C-reactive protein to albumin ratio predict responses to programmed cell death-1 inhibitors in hepatocellular carcinoma patients

BACKGROUND Over the years,programmed cell death-1(PD-1)inhibitors have been routinely used for hepatocellular carcinoma(HCC)treatment and yielded improved survival outcomes.Nonetheless,significant heterogeneity surrounds the outcomes of most studies.Therefore,it is critical to search for biomarkers that predict the efficacy of PD-1 inhibitors in patients with HCC.AIM To investigate the role of the C-reactive protein to albumin ratio(CAR)in evaluating the efficacy of PD-1 inhibitors for HCC.METHODS The clinical data of 160 patients with HCC treated with PD-1 inhibitors from January 2018 to November 2022 at the First Affiliated Hospital of Guangxi Medical University were retrospectively analyzed.RESULTS The optimal cut-off value for CAR based on progression-free survival(PFS)was determined to be 1.20 using x-tile software.Cox proportional risk model was used to determine the factors affecting prognosis.Eastern Cooperative Oncology Group performance status[hazard ratio(HR)=1.754,95%confidence interval(95%CI)=1.045-2.944,P=0.033],CAR(HR=2.118,95%CI=1.057-4.243,P=0.034)and tumor number(HR=2.932,95%CI=1.246-6.897,P=0.014)were independent prognostic factors for overall survival.CAR(HR=2.730,95%CI=1.502-4.961,P=0.001),tumor number(HR=1.584,95%CI=1.003-2.500,P=0.048)and neutrophil to lymphocyte ratio(HR=1.120,95%CI=1.022-1.228,P=0.015)were independent prognostic factors for PFS.Two nomograms were constructed based on independent prognostic factors.The C-index index and calibration plots confirmed that the nomogram is a reliable risk prediction tool.The ROC curve and decision curve analysis confirmed that the nomogram has a good predictive effect as well as a net clinical benefit.CONCLUSION Overall,we reveal that the CAR is a potential predictor of short-and long-term prognosis in patients with HCC treated with PD-1 inhibitors.If further verified,CAR-based nomogram may increase the number of markers that predict individualized prognosis.

Da Vinci robot-assisted pancreato-duodenectomy in a patient with situs inversus totalis:A case report and review of literature

BACKGROUND Situs inversus totalis(SIT)is an extremely rare congenital malformation characterized by mirror displacement of the thoracoabdominal organs such as the heart,liver,spleen,and stomach.Herein,we describe a patient with SIT complicated with cholangiocarcinoma who underwent successful pancreaticoduodenectomy with the assistance of a da Vinci robot.CASE SUMMARY A 58-year-old female presented to the hospital with paroxysmal pain in her left upper abdomen,accompanied by jaundice and staining of the sclera as chief complaints.Imaging examination detected a mass at the distal end of the common bile duct,with inverted thoracic and abdominal organs.Endoscopic retrograde cholangiopancreatography forceps biopsy revealed the presence of a well-differentiated adenocarcinoma.The patient successfully underwent robotic-assisted pancreaticoduodenectomy;the operation lasted 300 min,the intraoperative blood loss was 500 mL,and there were no intraoperative and postoperative complications.CONCLUSION SIT is not directly related to the formation of cholangiocarcinoma.Detailed preoperative imaging examination is conducive to disease diagnosis and also convenient for determining the feasibility of tumor resection.Robot-assisted pancreaticoduodenectomy for SIT complicated with cholangiocarcinoma provides a safe,feasible,minimally invasive,and complication-free alternative with adequate preoperative planning combined with meticulous intraoperative procedures.

