Sustained elevation of parathyroid hormone (PTH) levels is not uncommon post renal transplantation. Though in the majority of patients, it gradually normalizes, on average 5% of transplanted patients require parathyroidectomy (PTX). However, PTX itself has been associated with deterioration in allograft function and even completes graft loss seen with both total and subtotal PTX as well as an increased rate of acute rejection. The aim of this study was to determine the effect of post transplant partial PTX on allograft function in our patients as well as the incidence of acute rejection. Our results show that post transplantation, subtotal PTX, was successful in achieving metabolic control while preserving graft function without an increased incidence of acute rejection. Retention of sufficient residual parathyroid tissue with partial PTX might account for favorable outcome in our study. Despite this, surgery for advanced hyperparathyroidism should optimally take place in the pre-transplant period.

In this review article, various preferences in breast plastic surgery particularly after breast cancer will be discussed in view of the diverse indications for the different construction procedures. The various conditions that necessitate the need for reconstruction are appraised, the important reconstructive procedures are discussed. The most important indications procedures discussed in this review include;Prophylactic subcutaneous mastectomy, Lumpectomy and Radiation, modified mastectomy with Axillary Sampling, Nipple reconstruction, and the contralateral breast. These procedures are discussed in view of plastic surgeon practice and patients acceptability in Saudi Arabia. Data from Saudi Arabia in particular was identified through searches of the EmBASE, and mEDLINE database, using the keywords: Saudi Arabia, breast plastic Surgery, breast reconstruction, autologous breast reconstruction, breast augmentation. Advances in prosthetic technologies and modifications in autologous flap techniques, and the development of novel tissue alternatives have allowed for sustained developments in breast reconstruction results. A variety of attitudes has been accessible for addressing the difficulties that endure after resection of breast cancer in Saudi Arabia. Patients should be educated to accept different process in this context.

Revision total hip arthroplasty: An analysis of the quality and readability of information on the internet

The demand for revision total hip arthroplasty(THA)is *** quality on the internet has been extensively analysed in relation to primary THA but no such analysis has ever been performed for revision *** aim was to assess the quality and readability of this *** major internet search engines were searched for information on revision *** websites were assessed for quality of information using the DISCERN score,the Journal of the American medical Association benchmark criteria and a novel scoring system specific to revision THA[Vancouver Revision Arthroplasty Information(VRAI)score].Website readability was assessed,as was presence of the Health On the Net Foundation(HON)*** majority of websites(52%)were academic with a post-graduate reading *** 6.5%of websites had the HON *** percent of websites had a‘good’DISCERN score and only 28%had a‘good’score with the novel VRAI scoring *** information websites had significantly higher rates of‘good’VRAI scores(P=0.008).Websites with the HON seal had significantly higher DISCERN scores(P=0.01).all governmental websites were at a reading level suitable for patient *** on the internet relating to revision THA is of low quality,much lower than the quality of information on primary *** recommend governmental websites for their readability and health information websites for their quality of information specific to revision *** with the HON seal provide higher quality information and should be recommended to patients as reading material regarding revision THA.

The aim of this research was to explore the energy benefits and future potential of using Building Integrated Photovoltaic (BIPV) and Electrochromic Glazing (EG) within the climatic conditions of the city of Abu Dhabi. The Integrated Environmental Solutions (IES-VE) energy modeling software was used to assess the energy performance, mainly the reductions in HVAC and lighting, for different configurations and compare that to the base case scenario for south, east, west, and north facing facades. The results showed that the BIPV is most advantageous on the south fa?ade while the EC glazing performs best on the north facing windows. moreover, the change in sensor location increased the energy savings for both cases, although the change was very marginal compared to the change of the glass properties. Using an automated light control system with dimming for both models, compared against the standard on-off lighting mechanism in the base case, the BIPV proves to have a higher total annual energy saving potential for most orientations, upto 33.5% while dynamic EC was best suited for the North orientation resulting in 7.4% reduction in the total annual energy consumption.

