A feasible modified biopsy method for tissue diagnosis of gastric subepithelial tumors

AIM: To evaluate the diagnostic yield and safety of a modified technique for the histological diagnosis of subepithelial tumors (SETs). METHODS: A retrospective review of patients who underwent a modified technique for the histological diagnosis of gastric SETs, consisting of a mucosal incision with a fixed flexible snare (MIF) and deep-tissue biopsy under conventional endoscopic view, from January 2012 to January 2013 was performed. Eleven patients with gastric SETs 10-30 mm in diameter and originating from the third or fourth layer on endoscopic ultrasonography were included. RESULTS: The mean age was 59.8 (range, 45-76) years, and 5 patients were male. The mean size of the SETs was 21.8 (range, 11-30) mm. The number of biopsy specimens was 6.3 (range 5-8). The mean procedure time was 9.0 min (range, 4-17 min). The diagnostic yield of MIF biopsies was 90.9% (10/11). The histological diagnoses were leiomyoma (4/11, 36.4%), aberrant pancreas (3/11, 27.3%), gastrointestinal stromal tumors (2/11, 18.2%), an inflammatory fibrinoid tumor (1/11, 9.1%); one result was non-diagnostic (1/11, 9.1%). There were six mesenchymal tumors; the specimens obtained in each case were sufficient for an immunohistochemical diagnosis. There was no major bleeding, but one perforation occurred that was successfully controlled by endoscopic clipping. CONCLUSION: The MIF biopsy was simple to perform, safe, and required a shorter procedure time, with a high diagnostic yield for small SETs.

Biopathologic features and clinical significance of micrometatasis in the lymph node of early gastric cancer

AIM: To evaluate the biopathologic features and clinical significance of nodal micrometastasis(MI) in early gastric cancer(EGC).METHODS: Among 1022 EGC patients who underwent gastrectomy with lymphadenectomy of D1 + β or more from March 2001 to December 2005 at the Korean National Cancer Center, available nodal metastasis was found in 90 p T1N1 patients. Nodal metastasis was confirmed by immunohistochemistry(IHC) with cytokeratin and patients were classified into MI and macrometastasis(MA) groups based on the main tumor burden according to the 6th International Union Against Cancer/American Joint Committee on Cancer staging system; the main tumor burden with a diameter of greater than 0.2 mm but no greater than 2 mm as MI, and greater than 2 mm as MA of the representative metastatic node. Proliferative and apoptotic activities of the primary tumor and the nodal metastasis were measured by IHC with Ki-67 and terminal deoxynucleotidyl transferase d UTP nick end labeling, respectively. Biopathologic and clinical features of the patients were analyzed and compared between MI and MA groups. Patients with recurrence were compared with those without recurrence to identify risk factors for ***: Thirty-seven patients showed MI and the other 53 patients revealed MA in the lymph node; the incidence of patients with MI and MA was 41.1% and 58.9%. The main tumor burden was 0.9 and 4.6 mm in the representative metastatic node, respectively. Japanese N2 stations were more frequently involved in MA group(20.9%) than in MI group(10.3%) butthe difference was not statistically different(P = 0.338). Proliferative and apoptotic activities of MI were decreased than those of MA(26.7% vs 40.5%, P = 0.004 and 1.0% vs 3.0%, P < 0.001, respectively). However, nodal MI in the current study showed a relatively high proliferative activity and an equivalent apoptotic activity compared to other cancers in the previously published studies. 通过分析植物组合发酵物发酵前后的氨基酸、有机酸含量变化和对胶原蛋白的合成提高率的影响,并对一款含有该植物组合发酵物的精华露采用测定受试志愿者使用前后皮肤含水量、皮肤平滑度和光泽度等客观指标,结合问卷调查收集受试者使用感受等主观指标进行了人体功效评价。结果表明,植物组合发酵物与发酵前比较,氨基酸和有机酸的含量增加,同时胶原蛋白合成率提高。含有该植物组合发酵物的精华露能显著提高皮肤含水量,改善皮肤平滑度和光泽度。使用4周后97.2%的受试者对该产品整体效果表示满意,实验过程中无受试者出现不良反应。研究结果显示,该款产品具有一定的皮肤改善效果。

