Empowerment in chronic wound care——exploring the scope for patient contribution

Objective:In this study,we investigated the concept of empowerment in chronic wound care and propose to facilitate patient control by making use of degrees of freedom(DOF):that is,shaping of everyday wound care tasks initiated by patients and based on their wishes,mostly in terms of patients executing treatment steps,requesting or directing health care professionals to under take changes,or modifications of internal ***:As a first step,we conducted a systematic literature search,followed by an inductive form of qualitative content analysis,which resulted in the identification of 5 dimensions as main elements of empowerment:education and shared decision making,adherence to self-care behaviors,responsibility and control,general call for empowerment,and ***,the latter are noticeably absent in the *** investigate patients'freedom in shaping the wound care process,we conducted a second literature ***:A number of possibilities for patients to influence the wound care process could be identified,but experimental or clinical evidence about their effects is missing,their variety is limited,and they are only inadequately ***:however,DOF should be an indispensable aspect of genuine empowerment,since they allow patients to occupy the role of the agent in the treatment process and give rise to the subjective experience of feeling ***,in the third part,we develop a research proposal on how to investigate and include DOF in the clinical practice of wound ***,limitations about implementations are discussed(e.g.,patients being reluctant to overcome their passive role,resulting in frustration for health care professionals).

Are there new approaches for diagnosis, therapy guidance and outcome prediction of sepsis?

Beside many efforts to improve outcome, sepsis is still one of the most frequent causes of death in critically ill patients. It is the most common condition with high mortality in intensive care units. The complexity of the septic syndrome comprises immunological aspects- i.e.,sepsis induced immunosuppression- but is not restricted to this fact in modern concepts. So far, exact mechanisms and variables determining outcome and mortality stay unclear. Since there is no typical risk profile, early diagnosis and risk stratification remain difficult, which hinders rapid and effective treatment initiation. Due to the heterogeneous nature of sepsis, potential therapy options should be adapted to the individual. Biomarkers like C-reactive protein and procalcitonin are routinely used as complementary tools in clinical decision-making. Beyond the acute phase proteins, a wide bunch of promising substances and non-laboratory tools with potential diagnostic and prognostic value is under intensive investigation. So far, clinical decision just based on biomarker assessment is not yet feasible. however, biomarkers should be considered as a complementary approach.

The idea of using ultrashort X-ray pulses to obtain images of single proteins frozen in time has fascinated and inspired *** was one of the arguments for building X-ray free-electron *** to theory,the extremely intense pulses provide sufficient signal to dispense with using crystals as an amplifier,and the ultrashort pulse duration permits capturing the diffraction data before the sample inevitably *** was first demonstrated on biological samples a decade ago on the giant *** then,a large collaboration has been pushing the limit of the smallest sample that can be *** ability to capture snapshots on the timescale of atomic vibrations,while keeping the sample at room temperature,may allow probing the entire conformational phase space of *** we show the first observation of an X-ray diffraction pattern from a single protein,that of Escherichia coli GroEL which at 14 nm in diameter is the smallest biological sample ever imaged by X-rays,and demonstrate that the concept of diffraction before destruction extends to single *** the pattern,it is possible to determine the approximate orientation of the *** experiment demonstrates the feasibility of ultrafast imaging of single proteins,opening the way to single-molecule time-resolved studies on the femtosecond timescale.

基于2019—2020年京津冀地区不同天气系统影响下的降水过程,采用交叉相关法和光流法对快速更新多尺度分析和预报综合集成系统(Rapid-refresh Multi-Scale Analysis and Prediction System-Integration,RMAPS_IN)的降水分析产品进行0~2 h临近外推预报的批量试验。结果表明:由交叉相关法和光流法计算的两种外推矢量在大小和方向上存在一定差异,直接差异与影响降水的天气系统位置有明显的对应关系,而方向差异受地理位置的影响更明显,台风类降水呈弧形带状分布,低槽冷锋类、低涡类、气旋类、暖切变线类等几类降水均呈西北大东南小的特点;预报效果方面,总体上交叉相关法优于光流法,尤其是预报时效超过30 min以后,各种降水类型的批量检验结果显示交叉相关法的预报评分优于光流法,且预报时效越长、优势越明显,但预报时效为10 min时,光流法在低涡类、台风类、暖切变线类的空报率上优于交叉相关法。此外,基于外推的临近预报方法对京津冀地区台风类降水的预报效果最好,其次为暖切变线类、低涡类、低槽冷锋类、气旋类。

Disorder-enhanced nuclear spin relaxation at Landau level filling factor one

The nuclear spin relaxation rate （l/T1） is measured for GaAs two-dimensional （2D） electron systems in the quantum hall regime with an all-electrical technique for agitating and probing the nuclear spins. A ＂tilted plateau＂ feature is observed near the Landau level filling factor v = 1 in 1/T1 versus v. Both the width and magnitude of the plateau increase with decreasing electron density. At low temperatures, lIT1 exhibits an Arrhenius temperature dependence within the tilted plateau regime. The extracted energy gaps are up to two orders of magnitude smaller than the corresponding charge transport gaps. These results point to a nontrivial mechanism for the disorder-enhanced nuclear spin relaxation, in which microscopic inhomogeneities play a key role for the low energy spin excitations related to skyrmions.

