Predictive value of Th17 and Treg cells at baseline for HBsAg loss in chronic hepatitis B patients with low HBsAg quantification treated with pegylated interferon and nucleos(t)ide analogue

Background and aims:The primary goal of chronic hepatitis B(CHB)treatment is to reduce hepatitis B surface antigen(HBsAg).T helper 17(Th17)and regulatory T(Treg)cells are essential for the development of CHB.However,how Th17 and Treg cells contribute to HBsAg loss is still unknown.Therefore,this study aimed to search for the predictive value of Th17 and Treg cells for HBsAg loss in CHB patients with low HBsAg quantification.Methods:The study included 99 hepatitis B e antigen(HBeAg)-negative CHB patients who had completed a year of nucleos(t)ide analogue(NA)monotherapy and had received both NA and pegylated interferon(PEG-IFN)treatment for less than 96 weeks(96 wk).In the cured group,48 patients lost HBsAg within 48 wk,while 51 patients did not(uncured group).Blood samples and clinical data were collected for research.Results:During PEG-IFN and NA combination therapy,the proportion of Th17 cells in the cured group increased significantly,while the proportion of Treg cells in the uncured group increased.From 0 to 24 wk,the proportion of Th17 cells in the cured group was significantly higher than in the uncured group,while the opposite was true for Treg cells.Patients with alanine aminotransferase(ALT)2.5 upper limit of normal(ULN)at 12 wk had a higher proportion of Th17 cells and a lower proportion of Treg cells than those with ALTlinical cure index,including age,HBsAg quantification,and the proportions of Th17 and Treg cells,had a higher area under the curve(0.957)for predicting HBsAg loss when compared to the proportions of Th17 and Treg cells and HBsAg quantification alone.Conclusions:Combined with quantification of HBsAg,the proportions of Th17 cells and Treg cells at baseline can be used as good predictors of HBsAg loss in patients with low HBsAg quantification treated with NA and PEG-IFN.

To the Editor:Chronic hepatitis B virus(HBV)infection is common(7.4%)among people living with human immunodeficiency virus(PLWH)globally,with the highest prevalence rate being reported in sub-Saharan Africa and Asia.[1]Perinatal and horizontal transmission is the major route of HBV infection in these regions.However,human immunodeficiency virus(HIV)/HBV co-infection were more commonly seen among men who have sex with men or injecting drug users in developed countries.On the other hand,the hepatitis C virus(HCV)antibody was found in 6.2%of PLWH,meaning an estimate of 2.3 million out of 36.7 million PLWH globally have serological evidence of past or present HCV co-infection.[1]PLWH with HBV or HCV co-infection is more likely to develop chronic hepatitis infection,liver fibrosis,cirrhosis,end-stage liver disease(ESLD),and hepatocellular carcinoma.They also have a significantly greater risk of all-cause mortality,with ESLD being an important cause of death.Antiretroviral therapy(ART)with activity against both HIV and HBV significantly improves clinical outcomes of HBV co-infected PLWH.

BACKGROUND Mycobacterium mucogenicum(M.mucogenicum)belongs to the group of rapidly growing Nontuberculous mycobacteria.This microorganism is associated with a wide spectrum of infectious diseases.Due to a low detection rate or the time required for conventional culture methodology,a rapid and broad-spectrum method is necessary to identify rare pathogens.CASE SUMMARY A 12-year-old immunocompetent girl presented with painful masses for five months.The first mass was found in the right upper quadrant of the abdomen,and was about 1 cm×1.5 cm in size,tough but pliable in texture,with an irregular margin and tenderness.An abscess gradually formed and ulcerated with suppuration of the mass.Three new masses appeared on the back one by one.Chest computed tomography showed patchy and streaky cloudy opacities in both lungs.Needle aspiration of the abscess was performed,but the smear and conventional culture were negative,and the pathological examination showed no pathogens.We then performed next-generation sequencing using a formalinfixed,paraffin-embedded specimen to identify the pathogen.A significantly high abundance of M.mucogenicum was detected.The patient’s abscesses gradually decreased in size,while inflammation in both lungs improved following 12-wk of treatment.No recurrence was observed four months after the end of the one-year treatment period.CONCLUSION Next-generation sequencing is a promising tool for the rapid and accurate diagnosis of rare pathogens,even when using a formalin-fixed,paraffin-embedded specimen.

