Carcinosarcoma of the stomach:A case report and review of the literature

Carcinosarcomas are rare,malignant,biphasic tumors. We report the case of a 62-year-old man with gastric carcinosarcoma,along with its clinical,macroscopic and histopathological features. Macroscopically,a specimen of deformed stomach was obtained that measured 200 mm×150 mm×100 mm. A 150 mm×100 mm× 50 mm exophytic tumoral mass (Borrmann typeⅠ) was found,which involved the posterior wall from the cardia to the antrum. Histopathologically,a mixed type of malignancy was revealed: an adenocarcinoma with intestinal metaplasia,with interposed fascicles of fusiform atypical cells and numerous large,rounded and oval cells. The tumor showed positive histochemistry for cytokeratin 18,epithelial membrane antigen,carcinoembryonic antigen,chromogranin A and vimentin. Liver metastases were diagnosed 8 mo postoperatively,and the patient died 4 mo later. A review of the available literature is also presented.

Genetic differentiation in functional traits among wild cherry (Prunus avium L.) half-sib lines

Understanding intra-specific variation in leaf functional traits is one of the key requirements for the evaluation of species adaptive capacity to ongoing climate change, as well as for designing long-term breeding and conservation strategies. Hence, data of 19 functional traits describing plant physiology, antioxidant properties, anatomy and morphology were determined on 1-year-old seedlings of wild cherry (Prunus avium L.) half-sib lines. The variability within and among half-sib lines, as well as the estimation of multi-trait association, were examined using analysis of variance (ANOVA) followed by Tukey's honestly significant difference test and multivariate analyses: principal component analysis (PCA), canonical discriminant analysis (CDA) and stepwise discriminant analysis (SDA). Pearson’s correlation coefficient was used to evaluate linear correlation between the study parameters. The results of the ANOVA showed the presence of statistically significant differences (P r all study traits. The differences within half-sib lines, observed through the contribution of the examined sources of variation to the total variance (%), had higher impact on total variation in the majority of the examined traits. Pearson’s correlation analysis and PCA showed strong relationships between gas exchange in plants and leaf size and stomatal density, as well as between leaf biomass accumulation, intercellular CO_(2) concentration and parameters related to antioxidant capacity of plants. Likewise, the results of SDA indicate that transpiration and stomatal conductance contributed to the largest extent, to the discrimination of the wild cherry half-sib lines. In addition, PCA and CDA showed separation of the wild cherry half-sib lines along the first principal component and first canonical variable with regards to humidity of their original sites. Introduction: Polymicrogyria, the appearance of irregular small and over numbered gyri on the surface of the brain, has been reported as the most frequent finding, when exists, on the temporal lobe. Case Presentation: A patient, male, age of 35, came to the regular head and brain MrI scan due to psychiatric and hormonal treatment within transsexualism. There were no data of epilepsy in a patient’s history. MrI scans were acquired with a 1.5 T Siemens Magnetom system with a standard head coil. Three-dimensional 3D rAGE, contiguous 1.0 mm slice, and 1 acquisition sequence underwent final 3D rendering and subsequent volumetry. On the definitive 3D image, we have revealed an unusual gyrisation on the left temporal lobe which had a picture of partial polymicrogyria. Conclusion: This is a unique finding, to our knowledge, of the unilateral temporal polymicrogyria in a person with transsexualism. Although polymicrogyria is mostly related to epileptifom attacks, its impact on the transsexualism appearance is opened to be examined.

AIM: To assess whether depression and anxiety are more expressed in patients with the first episode of inflammatory bowel disease (IBD) than in individuals with newly discovered cancer of the colon (CCa). METHODS: A total of 32 patients with IBD including 13 males and 19 females, aged 27 to 74, and 30 patients with CCa including 20 males and 10 females, aged 39-78, underwent a structured interview, which comprised Hamilton’s Depression rating Inventory, Hamilton’s Anxiety rating Inventory and Paykel’s Stressful Events rating Scale. rESULTS: Patients of the IBD group expressed both depression and anxiety. Depressive mood, sense of guilt, psychomotor retardation and somatic anxiety were also more pronounced in IBD patients. The discriminant function analysis revealed the total depressive score was of high importance for the classification of a newly diagnosed patient into one of the groups. CONCLUSION: Newly diagnosed patients with IBD have higher levels of depression and anxiety. Moreover, a psychiatrist in the treatment team is advisable from the beginning.

