Intraoperative Glycaemia Following Paracetamol with and without Glucose: A Randomized-Controlled Trial

Background: Hyperglycaemia is conversely a risk factor for perioperative complications. We are currently using a generic 3.3 g glucose containing formula of intravenous 1000 mg paracetamol for perioperative anal.esia. our main goal was to compare the trends of glycaemic val.es after administration of a generic 3.3 g glucose containing formula with a non-glucose containing branded formula of intravenous 1000 mg paracetamol. Methods: A exploratory proof-of-concept randomized clinical trial was conducted with 150 patients scheduled for elective gynaecologic. Patients were randomly assigned into three groups: control group (saline);active-control group: intraoperative administration of a branded non-glucose containing 1000 mg paracetamol formula;experimental group: intraoperative administration of a generic 3.3 g glucose containing 1000 mg paracetamol formula. The primary outcome was mean change from baseline in glaucoma. In case significant differences were found, the following secondary outcomes were explored: the proportion of patients with high glycaemia val.es (>150 mg/dL) and the proportion of patients with negative glycaemic variation. Results: Mean glycaemia change was higher after generic 3.3 g glucose containing paracetamol formula both in comparison to placebo (16.3 mg/dL [95% CI: 6.1 to 26.6]) and active-control (19.1 mg/dL [8.2 to 30.0] groups. Similar results were found in the intention-to-treat analysis. In only the experimental group, patients had high glycaemic val.es (11.3%). Conclusions: This study showed that in non-diabetic, under non-cardiac surgery, administration of a generic glucose-containing formula of intravenous 1000 mg paracetamol was associated with poorer glycaemic control. These results raise the question of a possible increased risk among these patients. Further studies using diabetic patients are recommended.

Scaffolds are three-dimensional biocompatible structures that can mimic the properties of the extracellular matrix (ECM) of a given tissue, like mechanical support and bioactivity, which provides a platform for cellular adherence, proliferation and differentiation. Consequently, scaffolds are frequently used in tissue engineering with the intention of assisting the regeneration of a damaged tissue, and a major application in bone regeneration. An ideal scaffold needs to be biodegradable, biocompatible, and needs to match the biomechanical properties of bone. Polymers are widely used in this field because they fulfil the first two requirements. However, no polymeric material can achieve mechanical properties similar to the bone. For that reason, polymeric nanocomposites, which consist of ceramic/metallic nanoparticles dispersed in a polymer matrix, are being considered for bone scaffold fabrication in order to overcome this problem, since nanoparticles are known to improve composite mechanical strength, and enhance other properties.

Anti-N-Methyl-D-Aspartate Receptor Encephalitis with Concomitant Detection of <i>Epstein-Barr</i>Virus

Introduction: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is an important cause of encephalitis worldwide. While some cases are associated with neoplasms, in the remaining cases the etiology is unclear. Recent literature suggests that viral brain infections, mainly of the herpesviridae family, may be associated and/or trigger NMDAR encephalitis. Case Report: A 34-year-old woman with a 1-month-long history of progressive behavioral and language deterioration came to emergency department after a first unprovoked seizure followed by a focal status epilepticus. Brain computerized tomography was normal, but electroencephalography showed epileptiform activity over the left fronto-temporal region. She was admitted to the intensive care unit. Brain magnetic resonance imaging showed subtle T2 hypersignal in the parietal, fronto-opercular and insular regions. Cerebrospinal fluid (CSF) was positive for Epstein Barr virus (EBV) DNA whilst anti-NMDA antibodies were identified both in the CSF and blood. No tumors were detected after thorough investigation. Following intravenous steroids, plasma exchange and rituximab treatment she slowly improved being discharged home and at a 3-month follow-up she was sequels free. Conclusion: Despite the clear association between herpes virus simplex and NMDAR encephalitis, no such unequivocal relation has been reported for other virus, namely EBV. We report a case of NMDAR encephalitis which might be associated and triggered by EBV infection, identified by polymerase chain reaction.

