Hepatitis B virus(HBV)affects approximately two billion people worldwide and more than 240 million people in the world are currently chronic carrier that could develop serious complications in the future,like liver cirrhosis and hepatocellular *** an extended HBV immunization program is being carried out since the early‘80s,representing effective preventive measure,leading to a dramatic reduction of HBV hepatitis incidence,globally HBV infection still represents a major public health *** HBV virus is a DNA virus belongs to the Hepadnaviridae *** HBV-DNA is a circular,partial double strand *** coding information is on the minus DNA strand and it is organized into four open reading *** hepatitis B virus is a DNA virus,it has a high mutation rate due to its replicative strategy,that leads to the production of many nonidentical variants at each cycle of *** fact,it contains a polymerase without the proofreading activity,and uses an RNA intermediate(pg RNA)during its replication,so error frequencies are comparable to those seen in retroviruses and other RNA viruses rather than in more stable DNA *** to the low fidelity of the polymerase,the high replication rate and the overlapping reading frames,mutations occur throughout the genome and they have been identified both in the structural and not structural *** arise of mutations being to develop of a whole of viral variants called"quasi-species"and the prevalent population,which favors virus replication,was selected by viral fitness,host’s immune pressure and external pressure,i.e.,vaccination or antiviral *** occurring mutations were found both in acute and chronic *** the present review we examine and discuss the most recent available data about HBV genetic variability and its significance.

Tumor–stromal cross-talk modulating the therapeutic response in pancreatic cancer

Background: Pancreatic ductal adenocarcinoma(PDAC) is a highly malignant solid tumor with a dismal prognosis. The stroma component makes up to 90% of the tumor mass and is thought to be one of the main reasons for the tumor’s high chemoresistance. Cancer associated fibroblasts(CAFs) have previously been identified to be the key stromal players. This is the first time we provide detailed in vitro experiments investigating tumor–stromal interactions when exposed to three well-known chemotherapeutic agents. Methods: Monocultures, indirect and direct co-cultures of two PDAC cell lines(AsPC and Panc-1) and six primary patients derived CAFs were treated with gemcitabine, nab-paclitaxel and the γ-secretaseinhibitor(GSI) DAPT. The cell viability of each component was measured with XTT. Finally, IL-6 concentrations of the supernatants were analyzed. Results: On the contrary to PDAC cell lines, CAF monocultures hardly responded to any treatment which suggested that stroma(CAFs) itself is more resistant to standard chemo-treatments than the epithelial cancer cells. Moreover, only a weak chemotherapeutic response was observed in direct co-cultures of cancer cells with CAFs. A change in the morphology of direct co-cultures was accompanied with the chemoresistance. CAFs were observed to build cage-like structures around agglomerates of tumor cells. High levels of IL-6 were also associated with a reduced response to therapy. Indirect co-cultures make the tumor–stromal interaction more complex. Conclusions: CAFs are highly chemoresistant. Direct cell–cell contact and high levels of IL-6 correlate with a high chemoresistance.

Crohn's disease environmental factors in the developing world: A case-control study in a statewide catchment area in Brazil

To identify environmental risk factors associated with the development of Crohn’s disease (CD) in order to re-assess the hygiene hypothesis. METHODSA hospital-based, case-control study was carried out with CD patients (n = 145) and controls (n = 163) representing a socioeconomically diverse statewide catchment area in Brazil. Controls were recruited from caregivers of patients seen in different outpatient clinics at the same hospital. A multi-item survey with 94 questions regarding family history of CD, perinatal and childhood circumstances, living conditions, tobacco use and familial socioeconomic status was carried out by interviewers. RESULTSOn the univariate analysis, predictive variables for CD included being male, under age of 40, a high education level, urban dweller, smaller family size, exposure to enteric pathogens and user of treated water (P < 0.005). On the multivariate analysis, variables significantly associated with CD were male gender (OR = 2.09), under age 40 (OR = 3.10), white (OR = 2.32), from a small family in childhood (OR = 2.34) and adulthood (OR = 3.02), absence of viral infections in childhood (OR = 2.23), exposure to enteric pathogens (OR = 2.41), having had an appendectomy (OR = 2.47) and prior or current smoker (OR = 2.83/1.12). CONCLUSIONMost variables supporting the “hygiene hypothesis” are associated with the development of CD but are not independent predictors of the diagnosis.