目的研究实验感染旋毛虫(韩国分离株)的大鼠对成虫和肌期幼虫阶段感染的保护性免疫和IgG,IgG1,IgG2a抗体反应。方法46只大鼠随机分为7组,其中2组(A1、A2组,共10只)用于观察成虫阶段引起的保护性免疫,B组(B1、B2组,共14只)用于观察肌期幼虫阶段引起的保护性免疫,C组(C1、C2组,共17只)为感染对照组,D组(5只)为正常对照组。A、B和C组分别每鼠感染1000条旋毛虫肌幼虫,分别于感染后第7天(A1、A2组)和第30天(B1、B2组),用氟苯咪唑治疗(20mg/kg,10d)。治疗后第10天,A和B组每鼠再次感染500条旋毛虫肌幼虫,于感染后第7天剖杀A1和B1组大鼠,检测肠道内成虫数,于感染后第30天剖杀A2和B2组大鼠,检测横膈膜内肌期幼虫数,同时分别剖杀感染对照组和正常对照组大鼠。每组同期取血,ELISA检测特异性IgG、IgG1和IgG2a抗体水平。结果旋毛虫成虫阶段对成虫和肌幼虫的保护性免疫分别为100%和99.96%,肌幼虫阶段对成虫和肌幼虫的保护性免疫分别为99.92%和99.89%。肌幼虫感染阶段抗肌幼虫分泌排泄抗原的特异性IgG、IgG1和IgG2a抗体与正常对照组(分别为0.5、0.1和0.1)和成虫感染阶段(分别为0.5、0.09和0.09)比较,抗体反应均显著增高(分别为3.0、2.2和0.8)(P<0.01)。且幼虫期抗肌幼虫分泌排泄抗原特异性IgG1抗体(2.2)显著高于特异性IgG2a抗体(0.8)(P<0.01)。结论旋毛虫的成虫和肌幼虫阶段的感染均对成虫和幼虫的再感染产生保护性免疫。

AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,***: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. 通过原生质体融合技术得到侧耳菌亲和与非亲和菌种的融合产物，亲和菌种间体细胞杂交得到异核体，而非亲和菌种杂交得到保留有双亲基因的结合体（核）。结合核子实体发育受光照、温度、营养及培养基的物理状态等因素影响。结合体不能形成初始融合菌落，但在多数成熟的子实体中存在锁状联合，与异核体相比，结合体具生长速率缓慢，有丝分裂阶段形成锁状联合及子实体发育不同步等特点。体细胞杂交后代可以分离到四种遗传型菌落：原养型、一种亲本营养缺陷型、另一亲本营养缺陷型及重组子营养缺陷型。一些结合体具备异常等位基因，说明在受体细胞染色体重排中有大片段随机整合位点。

Background: The nutritional support after hematopoietic stem cell transplantation (HSCT) has not been well established due to the scarcity of clinical trials. To conduct international clinical trials in Asia, we performed the questionnaire survey to investigate the current standard of nutritional support after HSCT. Method: We sent the questionnaire to the physicians nominated by the Asia Pacific Blood and Marrow Transplantation (APBMT) members of each country/ region. Result: We received 15 responses from 7 different countries/regions. The target calorie amount is 1.0 - 1.3 × basal energy expenditure (BEE) in 11 institutes when partial parenteral nutrition is used. When total parenteral nutrition (TPN) is used, the target calorie amount is 1.0 - 1.3 × BEE in 9 institutes and 1.3 - 1.5 × BEE in 4 institutes. Lipid emulsion is routinely used in 12 institutes. Multivitamins and trace elements are routinely added to TPN used in most institutes. It is still uncommon to use the immunonutrition. Blood glucose levels are routinely monitored in all institutes, but the target range varies (<110 in 2 institutes, <150 in 4 institutes, and <200 in 8 institutes). Conclusions: Basic nutritional support is similar in participating institutes. However, the target glucose level varies and the use of immunonutrition is rather rare. These points can be the theme of future clinical trials.