Background:Effective management of imported cases is an important part of epidemic prevention and control.Hainan Province,China reported 168 coronavirus disease 2019(COVID-19),including 112 imported cases on February 19,2020,but successfully contained the epidemic within 1 month.We described the epidemiological and clinical characteristics of COVID-19 in Hainan and compared these features between imported and local cases to provide information for other international epidemic areas.Methods:We included 91 patients(56 imported and 35 local cases)from two designated hospitals for COVID-19 in Haikou,China,from January 20 to February 19,2020.Data on the demographic,epidemiological,clinical and laboratory characteristics were extracted from medical records.Patients were followed until April 21,2020,and the levels of antibodies at the follow-ups were also analysed by the Wilcoxon matched-pairs signed ranks test.Results:Of the 91 patients,78(85.7%)patients were diagnosed within the first three weeks after the first case was identified(Day 1:Jan 22,2020),while the number of local cases started to increase during the third week No new cases occurred after Day 29.Fever and cough were two main clinical manifestations.In total,15(16.5%)patients were severe,14(15.4%)had complicated infections,nine(9.9%)were admitted to the intensive care unit,and three died.The median duration of viral shedding in feces was longer than that in nasopharyngeal swabs(19 days vs 16 days,P=0.007).Compared with local cases,imported cases were older and had a higher incidence of fever and concurrent infections.There was no difference in outcomes between the two groups.IgG was positive in 92.8%patients(77/83)in the follow-up at week 2 after discharge,while 88.4%patients(38/43)had a reduction in IgG levels in the follow-up at week 4 after discharge,and the median level was lower than that in the follow-up at week 2(10.95 S/Cut Off(S/CO)vs 15.02 S/CO,P<0.001).Conclusion:Imported cases were more severe than local cases but had similar prognoses.The level

Venous diethylene glycol poisoning in patients with preexisting severe liver disease in China

AIM: To analyze the clinical presentation of venous diethylene glycol (DEG) poisoning in patients with preexisting severe liver disease and factors that correlate with DEG poisoning. METHODS: Retrospective chart review was performed to analyze the epidemiology, clinical presentation, hepatorenal functions, hemodynamics and pathological characteristics of 64 patients with severe liver disease who received intravenous armillarisin-A, the solvent of which was DEG. Comparative analyses of correlating factors and causes for poisoning were based on the presence or absence of poisoning. RESULTS: Of the 64 patients who received armillarisin-A, 15 were found to have DEG poisoning. Twelve poisoned patients died. After a mean of 5 d, the poisoned patients displayed acute renal failure. Metabolic acidosis occurred in 13 cases. BUN, Cr, and CO2 values were significantly elevated and exacerbation of digestive tract symptoms and/or symptom was noted in 11 cases. Neurological system impairment was observed in 10 cases after 2 wk. Compared to the 49 non-poisoned patients, the poisoned patients exhibited significantly lower RBC and Hb values and higher WBC count. Renal biopsy from the poisoned patientsrevealed acute tubular necrosis and interstitial nephritis. Significant differences in preexisting severe hepatitis, ascites, renal disease, and diuretic therapy were found between groups. Prior to diethylene glycol injections, the mean values for neutral granular cells, BUN, Cr, calcium and phosphorous ions differed significantly between groups. CONCLUSION: Venous diethylene glycol poisoning is characterized by oliguric acute renal failure, metabolic acidosis, digestive symptoms, nervous system impairment, and a high probability of anemia and WBC proliferation. Mortality is high. Correlative factors include preexisting severe liver disease, renal disease, and infection.

Kinetic-boosted CO_(2) electroreduction to formate via synergistic electric-thermal field on hierarchical bismuth with amorphous layer

Electrocatalytic converting CO_(2) into chemical products has emerged as a promising approach to achieving carbon neutrality.Herein,we report a bismuth-based catalyst with high curvature terminal and amorphous layer which fabricated via two-step electrodeposition achieves stable formate output in a wide voltage window of 600 mV.The Faraday efficiency(FE) of formate reached up to 99.4% at-0.8 V vs.RHE and it remained constant for more than 92 h at-15 mA cm^(-2).More intriguingly,FE formate of95.4% can be realized at a current density of industrial grade(-667.7 mA cm^(-2)) in flow cell.The special structure promoted CO_(2) adsorption and reduced its activation energy and enhanced the electric-thermal field and K^(+) enrichment which accelerated the reaction kinetics.In situ spectroscopy and theoretical calculation further confirmed that the introduction of amorphous structure is beneficial to adsorpting CO_(2)and stabling*OCHO intermediate.This work provides special insights to fabricate efficient electrocatalysts by means of structural and crystal engineering and makes efforts to realize the industrialization of bismuth-based catalysts.