Experiences in total knee arthtroplasty after distal femoral varus osteotomy

Introduction: There is only little information available about total knee arthroplasty (TKA) following distal femoral varus osteotomy (DFVO). The aim of our study was to show our experiences and mid-term results of TKA after a previous DFVO. Material and method: In a retrospective study we identified 36 consecutive patients who had undergone TKA after a previous distal femoral varus osteotomy. The average duration of follow-up after the TKA was 8.2 years (min: 5.0, max: 9.2). X-rays were taken in 2 planes before TKA, 1 week after TKA and at latest follow-up. Tibiofemoral alignment was measured on weightbearing long-leg anteroposterior radiographs. Ra- diolucent lines at latest follow-up were documented. Functional evaluations were performed preoperatively and postoperatively (at the time of latest follow-up). Results: The mean Knee Society knee score in- creased from 42 points before the arthroplasty to 91.3 points after the arthroplasty. The mean Knee Society function score increased from 27.4 points preoperatively to 93.2 points postoperatively. The mean overall Knee Society score increased from 91.3 points preoperatively to 163.4 points postoperatively. The mean radiographic alignment was 4.5? of valgus (10? of varus to 19? of valgus) before TKA and 3.1? of valgus (range, 3? of varus to 6? of valgus) at the time of latest follow-up. Postoperative complications included one deep vein thrombosis with non-lethal pulmonary embolism, one wound infection requiring revision and one septic loosening. Discussion: It is possible to perform TKA following DFVO with good mid-term results. In comparison to the literature there is no higher risk of complications in TKA following DFVO in comparison to primary TKA.

Sinus Lift versus Short Implants: Results of a Prospective Radiologic Study and Critical Review of Minimally Invasive Transcrestal Sinus Lift Procedures

Introduction: The current discussion about the use of short implants to avoid bone-augmentation (“sinus lift”) in the lateral maxilla remains a controversial topic and is increasingly at odds with the reality of evolutionary biology. Aim of the study was to determine the percentage of cases from a large routine patient-sample in which short implants might be suitable to avoid sinus lift procedures. Materials and Methods: From January 2012 to June 2015, all patients in three general dental practices in Austria with at least one subantral edentulous area were subjected to routine panoramic X-ray screening. The subantral alveolar ridge heights and the mesial extension of the maxillary sinus towards the canine fossa were measured. Statistics were performed by Excel data analysis (mean value, standard deviation). Results: 2837 patients were screened with 2837 panoramic radiographs presenting 3528 edentulous subantral regions and the subantral bone heights of 5674 maxillary sinuses were surveyed. 57.43% revealed subantral alveolar ridge heights of 4 mm or less;24.43% of all measure-points indicated a maximum alveolar ridge height of 6 mm. In 39.32% of cases, the pneumatisation of the maxillary sinuses with a subantral residual ridge height of 6 mm or less extended as far as the anatomical position of the second premolars, in 20.51% as far as the position of the first premolar and in 10.84% as far as the canine fossa. Discussion: The sinus lift procedure will continue to be one of the basic standard surgical procedures carried out by practice-based dental surgeons who perform implant surgery since in at least two thirds of the cases short implants with lengths of less than 6 mm cannot be applied. Preference should be given to sinus lift-procedures, which can be learned safely with a minimum of time-effort, least risk of failure and lowest possible level of patient morbidity. Genotoxicity testing for radon exposure： Dolichopoda （Orthoptera, Rhaphidophoridae） as potential bio-indicator of confined environments

