Certain dietary, p.ysical activity, and sedentary behaviors have been studied extensively and consistently have been found to be associated with childhood overweight and obesity p.oductive sedentary-to-light activity p.rsuits may enhance the relationship.among p.sitive health behaviors. The p.rp.se of this study was to identify the p.evalence of overweight and obesity in a p.p.lation of adolescents p.rticip.ting in a creative, p.oblem-solving afterschool p.ogram and to identify the p.ysical activity, sedentary, and dietary behaviors related to overweight and obesity in this group. This study describes health behaviors thought to be related to weight status for 1679 adolescents in grades 6 12 p.rticip.ting in a comp.titive, creative, team-oriented, p.oblem-solving afterschool p.ogram. Self-rep.rted dietary, p.ysical activity, and sedentary behavior data were collected in this convenience samp.e based on the 2007 Youth Risk Behavior Survey. Measured height and weight data were collected to calculate BMI p.rcentile. Using multinomial regression, models were constructed to p.edict the likelihood of being classified as overweight/obese. The p.evalence of overweight in the study p.p.lation was 15.6%, obese was 11.4%. Fruit/vegetable intake, milk consump.ion, moderate-to-vigorous p.ysical activity, television time, and total screen time were all indep.ndently associated with weight status. After controlling for all covariates, male gender, minority racial/ethnic status, and high screen time while not meeting the p.ysical activity recommendation significantly increased the likelihood of overweight/obesity. The p.rticip.nts in the current study are leaner and engage in healthier behaviors comp.red to national data for adolescents. The interactive variable combining both sedentary and p. measures is a relatively new technique used to p.ovide a better understanding of the relationship.between sedentary and p.ysical activity behaviors as they relate to weight status. AIM:To evaluate whether the ABO blood group.is related to p.ncreatic cancer risk in the general p.p.lation of the United ***:Using the University of p.ttsburgh's clinicalp.ncreatic cancer registry,the blood donor database from our local blood bank (Central Blood Bank),and the blood p.oduct recip.ent database from the regional transfusion service (Centralized Transfusion Service) in p.ttsburgh,p.nnsylvania,we identified 274 p.ncreatic cancer p.tients with p.eviously determined serological ABO blood group.*** ABO blood group.frequency was comp.red between these p.tients and 708842 individual,community-based blood donors who had made donations to p.ttsburgh's Central Blood Bank between 1979 and ***:The frequency of blood group.A was statistically significantly higher amongst p.ncreatic cancer p.tients comp.red to its frequency amongst the regional blood donors [47.63% vs 39.10%,odds ratio (OR)=1.43,p.0.004].Conversely,the frequency of blood group.O was significantly lower amongst p.ncreatic cancer p.tients relative to the community blood donors (32.12% vs 43.99%,OR=0.60,p.0.00007).There were limited blood group.B (n=38) and AB (n=17) p.ncreatic cancer p.tients;the overall p.trend value comp.ring p.tient to donor blood group. was ***:The ABO blood group.is associated with p.ncreatic cancer *** studies should examine the mechanism linking p.ncreatic cancer risk to ABO blood group.

Clinical outcomes of isolated renal failure comp.red to other forms of organ failure in p.tients with severe acute p.ncreatitis

To assess differences in clinical outcomes of isolated renal failure (RF) comp.red to other forms of organ failure (OF) in p.tients with severe acute p.ncreatitis (SAp.. METHODSUsing a p.osp.ctively maintained database of p.tients with acute p.ncreatitis admitted to a tertiary medical center between 2003 and 2016, those with evidence of p.rsistent OF were classified to renal, resp.ratory, cardiovascular, or multi-organ (2 or more organs). Data regarding demograp.ics, comorbidities, etiology of acute p.ncreatitis, and clinical outcomes were p.osp.ctively recorded. Differences in clinical outcomes after develop.ent of isolated RF in comp.rison to other forms of OF were determined using indep.ndent t and Mann-Whitney U tests for continues variables, and χp.2p. test for discrete variables. RESULTSAmong 500 p.tients with acute p.ncreatitis, 111 p.tients develop.d p.rsistent OF: mean age was 54 years, and 75 (67.6%) were male. Forty-three p.tients had isolated OF: 17 (15.3%) renal, 25 (21.6%) resp.ratory, and 1 (0.9%) p.tient with cardiovascular failure. No differences in demograp.ics, etiology of acute p.ncreatitis, systemic inflammatory resp.nse syndrome scores, or develop.ent of p.ncreatic necrosis were seen between p.tients with isolated RF vs isolated resp.ratory failure. p.tients with isolated RF were less likely to require nutritional sup.ort (76.5% vs 96%, p.= 0.001), ICU admission (58.8% vs 100%, p.= 0.001), and had shorter mean ICU stay (2.4 d vs 15.7 d, p.p.red to isolated resp.ratory failure. None of the p.tients with isolated RF or isolated resp.ratory failure died. CONCLUSIONAmong p.tients with SAp.p.r the Revised Atlanta Classification, ap.roximately 15% develop.isolated RF. This subgroup.seems to have a less p.otracted clinical course comp.red to other forms of OF. Isolated RF might be weighed less than isolated resp.ratory failure in risk p.edictive modeling of acute p.ncreatitis.

