Adherence to surveillance endoscopy following hospitalization for index esophageal variceal hemorrhage

AIM to investigate patient adherence to surveillance endoscopy after index esophageal variceal hemorrhage and the extent to which adherence influences *** We reviewed the records of patients with cirrhosis admitted to the medical intensive care unit between 2000 and 2014 for first time esophageal variceal hemorrhage treated with endoscopic variceal ligation who were subsequently discharged and scheduled for surveillance endoscopy at our medical center. Demographic and clinical data were obtained through the medical records, including etiology of cirrhosis, completion of variceal obliteration, attendance at surveillance endoscopy, zip code of primary residence, distance from home to hospital, insurance status, rehospitalization for variceal hemorrhage, beta-blocker at discharge, pharmacologically treated psychiatric disorder, and transplant free survival. RESULtS Of 99 consecutive survivors of esophageal variceal bleeding, the minority(33) completed variceal obliteration and fewer(12) adhered to annual surveillance. Completion of variceal obliteration was associated with fewer rehospitalizations for variceal rebleeding(27% vs 56%, P = 0.0099) and when rehospitalizations occurred, they occurred later in those who had completed obliteration(median 259 d vs 207 d, P = 0.0083). Incomplete adherence to endoscopic surveillance was associated with more rehospitalizations for variceal rebleeding compared to those fully adherent to annual endoscopic surveillance(51% vs 17%, P = 0.0328). those adherent to annual surveillance were more likely to be insured privately or through Medicare compared to those who did not attend post-hospital discharge endoscopy(100% vs 63%, P = 0.0119).CONCLUSION Most patients do not complete variceal obliteration after index esophageal variceal hemorrhage and fewer adhere to endoscopic surveillance, particularly the uninsured and those insured with Medicaid.

Focal adhesions are critical cell membrane components that regulate adhesion and migration and have cluster dimensions that correlate closely with adhesion engagement and migration *** utilized a label-free approach for dynamic,long-term,quantitative imaging of cell–surface interactions called photonic resonator outcoupler microscopy(PROM)in which membrane-associated protein aggregates outcoupled photons from the resonant evanescent field of a photonic crystal biosensor,resulting in a highly localized reduction of the reflected light *** mapping the changes in the resonant reflected peak intensity from the biosensor surface,we demonstrate the ability of PROM to detect focal adhesion *** spatial distributions can be observed between PROM images and fluorescence-labeled images of focal adhesion areas in dental epithelial stem *** particular,we demonstrate that cell–surface contacts and focal adhesion formation can be imaged by two orthogonal label-free modalities in PROM simultaneously,providing a general-purpose tool for kinetic,high axial-resolution monitoring of cell interactions with basement membranes.

Pseudosection modelling is rapidly becoming an essential part of a petrologist＇s toolkit and often forms the basis of interpreting the tectonothermal evolution of a rock sample, outcrop, or geological region. Of the several factors that can affect the accuracy and precision of such calculated phase diagrams, ＂geological＂ uncertainty related to natural petrographic variation at the hand sample- and/or thin section-scale is rarely considered. Such uncertainty influences the sample＇s bulk composition, which is the primary control on its equilibrium phase relationships and thus the interpreted pressure-temper- ature （P-t） conditions of formation. two case study examples--a garnet-cordierite granofels and a garnet-staurolite-kyanite schist--are used to compare the relative importance that geological uncer- tainty has on bulk compositions determined via （1） X-ray fluorescence （XRF） or （2） point counting techniques. We show that only minor mineralogical variation at the thin-section scale propagates through the phase equilibria modelling procedure and affects the absolute P-t conditions at which key assemblages are stable. Absolute displacements of equilibria can approach ＋l kbar for only a moderate degree of modal proportion uncertainty, thus being essentially similar to the magnitudes reported for analytical uncertainties in conventional thermobarometry. Bulk compositions determined from multiple thin sections of a heterogeneous garnet-staurolite-kyanite schist show a wide range in major-element oxides, owing to notable variation in mineral proportions. Pseudosections constructed for individual point count-derived bulks accurately reproduce this variability on a case-by-case basis, though averaged proportions do not correlate with those calculated at equivalent peak P-t conditions for a whole-rock XRF-derived bulk composition. the main discrepancies relate to varying proportions of matrix phases （primarily mica） relative to porphyroblasts （primarily staurolite and kyanit

the daily median ground surface flux densities per day from a continuously operating photomultiplier tube unit for four years in Sudbury, Ontario are presented for December 2010 to December 2014. Increases of about 2 to 4 PMt units (1 unit = 5 × 10-11 W·m-2) for median daily measures reliably occurred about two weeks before M ≥ 7.7 earthquakes anywhere on the planet. the PMt units, until June 2014, usually returned to baseline within a few days after the events. there has been a slow positive drift in flux power density since about 2012 and a conspicuous maintained increase after May, 2014. the equivalent energy per day if it were represented isotropically within the volume occupied by the earth is the same order of magnitude as the average daily global total seismic energy release. Spectral power densities revealed enhanced peaks (periodicities) between 100 to 150 days, 60 days, 30 days, and 25 days. Discriminable peaks in power were noted around 18 days, 14 days, and 4 to 6 days. these results suggest that continuous measurement of photonemissions within hyper-dark conditions may reveal geophysical processes that precede larger seismic events and could reflect the movement of the earth-solar system around the galactic center.

Fibrous dysplasia(FD)is a rare,disabling skeletal disease for which there are no established *** evidence supports inhibiting the osteoclastogenic factor receptor activator of nuclear kappa-B ligand(RANKL)as a potential treatment *** this study,we investigated the mechanisms underlying RANKL inhibition in FD tissue and its likely indirect effects on osteoprogenitors by evaluating human FD tissue pre-and post-treatment in a phase 2 clinical trial of denosumab(NCt03571191)and in murine in vivo and ex vivo preclinical *** analysis of human and mouse tissue demonstrated increased osteogenic maturation,reduced cellularity,and reduced expression of the pathogenic Gαs variant in FD lesions after RANKL *** sequencing of human and mouse tissue supported these *** interaction between osteoclasts and mutant osteoprogenitors was further assessed in an ex vivo lesion model,which indicated that the proliferation of abnormal FD osteoprogenitors was dependent on *** results from this study demonstrated that,in addition to its expected antiosteoclastic effect,denosumab reduces FD lesion activity by decreasing FD cell proliferation and increasing osteogenic maturation,leading to increased bone formation within *** findings highlight the unappreciated role of cellular crosstalk between osteoclasts and preosteoblasts/osteoblasts as a driver of FD pathology and demonstrate a novel mechanism of action of denosumab in the treatment of bone disease.
