Liver resections can be performed safely without Pringle maneuver: A prospective study

AIm To evaluate liver resections without Pringle maneuver, i.e., clamping of the portal triad. mETHODS Between 9/2002 and 7/2013, 175 consecutive liver resections(n = 101 major anatomical and n = 74 large atypical > 5 cm) without Pringle maneuver were performed in 127 patients(143 surgeries). Accompanying, 37 wedge resections(specimens m) and 43 radiofrequency ablations were performed. Preoperative volumetric calculation of the liver remnant preceeded all anatomical resections. The liver parenchyma was dissected by waterjet. The median central venous pressure was 4 mmH g(range: 5-14). Data was collected prospectively. RESULTS The median age of patients was 60 years(range: 16-85). Preoperative chemotherapy was used in 70 cases(49.0%). Liver cirrhosis was present in 6.3%, and liver steatosis of ≥ 10% in 28.0%. Blood loss was median 400 mL(range 50-5000 mL). Perioperative blood transfusions were given in 22/143 procedures(15%). The median weight of anatomically resected liver specimens was 525 g(range: 51-1850 g). One patient died postoperatively. Biliary leakages(n = 5) were treated conservatively. Temporary liver failure occurred in two *** major liver resections without Pringle maneuver are feasible and safe. The avoidance of liver inflow clamping might reduce liver damage and failure, and shorten the hospital stay.

Laparoscopic colorectal surgery(LCS) is an evolving *** studies show that LCS can not only offer safe surgery but evidence is growing that this new technique can be superior to classical open *** perioperative complications and faster postoperative recovery are regularly mentioned when studies of LCS are *** though the learning curve of LCS is frequently debated when limitations of laparoscopic surgeries are reviewed,studies show that in experienced hands LCS can be a safe procedure for colorectal cancer *** learning curve however,is associated with high conversion rates and economical aspects such as higher costs and prolonged hospital ***,laparoscopic colorectal cancer surgery(LCCR) offers several advantages such as less co-morbidity and less postoperative pain in comparison with open ***,the good exposure of the pelvic cavity by laparoscopy and the magnification of anatomical structures seem to facilitate pelvic dissection ***,recent studies describe no difference in safety and oncological radicalness in LCCR compared to the open total mesorectal excision(TmE).The oncological adequacy of LCCR still remains unproven today,because long-term results do not yet *** date,only a few studies have described the results of laparoscopic TmE combined with preoperative adjuvant treatment for colorectal *** aim of this review is to examine the various areas of development and controversy of LCCR in comparison to the conventional open approach.

Wet-cupping therapy(WCT)is one of the oldest known medical techniques,used as a traditional and complementary therapy with a wide application all around the world for general *** on the effects of WCT on sports performance are sparse and ***,we aimed to explore the effects of WCT on repeated sprint ability,wellness,and exertion in young active ***-nine active adult males(age:[28±5]years;body height[177±8]cm;body mass:[79±7]kg;body mass index:[25.4±1.8]kg/m^(2))were selected for the *** participants performed a running-based sprint test on two separate occasions(Control and Post-WCT).WCT was performed 24 h before the testing *** completed the Hooper questionnaire to assess their well-being(i.e.,sleep,stress,fatigue,and soreness)before each *** rating of perceived exertion(RPE)was collected after each testing session.A higher maximum power(pmean power(pminimum power(pmpared to Control session along with a better perceived sleep(pmpared to the Control *** present findings demonstrated that WCT moderately enhanced repeated sprint ability and had positive effects on perceived sleep,stress,and *** may be an effective ergogenic aid to improve repeated sprint ability and general wellbeing in young adult *** large-scale multicentric clinical studies are paramount to confirm the results of our study.

Endogenous neurogenesis can arise from a variety of physiological stimuli including exercise, learning, or ＂enriched environment＂ as well as pathological conditions such as ischemia, epilepsy or cortical spreading depression. Whether all these conditions use a common trigger to set off endogenous neurogenesis is yet unclear. We hypothesized that cortical spreading depression（CSD） induces neurogenesis in the cerebral cortex and dentate gyrus after cerebral venous ischemia. Forty-two Wistar rats alternatively underwent sham operation（Sham）, induction of ten CSDs or venous ischemia provoked via occlusion of two adjacent superficial cortical vein followed by ten induced CSDs（CSD ＋ 2-VO）. As an additional control, 15 na？ve rats received no intervention except 5-bromo-2′-deoxyuridine（Brd U） treatment for 7 days. Sagittal brain slices（40 μm thick） were co-stained for Brd U and doublecortin（DCX; new immature neuronal cells） on day 9 or Neu N（new mature neuronal cells） on day 28. On day 9 after sham operation, cell proliferation and neurogenesis occurred in the cortex in rats. The sole induction of CSD had no effect. But on days 9 and 28, more proliferating cells and newly formed neurons in the ipsilateral cortex were observed in rats subjected to CSD ＋ 2VO than in rats subjected to sham operation. On days 9 and 28, cell proliferation and neurogenesis in the ipsilateral dentate gyrus was increased in sham-operated rats than in na？ve rats. Our data supports the hypothesis that induced cortical neurogenesis after CSD ＋ 2-VO is a direct effect of ischemia, rather than of CSD alone.

First,we would like to congratulate Liu et *** their successful multicenter study including 1,032 patients undergoing robotic pancreaticoduodenectomy(RPD)and 1,154 open pancreaticoduodenectomy(OPD)in seven high volume centers in China over an 8 year period(1).To ensure the best possible outcomes of both techniques,RPD was performed by five surgeons from three centers after overcoming the individual learning curve that was defined as having performed>40 *** the other hand,OPD was performed by 9 surgeons from 6 centers after reaching the individual learning curve,defined as an experience of>60 OPD.
