Radiological and clinical outcomes of midline lumbar fusion on sagittal lumbar-pelvic parameters for degenerative lumbar diseases

BACKGROUND Improving the sagittal lumbar-pelvic parameters after fusion surgery is important for improving clinical outcomes.The impact of midline lumbar fusion(MIDLF)on sagittal lumbar-pelvic alignment for the management of degenerative lumbar diseases is still unknown.AIM To analyze the effects of short-segment MIDLF and minimally invasive transforaminal lumbar interbody fusion(MIS-TliF)on sagittal lumbar-pelvic parameters.METHODS We retrospectively analyzed 63 patients with degenerative lumbar diseases who underwent single-segment MIDLF or MIS-TliF.The imaging data of patients were collected before surgery and at the final follow-up.The radiological sagittal parameters included the lumbar lordosis(LL),lower LL,L4 slope(L4S),L5 slope(L5S),L5 incidence(L5I),L1 axis and S1 distance(LASD),pelvic incidence(PI),pelvic tilt(PT),sacral slope(SS),and PI-LL mismatch(PI-LL).Additionally,the clinical outcomes,including lower back and leg pain visual analog scale(VAS)and Oswestry disability index(ODI)scores,were also analyzed.RESULTS In both groups,LL and Lower LL significantly increased,while L5I and LASD significantly decreased at the final follow-up compared to that recorded prior to operation(PliF group,SS slightly increased and the mean PT value decreased compared to that recorded prior to operation,but without a statistically significant difference(P>0.05).However,the PI-LL in both groups was significantly reduced compared to that recorded prior to operation(P0.05).In addition,the change in sagittal lumbar-pelvic parameters did not differ significantly,except forΔLASD within the two groups(P>0.05).The mean lower back and

A robust vector tracking loop structure based on potential bias analysis

This paper proposes a robust vector tracking loop structure based on potential bias analysis. The influence of four kinds of biases on the existing two implementations of Vector Tracking Loops(VTLs) is illustrated by theoretical analysis and numerical simulations, and the following findings are obtained. Firstly, the initial user state bias leads to steady navigation solution bias in the relative VTL, while new measurements can eliminate it in the absolute VTL. Secondly, the initial code phase bias is transferred to the following navigation solutions in the relative VTL, while new measurements can eliminate it in the absolute VTL. Thirdly, the user state bias induced by erroneous navigation solution of VTLs can be eliminated by both of the two VTLs. Fourthly,the multipath/NLOS likely affects the two VTLs, and the induced tracking bias in the duration of the multipath/NLOS would decrease the performance of VTLs. Based on the above analysis,a robust VTL structure is proposed, where the absolute VTL is selected for its robustness to the two kinds of initialization biases;meanwhile, the instant bias detection and correction method is used to improve the performance of VTLs in the duration of the multipath/NLOS. Numerical simulations and experimental results verify the effectiveness of the proposed robust VTL structure.

Perspective of Calcium Imaging Technology Applied to Acupuncture Research

Acupuncture, a therapeutic treatment defined as the insertion of needles into the body at specific points(ie, acupoints), has growing in popularity world-wide to treat various diseases effectively, especially acute and chronic pain. In parallel, interest in the physiological mechanisms underlying acupuncture analgesia, particularly the neural mechanisms have been increasing. Over the past decades, our understanding of how the central nervous system and peripheral nervous system process signals induced by acupuncture has developed rapidly by using electrophysiological methods. However, with the development of neuroscience, electrophysiology is being challenged by calcium imaging in view field, neuron population and visualization in vivo. Owing to the outstanding spatial resolution, the novel imaging approaches provide opportunities to enrich our knowledge about the neurophysiological mechanisms of acupuncture analgesia at subcellular, cellular, and circuit levels in combination with new labeling, genetic and circuit tracing techniques. Therefore, this review will introduce the principle and the method of calcium imaging applied to acupuncture research. We will also review the current findings in pain research using calcium imaging from in vitro to in vivo experiments and discuss the potential methodological considerations in studying acupuncture analgesia.

Upadacitinib for refractory ulcerative colitis with primary nonresponse to infliximab and vedolizumab:A case report

BACKGROUND Many patients with ulcerative colitis(UC)do not respond well to,or tolerate conventional and biological therapies.There is currently no consensus on the treatment of refractory UC.Studies have demonstrated that the selective Janus kinase 1 inhibitor upadacitinib,a small-molecule drug,is effective and safe for treating UC.However,no studies have revealed that upadacitinib is effective in treating refractory UC with primary nonresponse to infliximab and vedolizumab.CASE SUMMARY We report the case of a 44-year-old male patient with a chief complaint of bloody diarrhoea with mucus and pus,in addition to dizziness.The patient had recurrent disease after receiving mesalazine,prednisone,azathioprine,infliximab and vedolizumab over four years.Based on the endoscopic findings and pathological biopsy,the patient was diagnosed with refractory UC.In particular,the patient showed primary nonresponse to infliximab and vedolizumab.Based on the patient’s history and recurrent disease,we decided to administer upadacitinib.During hospitalisation,the patient was received upadacitinib under our guidance.Eight weeks after the initiation of upadacitinib treatment,the patient’s symptoms and endoscopic findings improved significantly.No notable adverse reactions have been reported to date.CONCLUSION Our case report suggests that upadacitinib may represent a valuable strategy for treating refractory UC with primary nonresponse.

