Pathophysiology, epidemiology, classification and treatment options for polycystic liver diseases

Polycystic liver diseases(PLD)represent a group of genetic disorders in which cysts occur in the liver(autosomal dominant polycystic liver disease)or in combination with cysts in the kidneys(autosomal dominant polycystic kidney disease).Regardless of the genetic mutations,the natural history of these disorders is alike.The natural history of PLD is characterized by a continuous increase in the volume and the number of cysts.Both genders are affected;however,women have a higher prevalence.Most patients with PLD are asymptomatic and can be managed conservatively.Severe symptoms can affect 20%of patients who develop massive hepatomegaly with compression of the surrounding organs.Rrarely,patients with PLD suffer from acutecomplications caused by the torsion of hepatic cysts,intraluminal cystic hemorrhage and infections.The most common methods for the diagnosis of PLD are cross sectional imaging studies.Abdominal ultrasound and computerized tomography are the two most frequently used investigations.Magnetic resonance imaging is more sensitive and specific,and it is a valuable test for patients with intravenous contrast allergies or renal dysfunction.Different treatment modalities are available to physicians caring for these patients.Medical treatment has been ineffective.Percutaneous sclerotherapy,transarterial embolization,cyst fenestration,hepatic resection and liver transplantation are indicated to specific groups of patients and have to be tailored according to the extent of disease.This review outlines the current knowledge of the pathophysiology,clinical course,diagnosis and treatment strategies of PLD.

Measurement of the In-Plane Thermal Conductivity of Long Fiber Composites by Inverse Analysis

In the present work, inverse thermal analysis of heat conduction is carried out to estimate the in-plane thermal conductivity of composites. Numerical simulations were performed to determine the optimal configuration of the heating system to ensure a unidirectional heat transfer in the composite sample. Composite plates made of unsaturated polyester resin and unidirectional glass fibers were fabricated by injection to validate the methodology. A heating and cooling cycle is applied at the bottom and top surfaces of the sample. The thermal conductivity can be deduced from transient temperature measurements given by thermocouples positioned at three chosen locations along the fibers direction. The inverse analysis algorithm is initiated by solving the direct problem defined by the one-dimensional transient heat conduction equation using a first estimate of thermal conductivity. The integral in time of the square distance between the measured and predicted values is the criterion minimized in the inverse analysis algorithm. Finally, the evolution of the in-plane composite thermal conductivity can be deduced from the experimental results by the rule of mixture.

Clinical and epidemiological characteristics of norovirus gastroenteritis among hospitalized children in Lebanon

AIM To assess the burden of norovirus(No V) and to determine the diversity of circulating strains among hospitalized children in Lebanon. METHODS Stool samples were collected from children presenting with acute gastroenteritis to six major hospitals in Lebanon. A total of 739 eligible stool samples, testing negative for diarrhea caused by rotavirus as a possible viral pathogen, were collected between January 2011 and June 2013. A standardized questionnaire including demographic, epidemiological and clinical observations was used at the time of hospitalization of children presenting with diarrhea. Viral RNA was extracted from stool samples followed by reverse transcription polymerase chain reaction and nucleotide sequencing of a fragment of the viral protein 1 capsid gene. Multiple sequence alignments were carried out and phylogenetic trees were constructed using the MEGA 6 software.RESULTS Overall, 11.2% of stool samples collected from children aged < 5 years tested positive for No V genogroups Ⅰ(GⅠ) and Ⅱ(GⅡ). GⅡ accounted for 10.6% of the gastroenteritis cases with only five samples being positive for GⅠ(0.7%). The majority of hospitalized children showed symptoms of diarrhea, dehydration, vomiting and fever. Upon sequencing of positive samples and based on their clustering in the phylogenetic tree, 4/5 of GⅠ gastroenteritis cases were designated GⅠ.3 and one case as GⅠ.4. GⅡ.4 was predominantly detected in stool of our study participants(68%). We report a JB-15/KOR/2008 GⅡ.4 Apeldoorn 2008-like variant strain circulating in 2011; this strain was replaced between 2012 and 2013 by a variant sharing homology with the Sydney/NSW0514/2012/AUS GⅡ.4 Sydney 2012 and Sydney 2012/FRA GⅡ.4 strains. We also report the co-circulation of non-GⅡ.4 genotypes among hospitalized children. Applying Neural Network Architecture for Inverse Kinematics Problem in Robotics

One of the most important problems in robot kinematics and control is, finding the solution of Inverse Kinematics. Inverse kinematics computation has been one of the main problems in robotics research. As the Complexity of robot increases, obtaining the inverse kinematics is difficult and computationally expensive. Traditional methods such as geometric, iterative and algebraic are inadequate if the joint structure of the manipulator is more complex. As alternative approaches, neural networks and optimal search methods have been widely used for inverse kinematics modeling and control in robotics This paper proposes neural network architecture that consists of 6 sub-neural networks to solve the inverse kinematics problem for robotics manipulators with 2 or higher degrees of freedom. The neural networks utilized are multi-layered perceptron (MLP) with a back-propagation training algorithm. This approach will reduce the complexity of the algorithm and calculation (matrix inversion) faced when using the Inverse Geometric Models implementation (IGM) in robotics. The obtained results are presented and analyzed in order to prove the efficiency of the proposed approach.