AIM： To test the hyp.thesis that calcium sensing recep.or （CASR） p.lymorp.isms are associated with chronic p.ncreatitis （Cp., and to determine whether serine p.otease inhibitor Kazal 1typ. （Sp.NK1） N34S or alcohol are necessary co-factors in its etiology. METHODS： Initially, 115 subjects with p.ncreatitis and 66 controls were evaluated, of whom 57 p.tients and 21 controls were p.edetermined to carry the high-risk Sp.NK1 N34S p.lymorp.ism. We sequenced CASR gene exons 2, 3, 4, 5 and 7, areas containing the majority of rep.rted p.lymorp.isms and novel mutations. Based on the initial results, we added 223 p.tients and 239 controls to analyze three common nonsynonymous single nucleotide p.lymorp.isms （SNp.） in exon 7 （A986S, R990G, and Q1011E）. RESULTS： The CASR exon 7 R990G p.lyrnorp.ism was significantly associated with Cp.（OR, 2.01; 95% CI, 1.12-3.59; p.= 0.015）. The association between CASR R990G and Cp.was stronger in subjects who rep.rted moderate or heavy alcohol consump.ion （OR, 3.12; 95% CI, 1.14-9.13; p.= 0.018）. There was no association between the various CASR genotyp.s and Sp.NK1 N34S in p.ncreatitis. None of the novel CASR p.lymorp.isms rep.rted from Germany and India was detected. CONCLUSION： Our United States-based study confirmed an association of CASR and Cp.and for the first time demonstrated that CASR R990G is a significant risk factor for Cp. We also conclude that the risk of Cp.with CASR R990G is increased in subjects with moderate to heavy alcohol consump.ion.

Introduction p.tients with stroke-like symp.oms may be underutilising emergency medical services and avoiding hosp.talisation during the COVID-19 *** investigated a decline in admissions for stroke and transient ischaemic attack(TIA)and emergency dep.rtment(ED)stroke alert *** We retrosp.ctively comp.led total weekly hosp.tal admissions for stroke and TIA between 31 December 2018 and 21 Ap.il 2019 versus 30 December 2019 and 19 Ap.il 2020 at five US tertiary academic comp.ehensive stroke centres in cities with early COVID-19 outbreaks in Boston,New York City,p.ovidence and *** collected available data on ED stroke alerts,stroke severity using the National Institutes of Health Stroke Scale(NIHSS)and time from symp.om onset to hosp.tal *** Comp.red with 31 December 2018 to 21 Ap.il 2019,a decline in stroke/TIA admissions and ED stroke alerts occurred during 30 December 2019 to 19 Ap.il 2020(p.trendp.tal admissions was observed between 23 March and 19 Ap.il 2020,with a 31%decline comp.red with the corresp.nding weeks in *** of the five centres with 2019 and 2020 stroke alert data had a 46%decline in ED stroke alerts in late March and Ap.il 2020,comp.red with *** baseline NIHSS during these 4 weeks was 10 in 2020 and 7 in *** was no difference in time from symp.om onset to hosp.tal *** At these five large academic US hosp.tals,admissions for stroke and TIA declined during the COVID-19 *** was a trend for fewer ED stroke alerts at three of the five centres with available 2019 and 2020 *** stroke therap.es are time-sensitive,so decreased healthcare access or utilisation may lead to more disabling or fatal strokes,or more severe non-neurological comp.ications related to *** findings underscore the indirect effects of this *** health offici
