Intrapartum application of the continuous glucose monitoring system in pregnancies complicated with diabetes: A review and feasibility study

Intrapartum maternal normoglycemia seems to play an important role in the prevention of adverse perinatal, maternal and neonatal outcomes. Several glucose monitoring protocols have been developed, aiming to achieve a tight glucose monitoring and control. Depending on the type of diabetes and the optimal or suboptimal glycemic control, the treatment options include fasting status of the parturient, frequent monitoring of capillary blood glucose, intravenous dextrose infusion and subcutaneous or intravenous use of insulin. continuous glucose monitoring system(cGMS) is a relatively new technology that measures interstitial glucose at very short time intervals over a specifi c period of time. The resulting profi le provides a more comprehensive measure of glycemic excursions than intermittent home blood glucose monitoring. Results of studies applying the cGMS technology in patients with or without diabetes mellitus(DM) have revealed new insights in glucose metabolism. Moreover, cGMS have a potential role in the improvement of glycemic control during pregnancy and labor, which may lead to a decrease in perinatal morbidity and mortality. In conclusion, the use of cGMS, with its important technical advantages compared to the conventional way of monitoring, may lead into a more etiological intrapartum management of both the mother and her fetus/infant in pregnancies complicated with DM.

Structural Geology and chronology of Shear Zones along the Shangdan Suture in Qinling Orogenic Belt,china:Implications for Late Mesozoic Intra-continental Deformation of East Asia

The Shangdan suture zone(SDZ)in the Qinling orogenic belt(QOB)is a key to understanding the East Asia tectonic *** SDZ gives information about convergent processes between the North china Block(NcB)and South china Block(ScB).In the Late Mesozoic,several shear zones evolved along the SDZ boundary that helps us comprehend the collisional deformation between the NcB and ScB,which was neglected in previous *** shear zones play an essential role in the tectonic evolution of the East Asia *** study focuses on the deformation and geochronology of two shear zones distributed along the SDZ,identified in the Shaliangzi and Maanqiao *** shear sense indicators and kinematic vorticity numbers(0.54–0.90)suggest these shear zones have sinistral shear and sub-simple shear deformation *** quartz’s dynamic recrystallization and c-axis fabric analysis in the Maanqiao shear zone(MSZ)revealed that the MSZ experienced deformation under green-schist facies conditions at∼400–500℃.The Shaliangzi shear zone deformed under amphibolite facies at∼500–700℃.The^(40)Ar/^(39)Ar(muscovite-biotite)dating of samples provided a plateau age of 121–123 *** with previously published data,our results concluded that QOB was dominated by compressional tectonics during the Late Early ***,we suggested that the Siberian Block moved back to the south and Lhasa-Qiantang-Indochina Block to the north,which promoted intra-continental compressional tectonics.

consensus-based dispatch optimization of a microgrid considering meta-heuristic-based demand response scheduling and network packet loss characterization

The uncertainty inherent in power load forecasts represents a major factor in the mismatches between supply and demand in renewables-rich electricity networks, which consequently increases the energy bills and curtailed generation. As the transition to a power grid founded on the so-called grid-of-grids becomes more evident, the need for distributed control algorithms capable of handling computationally challenging problems in the energy sector does so as well. In this light, the consensus-based distributed algorithm has recently been shown to provide an effective platform for solving the complex energy management problem in microgrids. More specifically, in a microgrid context, the consensus-based distributed algorithm requires reliable information exchange with customers to achieve convergence. However, packet losses remain an important issue, which can potentially result in the failure of the overall system. In this setting, this paper introduces a novel method to effectively characterize such packet losses during information exchange between the customers and the microgrid operator, whilst solving the microgrid scheduling optimization problem for a multi-agent-based microgrid. More specifically, the proposed framework leverages the virulence optimization algorithm and the earth-worm optimization algorithm to optimally shift the energy consumption during peak periods to lower-priced off-peak hours. The effectiveness of the proposed method in minimizing the overall active power mismatches in the presence of packet losses has also been demonstrated based on benchmarking the results against the business-as-usual iterative scheduling algorithm. Also, the robustness of the overall meta-heuristic- and multi-agent-based method in producing optimal results is confirmed based on comparing the results obtained by several well-established meta-heuristic optimization algorithms, including the binary particle swarm optimization, the genetic algorithm, and the cuckoo search op

