Background: Retroperitoneal heamorrahge is an acute surgical condition that is associated with difficulty of diagnosis, organ injuries and mortality. Materials and Methods: One hundred and two patients with traumatic retroperitoneal hematoma treated in Al Yarmuk teaching hospital from May 2012 to January 2013 were reviewed retrospectively. The data include patient’s age, type of injury, presenting symptoms, associated injuries and common site of hematoma. Results: In 102 patients, 69 were males (67.6%) and 33 were females (32.3%), mean age of 22 years. The comments presenting symptoms was pain (85.2%), followed by tenderness (73.5%) and shock (58.8%). Around 38.2% were injured by blunt trauma and 61.8% by penetrating trauma. The large bowel was the most common affected organ 26.4%, spleen and kidneys were the second affected organ (23.5%) while the jejunum and ileum were the third associated organs to be affected (14.7%). The commonest complication was septicemia, followed by wound infection. The least reported complications were Pulmonary embolism, Fistula, and Intestinal Obstruction. Conclusion: Traumatic retroperitoneal hematoma is life-threatening condition, early diagnosis and correct treatment is of upmost importance.

Background: Laparoscopic cholecystectomy has become the standard treatment for symptomatic gall bladder disease. However, there still a substantial proportion of patients in whom Laparoscopic cholecystectomy cannot be successfully performed, and for whom conversion to open surgery is required. Method: In this study, 1600 laparoscopic cholecystectomy performed at Al karamah teaching hospital from January 2010 to January 2015, were prospectively analyzed. The patients studied included 1600, 1245 where females (78.4%) and 346 where males (21.6%) with a mean age of 41.2 years. From the data collected, only factors available to surgeon preoperatively were considered for analysis. These factors included: age, gender, history of acute Cholecystitis, jaundice, previous abdominal surgery, obesity and concomitant disease, ultrasound preoperatively & ERCP. Results: Of the 1600 patients in whom Laparoscopic cholecystectomy was attempted 56 patients (3.5%) required conversion to open surgery. The most common reason for conversion was difficult to define anatomy in patients with inflamed, contracted gall bladder (n = 42). Significant predictor factors for conversion were male gender, previous abdominal surgery, acute Cholecystitis, and obesity. Conclusion: An appreciation for these predictors for conversion will allow appropriate planning by the patient, the institution and the surgeon.

Efficiency of Tamsulosin after Shockwave Lithotripsy for the Treatment of Lower Ureteric Stone: Alkaramah Teaching Hospital Experience, Iraq

Background: Stone disease is a wide health problem;certain drugs have been used as supplement with ESWL for the treatment of symptomatic uncomplicated lower ureteric stone like Nefidipine, Alfuzosin and Tamsulosin. The purpose of this study is to evaluate the efficacy of Tamsulosin for the treatment of lower ureteric stone of 5 - 10 mm diameter after ESWL. Methods: A total of 105 patients with lower ureteric stone of less than 10 mm diameters were divided into two groups. Group 1 including 48 patients received 0.4 mg Tamsulosin and diclofenac sodium analgesia as needed immediately after ESWL and continued until stone expulsion was confirmed up to maximum of 30 days. Group 2 including 47 patients was given diclofenac sodium only as needed after ESWL. Result: 46 of 49 patients in group 1 and 44 of 48 patients in group 2 ultimately passed stones. The number of ESWL sessions was 1.4 ± 0.6 in group 1 & 1.42 ± 0.75 in group 2. There were 10 patients who needed analgesia after ESWL in group 1 and 17 patients in group 2. Visual analogue scale pain severity score was 5.32 ± 1.23 and 6.41 ± 1.26 in group 1 and 2, respectively. The time to stone expulsion in group 1 and 2 was 8 ± 5.4 days and 12 ± 10.3 days, respectively. No significant complications or side effects occurred. Conclusion: The use of Tamsulosin in combination with ESWL in patients with lower ureteric stone of 5 - 10 mm was effective for reducing the time to stone expulsion, decreasing need for analgesia, and facilitating stone passage without significant improvement in stone free rate.