Strain-promoted S-arylation and alkenylation of sulfinamides using arynes and cyclic alkynes

The conversion of commercially available chiral sulfinamides into pharmaceutically useful chiral sulfoximines via direct SIVfunctionalization is synthetically attractive but challenging due to the competitive reaction of N-functionalization. Herein, we disclose a novel strain-release strategy to access stereospecific and chemoselective SIV-arylation and alkenylation of sulfinamides using arynes and strained cyclic alkynes. This method tolerates an unprecedented chemical diversity of functional groups attached to the nitrogen center(N-R). The origin of the high SIV-selectivity is elucidated by density functional theory calculations, suggesting a stepwise mechanism for the aryne substrates and a concerted mechanism for the cyclic alkynes.

Analysis and experimental study on resistance-increasing behavior of composite high efficiency autonomous inflow control device

Bottom water coning is the main reason to reduce the recovery of horizontal bottom water reservoir. By water coning, we mean the oil-water interface changes from a horizontal state to a mound-shaped cone and breaks through to the wellbore. Autonomous inflow control device(AICD) is an important instrument maintain normal production after bottom water coning, however, the resistance increasing ability of the swirl type AICD is insufficient at present, which seriously affects the water control effect. Aiming this problem, this paper designs a multi-stage resistance-increasing and composite type AICD. The separation mechanism of oil-water two phases in this structure, the resistance form of oil-water single phase and the resistance-increasing principle of water phase are analyzed. Establishing the dual-phase multi-stage separation and resistance-increasing model, and verified by measuring the throttling pressure drop and oil-water volume fraction of the AICD, it is found that the composite type AICD has the effect of ICD and AICD at the same time, which can balance the production rate of each well section at the initial stage of production, delay the occurrence of bottom water coning. In the middle and later stages of production, water-blocking can be effectively increased to achieve water control and stable production.After structural sensitivity analysis, the influence law of various structural parameters on the water control performance of composite AICD was obtained. The simulation calculation results show that,compared with the existing swirl type AICD, composite AICD has higher sensitivity to moisture content,the water phase throttling pressure drop is increased by 4.5 times on average. The composite AICD is suitable for the entire stage of horizontal well production.

Legumes have evolved specific inventions to enhance nitrogen(N)acquisition by establishing symbiotic interactions with N-fixing rhizobial bacteria.Because symbiotic N fixation is energetically costly,legumes have developed sophisticated mechanisms to ensure carbon-nitrogen balance,in a variable environment,both locally and at the whole plant level,by monitoring nodule number,nodule development,and nodular nitrogenase activity,as well as controlling nodule senescence.Studies of the autoregulation of nodulation and regulation of nodulation by nodule inception(NIN)and NIN-LIKE PROTEINs(NLPs)have provided great insights into the genetic mechanisms underlying the nitrateinduced regulation of root nodulation for adapting to N availability in the rhizosphere.However,many aspects of N-induced pleiotropic regulation remain to be fully explained,such as N-triggered senescence in mature nodules.Wang et al.determined that this process is governed by a transcriptional network regulated by NAC-type transcription factors.Characterization and dissection of these soybean nitrogenassociated NAPs(SNAPs)transcription factor-mastered networks have yielded a roadmap for exploring how legumes rewire nodule functions across a range of N levels,laying the foundation for enhancing the growth of N-deprived crops in agricultural settings.

Thrombophilia denotes a condition,whether acquired or hereditary,characterized by increased susceptibility to hypercoagulation.[1]This condition was first described in 1965,coinciding with the discovery of an inherited predisposition to venous thromboembolism(VTE)in patients deficient in antithrombin III.[2]While arterial and venous thromboses are common in hospitalized patients,acute myocardial infarction(AMI)and pulmonary embolism(PE)stand out as lifethreateningconditions.However,theoccurrenceof AMI complicated by PE is exceedingly rare,especially when considering cases where paradoxical embolism originating from a patent foramen ovale is absent.This report presents a case of AMI complicated with PE.A comprehensive understanding of the pathophysiology of this rare yet critical condition is important for ensuring prompt diagnosis and treatment.