Use of anesthesia on the rise in gastrointestinal endoscopy

conscious sedation has been the standard of care for many years for gastrointestinal endoscopic procedures. As procedures have become more complex and lengthy, additional medications became essential for adequate sedation. Often time's deep sedation is required for procedures such as endoscopic retrograde cholangiography which necessitates higher doses of narcotics and benzodiazepines or even use of other medications such as ketamine. Given its pharmacologic properties, pro-pofol was rapidly adopted worldwide to gastrointestinal endoscopy for complex procedures and more recently to routine upper and lower endoscopy. Many studies have shown superiority for both the physician and patient compared to standard sedation. Nevertheless, its use remains highly controversial. A number of studies worldwide show that propofol can be given safely by endoscopists or nurses when well trained. Despite this wealth of data, at many centers its use has been pro-hibited unless administered by anesthesiology. In this commentary, we review the use of anesthesia support for endoscopy in the United States based on recent data and its implications for gastroenterologists world-wide.

Below-ground growth characteristics of 13-year-old Pinus densiflora on different contour conditions in a post-fire plantation in Samcheuk,Korea

We estimated the growth volume of artificially reforested Pinus densiflora in a post-fire area on three different contour conditions by comparing and analyzing the vertical and horizontal distributions of below-ground roots on each contour. The main roots at the south-facing slope (SS) developed in a long and straight form, and those on the north-facing slope (NS) in a twisted form. The side roots developed more than the main roots on the Ridge. The depth of taproots decreased in the following order: SS > NS > Ridge. The roots on the SS developed in a pile-form root structure whereas those in the Ridge and NS developed concentrically near the root collar. The amount of root development decreased in the following order: SS > NS > Ridge. The ratio of fine roots from the whole-root development decreased in the following order: Ridge > SS > NS. These results can guide considerations of growth differences according to the planting contour conditions for future establishment of P. densiflora artificial plantations.

Assessment of Foot care Knowledge among Diabetic Amputees at National Orthopaedic Hospital Enugu

Diabetics with unilateral lower extremity amputation (LEA) have an increased risk of contralateral re-amputation. Foot care knowledge and practice has been shown to reduce the incidence of amputation. The study aims to assess the level of foot care knowledge among diabetics with unilateral LEA. Method: A questionnaire based study involving diabetics with unilateral LEA from August 2015 to August 2017. Demographic data and level of foot care knowledge were assessed. Associations between age, gender, educational level, prior amputation and duration of diabetes with level of foot care knowledge were investigated. The presence of peripheral neuropathy in the contralateral leg was also assessed. Results: A total of 64 patients had unilateral LEA within this period, but only 42 patients completed the study. Twenty-four, 24 (57.1%) were males while 18 (42.9%) were females. Mean age was 58.3 years (SD = 12.0). Fifteen, 15 (35.7%) had secondary, 15 (35.7%) had tertiary education, while 3 (7.1%) and 9 (21.4%) had none and primary education respectively. Below knee amputation (BKA) was the commonest (78.6%), and 35.7% have had a prior amputation. Peripheral neuropathy was present in 71.4% of cases. No participant had a good level of foot care knowledge, while 13 (31%) have a fair knowledge and 29 (69%) have poor knowledge. Only 18 (42%) said they have been taught on foot care. Only duration of diabetes and educational level were significantly associated with level of foot care knowledge. conclusion: Diabetics with unilateral LEA have poor knowledge of foot care and a high prevalence of peripheral neuropathy. Efforts at education of these susceptible cohorts should be intensified.

Objective:To investigate the antimalarial activity of ethanol extract of Aspilin africana (***) ***:The ethanol extract of *** leaf(100-400 mg/kg) was screened for blood schizonticidal effect against chloroquine-sensitive Plasmodium berghei(P. berghei) in mice both in early and established models of antimalarial ***:The leaf extract exhibited significant(Pctivity in 4-day early infection and in established infection tests with a considerable mean survival time comparable to that of standard drug,chloroquine(10 mg/kg).conclusions:The findings show thai ethanol extract of *** leaf possesses potent antiplasmodial activity which justify the use in ethnomedicine and can be developed in malaria therapy.