Background: Extracorporeal shock wave lithotripsy (ESWL) became the first line in the treatment of ureteric stone after failure of conservative treatment because of its safety, simplicity and effectiveness. It is not invasive procedure and can be done on outpatient basis without anesthesia and with few complications which is most probably temporary and treatable. The objective of this study is to evaluate the efficiency and safety of ESWL in treatment of ureteric stone in Iraq. Materials and Methods: A total of 112 Iraqi patients with ureteric stones were participated in this prospective observational study in which patients scheduled for ESWL treatment for a period of 6 months. Patients were divided into 2 groups: 1) Group 1: 52 patients with proximal ureteric stone;2) Group 2: including 60 patients with distal ureteric stone. Preoperatively all patient underwent bowel preparation and were asked to fast for 8 hours before the procedure. Results: The age ranged between 22 and 55 with mean of 42 (SD = 5) years. Around 46% had proximal ureteric stone and the rest were in distal ureter. Around 44% needed one session and 40% needed two sessions to be stone-free respectively. In regards to associated symptoms, 74% had ureteric colic, 3% haematuria, 43% microhematuria and 12% UTI. Mild hydronephrosis was found in 90% of the cases and 30 reported had previous intervention. Success rate was 90%. Conclusions: ESWL is safe and effective in treatment of ureteric stone with few complications and must be regarded first choice after conservative treatment in a patient with uncomplicated ureteric stone.

The rate of breakage of drops and bubbles(fluid particles) in stirred systems is of great consequence on mass,heat, and momentum transport phenomena. Therefore, over the years, the topic has gained a considerable attention from the researchers to study and characterize this phenomenon. In present work, the experimental studies of breakage phenomenon in a stirred tank in the last four decades were reviewed. The review highlighted the investigated experimental conditions and the major findings concerning the breakage mechanism and the effect of operating conditions. The discrepancy and contradictions of the outcomes of those studies were specified and discussed. Experimental observations and conclusions concerning the breakage characterization parameters,such as deformation, breakage probability, breakage time, and breakage location were presented and commented on. Possible future refinements and prospective were recommended. The review indicated that there are clear discrepancies and contradictions between previous studies regarding the effect of some operating parameters and the values of breakage time, breakage probability, number of daughter particles, and breakage location relative to the impeller. In addition, there are still many scientific gaps that need to be studied and characterized in future by overcoming specific experimental difficulties and uncertainties.

This advanced paper presents a new approach to improving image steganography using the Ant Colony Optimization(ACO)*** steganography,a technique of embedding hidden information in digital photographs,should ideally achieve the dual purposes of maximum data hiding and maintenance of the integrity of the cover media so that it is least *** contemporary methods of steganography are at best a compromise between these *** this paper,we present our approach,entitled Ant Colony Optimization(ACO)-Least Significant Bit(LSB),which attempts to optimize the capacity in steganographic *** approach makes use of a grayscale cover image to hide the confidential data with an additional bit pair per byte,both for integrity verification and the file checksumof the secret *** approach encodes confidential information into four pairs of bits and embeds it within uncompressed grayscale *** ACO algorithm uses adaptive exploration to select some pixels,maximizing the capacity of data embedding whileminimizing the degradation of visual *** evaporation is introduced through iterations to avoid stagnation in solution *** levels of pheromone are modified to reinforce successful pixel *** results obtained through the ACO-LSB method reveal that it clearly improves image steganography capabilities by providing an increase of up to 30%in the embedding capacity compared with traditional approaches;the average Peak Signal to Noise Ratio(PSNR)is 40.5 dB with a Structural Index Similarity(SSIM)of *** approach also demonstrates very high resistance to detection,cutting down the rate by 20%.Implemented in MATLAB R2023a,the model was tested against one thousand publicly available grayscale images,thus providing robust evidence of its effectiveness.