The Plastid-Localized NAD-Dependent Malate Dehydrogenase Is Crucial for Energy homeostasis in Developing Arabidopsis thaliana Seeds

In the absence of photosynthesis, ATP is imported into chloroplasts and non-green plastids by ATP/ADP transporters or formed during glycolysis, the latter requiring continuous regeneration of NAD＋, supplied by the plastidial isoform of NAD-MDh. During screening for T-DNA insertion mutants in the plNAD-MDh gene of Arabidopsis, only heterozygous plants could be isolated and homozygous knockout mutants grew only after complementation. These heterozygous plants show higher transcript levels of an alternative NAD＋-regenerating enzyme, NADh-GOGAT, and, remarkably, improved growth when ammonium is the sole N-source. In situ hybridization and GUS-histochemical stain- ing revealed that plNAD-MDh was particularly abundant in male and female gametophytes. Knockout plNAD-MDh pollen exhibit impaired tube growth in vitro, which can be overcome by adding the substrates of NADh-GOGAT. In vivo, knockout pollen is able to fertilize the egg cell. Young siliques of selfed heterozygous plants contain both green and white seeds corresponding to wild-type/heterozygous （green） and homozygous knockout mutants （white） in a （1：2）：1 ratio. Embryos of the homozygous knockout seeds only reached the globular stage, did not green, and developed to tiny wrinkled seeds. Complementation with the gene under the native promoter rescued this defect, and all seeds developed as wild-type. This suggests that a blocked major physiological process in plNAD-MDh mutants stops both embryo and endosperm development, thus avoiding assimilate investment in compromised offspring.

Objectives:To test for differences in overall and recurrence-free survival between laparoscopic and open surgical approaches in patients undergoing radical nephroureterectomy(RNU)for upper tract urothelial carcinoma(UTUC).Materials and methods:We retrospectively identified patients treated for UTUC from 2010 to 2020 from our institutional *** undergoing laparoscopic or open RNU with no suspicion of metastasis(cM0)were for the current study *** with suspected metastases at diagnosis(cM1)or those undergoing other surgical treatments were *** was performed according to the laparoscopic versus open surgical ***-Meier plots were used to test for differences in overall and recurrence-free survival with regard to the surgical ***,separate Kaplan-Meier plots were used to test the effect of preoperative ureterorenoscopy on overall and recurrence-free survival within the overall study ***:Of the 59 patients who underwent nephroureterectomy,29%(n=17)underwent laparoscopic nephroureterectomy,whereas 71%(n=42)underwent open *** and tumor characteristics were comparable between groups(p≥0.2).The median overall survival was 93 and 73 months in the laparoscopic nephroureterectomy group compared to the open nephroureterectomy group(p=0.5),*** median recurrence-free survival did not differ between open and laparoscopic nephroureterectomies(73 months for both groups;p=0.9).Furthermore,the median overall and recurrence-free survival rates did not differ between patients treated with and without preoperative ***:The results of this retrospective,single-center institution showed that overall and recurrence-free survival rates did not differ between patients with UTUC treated with laparoscopic and open ***,preoperative ureterorenoscopy before RNU was not associated with higher overall or recurrence-free survival rates.

First determination of the highly genotoxic fungal contaminant altertoxin II in a naturally infested apple sample

Altertoxin II is an epoxide-holding perylene quinone and secondary metabolite produced by Alternaria *** genotoxic potential and the high concentrations determined in laboratory fungal cultures resulted in an increased scientific interest in this emerging ***,no natural occurrence in food has been reported to date to the best of our ***,we unambiguously identified altertoxin II in a naturally infected,unprocessed *** a validated LC-MS/MS method,fruit peel spots(approx.1 mm in diameter),typically associated with Alternaria infestation,were tested for 17 mycotoxins produced by this *** concentrations of tentoxin were determined 24 h after harvest,and the dibenzopyrones alternariol,alternariol monomethyl ether(AME)and AME-3-sulfate after six months of ***,these contamination patterns can be considered unlikely to be of toxicological concern for ***,an inconspicuous peel lesion(0.5×1 cm)separately sampled from a stored apple,exhibited considerably high concentrations of 14 Alternaria toxins,including dibenzopyrones,their barely studied modified forms(glucosides and sulfates),as well as genotoxic perylene quinones including altertoxin *** the toxicological potential of this compound,contaminations in such infected fruit parts should not be underestimated,*** the case of exposure through the consumption of fresh smoothies or fruit juices.