Image J Analysis of Six Different Annealed Temperatures of 0.17% c of HSLA Steels

Annealing is a heat treatment procedure in which the mechanical properties of a material are being altered, and such alterations cause changes in its properties such as strength and hardness. It is usually carried out to improve ductility and toughness, to reduce hardness and to remove carbides. This study deals with the use of image analysis processing method for the measurement of structure of six annealed samples of 0.17% High Strength Low Alloy (HSLA) Steels (840°c - 990°c) with 30°c interval and 30 minutes soaking time. From the optical microscope images using Image J program, some parameters like calculation of area, pixel value statistics, distances and angles measurements, edge detection, such as the circularity, ferret angle, solidity and perimeter, average area and the percentage area were examined. The annealed sample of 960°c gave the highest grain count of 543 with a perimeter of 41.518;standard deviation 7.057 and a mean of 28.722 which shows that annealing greatly improved the fatigue life of the steel. From the mechanical analysis result, the annealed sample at 840°c has a highest hardness value of 129.4 BHN and gave the lowest impact value of 58.75 J. This study shows a significant decrease in the hardness value as the annealing temperature increased which signifies that annealing temperature enhanced the fatigue property and tensile strength (wear strength) of the steel in study.

Genome characteristics of the cyclophragma Undans Nucleopolyhedrovirus: A Distinct Species in Group Ⅰ of Alphabaculovirus

The cyclophragma undans nucleopolyhedrovirus （cyunNPV）, a potential pest control agent, was isolated from cyclophragma undans （Lepidoptera： Lasiocampidae）, an important forest pest. In the present study, we performed detailed genome analysis of cyunNPV and compared its genome to those of other Group I alphabaculoviruses. Sequencing of the cyunNPV genome using the Roche 454 sequencing system generated 142,900 bp with a G ＋ c content of 45%. Genome analysis predicted a total of 147 hypothetical open reading frames comprising 38 baculoviral core genes, 24 lepidopteran baculovirus conserved genes, nine Group I Alphabaculovirus conserved genes, 71 common genes, and five genes that are unique to cyunNPV. In addition, the genome contains 13 homologous repeated sequences （hrs）. Phylogenetic analysis groups cyunNPV under a distinct branch within clade ＂a＂ of Group I in the genus Alphabaculovirus. Unlike other members of Group I, cyunNPV harbors only nine of the 11 genes previously determined to be specific to Group I viruses. Furthermore, the cyunNPV lacks the tyrosine phosphatase gene and the ac30 gene. The cyunNPV F-like protein contains two insertions of continuous polar amino acids, one at the conventional fusion peptide and a second insertion at the pretransmembrane domain. The insertions are likely to affect the fusion function and suggest an evolutionary process that led to inactivation of the F-like protein. The above findings imply that cyunNPV is a distinct species under Group I Alphabaculovirus.

Methylenetetrahydrofolate reductase(MTHFR)deficiency is the most common genetic cause of hyperhomocysteinemia,which has been implicated in the etiology of recurrent spontaneous abortion(RSA).This study was designed to investigate the association between two single nucleotide polymorphisms(SNP)(rs1801133[c677T]and rs1801131[A1298c])in the MTHFR gene and RSA,in *** two SNPs were selected as these polymorphisms have a different effect on the activity and stability of the enzyme,and significantly diverse effects have been reported in relation to the association with *** approval was acquired from the IRB at King Saud University(KKUH),Saudi Arabia,and written informed consent was obtained from each *** study group comprised of 100 Saudi women with unexplained RSA and 100 age-matched controls,both attending KKUH for a routine *** was drawn in EDTA tubes,and DNA was *** was conducted using TaqMan SNP genotyping assay *** frequency of the T allele of c677T was 0.165 in patients and 0.17 in *** frequencies for cc,cT and TT genotypes were 70%,27%and 3%,respectively in RSA,and 71%,24%and 5%,respectively,in the controls(p>0.05).For the A1298c polymorphism,the c allele frequencies were 0.345 in patients and 0.28 in controls,while genotype frequencies for AA,Ac and cc genotypes were 44%,43%,and 13%,respectively,in patients,and 54%,36%,and 10%,respectively,in controls(p>0.05).The frequency of cc genotype and c allele of A1298c were higher in the patients with RSA,but not significantly,while c677T genotypes and allele frequencies did not differ between patients and *** results suggested that MTHFR gene polymorphisms are population-specific and may not associate with RSA in Saudi women.
