COVID-19 and HIV Treatment Interruption: A Case Study of the AIDS Support Organization (TASO) Mbale Clinic

Introduction: COVID-19 pandemic caught many HIV programs completely unprepared, leading to massive interruptions in HIV treatment. Fear and anxiety caused by another infectious and potentially deadly virus kept many PLHIV away from accessing ART services. Besides, the COVID-19 control measures imposed by the Government of Uganda, such as restrictions on movement due to the ban on both private and public transport, shortened travel hours due to the curfew imposed from 7 p.m. local time and limited resources at The AIDS Support Organization (TASO) Mbale clinic further frustrated access to ART services. The actual burden of treatment interruption in Uganda remains unclear. As such, this paper provides the magnitude of treatment interruption in TASO Mbale clinic during the April-June 2020 quarter—the COVID-19 pandemic peak period in Uganda. Methodology: We analyzed secondary and routine program data for all PLHIV on scheduled appointment in the quarters of January-March 2020 and April 2020-June 2020. We abstracted data from Uganda Electronic Medical Records (EMR) and linked with that from TASO Management Information system to make one dataset. This was then exported for final analysis in STATA version 15. Results: Out of 6744 PLHIV scheduled on appointment during April-June 2020 quarter, 1710 (25.3%) individuals missed their appointments, with the facility-based clients more affected than community-based (56.1% vs 43.9%) (p < 0.001), individuals with up-to-date viral load (VL) status were less likely to miss their appointment (p < 0.001) while sex was not associated with missed appointment (p-value = 0.269). Overall, there was a 356% increase in rate of missed appointment compared with that of January-March 2020 quarter (25.3% vs 7.1%) before COVID-19 pandemic hit Uganda. Moreover, PLHIV who received six-month’s drug refills were less likely to miss their appointment (p < 0.001) compared to those who received less.

Interventions to Improve HIV Viral Load Suppression among the Adolescents: Evidence of Improvement Science through a Quality Improvement Approach in Eastern Uganda

Introduction: Achieving viral load suppression among the adolescents living with HIV continues to hold back attainment of sustainable development goals. TASO Mbale realized a viral load suppression rate of 63.1% among the adolescents living with HIV in care in quarter 4 of 2016. We therefore, instituted a quality imrpovement project to improve Viral load suppression from 63.1% in quarter 4 2016 to 90% by the end of quarter 4 2017. Method: Baseline data from the Uganda viral load dashboard were analyzed, and fishbone diagram was utilized to provide root causes of low viral load suppression among the adolescents living with HIV at TASO Mbale. The identified barriers were Knowlegde gap, among the adolescents, on positive living, Missing clinic appointments, Sub-optimal adherence, Poorly planned adolescent HIV clinic, Inadequate follow-up and Low use of data for informed decisions. A plan-do-study-act (PDSA) model was applied to implement tested changes. Strategies that worked included introduction of appointment register to track appointment behaviour of the adolescents, generating lists of clients on appointment who were due for Viral Load bleeding, telephone calls for follow up, increasing the frequency of reviewing adolescents from once a month to twice a week, committing a dedicated team responsible for adolescent care. Results: The viral load suppression improved from 63.1% in quarter 4 of 2016 to 63.8% in the first quarter of 2017, to 87.5% in quarter 2 of 2017, 97.6% in the third quarter and 91.4% in quarter 4 of 2017. Conclusion: The use of quality improvement in addressing gaps in HIV service delivery is highly effective.

Iron deficiency(ID)and ID anemia(IDA)pose significant public health concerns in *** iron sucrose(IS)treatment is well-established in the country,ferric carboxymaltose(FCM)offers the advantage of higher doses and fewer *** open label,randomized,controlled,non-inferiority trial was conducted at multiple sites in China to compare the outcomes of FCM(maximum of 2 doses,500 or 1000 mg iron)and IS(up to 11 infusions,200 mg iron)treatments in subjects with *** primary endpoint was the achievement of hemoglobin(Hb)response(an increase of⩾2 g/dL from baseline)within 8 weeks,whereas secondary endpoints included changes in Hb,transferrin saturation,and serum ferritin *** the 371 randomized subjects,a similar percentage of subjects treated with FCM and IS achieved Hb-response(FCM 99.4%,IS 98.3%),thereby confirming the non-inferiority of FCM compared with IS(difference 1.12(−2.15,4.71;95%confidence interval(CI))).Furthermore,a significantly higher proportion of FCM-treated subjects achieved early Hb-response at Week 2(FCM 85.2%,IS 73.2%;difference 12.1(3.31,20.65;95%CI)).Additionally,the increase in TSAT and serum ferritin levels from baseline was significantly greater at all time points for FCM-treated *** safety profiles of FCM and IS were comparable,with the exception of transient hypophosphatemia and pyrexia,which are consistent with FCM’s known safety *** conclusion,FCM proves to be an efficacious treatment for IDA,providing faster Hb-response and correction of ID with fewer administrations than IS.

Perikinetics and sludge study for the decontamination of petroleum produced water(PW) using novel mucuna seed extract

In this study, Mucuna flagellipes seed extract was applied in the coagulation-flocculation of produced water （PW）. Process parameters such as pH, dosage, and settling time were investigated. Process kinetics was also studied. Instrumental characterization of mucuna seed （MS）, mucuna seed coagulant （MSC）, and post effluent treatment settled sludge （PTSS） were carried out. The optimum decontamination efficiency of 95 % was obtained at 1 g/L MSC dosage, PW pH of 2, and rate constant of 0.0001 （L/g/s）. Characterization results indicated that MS, MSC, and PTSS were of network structure, primitive lat- tice, and thermally stable. It could be concluded that MSC would be potential biomass for the treatment of produced water under the experimental conditions.

The RNA-binding pentatricopeptide repeat(PPR)family comprises hundreds to thousands of genes in most plants,but only a few dozen in algae,indicating massive gene expansions during land plant *** nature and timing of these expansions has not been well defined due to the sparse sequence data available from early-diverging land plant *** this study,we exploit the comprehensive OneKP datasets of over 1000 transcriptomes from diverse plants and algae toward establishing a clear picture of the evolution of this massive gene family,focusing on the proteins typically associated with RNA editing,which show the most spectacular variation in numbers and domain composition across the plant *** characterize over 2250000 PPR motifs in over 400000 *** lycophytes,polypod ferns,and hornworts,nearly 10%of expressed protein-coding genes encode putative PPR editing factors,whereas they are absent from algae and complex-thalloid *** show that rather than a single expansion,most land plant lineages with high numbers of editing factors have continued to generate novel sequence *** identify sequence variations that imply functional differences between PPR proteins in seed plants versus non-seed plants and variations we propose to be linked to seed-plant-specific editing ***,using the sequence variations across the datasets,we develop a structural model of the catalytic DYW domain associated with C-to-U editing and identify a clade of unique DYW variants that are strong candidates as U-to-C RNA-editing factors,given their phylogenetic distribution and sequence characteristics.