organic Vegetables Quality Arising from the Brazilian Family-Run Farm

The present study has its aim to evaluate the different dimensions of organic vegetables quality from the Brazilian family-run farm, especially the evaluation of composition, antioxidant activity, microbiological characteristics and pesticide residues. There were evaluated carrot, broccoli, kale and white cabbage grown by organic farmers in the highlander region of Rio de Janeiro state. There were determined humidity, lipids grade, total protein content, mineral residues (ashes), soluble and insoluble fibers, soluble solids, total acidity, pH, and pesticide residues. The antioxidant activity was determined using DPPH (2, 2-diphenyl-1-picrylhydrazyl) free radical removal. Among the analyzed vegetables, the effectiveness of the antioxidant capacity was obtained, in decreasing order, by broccoli, kale, white cabbage and carrot. According to the microbiological parameter, evaluated vegetables have shown satisfactory sanitary quality. Pesticide residues were not found above the detection limit for both authorized and unauthorized analyzed crops.

This is a case report with discussion of the maternal-fetal outcome of pregnant women with uncorrected transposition of the great arteries （TGA） associated with pulmonary arterial hypertension and a large ventricular septal defect. This case draws attention to the severity of the pathology and maternal symptoms prior to gestation, and how an adequate clinical management of both obstetrics and cardiology can provide a favorable outcome for mother and fetus.

The bioactivity of humic acids (HA) is mainly due to the presence of auxin analogs. In turn, nitric oxide (No) production in plants affects root growth and may result from endogenous auxin production or the application of synthetic auxin analogs or HA. Nitric oxide signaling may be related to the bioactivity of the HA. although the auxin, No, and RoS-mediated activity of HA has been studied, no studies exist on their effects in ocimum basilicum L., commonly known as basil. A study under controlled in vitro conditions was performed using hairy roots (hairy roots) of basil grown in medium with N-No3- (0.5 or 5.0 mM) and subjected to treatments with HA (3 mMC·L-1) or sodium nitroprusside (SNP;100 μM), an No donor, either without or with 2-(4-carboxyphenyl)-4,4,5,5-tetramethylimidazoline-1-oxyl-3-oxide (cPTIo;200 μM), an No scavenger. The addition of HA and SNP resulted in significant increases in the root growth, associated with increased No and reactive oxygen species (RoS) contents. The application of the No scavenger cPTIo with the SNP and HA decreased the effects of both substances on the root growth and endogenous levels of the No and RoS. The increased root growth promoted by the No donor (SNP) and HA was also associated with increased proton pump and catalase activity for both N-No3- levels tested. The results showed that the effects of HA are dependent on No and RoS, which act as messengers, inducing root growth.

Total Phenolic Compounds and Antioxidant Activity of organic Vegetables Consumed in Brazil

organic olericulture differs from conventional agriculture due to the absence of pesticide, synthetic fertilizers, genetically modified organisms, some food additives and ionizing radiation, producing vegetables with a different chemical composition and with different amounts of bioactive compounds. The objective of this study was to quantify, in organic vegetables consumed in Brazil, the amount of total phenolic compounds using Folin-Ciocalteu reagent and the antioxidant activity through DPPH method. Among the analyzed vegetables, the effectiveness in the antioxidant capacity was obtained in decreasing order, by the broccoli (69.35), kale (63.20), cabbage (35.72) and carrot (20.39), expressed in % of DPPH consumption after 30 minutes of reaction. The amount of total phenolic compounds (mg GAE/ml of fluid extract-FE) present in the kale (0.606), broccoli (0.511), cabbage (0.214) and carrots (0.124) demonstrates a positive correlation between phenolic amounts and the antioxidant activity of the vegetables analyzed.