Phishing,an Internet fraudwhere individuals are deceived into revealing critical personal and account information,poses a significant risk to both consumers and web-based *** indicates a persistent rise in phishing ***,these fraudulent schemes are progressively becoming more intricate,thereby rendering them more challenging to ***,it is imperative to utilize sophisticated algorithms to address this *** learning is a highly effective approach for identifying and uncovering these harmful *** learning(mL)approaches can identify common characteristics in most phishing *** this paper,we propose an ensemble approach and compare it with six machine learning techniques to determine the type of website and whether it is normal or not based on two phishing *** that,we used the normalization technique on the dataset to transform the range of all the features into the same *** findings of this paper for all algorithms are as follows in the first dataset based on accuracy,precision,recall,and F1-score,respectively:Decision Tree(DT)(0.964,0.961,0.976,0.968),Random Forest(RF)(0.970,0.964,0.984,0.974),Gradient Boosting(GB)(0.960,0.959,0.971,0.965),XGBoost(XGB)(0.973,0.976,0.976,0.976),AdaBoost(0.934,0.934,0.950,0.942),multi Layer Perceptron(mLP)(0.970,0.971,0.976,0.974)and Voting(0.978,0.975,0.987,0.981).So,the Voting classifier gave the best *** in the second dataset,all the algorithms gave the same results in four evaluation metrics,which indicates that each of them can effectively accomplish the prediction ***,this approach outperformed the previous work in detecting phishing websites with high accuracy,a lower false negative rate,a shorter prediction time,and a lower false positive rate.

Indoor Surveillance of Airborne Fungi Contaminating Intensive Care Units and Operation Rooms in Assiut University Hospitals, Egypt

mycoflora of atmospheric air and dust samples collected from air conditioning systems in 12 of each I.C.U. （intensive care units） and O.R. （operation rooms） were tested using settle and dilution plate methods on four types of agar media and incubated at 25℃. Forty-five fungal species representing 23 genera were isolated and identified. The most prevalent genera recorded were Cladosporium, Aspergillus, Penicillium and Fusarium. The total colony forming units of airborne fungi recovered in I.C.U. and O.R. ranged between 31.13-49.61 colonies/m3 on the four types of media usedl The fungal total catch of the dust samples collected from the air conditioning system filters in I.C.U. and O.R. were ranged from 65.5-170 colonies/mg dust. Since, the interest to replace synthetic xenobiotics by natural compounds with low environmental persistence and biodegradable to control such airborne fungal contaminants is needed. In this respect, essential oils showed to possess a broad spectrum of antifungal activity. Fungal static ability of six oils was tested on 30 different fungal isolates. Vapors of common thyme oil exhibited the strongest inhibitory effects on the tested isolates, whereas the headspace vapors of blue gum and ginger had no inhibitory effects on the tested fungal isolates. These data revealed that the air conditioning systems may be an important source of contamination in I.C.U. and O.R. of Assiut university hospitals. Thus, patients may be in risk of being exposed to contaminated atmospheric air by opportunistic fungi and the use of essential oils as an alternative option to control hospital wards from fungal contaminants needs further studies.

In the present work, inverse thermal analysis of heat conduction is carried out to estimate the in-plane thermal conductivity of composites. Numerical simulations were performed to determine the optimal configuration of the heating system to ensure a unidirectional heat transfer in the composite sample. Composite plates made of unsaturated polyester resin and unidirectional glass fibers were fabricated by injection to validate the methodology. A heating and cooling cycle is applied at the bottom and top surfaces of the sample. The thermal conductivity can be deduced from transient temperature measurements given by thermocouples positioned at three chosen locations along the fibers direction. The inverse analysis algorithm is initiated by solving the direct problem defined by the one-dimensional transient heat conduction equation using a first estimate of thermal conductivity. The integral in time of the square distance between the measured and predicted values is the criterion minimized in the inverse analysis algorithm. Finally, the evolution of the in-plane composite thermal conductivity can be deduced from the experimental results by the rule of mixture.