Electrodeposition of functionally graded Ni-W/Er2O3 rare earth nanoparticle composite film

Multi-layered functionally graded(FG)structure Ni-W/Er2O3 nanocomposite films were prepared by continuously changing the deposition parameters,in which the Er2O3 and W contents varied with thickness.The microstructure and chemical composition of the electrodeposited Ni-W/Er2O3 films were determined by scanning electron microscopy(SEM)and energy-dispersive X-ray spectroscopy(EDS).The anti-corrosion and wear properties of the electrodeposition films were investigated by electrochemical measurement and ball-on-disk friction test.The microhardness distribution of the cross section of nanocomposites was measured by nanoindentation.The results showed that with decreasing agitation rate or increasing average current density,the contents of Er2O3 nanoparticles and tungsten were distributed in a gradient along the thickness,and the contents on the surface were larger.By comparison,FG Ni-W/Er2O3 films had better anti-corrosion and wear properties than the uniform Ni-W/Er2O3 films.Atomic force microscopy(AFM)and profilometry measurements indicated that Er2O3 nanoparticles had an effect on the surface roughness.

The complement system is a key component of the body's immune system. When abnormally activated, this system can induce inflammation and damage to normal tissues and participate in the development and progression of a variety of diseases. In the past, many scholars believed that alcoholic liver disease(ALD) is induced by the stress of ethanol on liver cells, including oxidative stress and dysfunction of mitochondria and protease bodies, causing hepatocyte injury and apoptosis. Recent studies have shown that complement activation is also involved in the genesis and development of ALD. This review focuses on the roles of complement activation in ALD and of therapeutic intervention in complement-activation pathways. We intend to provide new ideas on the diagnosis and treatment of ALD.

With the development of millimeter wave technology and the increasingly complex electromagnetic environment, the traditional method of DOA estimation based on subspace technology can not meet the high-precision position requirements of space-time networks in various application scenarios. For this reason, scholars have introduced compressed sensing and sparse reconstruction technology into the problem of array signal estimation, and used sparse Bayesian(SBL) reconstruction based methods to estimate the angle of arrival(DOA), which often faces the problems of estimation performance and computation adjustment. In this paper, we propose a Bayesian grid iteration algorithm with low computational complexity while ensuring high estimation performance. Firstly, the SBL with the lowest computational complexity is used for rough estimation of the direction of arrival, and the grid is divided unevenly according to the rough estimation results. After the grid division termination condition is reached, the MDL criterion is used to estimate the number of sources, and the signal is decomposed into SVD. Finally, the updated grid division is combined with the SVD decomposition results, and the DOA is fine estimated using Root SBL. Simulation results show that this algorithm has low computational complexity and high estimation performance.

Numerical Simulation of Charging Performance of Combined Sensible-Latent Heat Storage System with a Macro-Encapsulation Method

In recent years,heat storage system combining sensible and latent heat materials has received more and more attentions.In this paper,we proposed the hybrid configuration with a macro-encapsulation,and analyzed its charging performance with different influencing factors by CFD simulation.In the case,the sensible heat storage materials are magnesia brick or HT concrete and the phase change materials(PCMs)are mixed molten salts.Firstly,we analyzed the heat transfer characteristics of the hybrid configuration in charging process.Then,we analyzed the effect of heating power on charging performance.The maximum temperature of the heating surface shall not exceed 500℃as the constraint condition,the heat storage capacity increases at first and then decreases with the heating power.Then,we compared the charging performance of different solid structure and the hybrid configurations.Whether magnesia brick or HT concrete,the charging performance of the solid structure is better than that of the hybrid configuration,because the thermal conductivity of the molten salt is significantly lower than that of the two sensible heat storage materials.Then,we compared the charging performance of different molten salts.The hybrid configuration with lower melting point molten salt has better performance because of more intensity natural convection.Finally,we analyzed the charging performance of the hybrid configuration used the composite phase change material(CPCM)with high thermal conductivity and specific heat.From the result,the charging performance increases by 22.5%compared with the solid structure.These results indicate that the hybrid configuration with the macro-encapsulation method is a potential form of thermal energy storage,but it needs to be further optimized.