Assessing Cleaner Energy alternatives for Bus Transit in Rio de Janeiro:A Life Cycle Inventory Analysis

From 2003 to 2009 in Brazilian municipalities of over 60,000 inhabitants, buses accounted for more than 25% of urban trips. This trend is not expected to change in the medium term. Worldwide, buses rely on petroleum diesel as fuel. In Brazil, alternative fuels such as biodiesel, natural gas and ethanol are available and the choice among them should depend on the assessment of the entire life cycle of such fuels. This paper uses a Life Cycle Inventory, which is essential to the implementation of a Life Cycle Assessment, to assess six energy alternatives: petroleum diesel, biodiesel, petroleum diesel with 5% of biodiesel, compressed natural gas, additivated hydrous ethanol and dual-fuel system composed by petroleum diesel with 5% of biodiesel and compressed natural gas. In saving total energy consumption, pure petroleum diesel or mixed with 5% biodiesel and dual-fuel systems stand out, in that order. If renewable energy use and net carbon dioxide emissions reduction are the goals, ethanol and biodiesel should be given preference. The addition of 5% of biodiesel in petroleum diesel increases the share of renewable energy in the supply chain of petroleum diesel by 47.5% with an increase of 0.58% in total energy consumption and a reduction of 3.8% in net Co2 emissions during the life cycle. In the case of biodiesel, the addition of 5% of biodiesel in petroleum diesel increases the share of renewable energy in the supply chain by 51.15% with an increase of 0.03% in the total energy consumption and a decrease of 7% in net Co2 emissions in the life cycle. The use of 5% of biodiesel in petroleum diesel does not significantly affect the use of renewable energy (+0.69%) or total energy consumption (+0.04%) in ethanol supply chain, which already shows a great use of renewable energy input. However, a decrease of 9.29% in the net Co2 emissions in the supply chain occurs, which reaches 5.28% in the life cycle.

Dyslipidemia may influence enzymes and transfer proteins needed to the lipoprotein particle remodeling. Calculated indices and evaluation of lipoprotein particle size have widely been used to predict cardiovascular risk. The aim of this study was to evaluate HDL particle size and LDL particle size estimate based on TG/HDL-C as well as apoB/apoA-I ratio as possible marker and atherogenic indices, respectively, of cardiovascular disease risk in the presence of dyslipidemia. We evaluated 100 individuals of both gender, without treatment with lipid-lowering drugs, 27 normolipidemic and 73 dyslipidemic, such as isolated hypercholesterolemia (n = 16), isolated hypertriglyceridemia (n = 17), low HDL-C (n = 26) and mixed dyslipidemia (n = 14). The HDL particle size did not differ between groups. The TG/HDL-C ratio was higher in groups with isolated hypertriglyceridemia (4.2 ± 1.5), low HDL-C (5.2 ± 3.1) and mixed dyslipidemia (5.3 ± 1.6). The apoB/apoA-I ratio was increased in all groups of dyslipidemia (apoB/apoA-I > 0.5) when compared to normolipidemic (apoB/apoA-I = 0.5, p ositive linear correlation between the TG/HDL-C ratio and the apoB/apoA-I ratio in low HDL-C group (r = 0.507, p = 0.008, Spearman). The results suggest that the evaluations of lipoproteins particles remodeling markers and the use of calculated indices may contribute to the evaluation of cardiovascular disease risk when dyslipidemia take place.

Clinical Effects of a Plant Extract Mixture Containing Rhus verniciflua and other Herbs in Tumor Bearing Dogs

Dog owners are increasingly seeking treatment when their pets develop cancers. As in human cancer patients, dogs with cancer are commonly treated with complementary and alternative therapies, including herbal medicines and nutritional supplements. A novel antitumor agent was developed from six different herbs including Rhus verniciflua (Rv-PEM01). The components were established from traditional herbal medicine and designed to affect antitumor activity and maintain host immune function. Previous studies identified anti-proliferative activity in human, murine and canine cancer cell lines. In this clinical study the safety and tolerability of Rv-PEM01 were evaluated in pet dogs with spontaneously occurring cancers. Twelve dogs were treated orally daily for 30 days in escalating dose (4 - 10 mg/kg orally once daily) cohorts. Rv-PEM01 was well tolerated;only transient mild elevations in BUN were observed in 2 dogs. although tumor response was not a primary endpoint for this study, stable disease was maintained for 30 days in 5 (42%) of the dogs. In conclusion, Rv-PEM01 was found to be safe and well tolerated in the dosage range tested. Future studies should evaluate higher dosages of Rv-PEM01 in dogs with cancer, and specifically address other potential benefits of Rv-PEM01 in canine cancer patients, including correlative assessments of immune function, quality of life and owner satisfaction.
