+were also tested for PD-L1 expression in their ***-L1(-)GC with pN+served as the comparison *** Among 284 GC patients included,45 had PD-L1+PT and 24 of them had pN+.For comparison,44 PD-L1(-)ca.es with pN+were included(sample loss of 4 ca.es).of the PD-L1+PT,54.2%(13/24 ca.es)were also PD-L1+in the *** PD-L1(-)PT,9.1%(4/44)had PD-L1+in the *** agreement between PT and LNM had a kappa value of *** tumor size and moderate/severe peritumoral inflammatory response were associated with PD-L1 positivity in both *** was no statistica. difference in overall survival for PT and LNM according to the PD-L1 status(P=0.166 and P=0.837,respectively).CoNCLUSIoN Intra-patient heterogeneity in PD-L1 expression was observed between the PT and matched *** disagreement in PD-L1 status may emphasize the importance of considering different tumor sites for analyses to select patients for immunotherapy.

Computed tomography-guided percutaneous core needle biopsy in pancreatic tumor diagnosis

AIM: To evaluate the techniques, results, and complica.ions related to computed tomography(CT)-guided percutaneous core needle biopsies of solid pancreatic ***: CT-guided percutaneous biopsies of solid pancreatic lesions performed at a ca.cer reference center between January 2012 and September 2013 were retrospectively analyzed. Biopsy material was collected with a 16-20 G Tru-Core needle(10-15 cm; Angiotech, Vancouver, ca. using a coaxial system and automatic biopsy gun. When direct access to the lesion was not possible, indirect(transgastric or transhepatic) access or hydrodissection and/or pneumodissection maneuvers were used. Characteristics of the patients, lesions, procedures, and histologic results were recorded using a standardized form. RESULTS: A total of 103 procedures included in the study were performed on patients with a mean age of 64.8 year(range: 39-94 year). The mean size of the pancreatic lesions was 45.5 mm(range: 15-195 mm). Most(75/103, 72.8%) procedures were performed via direct access, though hydrodissection and/or pneumodissection were used in 22.2%(23/103) of ca.es and indirect transhepatic or transgastric access was used in 4.8%(5/103) of ca.es. Histologic analysis was performed on all biopsies, and diagnoses were conclusive in 98.1%(101/103) of ca.es, confirming3.9%(4/103) of tumors were benign and 94.2%(97/103) were malignant; results were atypica. in 1.9%(2/103) of ca.es, requiring a repeat biopsy to diagnose a neuroendocrine tumor, and surgica. resection to confirm a primary adenoca.cinoma. only mild/moderate complica.ions were observed in 9/103 patients(8.7%),and they were more commonly associated with biopsies of lesions loca.ed in the head/uncinate process(n =8), than of those loca.ed in the body/tail(n = 1) of the pancreas, but this difference was not ***: CT-guided biopsy of a pancreatic lesion is a safe procedure with a high success rate, and is an excellent option for minimally invasive diagnosis.

objective. The aim of the present study was to investigate the usefulness of the ca. 125 area under the curve (AUC) as a new kinetic parameter for predicting overall survival in patients with ovarian ca.cer. In addition, the relationship of ca. 125 AUC with other prognostic factors of ovarian ca.cer was evaluated. Methods. Ninety- two patients that underwent primary line chemotherapy within 4 months after submission to cytoreductive surgery were included. For each patient, ca. 125 AUC was ca.culated and a statistica. analysis was conducted to compare ca. 125 AUC behavior among patients according to several covariates. Results. The mean age at diagnostic time was found to be 55.5 (16.1- 82.4) years with a mean survival of 39.2 (3.5- 100.1; SE = 2.6) months. Across FIGo stage I, II, III, and IV patients had a mean ca. 125 AUC of 18.2, 24.6, 147.8, and 574.6 IU/***, respectively (P on date, living patients had a mean ca. 125 AUC of 40.1 in contrast to 234.1 IU/*** (P or deceased ones. Patients with a complete response to primary chemotherapy had a mean ca. 125 AUC of 48.8, while patients with a partial response had a mean of 251.7 IU/***, and patients with no response or disease progression had a mean of 316.5 IU/*** (P ca. 125 AUC performance is in predicting patient complete response to chemotherapy with a cut- off of 100 IU/*** and an accuracy of 82% . Conclusions. Despite ca. 125 AUC high correlation with the FIGo stage, residual disease, and patient final outcome, the main interest of ca. 125 AUC ca.culation is to evaluate the treatment effica.y and to foresee a full chemotherapy response. Further studies should be ca.ried out before extrapolating these results to other data sets.