膨胀机性能对压缩空气储能系统的影响已被广泛研究。然而,仅有少数研究是集中于不可逆损失的影响,尤其是压力损失对膨胀机性能的影响往往被忽略。润滑油的加入可以达到改善压力损失的目的,为了阐明润滑油对压力损失的影响关系,以压缩空气为工作介质,采用6种不同运动黏度的润滑油对3台不同结构参数的单螺杆膨胀机在不同运行条件下进行了对比实验研究。结果表明,提高润滑油的运动黏度可以大幅减小压力损失及占比以及有效降低压力能量损失对轴效率的影响率。相反,虽然提高润滑油的运动黏度可以在一定程度上有效提高轴效率,但两者之间并不存在正比关系。

Seismic safety assessment with non-Gaussian random processes for train-bridge coupled systems

Extensive high-speed railway(HSR)network resembled the intricate vascular system of the human body,crisscrossing mainlands.Seismic events,known for their unpredictability,pose a significant threat to both trains and bridges,given the HSR’s extended operational duration.Therefore,ensuring the running safety of train-bridge coupled(TBC)system,primarily composed of simply supported beam bridges,is paramount.Traditional methods like the Monte Carlo method fall short in analyzing this intricate system efficiently.Instead,efficient algorithm like the new point estimate method combined with moment expansion approximation(NPEM-MEA)is applied to study random responses of numerical simulation TBC systems.Validation of the NPEM-MEA’s feasibility is conducted using the Monte Carlo method.Comparative analysis confirms the accuracy and efficiency of the method,with a recommended truncation order of four to six for the NPEM-MEA.Additionally,the influences of seismic magnitude and epicentral distance are discussed based on the random dynamic responses in the TBC system.This methodology not only facilitates seismic safety assessments for TBC systems but also contributes to standard-setting for these systems under earthquake conditions.

Energy Harvesting in the Wake of An Inverted C-Shaped Bluff Body

This paper proposes a novel wake-induced vibration(WIV)-based energy harvesting system consisting of two bluff bodies.An inverted C-shaped bluff body is stationary installed at the upstream position to generate an interference wake street,and a cylinder bluff body equipped with a transducer is elastically suspended at the downstream position to harness WIV energy.The hydrodynamics and energy harvesting(EH)performance of the proposed system are investigated via experimental studies.The reduced velocity(U*)ranging from 2 to 14(the corresponding Reynolds number ranging from 15100 to 106200)is considered in the present study.It is found that the wake generated by the inverted C-shaped bluff body significantly affects the EH performance.Enlarging the opening angle(α)of the C-shaped bluff body increases the vibration amplitude of the downstream cylinder,thereby increasing the harvested power.When the spacing(L)between two bluff bodies is two times the cylinder diameter(D),the wake-induced vibration(WIV)mode is observed,while the combined WIV and wake galloping(WG)mode occurs whenαis 150°,and L equals 3D or 4D.The average drag coefficient becomes negative when L is 2D,3D,or 4D.By carefully configuring a C-shaped bluff body,the wake generated by it can bring an augmenting effect on the vibration of the downstream EH cylinder.For example,the RMS power output of the proposed EH system reaches a maximum of 0.31 W at U*=8 and L=4D,which is 300%greater than that of its traditional counterpart.Furthermore,after a number of case stud-ies,it is identified that the proposed EH system can achieve the best performance whenαis 150°and L=2D.

目的观察经皮耳甲迷走神经刺激(taVNS)治疗慢性意识障碍(pDoC)的临床效果。方法选取宝鸡第三医院2020-06—2022-12收治的68例pDoC患者,随机分成对照组与治疗组各34例。对照组接受常规促醒治疗,治疗组在对照组的基础上接受taVNS治疗,2组患者均治疗4周。对比2组患者格拉斯哥昏迷量表(GCS)评分、昏迷恢复量表(CRS-R)评分、全身无反应性量表(FOUR)评分、安全性及临床疗效。结果治疗后,2组患者的GCS单项评分(睁眼、言语和运动)及总分、CRS-R单项评分(听觉、视觉、运动、言语、交流和唤醒)及总分和FOUR单项评分(眼部、运动、脑干和呼吸)及总分均升高(均P0.05)。2组患者均未发生不良事件。治疗组总有效率(79.41%)显著高于对照组(61.76%)(P<0.05)。结论 taVNS治疗pDoC有一定的疗效,可改善患者意识障碍程度,且安全可靠。