自动时报漏刻(自击宫漏)是宫中都工匠护军蒋英实奉世宗大王(李祹,1397—1450年;1418—1450年在位)之命于1433年制造的。从1434年7月1日开始它成为国家的标准计时仪器。对民众报时的午正、人定和罢漏就是根据这套计时仪器报的。它安装在景福宫的报漏阁里,因此后称报漏阁漏.景福宫是国家天文台"简仪台"所在地。它能够通过操控声音和视觉信号发生系统依朝鲜时代的二元化时制——即十二时辰和五更点法——自动报时。系统自动发出报时声同时显示指示,报出十二时、百刻,以及五更时刻。世宗大王创制的报漏阁自击漏成为模型在朝鲜(1392—1910年)经历过3次复制或修复——1433年的初创,1536年的再创以及1618年的重创。初创之后574年,2004年为了纪念韩国国立古宫博物馆在首尔开馆,文化财厅,博物馆与建国大学校和自击漏硏究会签订了自击漏复原计划,到2007年11月展示了复原的自击漏。复原计划是按照《世宗实录.报漏阁记》中的记载以及原器物的一些残余部分——包括1536年和1618年仪器的现存部分——进行的。用了一年半的时间复制漏器和自击装置。文章从机械学和功能方面的细节系统地描述了复原的过程,包括根据当时的教范《漏筹通义》的记载刻制十一支箭.《漏筹通义》是根据授时历和经过汉阳(当时朝鲜首都)地方时校正的《七政算内篇》编纂的。自击漏包括4个主要部分:(1)漏器部,通过安排三级播水壶,用于两个受水壶递水的计时壶来计时;(2)方木机构,通过在相应的时间落下相应的小铜丸将水平面转换到以数字显示的二元报时时刻标示信号;(3)铁丸放出机构,由复杂的杠杆系统操纵的机构放大动力报时机来操作。(4)视听觉报时部,由两个次一级结构组成。一个是十二时辰报时机部,包括一个钟和木制的小雕像系统,以不同次数的敲钟声和相应的木制的十二支神小雕像举出的时辰牌来报时。另一个是更点机,包括一面鼓、一面锣、以及小雕像,小雕像在不同的更点敲击出相应次数的锣鼓声以报更点。我们对已经失传几个世纪的铁丸作动人形机构的详细复制表明,蒋英实受到了阿拉伯铁丸利用报时法的启发,发明了丸落下时刻标时法(通过落丸来标示时刻的机械装置),并通过改进13世纪阿拉伯时计师al-Jazari的仪器来实现将动力传到对人形机构的推动中。另一方面,苏颂的《新仪象法要》,《元史》中描述的郭守敬的大明殿灯漏以及顺帝的宫漏可能影响了世宗朝的宫廷学者。总的来说,这次复原源于对当时的文献记载和遗物的重建,中国-阿拉伯钟表技术对我们也很有帮助。全部系统现在运行良好。我们的复原将有助于重建古代东亚、甚至阿拉伯的机械水钟。

＜正＞Twenty-one patients with primary high-grade advanced gastric cancer in whom the presence of stage ? was con- firmed by preoperative diagnostic imaging,were treated with a wide-spectrum regimen,designated as FEPMTX therapy to establish an effective salvage *** therapy consisted of 5-fluorouracil（5-FU）and the triple biochemical modulators;methotrexate（MTX）,calcium leucovorin（LV）,cisplatin（CDDP）,and in addition of epiru- bicin（EPI）.The schedule comprised 3 days continuous administration of 5-FU（350 mg/m2/day）and MTX（35 mg/ m2）on day 1,LV（30 mg/m2）on the day 2,LV（30 mg/m2）and CDDP（30 mg/m2）and EPI（20 mg/m2）on day *** treatment was performed two or more courses with 2 weeks interval in *** partial responses（PR）, five no changes（NC）and five progressive diseases（PD）were obtained,and the response rate was 52%.Ten patients （PR 7,NC 2,PD 1）were received gastrectomy（resectability rate 48%）.The survival in all cases resulted in median survival of 184 days and one-year survival rate of 23%.While there is no significant difference insurvival between the cases with or without the resection of the primary lesion,the survival of responders was significantly longer than that of non-responders（median survival time;responder 356days,non-responder 152days）.Adverse effects such as myelo- suppression and anorexia and fatigue sometimes occurred,but were mild and all the patients well *** therapy is thought to be an effective regimen for neoadjuvant chemotherapy with longer survival and little toxic for patients with stage?gastric cancer.