The rapid movement of heat transferred goes to be the most important target for researchers to deals with the thermal problems of electronic ***,that has high packing densities in limited ***,vertical combination between two shapes of fins was considered to improve the thermal performance of heat ***,many models(longitudinal-pin fins)were created as a new approach based on the stable both of material and *** the analytical solution,Finite element techniques used to solve the mathematical modeling and signum-sinusoidal-signum function was modified to model both the variable heat transfer area and convection *** in the numerical procedure,ANSYS simulation used as the validation of temperature *** addition,results from previous work were used as second method of validation *** results show a high level of agreement by maximum difference does not exceed(3.52%).In the calculation process,natural convection,range of Ra number(105–107)were applied for all models of which each one is distinguished by the parameter of area ratio(A∗)between hybrid model and longitudinal *** results show many advantages;a significant drop in temperature profile about(2.7%–8.8%).Also,decrease in thermal resistance by(23%–43%)and increases in heat transferred(29%–78%).Furthermore,all parameters have a significant improve for the models that have A∗1.48 as a result of overlapping between the effects of miscellaneous *** that associated with the entropy generation was *** this step,it becomes important to find the model that achieves maximize of the thermal performance and minimize of the entropy *** that,the optimization procedure using genetic algorithm was ***'s clearly the best model exist within a close range between 1.509 and 1.518 based on the behavior of the best fitness function.

Beamforming in stand-alone Millimeter-Wave(mmWave)communications results in prolonged access times and latencies due to the increased number of measurements required to determine the optimal beam directions at the Mobile Station(MS)and Base Station(BS),returning the highest received signal ***,dynamic and fast access schemes that meet the Third-Generation Partnership Project(3GPP)specifications are required ***,in this paper,a novel initial access scheme is proposed for multiple MS users by leveraging for the first time a digital compass in the access ***,when a new MS joins the footprint of a BS,it probes the channel for beacon signaling about the BS direction,i.e.,broadcasted by a neighboring MS that completed beam association at previous time ***,a digital compass is utilized to adjust the coordinates of the BS according to the location of the new *** proposed scheme is applied for a single and multi-user settings at various broadcasting *** includes a single associated MS user that broadcasts information to a single incoming MS user,a single user that broadcasts signals to multiple incoming users,or all multiple associated users broadcast to multiple incoming ***,the proposed schemes yield in notable efficiency in terms of the computational complexity,access times,power and energy consumption as compared to existing access ***,high success rates are achieved at the detriment of relatively higher cost.

Date palm pit(DPP)-filled poly(-hydroxybutyrate)(PHB)composites were prepared,evaluated,and characterized to determine their thermal insulation *** conductivity values ranged between 0.086 and 0.100 W/(m·K).At a maximum filler concentration(50%(w)),the specific heat capacity and thermal diffusivity were 1183 J/(kg·K)and 0.0689 mm^(2)/s,*** DPP increased the thermal stability,and the highest compressive strength obtained was 80 MPa at 30%filler *** PHB-DPP composites exhibited promising water absorption(less than 6%)and tensile strength(6-14 MPa).Date-pit-based PHB composites could be used in sustainable building engineering and cleaner production.

In reality,the flipped classroom has gained popularity as a modern way of structuring teaching,where lectures move from in-class procedures to digitally-based assignments,freeing up the debate,and practice exercises class ***,it is essential to implement and analyze a way of teaching that will improve student *** paper aims to develop a model of the method of teaching science in Iraqi schools,and to assess whether teaching flipped classroom affects the achievement,motivation,and creative thinking of students by using the methodology of Multi-Criteria Decision Making(MCDM)in the Analytic Hierarchy Process(AHP).The AHP approach includes several steps,including setting assessment criteria and their weights,and by assessing the methodology of the flipped classroom as compared to the conventional cognitive learning *** experiment was carried out in Iraqi secondary schools to examine the attitude of the students towards the subject of *** findings have indicated that the students and teachers favored flipped classroom learning more than conventional cognitive *** study took the following parameters compared to the traditional approach:teaching techniques,learning flexibility,teaching aids effectiveness,student participation and working *** paper indicates that the teachers in Iraqi schools will be able to improve and do more preparation to shift towards flipped learning in the classroom.
