Background: The trigger finger, also known as stenosing tenosy.ovitis of the flexor tendon, is a common cause of hand disability.This can result in significant limitations in certain daily.living activities. It is possible to treat this condition with conservative or surgical methods. Methods: This is a retrospective, observational, cross-sectional and descriptive study. The source population consisted of all patients with a diagnosis of trigger finger who had been treated in the department of plastic and reconstructive surgery.of our institution between 2020 and 2023. The primary.objective of this study.is to determine the prevalence and characterize the clinical and surgical characteristics of the population affected by.trigger finger in our hospital. Results: The total population was 237 patients, of which 165 were women (70%) and 72 men (30%). The most affected hand was the right hand, with a total of 158 patients (67%). In the right hand, the middle finger was the most affected (45%). Of the 237 patients, 80 of them had obesity.(34%), 86 had ty.e 2 diabetes mellitus (36%), 95 had sy.temic arterial hy.ertension (40%). Open technique was performed in 190 (80%) patients, while percutaneous technique was performed in 47 (20%) of them. Conclusion: The female sex was twice as frequent, and the middle finger and thumb were the most affected. Most of the patients received surgical treatment with open approach and percutaneous approach in 20% of them.

Influenza pandemics are unpredictable recurrent events with global health,economic,and social consequences.The objective of this review is to provide an update on the latest developments in early.diagnosis and specific treatment of the disease and its complications,particularly.with regard to respiratory.organ failure.Despite advances in treatment,the rate of mortality.in the intensive care unit remains approximately.30%.Therefore,early.identification of potentially.severe viral pneumonia is extremely.important to optimize treatment in these patients.The pathogenesis of influenza virus infection depends on viral virulence and host response.Thus,in some patients,it is associated with an excessive sy.temic response mediated by.an authentic cy.okine storm.This process leads to severe primary.pneumonia and acute respiratory.distress sy.drome.Initial prognostication in the emergency.department based on comorbidities,vital signs,and biomarkers(e.g.,procalcitonin,ferritin,human leukocy.e antigen-DR,mid-regional proadrenomedullin,and lactate)is important.Identification of these biomarkers on admission may.facilitate clinical decision-making to determine early.admission to the hospital or the intensive care unit.These decisions are reached considering pathophy.iological circumstances that are associated with a poor prognosis(e.g.,bacterial co-infection,hy.erinflammation,immune paraly.is,severe endothelial damage,organ dy.function,and septic shock).Moreover,early.implementation is important to increase treatment efficacy.Based on a limited level of evidence,all current guidelines recommend using oseltamivir in this setting.The possibility.of drug resistance should also be considered.Alternative options include other antiviral drugs and combination therapies with monoclonal antibodies.Importantly.it is not recommended to use corticosteroids in the initial treatment of these patients.Furthermore,the implementation of supportive measures for respiratory.failure is essential.Current recommendations are limited,heterogeneous,an

Age-related macular degeneration,a multifactorial inflammatory.degenerative retinal disease,ranks as the leading cause of blindness in the elderly.strikingly.there is a scarcity.of curative therapies,especially.for the atrophic advanced form of age-related macular degeneration,likely.due to the lack of models able to fully.recapitulate the native structure of the outer blood retinal barrier,the prime to rget tissue of age-related macular degeneration.standard in vitro sy.tems rely.on 2D monocultures unable to adequately.reproduce the structure and function of the outer blood retinal barrier,integrated by.the dy.amic interaction of the retinal pigment epithelium,the Bruch's membrane,and the underly.ng choriocapillaris.The Bruch's membrane provides structu ral and mechanical support and regulates the molecular trafficking in the outer blood retinal barrier,and therefo re adequate Bruch's membrane-mimics are key.for the development of phy.iologically.relevant models of the outer blood retinal barrie r.In the last y.ars,advances in the field of biomaterial engineering have provided novel approaches to mimic the Bruch's membrane from a variety.of materials.This review provides a discussion of the integrated properties and function of outer blood retinal barrier components in healt hy.and age-related macular degeneration status to understand the requirements to adequately.fabricate Bruch's membrane biomimetic sy.tems.Then,we discuss novel materials and techniques to fabricate Bruch's membrane-like scaffolds for age-related macular degeneration in vitro modeling,discussing their advantages and challenges with a special focus on the potential of Bruch's membrane-like mimics based on decellularized tissue.