Radon represents the major source of natural radioactivity in confined environments. Despite the clear evidence of a direct association between residential exposure and human lung cancer provided by case-control studies, results relating indoor exposure and genotoxic/mutagenic effect induction are still contradictory. The present study attempts to estimate the genotoxic effects induced by exposure to radioactive radon in wild cricket populations sampled from caves where varying concentrations of radon are present. Cave crickets are also tested as possible bio-indicator organisms of genotoxic potential of contaminated residential and confined environments. Six caves in Central Italy are considered covering a broad spectrum of radon radioactivity concentration （221-26,000 Bq/m3）. Dolichopoda specimens were sampled from each cave; both haemocytes and brain cells taken from individuals were tested for responsiveness to DNA damage induced by radon through the Comet assay. Specimens from the least radioactive cave, housed in controlled conditions for 60 days before analysis, were used as control group. Statistically significant increase of DNA damage was found in all groups of individuals from each cave, for both cell types. Very low values of all Comet parameters were found in control group individuals, which gave indications of a good responsiveness of the organism to the variable environmental levels of radioactive contamination. Results indicate that cave crickets represent a reliable tool for the detection of genotoxic potential induced by radioactive contamination of confined environments and can be proposed as a possible bio-indicator system for air （-radioactive） pollution related to indoor exposure [Current Zoology 60 （2）： 299-307, 2014].

Imaging of skeletal muscle in vitamin D deficiency

Elderly people are prone to accidental falls and one of the main risk factor is considered muscle weakness. Several studies focused on muscle weakness and muscle morphology changes in the elderly that may be associated with vitamin D deficiency. The prevalence of vitamin D deficiency is higher than previously though representing an important issue for public health and prevention. There is an increased interest in vitamin D effects in skeletal muscle and imaging modalities are particularly involved in this field. In patients with vitamin D deficiency, ultrasound, computed tomography, densitometry and magnetic resonance imaging(MRI) can efficiently describe changes in muscle morphology and size. Moreover, new imaging modalities, such as MRI spectroscopy, may improve knowledge about the metabolic effects of vitamin D in skeletal muscle. In this narrative review we will discuss the role of skeletal muscle imaging in vitamin D-deficient individuals. The aim of this paper is to improve and encourage the role of radiologists in this field.

Role of lenalidomide in the treatment of peripheral T-cell non-Hodgkin lymphomas

Structural Analysis Programme Based on the Equivalent Column＇s Method

The focus of this article is to accomplish a program using MatLab software, which will determine the global behavior of the structure, the stress, stability and frequency of the tall building with ＂n＂ levels. The program starts with the global analyses based on the equivalent column theory. The central core consists of reinforced concrete shear walls of the elevator＇s tubes and of the staircase, design to resist the lateral loads. Applying the equivalent column＇ s theory, the bracing system will be replaced by an equivalent column, considered to be a vertical cantilever fixed at the base and the loads will be applied in the shear center axis. The program using mathematical and physical relations, has as input dates the geometrical and stiffness characteristics of the plan of the building and as output dates the critical load, the fundamental frequency, the maximum stresses and deformations of the structure.

Optimum Design of Outrigger and Belt Truss Systems Using Genetic Algorithm

There are many structural lateral systems used in tall buildings： rigid frames, braced frames, shear walls, tubular structures and core structures. The outrigger and belt truss systems are efficient structures for drift control and base moment reduction in tall buildings where the core alone is not rigid enough to resist lateral loads. Perimeter columns are mobilized for increasing the effective width of the structure, and they developed tension in the windward columns and compression in the leeward columns. Optimum locations for the outriggers have been studied because of the influence on the top displacement and base moment in the core. It was analyzed the optimal position for two to seven outriggers and belt trusses, aiming to achieve minimum bending moment and minimum drift.
