monosegment 4±1 Associating Liver Partition and Portal Vein ligation for Staged hepatectomy(alPPS)for bilateral colorectal liver metastases decreases the risk of drop-out of two stage hepatectomy,triggering a rapid and significant increase in future liver remnant(FLR)with promising oncological *** report two cases of segment 4+1 monosegment alPPS for multiple colorectal liver metastases performed at our *** the literature,seven similar cases have been *** and long term outcomes of our two patients were reported along with a review of data from the *** patients showed a FLR increase from 13%to 37%and from 14%to 41%of total liver volume,*** was compared to a median growth from 19%at baseline to 34%before stage 2,in the *** 20 and 27 months since resection both patients are alive and *** the literature,median overall survival and disease free survival were 13 months(range,5–24 months)and 5 months(range,3–23 months),*** 4±1 alPPS is associated with promising oncological outcomes and a significant FLR *** may be safely performed in selected patients as a salvage procedure,reducing the risk of the dropout of two-stage hepatectomy.

The Deep Neural Networks(DNN)training process is widely affected by backdoor *** backdoor attack is excellent at concealing its identity in the DNN by performing well on regular samples and displaying malicious behavior with data poisoning *** state-of-art backdoor attacks mainly follow a certain assumption that the trigger is sample-agnostic and different poisoned samples use the same *** overcome this problem,in this work we are creating a backdoor attack to check their strength to withstand complex defense strategies,and in order to achieve this objective,we are developing an improved Convolutional Neural Network(ICNN)model optimized using a Gradient-based Optimization(GBO)(ICNN-GBO)*** the ICNN-GBO model,we are injecting the triggers via a steganography and regularization *** are generating triggers using a single-pixel,irregular shape,and different *** performance of the proposed methodology is evaluated using different performance metrics such as Attack success rate,stealthiness,pollution index,anomaly index,entropy index,and *** the CNN-GBO model is trained with the poisoned dataset,it will map the malicious code to the target *** proposed scheme’s effectiveness is verified by the experiments conducted on both the benchmark datasets namely CIDAR-10 andmSCELEB 1m *** results demonstrate that the proposed methodology offers significant defense against the conventional backdoor attack detection frameworks such as STRIP and Neutral cleanse.

Abdominal cocoon,the idiopathic form of sclerosing encapsulating peritonitis,is a rare condition of unknown etiology that results in an intestinal obstruction due to total or partial encapsulation of the small bowel by a f ibrocollagenous *** diagnosis requires a high index of clinical *** early clinical features are nonspecif ic,are often not recognized and it is diff icult to make a def inite pre-operative *** suspicion may be generated by the recurrent episodes of small intestinal obstruction combined with relevant imaging f indings and lack of other plausible *** radiological diagnosis of abdominal cocoon may now be conf idently made on computed tomography *** is important in the management of this *** dissection and excision of the thick sac with the release of the small intestine leads to complete recovery in the vast majority of cases.

This research evaluated the efficiency of Atrium in hot,arid regions and investigated the parameters influencing the total energy consumption of an existing shopping mall in Dubai,UAE using the IES-VE energy modeling *** parameters tested included:Roof profile,Roof ventilation,Number of floors,glazing’s Light to Solar Gain ratio(LSG)and the Thermal insulation of the building *** impact of using light sensors was also *** light sensors and the roof’s thermal insulation resulted in the highest reduction of the atrium’s energy,8.5%and 7.5%,*** the number of floors reduced the energy consumption by 5%per unit *** roof profile,external wall’s thermal insulation and height of the atrium resulted in similar energy reductions of 3.5%*** external glazing’s LSG reduced the energy consumption by 3.25%while the roof ventilation resulted in a reduction of 1.3%.The internal glazing’s LSG did not affect the atrium’s total energy *** optimal combination of the parameters studies resulted in an energy reduction of about 20%.Thus,this paper presents the optimal design configuration to reduce the energy consumption of an atrium located in a hot and arid climate.