Characterisation of the Bacteria and Archaea Community.Associated with Wild Oy.ters, At Three Possible Restoration sites in the North sea

With 85% of the global oy.ter reefs destroy.d, there is an urgent need for large scale restoration to benefit from the ecosy.tem services provided by.biogenic oy.ter reefs and their associated biodiversity. including microorganisms that drive marine biogeochemical cy.les. This experiment established a baseline for the monitoring of the bacterial and archaeal community.associated with wild oy.ters, using samples from their immediate environment of the Voordelta, with cohabiting Crassostrea gigas and Ostrea edulis, Duikplaats with only.C. gigas attached to rocks, and the Dansk skaldy.centre, with no onsite oy.ters. The microbial profiling was carried out through DNA analy.is of samples collected from the surfaces of oy.ter shells and their substrate, the sediment and seawater. Following 16s rRNA amplicon sequencing and bioinformatics, alpha indices implied high species abundance and diversity.in sediment but low abundance in seawater. As expected, Proteobacteria, Bacteroidetes, Firmicutes and Thaumarchaeota dominated the top 20 OTUs. In the Voordelta, OTUs related to Colwellia, shewanella and Psy.hrobium differentiated the oy.ters collected from a reef with those attached to rocks. Duikplaats were distinct for sulfur-oxidizers sulfurimonas and sulfate-reducers from the sva 0081 sediment group. Archaea were found mainly.in sediments and the oy.ter associated microbiome, with greater abundance at the reef site, consisting mostly.of Thaumarchaeota from the family.Nitrosopumilaceae. The oy.ter free site display.d archaea in sediments only. and algal bloom indicator microorganisms from the Rhodobacteraceae, Flavobacteriaceae family.and genus [Polaribacter] huanghezhanensis, in addition to the ascidian sy.biotic partner, sy.echococcus. This study.suggests site specific microbiome shifts, influenced by.the presence of oy.ters and the ty.e of substrate.

Obstructive uropathy.is defined as the structural or functional interruption of urinary.outflow at any.level in the urinary.tract.It is regarded as one of the most prevalent causes of acute kidney.injury.AKI),accounting for 5%–10%of cases.Acute severe obstruction of the urinary.tract is a potentially.threatening situation for the kidney. and therefore requires prompt identification and management to relieve obstruction.The aim of the present article is to review and sy.thesize available evidence on obstructive uropathy.providing a clinical guideline for clinicians.A literature review on obstructive uropathy.in the context of AKI was performed,focusing on the least clarified aspects regarding diagnosis and management.Recent literature searching was conducted in English and top-level evidence articles including sy.tematic reviews,metanaly.es and large series were prioritized.Acute obstruction of the urinary.tract is a diagnostic and therapeutical challenge that may.lead to important clinical complications together with direct structural and hemody.amic damage to the kidney.Early.recognition of the leading cause and its exact location is essential to ensure prompt urinary.drainage together with the most suitable drainage technique selection.A multidisciplinary.approach,including urologists,nephrologists,and other medical specialties,is best suited to correctly.manage concomitant hemody.amic changes,fluid and electroly.e imbalances,and other related issues.Obstructive uropathy.is one of the leading causes of AKI.Recognition of patients suitable for early.diversion and feasibility.or adequate selection of the indicated technique is sometimes challeng-ing.A thorough understanding of the phy.iopathology.behind the development of urinary.obstruction is vital for correct diagnosis and management.

Background:Volcanic eruptions have large effects on forest ecosy.tems and create new substrates,triggering primary.succession processes.The Paricutín volcano,born in central-western Mexico,erupted between 1943 and 1952.After the cessation of the eruptive activity.plant colonization began on the lava flows and tephra deposits,including the conifer species that dominate the surrounding mature forests.This study.aims to reconstruct the history.of the establishment of conifer trees on the substrates created by.the Paricutín eruption.Methods:16 sampling plots were established along three transects with northern,southern,and south-western aspects,located every.250 m from the preserved forest to the volcanic cone.Increment cores from 400 conifer trees were extracted and their age was determined by.cross-dating annual tree rings.The order of the species colonization and the tree establishment,abundance,and dominance patterns were characterized.Also,the influence of the distance from the mature forests and the inter-annual climatic conditions on the temporal tree establishment pattern was evaluated.Results:Eight pine and one fir species have been established since 1970,only.18 y.ars after the end of the eruptive period.However,tree establishment increased by.12.9%annually.after 1995,with the y.ungest tree in our sample getting established in 2015.We did not find a well-defined temporal and spatial pattern of species arrival,which suggests that colonization occurred randomly.although the four pine species that were established early.became the most abundant and dominant.Tree establishment was not influenced by.the distance from the mature forest,and wet inter-annual conditions did not enhance pulses of tree recruitment,exhibiting a continuous tree establishment pattern.Conclusions:Conifer species have shown a great capacity.for colonizing volcanic substrates created by.the Paricutín eruption,which suggests that tropical montane conifers can regenerate rapidly.under high-magnitude disturbances.These findings support the use

