目的分析研究罹患鼓膜前缘穿孔治疗中采取耳内镜下嵌入蝶形软骨鼓膜成形术的应用价值。方法选取2020.09-2022.09区间沧州市人民医院耳鼻喉科收治70例鼓膜前缘穿孔患者作为主体展开对照分析,依据治疗方式差异性展开规范分组,记对照组(n=35)、观察组(n=35),前组予以常规治疗,后组耳内镜下嵌入蝶形软骨鼓膜成形术,统计两组各项指标(治疗效果评估值、并发症发生率评估值、听力测评值、临床指标测评值)展开细致观测并实施对比分析。结果观察组治疗有效率占比高于对照组,P0.05;干预后,观察组低于对照组,P<0.05。观察组临床指标测评值低于对照组,P<0.05。结论在鼓膜前缘穿孔患者治疗期间通过予以耳内镜下嵌入蝶形软骨鼓膜成形术并安全合理开展,具有降低并发症发生率、提高听力水平、改善临床指标的效果,很大程度上提高了治疗水平。

With the rapid growth of real-world graphs,the size of which can easily exceed the on-chip(board)storage capacity of an accelerator,processing large-scale graphs on a single Field Programmable Gate Array(FPGA)becomes *** multi-FPGA acceleration is of great necessity and *** cloud providers(e.g.,Amazon,Microsoft,and Baidu)now expose FPGAs to users in their data centers,providing opportunities to accelerate large-scale graph *** this paper,we present a communication library,called FDGLib,which can easily scale out any existing single FPGA-based graph accelerator to a distributed version in a data center,with minimal hardware engineering *** provides six APIs that can be easily used and integrated into any FPGA-based graph accelerator with only a few lines of code *** the torus-based FPGA interconnection in data centers,FDGLib also improves communication efficiency using simple yet effective torus-friendly graph partition and placement *** interface FDGLib into AccuGraph,a state-of-the-art graph *** results on a 32-node Microsoft Catapult-like data center show that the distributed AccuGraph can be 2.32x and 4.77x faster than a state-of-the-art distributed FPGA-based graph accelerator ForeGraph and a distributed CPU-based graph system Gemini,with better scalability.

BACKGROUND: Poorer prognosis is seen in patients with hepatocellular carcinoma (HCC) after curative hepatic resection with early recurrence (1 year). This study aimed to identify risk factors for postoperative early recurrence of small HCC (100 ng/ml, lack of tumor capsule formation, microscopic vascular invasion, high Edmonson-Steiner grades, and cytokeratin-19 (CK-19) expression (P100 ng/ml (odds ratio 2.561, 95% confidence interval 1.057 to 6.206, P=0.037) and microscopic vascular invasion (odds ratio 4.549, 95% confidence interval 1.865 to 11.097, P=0.001) were independent factors. CONCLUSIONS: Postoperative early recurrence is related to serum AFP level >100 ng/ml and microscopic vascular invasion in patients with small HCC. Adjuvant therapy and careful follow-up are required for patients with these risk factors.