Objective:To evaluate the possible in vitro interaction between ethanolic extracts of Rhus coriaria[***)(seed),Sacropoterium spinosum(***)(seed),Rosa damascena(R. damascene)(flower) and certain known antimicrobial drugs including oxytetracycline HCl, penicillin C,cephalexin,sulfadimethoxine as sodium,and enr of *** synergy study was carried out against 3 clinical strains of multidrug-resistant Pseudomonas aeruginosa (***).methods:Evaluation of synergy interaction between plant extracts and antimicrobial agents was carried out using microdilution ***:The results of this study showed that there is a decrease in the mIC in case of combination of ethanolic plant extracts and test antimicrobial *** most interesting result was that the combination between R. coriaria and these antibiotics,showed a high decrease in minimum inhibitory concentration(mIC), and a strong bactericidal activity against these ***:These results may indicate that combinations between *** extract and these antibiotics could be useful in fighting emerging drug-resistance ***,which may due to that *** extract contain natural inhibitors working by different mechanisms or inhibiting efflux *** we have experiments underway leading to the identification of the active molecules present in ***,in vivo experiments are needed to confirm pseudomonal protection.

mounting awareness of climate change in recent years has led the construction industry to initiate new approaches toward sustainable building design, yet stakeholders in hospitality development stilt hesitate to build green. This reluctance is due to perceived high guest expectations of comfort levels and amenity access among these properties. Research was conducted to determine precisely which green building innovations present the most significant barriers to incorporating sustainability into design for hospitality. Average LEED credit imple- mentation rates among 28 existing LEED hotel projects were calculated and compared with average credits employed among common commercial building projects. 15 of those projects＇ designers also offered survey opinions on which sustainable innovations were most commonly avoided in their approaches for hospitality. The results indicate that certain credits do expe- rience decreased popularity among hospitality projects, yet guest comfort was not the only barrier identified. Cost of implementation and local applicability affected by climate and local bylaws were also found as major role players in the selection of credits specific to hospitality design. Conclusions are presented according to the data and recommendations made to support further growth and success in future applications of LEED sustainable design in hospitality.

Cervical cancer is an intrusive cancer that imitates various women around the world. Cervical cancer ranks in thefourth position because of the leading death cause in its premature stages. The cervix which is the lower end of thevagina that connects the uterus and vagina forms a cancerous tumor very slowly. This pre-mature cancerous tumorin the cervix is deadly if it cannot be detected in the early stages. So, in this delineated study, the proposed approachuses federated machine learning with numerous machine learning solvers for the prediction of cervical cancer totrain the weights with varying neurons empowered fuzzed techniques to align the neurons, Internet of medicalThings (IomT) to fetch data and blockchain technology for data privacy and models protection from hazardousattacks. The proposed approach achieves the highest cervical cancer prediction accuracy of 99.26% and a 0.74%misprediction rate. So, the proposed approach shows the best prediction results of cervical cancer in its early stageswith the help of patient clinical records, and all medical professionals will get beneficial diagnosing approachesfrom this study and detect cervical cancer in its early stages which reduce the overall death ratio of women due tocervical cancer.

Nasal flap preservation in endoscopic dacryocystorhinostomy for nasolacrimal duct obstruction

AIm:To compare surgical outcomes between the conventional endoscopic dacryocystorhinostomy(DCR)and a modified endoscopic DCR for the treatment of nasolacrimal duct obstruction(NLDO),and evaluate factors associated with the surgical success ***:medical records of patients who underwent primary DCR surgery between January 2016 and July 2020 at the Otorhinolaryngology Department of Eye and Ear International Hospital,Lebanon were ***:The study group consisted of 50 consecutive modified endoscopic DCR and the control group consisted of 138 consecutive conventional endoscopic *** success rates at 1y were 98.0%(49 out of 50)for modified DCR,significantly higher compared to 84.8%(117/138)for the conventional DCR;there was no significant difference in the success rate throughout the years in terms of both surgical *** modified surgery vs traditional[adjusted odds ratio(aOR)=14.96]and having an adjunctive septoplasty surgery vs not(aOR=3.99)were significantly associated with higher odds of ***:mucosal flap preservation and apposition shows significant improvement in the surgical success ***,there is no statistically significant difference found in terms of complication rate and mean operative time between the conventional and the modified techniques.