Methyl acetate is considered low toxicity volatile solvent produced either as a by-product during methanol carbonylation or via acetic acid esterification with methanol. In both cases, pure methyl acetate has to be isolated from the reaction mixture. Simulation of methyl acetate separation from its mixture with methanol by extraction distillation was carried out in ASPEN + software. In total three case studies were assumed using two different extraction solvents and two solvent regeneration strategies. In case A, novel extraction solvent 1-ethyl-3-methylimidazolium trifluoromethanesulfonate ionic liquid, was considered. raw material separation was achieved in an extraction distillation column while the solvent regeneration was accomplished in a second distillation column in this case. In case study B, the same extraction solvent was used; however, its regeneration was carried out in a single-effect evaporator. Dimethyl sulfoxide was the second extraction solvent selected. Its use in methyl acetate-methanol separation is presented in case study C. As high purity of dimethyl sulfoxide was required for the methyl acetate-methanol azeotrope breaking, its regeneration was carried out in the second distillation column only. To simulate the ternary methyl acetate-methanol-extraction solvent mixtures separation, vapor-liquid equilibrium was predicted based on the NrTL equation. Further, unknown properties of the considered ionic liquid and variation of these properties with temperature were predicted and introduced into the ASPEN + components properties database. Based on these data, optimum operation parameters of the respective separation equipment were established. AIM: To investigate autonomic nervous function in patients with a diagnosis of gastroesophageal reflux disease(GErD).METHODS: The investigation was performed on 29patients(14 men), aged 18-80 years(51.14 ± 18.34),who were referred to our Neurocardiology Laboratory at the Clinical and Hospital Center "Bezanijska Kosa"with a diagnosis of GErD. One hundred sixteen healthy volunteers matched in age and sex with the examinees served as the control group. The study protocol included the evaluation of autonomic function and hemodynamic status, short-term heart rate variability(HrV) analysis, 24 h ambulatory ECG monitoring with long-term HrV analysis and 24 h ambulatory blood pressure ***: Pathologic results of cardiovascular reflex test were more common among patients with reflux compared to the control group. Severe autonomic dysfunction was detected in 44.4% of patients and in7.9% of controls(P rameters of short-term analysis of rr variability, which are the indicators ofvagal activity, had lower values in patients with GErD than in the control group. Long-term HrV analysis of time-domain parameters indicated lower values in patients with reflux disease when compared to the control group. Power spectral analysis of long-term HrV revealed lower low- and high-frequency *** 24 h ambulatory blood pressure analysis showed significantly higher values of systolic blood pressure and pulse pressure in the reflux group than in the control ***: Patients with GErD have distortion of sympathetic and parasympathetic components of the autonomic nervous system, but impaired parasympathetic function appears more congruent to GErD.

AIM:To investigate and compare efficacy and differences in the nutritional status evaluation of gastroenterology patients by application of two methods:subjective global assessment(SGA) and nutritional risk index(NrI).METHODS:The investigation was performed on 299 hospitalized patients,aged 18-84 years(average life span 55.57 ± 12.84),with different gastrointe-stinal pathology,admitted to the Department of Gastroenterohepatology,Clinical and Hospital Center "Bezanijska Kosa" during a period of 180 *** the patients,after being informed in detail about the study and signing a written consent,underwent nutritional status analysis,which included two different nutritional indices:SGA and NrI,anthropometric parameters,bioelectrical impedance analysis,and biochemical markers,within 24 h of ***:In our sample of 299 hospitalized patients,global malnutrition prevalence upon admission varied from 45.7% as assessed by the SGA to 63.9% by *** applied methods required different parameters for an adequate approach:glucose level(5.68 ± 1.06 mmol/L vs 4.83 ± 1.14 mmol/L,F = 10.63,P = 0.001);body mass index(26.03 ± 4.53 kg/m2 vs 18.17 ± 1.52 kg/m2,F = 58.36,P r(42.62 ± 7.98 kg vs 36.22 ± 9.32 kg,F = 7.95,P = 0.005);basal metabolic rate(1625.14 ± 304.91 kcal vs 1344.62 ± 219.08 kcal,F = 9.06,P = 0.003) were very important for SGA,and lymphocyte count was relevant for NrI:25.56% ± 8.94% vs 21.77% ± 10.08%,F = 11.55,P = *** number of malnourished patients rose with the length of hospital stay according to both nutritional *** discriminative function analysis(DFA) delineated the following parameters as important for prediction of nutritional status according to SGA assessment:concentration of albumins,level of proteins,SGA score and body *** DFA extracted MAMC,glucose level and NrI scores were variables of importance for the prediction of whether admitted patients would be classif ied as well or ***:SGA