Management of Left Ventricular Aneurysm: A Study from Iraq

Background: The most appropriate surgical approach for post-myocardial infarction left ventricular aneurysm (LVA) is controversial. This study aims to display the results of surgical treatment of LVA in a major Iraqi cardiac surgical center. Methods: The surgical management of LVAs over the period 2001 to 2011 was retrospectively reviewed. The presenting signs and symptoms, results of investigations, operative findings, and outcomes of patients were determined. Results: Twenty-seven true LVAs associated with 4 ventricular septal defects (VSDs) were treated surgically. During the same period, 1136 coronary artery bypass graft (CABG) operations were done, thus LVA represented 2.4%. Males constituted the majority (74.1%). The mean age was 54.6 years old. The typical ECG changes were seen in 42.1%. Apical and antero-apical locations predominated. The majority of patients (84.2%) had subnormal values of ejection fraction (EF). Most patients had multi-vessel coronary artery disease (CAD). The most frequent was the left anterior descending artery (LAD). All patients had CABG except 3. Linear repair and Dor technique were used equally. The commonest postoperative complication was bleeding (38.4%). The overall hospital mortality was 18.5%. Conclusion: Concomitant CABG improves early postoperative course and must be added when significant lesions in coronary arteries particularly the LAD are present.

Pancreatic cancer(PC)remains one of the most challenging diseases,with a very poor 5-year overall survival of around 11.5%.Kirsten rat sarcoma virus(KRAS)mutation is seen in 90%-95%of PC patients and plays an important role in cancer cell proliferation,differentiation,metabolism,and survival,making it an essential mutation for targeted therapy.Despite extensive efforts in studying this oncogene,there has been little success in finding a drug to target this pathway,labelling it for decades as“undruggable”.In this article we summarize some of the efforts made to target the KRAS pathway in PC,discuss the challenges,and shed light on promising clinical trials.

给出空气和燃料在3种初始混合模式（非预混、部分预混和完全预混）下无焰燃烧的实验结果，介绍了无焰燃烧的实现方法，研究当量比和初始混合模式对无焰燃烧的影响。实验炉在功率为7.5~15 kW下运行，当量比的变化范围为0.5~1。实验结果表明，无焰燃烧状态一旦建立则很稳定，很容易维持。无论是预混、非预混还是部分预混燃烧，其炉温差别不大，CO和NOx排放低。当量比接近1时，虽然炉内仍处于无焰燃烧状态且NOx排放量少，但CO和H2排放量都极多，此时并不是清洁燃烧。当量比小于0.98时，3种混合模式下烟气中的CO和H2含量都极低，但部分预混无焰燃烧产生的NOx排放量相对较多，而完全预混无焰燃烧的NOx排放量最低。文中还对当量比对NOx排放量的影响进行了详细分析。

Sleep breathing disorders in female population of Dubai, UAE

PURPOSE: United Arab Emirates (UAE) is part of the GCC countries and ranks 18 on 2007 WHO list of the fattest countries with 68.3% of its citizens with an unhealthy weight. The WHO data in 2008 state that the prevalence of overweight in UAE is 71%, while it is 61% in the UK, 54% in Germany, and 45% in France. The increasing overweight and obesity in the UAE are closely related to high social-economic development. Accordingly, the medical authority has observed that the rates of hypertension and diabetes mellitus type 2 appear to be one of the highest rates worldwide. We presume that the increase prevalence of obesity in the UAE would be linked to the increase in prevalence of obstructive sleep apnea syndrome (OSAS). The purpose of this study is to estimate the prevalence of symptoms and risk of OSAS in women followed up in the primary health care (PHC) setting in Dubai and the relationship between obesity and sleep apnea in females in the UAE. DESIGN: Prospective observational community-based survey. SETTING: 20 primary health centers in Dubai. PARTICI-PANTS: Consecutive female patients who were older than 14 years, regardless of the reason of their visit. METHODS: In this prospective survey, trained medical nurse administered the Berlin Questionnaire (which includes questions about self-reported snoring, witnessed apneas, daytime sleepiness, hypertension, and obesity) to a consecutive random sample of female patients in the age group older than 14 years, attending the PHC center in Dubai Health Authority, Dubai, UAE, from September 2011 to March 2012. Based on the questionnaire, individuals were classified into high-risk and low-risk groups for OSAS. RESULTS: Based on the responses and measurement of the Berlin Questionnaire of 704 female subjects studied, 137 respondents met the criteria for the high-risk scoring. This gives a prevalence rate of 19.5% while the remainders of the participants were classified as low risk. The overall mean age of the high risk for OSAS female respondents was 39.95 ± 11.73 years. A New Kinetic Spectrophotometric Method for Determination of Cefadroxil in Pharmaceutical Formulations Using <i>Lawsonia inermis</i>(Henna) as Natural Reagent

A new simple and sensitive kinetic spectrophotometric method has been proposed for the determination of cefadroxil in pure form and pharmaceutical formulations. The proposed method is based on the formation of yellow coloured product (λmax = 410 nm) that resulted after addition of the reagent, which is composed of a mixture of Henna extract, sodium hydroxide and potassium permanganate, to cefadroxil aqueous solution. The experimental conditions have been optimized. Beer’s law is obeyed over the concentrations of 100 - 35 μg/ml, with a linear regression correlation coefficient of 0.996. The proposed method has been successfully applied for the determination of cefadroxil in pharmaceutical dosage forms.