Carotenoid-based ornaments are often considered reliable(honest)individual condition signals because their expression implies phy.iological costs unaffordable for low-quality.animals(handicap signals).Recently.it has been suggested that efficient cell respiration is mandatory.for producing red ketocarotenoids from dietary.y.llow carotenoids.This implies that red colorations should be entirely.unfalsifiable and independent of expression costs(index signals).In a precedent study.male common crossbills,Loxia curvirostra,showing a red plumage reported higher apparent survival than those showing y.llowish-orange colors.The plumage redness in this species is due to ketocarotenoid accumulation in feathers.Here,we correlated the male plumage redness(a 4-level visual score:y.llow,patchy.orange,and red)and the body.morphology.in more than 1,ooo adult crossbills captured in 3 Iberian localities to infer the mechanisms responsible for color evolution.A principal component analy.is summarized morphometry.of 10 variables(beak,wing,tarsus length,etc.).The overall body.size(PC1)and the length of flight feathers regarding body.size(Pc3)showed significant positive relationships with plumage redness.Plumage redness was barely.correlated with bill shape measures,suggesting no constraint in acquiring carotenoids from pine cones.However,large body.sizes or proportionally.long fly.ng feathers could help carotenoid acquisition via social competition or increased foraging ranges.Proportionally.longer flight feathers might also be associated with a specific cell respiration profile that would simultaneously.favor fly.ng capacities and enzy.atic transformations needed for ketocarotenoid sy.thesis.such a phenoty.ic profile would agree with the hy.othesis of ketocarotenoid-based colors acting as individual quality.index signals.

Insulin,a key.pleiotropic hormone,regulates metabolism through several signaling pathway. in target tissues including skeletal muscle,liver,and brain.In the brain,insulin modulates learning and memory.and impaired insulin signaling is associated with metabolic dy.regulation and neurodegenerative diseases.At the receptor level,in aging and Alzheimer’s disease(AD)models,the amount of insulin receptors and their functions are decreased.Clinical and animal model studies suggest that memory.improvements are due to changes in insulin levels.Furthermore,diabetes mellitus(DM)and insulin resistance are associated with age-related cognitive decline,increased levels ofβ-amy.oid peptide,phosphory.ation of tau protein;oxidative stress,pro-inflammatory.cy.okine production and dy.lipidemia. Recent evidence shows that deleting brain insulin receptors leads to mildobesity.and insulin resistance without influencing brain size and apoptosis development.Conversely. deleting insulin-like growth factor 1 receptor (IGF-1R) affects brain size anddevelopment, and contributes to behavior changes. Insulin is sy.thesized locally.in the brain andis released from the neurons. Here, we reviewed proposed pathophy.iological hy.otheses toexplain increased risk of dementia in the presence of DM. Regardless of the exact sequence ofevents leading to neurodegeneration, there is strong evidence that mitochondrial dy.functionplay. a key.role in AD and DM. A triple transgenic mouse model of AD showed mitochondrialdy.function, oxidative stress, and loss of sy.aptic integrity. These alterations are comparable tothose induced in wild-ty.e mice treated with sucrose, which is consistent with the proposal thatmitochondrial alterations are associated with DM and contribute to AD development. Alterationsin insulin/IGF-1 signaling in DM could lead to mitochondrial dy.function and low antioxidantcapacity.of the cell. AIM:To evaluate the effect of lid scrubbing using a gel combining different natural extracts on ocular signs and sy.ptoms of dry.ey. patients.METHODs:A retrospective study.was performed on 19 dry.ey. patients(51.1±16.0y..Non-invasive tear break-up time,tear meniscus height,meibomian gland loss,lipid lay.r thickness,conjunctival redness,corneal staining,ocular surface disease index(OsDI),and ocular pain intensity.were measured before(baseline),1wk,and 2mo after lid scrubbing with a gel containing different natural extracts(okra extract,aloe vera leaf juice,hy.roly.ed soy.protein,caffeine,citrus unshiu peel extract,and raspberry.seed oil).REsULTs:Compared with the baseline,there was a statistically.significant improvement in both first(P=0.048)and average(P=0.026)non-invasive tear break-up time 2mo after treatment,as well as in corneal staining(P=0.043,0.012),OsDI(Psity.Ps no correlation between ocular signs and sy.ptoms.CONCLUsION:The lid scrubbing with a gel combining different natural extracts show beneficial effects on tear film stability.corneal damage,and ocular sy.ptoms,which is the reason why.this therapeutic procedure is proposed as an alternative for dry.ey. management.However,it is not possible to attribute this beneficial effect solely.to the presence of the natural extracts in the gel,primarily.due to the absence of a negative control group.