Microbiome changes in esophageal cancer:implications for pathogenesis and prognosis

Esophageal cancer(EC)is an aggressive malignancy with a poor prognosis.Various factors,including dietary habits,and antacid and antibiotic use,have been shown to influence the esophageal microbiome.Conversely,enrichment and diversity of the esophageal microbiome can also impact its function.Recent studies have revealed prevalent changes in the esophageal microbiome among patients with EC,thus suggesting the potential contribution of the esophageal microbiome to EC development.Additionally,distinct microbiome compositions have been observed in patients with different responses to radiotherapy and chemotherapy,indicating the role of the esophageal microbiome in modulating treatment outcomes.In this review,we have examined previous studies on the esophageal microbiome in healthy individuals and patients with EC or other esophageal diseases,with a focus on identifying microbial communities associated with EC pathogenesis and prognosis.Understanding the role of the microbiome in EC may aid in early detection and optimized treatment strategies,ultimately leading to better outcomes for patients.

Osmotic-Enhanced-Photoelectrochemical Hydrogen Production Based on Nanofluidics

Harvesting clean energy such as solar energy and salinity gradient energy directly from the surrounding environment has attracted great attention.A promising proof-of-concept combination of cation-selective membrane-based osmotic energy with photoelectrochemical-based solar energy has been developed,highlighting the great potential for the direct conversion of osmotic energy to hydrogen energy.With the help of a 50-fold concentration gradient,the MXene-CdSe quantum dots system exhibits the highest photocurrent enhancement ratio(Δ/_(L-H)/Δ/_(L-L)),and the hydrogen production is increased by about 33%at a bias of 0 V versus reversible hydrogen electrode.Directly converting osmotic energy and solar energy into hydrogen energy suggests the possibility of coupling osmotic energy with other renewable energy sources.

The crosstalk between enteric nervous system and immune system in intestinal development,homeostasis and diseases

The gut is the largest digestive and absorptive organ,which is essential for induction of mucosal and systemic immune responses,and maintenance of metabolic-immune homeostasis.The intestinal components contain the epithelium,stromal cells,immune cells,and enteric nervous system(ENS),as well as the outers,such as gut microbiota,metabolites,and nutrients.The dyshomeostasis of intestinal microenvironment induces abnormal intestinal development and functions,even colon diseases including dysplasia,inflammation and tumor.Several recent studies have identified that ENS plays a crucial role in maintaining the immune homeostasis of gastrointestinal(GI)microenvironment.The crosstalk between ENS and immune cells,mainly macrophages,T cells,and innate lymphoid cells(ILCs),has been found to exert important regulatory roles in intestinal tissue programming,homeostasis,function,and inflammation.In this review,we mainly summarize the critical roles of the interactions between ENS and immune cells in intestinal homeostasis during intestinal development and diseases progression,to provide theoretical bases and ideas for the exploration of immunotherapy for gastrointestinal diseases with the ENS as potential novel targets.

One-step hydrothermal synthesis of few-layered metallic phase MoS2 for high-performance supercapacitors

In recent years,molybdenum disulfide（MoS2） has gained significant attention in the scientific community.Fewlayered MoS2demonstrates unique properties and potential applications.However,the synthesis of few-layered and high-purity 1T-MoS2is still a challenge.In this study,we successfully employed a hydrothermal method to synthesize few-layered and high-purity 1T-MoS2.The purity of the material is controlled by a combination of sodium borohydride and ethanol.Notably,the few-layered 1T-MoS2exhibits exceptional performance as a supercapacitor,including the high specific capacitance(250.3 F g-1at a current density of 1 A g-1) and excellent long-trem cycling stability（90.7 % after 5000 cycles）.Meanwhile,the asymmetric device assembled by 6-FL-1TMoS2and active carbon on carbon cloths exhibits excellent flexibility and high energy and power density(23.1μWh cm-2at 600 μW cm-2,55 μWh cm-2at 12000 μW cm-2).This work provides valuable insights into the synthesis of few-layered and high-purity 1T-MoS2,opening up new avenues for further research and applications.

Hyperuricemia(HUA)is a vital risk factor for chronic kidney diseases(CKD)and development of functional foods capable of protecting CKD is of importance.This paper aimed to explore the amelioration effects and mechanism of Andrias davidianus bone peptides(ADBP)on HUA-induced kidney damage.In the present study,we generated the standard ADBP which contained high hydrophobic amino acid and low molecular peptide contents.In vitro results found that ADBP protected uric acid(UA)-induced HK-2 cells from damage by modulating urate transporters and antioxidant defense.In vivo results indicated that ADBP effectively ameliorated renal injury in HUA-induced CKD mice,evidenced by a remarkable decrease in serum UA,creatinine and blood urea nitrogen,improving kidney UA excretion,antioxidant defense and histological kidney deterioration.Metabolomic analysis highlighted 14 metabolites that could be selected as potential biomarkers and attributed to the amelioration effects of ADBP on CKD mice kidney dysfunction.Intriguingly,ADBP restored the gut microbiome homeostasis in CKD mice,especially with respect to the elevated helpful microbial abundance,and the decreased harmful bacterial abundance.This study demonstrated that ADBP displayed great nephroprotective effects,and has great promise as a food or functional food ingredient for the prevention and treatment of HUA-induced CKD.
