Carcinosarcoma of the stomach:A case report and review of the literature

Carcinosarcomas are rare,malignant,biphasic tumors. We report the case of a 62-year-old man with gastric carcinosarcoma,along with its clinical,macroscopic and histopathological features. Macroscopically,a specimen of deformed stomach was obtained that measured 200 mm×150 mm×100 mm. A 150 mm×100 mm× 50 mm exophytic tumoral mass (Borrmann typeⅠ) was found,which involved the posterior wall from the cardia to the antrum. Histopathologically,a mixed type of malignancy was revealed: an adenocarcinoma with intestinal metaplasia,with interposed fascicles of fusiform atypical cells and numerous large,rounded and oval cells. The tumor showed positive histochemistry for cytokeratin 18,epithelial membrane antigen,carcinoembryonic antigen,chromogranin A and vimentin. Liver metastases were diagnosed 8 mo postoperatively,and the patient died 4 mo later. A review of the available literature is also presented.

AIM: To investigate autonomic nervous function in patients with a diagnosis of gastroesophageal reflux disease(GERD).METHODS: The investigation was performed on 29patients(14 men), aged 18-80 years(51.14 ± 18.34),who were referred to our Neurocardiology Laboratory at the Clinical and Hospital Center "Bezanijska Kosa"with a diagnosis of GERD. One hundred sixteen healthy volunteers matched in age and sex with the examinees served as the control group. The study protocol included the evaluation of autonomic function and hemodynamic status, short-term heart rate variability(HRV) analysis, 24 h ambulatory ECG monitoring with long-term HRV analysis and 24 h ambulatory blood pressure ***: Pathologic results of cardiovascular reflex test were more common among patients with reflux compared to the control group. Severe autonomic dysfunction was detected in 44.4% of patients and in7.9% of controls(P < 0.001). Parameters of short-term analysis of RR variability, which are the indicators ofvagal activity, had lower values in patients with GERD than in the control group. Long-term HRV analysis of time-domain parameters indicated lower values in patients with reflux disease when compared to the control group. Power spectral analysis of long-term HRV revealed lower low- and high-frequency *** 24 h ambulatory blood pressure analysis showed significantly higher values of systolic blood pressure and pulse pressure in the reflux group than in the control ***: Patients with GERD have distortion of sympathetic and parasympathetic components of the autonomic nervous system, but impaired parasympathetic function appears more congruent to GERD.

Schreiber’s bent-winged bat Miniopterus schreibersii and the greater horseshoe bat Rhinolophus ferrumequinum are widespread and common cavernicolous species across southern Europe that host numerous specialized ectoparasite *** objective of this study was to characterize the species assemblage,genetic diversity,and host specificity of bat flies(Nycteribiidae,Diptera)and wing mites(Spinturnicidae,Acari)found on these bat hosts in Serbia and Bosnia and ***,while bat flies lay puparia on the cave walls and can thus be transmitted indirectly,wing mites require direct body contact for *** identification and sequencing of a 710-bp fragment of cytochrome oxidase I gene of 207 bat flies yielded 4 species,3 on *** and 1 on *** of a 460-bp small subunit ribosomal RNA fragment,in all 190 collected wing mites revealed 2 species,1 per *** no case was a parasite associated with 1 host found on the other *** and genetic diversity of flies were higher in ***,likely reflecting their host’s larger colony sizes and migratory *** species of both hosts showed similarly low diversity,likely due to their faster life history and lower winter *** findings highlight a remarkably high host-specificity and segregation of ectoparasite species despite direct contact among their hosts in the roost,suggesting a defined host preference in the investigated ectoparasite ***,the differences in ectoparasite genetic diversity exemplify the interplay between host and parasite life histories in shaping parasite population genetic structure.

Mobile Edge Computing(MEC)-based computation offloading is a promising application paradigm for serving large numbers of users with various delay and energy *** this paper,we propose a flexible MECbased requirement-adaptive partial offloading model to accommodate each user's specific preference regarding delay and energy *** address the dimensional differences between time and energy,we introduce two normalized parameters and then derive the computational overhead of processing *** from existing works,this paper considers practical variations in the user request patterns,and exploits a flexible partial offloading mode to minimize computation overheads subject to tolerable delay,task workload and power *** the resulting problem is non-convex,we decouple it into two convex subproblems and present an iterative algorithm to obtain a feasible offloading *** experiments show that our proposed scheme achieves a significant improvement in computation overheads compared with existing schemes.

Enteropathy-associated T-cell lymphoma （EATCL） is a high grade, pleomorphic peripheral T-cell lymphoma usually with cytotoxic phenotypes. We describe a first case of patient with EATCL that is remarkable for its fulminant course and invasion of both kidneys manifested as acute renal failure. The patient was a 23 year old woman with a long history of celiac disease. She was presented with acute renal failure and enlarged mononuclear infiltrated kidneys. Diagnosis of tubuloi-nterstitial nephritis and polyserositis was confirmed with consecutive pulse doses of steroid therapy. After reco-very, she had disseminated disease two months later. Magnetic resonance imaging showed thickened intestine wall, extremely augmented kidneys, enlarged intra-abdominal lymph nodes with extra-luminal compression of common bile duct. Laparotomy with mesenterial adipous tissue and lymph glands biopsy was done. Consecutive pathophysiological and immunohistochemical analyses confirmed the diagnosis of EATCL： CD45RO＋, CD43＋, CD3＋. The revision of renal pathophysiology sub-stantiated the diagnosis. The patient received chemotherapy, but unfortunately she died manifesting signs of pulmonary embolism caused by tumor cells.
