The development of maintenance hemodialysis(HD)for end stage kidney disease patients is a success story that continues to save many ***,intermittent renal replacement therapy is also a source of recurrent stress for *** thrice weekly short HD is an imperfect treatment that only partially corrects uremic abnormalities,increases cardiovascular risk,and exacerbates disease *** cycles of fluid loading associated with cardiac stretching(interdialytic phase)and then fluid unloading(intradialytic phase)likely contribute to cardiac and vascular *** unphysiologic treatment profile combined with cyclic disturbances including osmotic and electrolytic shifts may contribute to morbidity in dialysis patients and augment the health burdenof treatment. As such, HD patients are exposed to multiple stressors including cardiocirculatory,inflammatory, biologic, hypoxemic, and nutritional. This cascade of events can be termed thedialysis stress storm and sickness syndrome. mitigating cardiovascular risk and morbidityassociated with conventional intermittent HD appears to be a priority for improving patientexperience and reducing disease burden. In this in-depth review, we summarize the hidden effectsof intermittent HD therapy, and call for action to improve delivered HD and develop treatmentschedules that are better tolerated and associated with fewer adverse effects.

Convergence of random zeros on complex manifolds

We show that the zeros of random sequences of Gaussian systems of polynomials of in- creasing degree almost surely converge to the expected limit distribution under very general hypotheses. In particular,the normalized distribution of zeros of systems of m polynomials of degree N,orthonor- malized on a regular compact set K(?)C^m,almost surely converge to the equilibrium measure on K as N→∞.

Effect of Bimodal Grain Size Distribution on the Strain Hardening Behavior of a medium-Entropy Alloy

The evolution of strain hardening behavior of the Fe_(50)(CoCrmnNi)_(50) medium-entropy alloy as a function of the fraction of recrystallized microstructure and the grain size was studied using the Hollomon and Ludwigson *** specimens under study were partially recrystallized,fully recrystallized with ultrafine-grained microstructure,and fully recrystallized with coarse *** yield strength decreases steadily as the fraction of recry stallized micro structure and grain size increases due to the recovery process and the Hall-Petch ***,the bimodal grain distribution was found to have a significant impact on strain hardening during plastic *** instance,the highest ultimate tensile strength was exhibited by a 0.97 μm specimen,which was observed to contain a bimodal grain ***,using the Ludwigson equation,the effect of the bimodal grain distribution was established from the behavior of K_(2) and n_(1) *** curves tend to show very high values in the specimens with a bimodal grain distribution compared to those that show a homogenous grain ***,the bimodal grain distribution contributes to the extensive Luders strain observed in the 0.97 μm specimen,which induces a significant deviation of the Hollomon equation at lower strains.

The classification of ATP-binding cassette subfamily A member 3 mutations using the cystic fibrosis transmembrane conductance regulator classification system

Importance: The ATP-binding cassette subfamily A member 3 (ABCA3) protein plays a vital role in surfactant homeostasis. mutations in the ABCA3 gene lead to the development of interstitial lung disease. In the most severe manifestation, mutations can lead to a fatal respiratory distress syndrome in neonates. ABCA3 belongs to the same ATP-binding cassette transporter superfamily as the cystic fibrosis transmembrane conductance regulator (CFTR), the gene that causes cystic fibrosis. Objective: To classify ABCA3 mutations in a manner similar to CFTR mutations in order to take advantage of recent advances in therapeutics. methods: Sequence homology between the CFTR protein and the ABCA3 protein was established. The region of CFTR that is a target for the new potentiator class of drugs was of particular interest. We performed a literature search to obtain all published mutations that were thought to be disease causing. We classified these mutations using the established CFTR classification system. When possible, we drew on previous experimental classification of ABCA3 mutations. Results: Although the proteins share the same overall structure, only a 19％identity was established between CFTR and ABCA3. The CFTR therapeutic target region has a 22％ homology with the corresponding ABCA3 region. Totally 233 unique protein mutations were identified. All protein mutations were classified and mapped to a schematic diagram of the ABCA3 protein. Interpretation: This new classification system for ABCA3, based on CFTR classification, will likely aid further research of clinical outcomes and identification of mutation-tailored therapeutics, with the aim for improving clinical care for patients with ABCA3 mutations.

Inhibition of mitochondrial fatty acid oxidation in drug-induced hepatic steatosis

mitochondrial fatty acid oxidation(mtFAO)is a key metabolic pathway required for energy production in the liver,in particular during periods of *** major consequence of drug-induced impairment of mtFAO is hepatic steatosis,which is characterized by an accumulation of triglycerides and other lipid species,such as ***,the severity of this liver lesion is dependent on the residual mitochondrial b-oxidation ***,a severe inhibition of mtFAO leads to microvesicular steatosis,hypoglycemia and liver *** contrast,moderate impairment of mtFAO can cause macrovacuolar steatosis,which is a benign lesion in the short *** some drugs can induce both microvesicular and macrovacuolar steatosis,it is surmised that severe mitochondrial dysfunction could be favored in some patients by non-genetic factors(e.g.,high doses and polymedication),or genetic predispositions involving genes that encode proteins playing directly or indirectly a role in the mtFAO *** of drugs inducing steatosis include acetaminophen(APAP),amiodarone,ibuprofen,linezolid,nucleoside reverse transcriptase inhibitors,such as stavudine and didanosine,perhexiline,tamoxifen,tetracyclines,troglitazone and valproic *** several previous articles reviewed in depth the mechanism(s)whereby most of these drugs are able to inhibit mtFAO and induce steatosis,the present review is rather focused on APAP,linezolid and *** steatogenic drugs are indeed rarely discussed in the literature as regards their ability to impair mtFAO.