目的探讨初始可切除型胃癌肝转移(GCLM)不同治疗方式的预后情况,分析预后的影响因素。方法采用回顾性队列研究方法。收集2010年1月至2019年12月基于真实世界数据的GCLM诊疗全国多中心回顾性队列研究数据库中327例初始可切除型GCLM患者的临床病理资料;男267例,女60例;年龄为61(54,68)岁。根据患者具体情况分别行根治性手术联合系统治疗、姑息性手术联合系统治疗、单纯系统治疗。观察指标:(1)不同治疗方式患者临床特征情况。(2)不同治疗方式患者预后情况。(3)初始可切除型GCLM患者预后的影响因素分析。(4)根治性手术联合系统治疗和姑息性手术联合系统治疗潜在获益人群筛选。正态分布的计量资料以x±s表示,组间比较采用独立样本t检验。偏态分布的计量资料以M(Q1,Q3)表示,组间比较采用秩和检验。计数资料以绝对数或百分比表示,组间比较采用χ2检验。采用Kaplan-Meier法计算生存率并绘制生存曲线,Log-Rank检验进行生存分析。采用COX风险比例回归模型进行单因素和多因素分析。倾向评分匹配按1∶1最近邻匹配法匹配,卡钳值为0.1。采用森林图筛选不同手术联合系统治疗的潜在获益人群。结果 (1)不同治疗方式患者临床特征情况。327例患者中,行根治性手术联合系统治疗118例,行姑息性手术联合系统治疗164例,行单纯系统治疗45例;3者吸烟、饮酒、胃原发肿瘤部位和肿瘤最大径、肝转移肿瘤部位、转移时限比较,差异均有统计学意义(P<0.05)。(2)不同治疗方式患者预后情况。327例患者的中位总生存时间为19.9个月(95%可信区间为14.9~24.9个月),1、3年总生存率分别为61.3%、32.7%。行根治性手术联合系统治疗、姑息性手术联合系统治疗、单纯系统治疗患者的1年总生存率分别为68.3%、63.1%、30.6%,3年总生存率分别为41.1%、29.9%、11.9%,3者总生存率比较,差异有统计学意义(χ2=19.46,P<0.05)。进一步分析,行根治性手术联合系统治疗和单纯系统治疗患者总生存率比较,差异有统计学意义(风险比=0.40,95%可信区间为0.26~0.61,P<0.05);行姑息性手术联合系统治疗和单纯系统治疗患者总生存率比较,差异有统计学意义(风险比=0.47,95%可信区间为0.32~0.71,P<0.05)。(3)初始可切除型GCLM患者预后的影响因素分析。多因素分析结果显示:胃原发肿瘤越大、肿瘤分化程度为低分化、肝转移肿瘤越大、肝转移肿瘤数目为多发是影响初始可切除型GCLM患者预后的独立危险因素(风险比=1.20,1.70,1.20,2.06,95%可信区间为1.14~1.27,1.25~2.31,1.04~1.42,1.45~2.92,P<0.05);免疫或靶向治疗、根治性手术联合系统治疗、姑息性手术联合系统治疗是影响初始可切除型GCLM患者预后的独立保护因素(风险比=0.60,0.39,0.46,95%可信区间为0.42~0.87,0.25~0.60,0.30~0.70,P<0.05)。(4)根治性手术联合系统治疗和姑息性手术联合系统治疗潜在获益人群筛选。森林图分析结果显示:肿瘤分化程度为高-中分化、肝转移肿瘤部位为左半肝患者行根治性手术联合系统治疗总生存率均优于行姑息性手术联合系统治疗(风险比=0.21,0.42,95%可信区间为0.09~0.48,0.23~0.78,P<0.05)。结论与行单纯系统治疗比较,行根治性手术联合系统治疗或姑息性手术联合系统治疗可以改善初始可切除型GCLM患者预后。胃原发肿瘤越大、肿瘤分化程度为低分化、肝转移肿瘤越大、肝转移肿瘤数目为多发是影响初始可切除型GCLM患者预后的独立危险因素;免疫或靶向治疗、根治性手术联合系统治疗、姑息性手术联合系统治疗是影响初始可切除型GCLM患者预后的独立保护因素。

Objective: Human embryonic stem cells (hESCs) have recently been reported as an unlimited source of mesenchymal stem cells (MSCs).The present study not only provides an identical and clinically compliant MSC source derived from hESCs (hESC-MSCs),but also describes the immunomodulative effects of hESC-MSCs in vitro and in vivo for a carbon tetrachloride (CCl4)-induced liver inflammation ***: Undifferentiated hESCs were treated with Rho-associated kinase (ROCK) inhibitor and induced to fibroblast-looking *** cells were tested for their surface markers and multilineage differentiation *** more,we analyzed their immune characteristics by mixed lymphocyte reactions (MLRs) and animal ***: hESC-MSCs show a homogenous fibroblastic morphology that resembles bone marrow-derived MSCs (BM-MSCs).The cell markers and differentiation potential of hESC-MSCs are also similar to those of *** their original cells,hESC-MSCs possess poor immunogenicity and can survive and be engrafted into a xenogenic immunocompetent ***: The hESC-MSCs demonstrate strong inhibitory effects on lymphocyte proliferation in vitro and anti-inflammatory infiltration properties in *** study offers information essential to the applications of hESC-MSC-based therapies and evidence for the therapeutic mechanisms of action.

A polyhedral oligomeric silsesquioxane-[60]fullerene (POSS-C60) dyad was designed and used as a novel electron acceptor for bulk heterojunction (BHJ) polymer solar cells (PSCs) with an inverted device configuration. The studies of time-resolved photoinduced absorption of the pristine thin film of poly[(4,4'-bis(2-ethylhexyl)dithieno[3,2-b:2',3'-d]silole)-2,6-diyl-alt-(4,7-bis (2-thienyl)-2,1,3-benzothiadiazole)-5,5'-diyl] (SiPCPDTBT) and the composite thin film of SiPCPDTBT:POSS-C60 indicated efficient electron transfer from SiPCPDTBT to POSS-C60 with inhibited back-transfer. BHJ PSCs made by SiPCPDTBT mixed with POSS-C60 yielded the power conversion efficiencies (PCEs) of 1.50%. Under the same operational conditions, PCEs observed from BHJ PSCs made by SiPCPDTBT mixed with [6,6]-phenyl-C61-butyric acid methyl ester were 0.92%. These results demonstrated that POSS-C60 is a potentially good electron acceptor for inverted BHJ PSCs.
