Anemia and long-term outcome in adjuvant and neoadjuvant radiochemotherapy of stage Ⅱ and Ⅲ rectal adenocarcinoma:The Freiburg experience(1989-2002)

AIM： To evaluate the long-term outcome of standard 5-FU based adjuvant or neoadjuvant radiochemotherapy and to identify the predictive factors, especially anemia before and after radiotherapy as well as hemoglobin increase or decrease during radiotherapy. METHODS： Two hundred and eighty-six patients with Union International Contre Cancer （UICC） stage Ⅱ and Ⅲ rectal adenocarcinomas, who underwent resection by conventional surgical techniques （low anterior or abdominoperineal resection）, received either postoperative （n = 233） or preoperative （n = 53） radiochemotherapy from January 1989 until July 2002. Overall survival （OAS）, cancer-specific survival （CSS）, disease-free survival （DFS）, local-relapse-free （LRS） and distant-relapse-free survival （DRS） were evaluated using Kaplan-Meier, Log-rank test and Cox＇s proportional hazards as statistical methods. Multivariate analysis was used to identify prognostic factors. Median follow-up time was 8 years. RESULTS： Anemia before radiochemotherapy was an independent prognostic factor for improved DFS （risk ratio 0.76, P= 0.04） as well as stage, grading, R status （free radial margins）, type of surgery, carcinoembryonic antigen （CEA） levels, and gender. The univariate analysis revealed that anemia was associated with impaired LRS（better local control） but with improved DFS. In contrast, hemoglobin decrease during radiotherapy was an independent risk factor for DFS （risk ratio 1.97, P= 0.04）. During radiotherapy, only 30.8% of R0-resected patients suffered from hemoglobin decrease compared to 55.6% if R1/2 resection was performed （P= 0.04）. The 5-year OAS, CSS, DFS, LRS and DRS were 47.0%, 60.0%, 41.4%, 67.2%, and 84.3%, respectively. Significant differences between preoperative and postoperative radiochemotherapy were not found. Between fighting and tolerance： reproductive biology of wingless males in the ant Cardiocondyla venustula

Male reproductive tactics vary widely across the species of the ant genus Cardiocondyla, from obligatory lethal combat among co-occurring males to complete mutual tolerance. The African species C. venustula Wheeler, 1908 has an intermediate phylogenetic position between taxa with fighting males and taxa with tolerant males and also shows an intermediate male behavior. Males from 2 native populations in South Africa and a population introduced to Puerto Rico attacked and killed freshly eclosing rivals but rarely engaged in deadly fights with adult competitors, Instead, several males per colony established small ＂territories＂ in their natal nests and defended them against other males. Males with a stable territory had more contact with female sexuals than nonterritorial males and more frequently engaged in mating attempts. In controlled choice experiments, female sexuals did not show any preference for particular males. We suggest that male territoriality in C. venustula is an adaptation to the seasonal production of large numbers of female sexuals by multiple mothers.

3D desktop-based virtual environments provide a means for displaying quantitative data in *** that are inherently spatial in three dimensions may benefit from visual exploration and analysis in relation to the environment in which they were collected and to which they *** empirically evaluate how effectively and efficiently such data can be visually analyzed in relation to location and landform in 3D versus 2D *** two experiments,participants performed visual analysis tasks in 2D and 3D visualizations and reported insights and their confidence in *** results showed only small differences between the 2D and 3D visualizations in the performance measures that we evaluated:task completion time,confidence,complexity,and insight ***,we found differences for different datasets and settings suggesting that 3D visualizations or 2D representations,respectively,may be more or less useful for particular datasets and contexts.

Mechanisms of intrahepatic triglyceride accumulation

Hepatic steatosis defined as lipid accumulation in hepatocytes is very frequently found in adults and obese adolescents in the Western World. Etiologically, obesity and associated insulin resistance or excess alcohol intake are the most frequent causes of hepatic steatosis. However, steatosis also often occurs with chronic hepatitis C virus(HCV) infection and is also found in rare but potentially life-threatening liver diseases of pregnancy. Clinical significance and outcome of hepatic triglyceride accumulation are highly dependent on etiology and histological pattern of steatosis. This review summarizes current concepts of pathophysiology of common causes of hepatic steatosis, including non-alcoholic fatty liver disease(NAFLD), alcoholic fatty liver disease, chronic HCV infections, drug-induced forms of hepatic steatosis, and acute fatty liver of pregnancy. Regarding the pathophysiology of NAFLD, this work focuses on the close correlation between insulin resistance and hepatic triglyceride accumulation, highlighting the potential harmful effects of systemic insulin resistance on hepatic metabolism of fatty acids on the one side and the role of lipid intermediates on insulin signalling on the other side. Current studies on lipid droplet morphogenesis have identified novel candidate proteins and enzymes in NAFLD.

A recent study by Chen et *** in Nature reported the presence of activated T cells in the brains of transgenic mice with frontotemporal dementia(FTD)-like Tau protein pathology and in postmortem Alzheimer’s disease(AD)brains.1 The infiltration of T cells correlated with the degree of Tau pathology and microglia activation,and appeared to promote *** progressive aggregation of amyloid-β in extracellular amyloid plaques and of Tau in neurofibrillary tangles are the two main protein pathological hallmarks of *** are accompanied by synaptic loss,neuroinflammation,and neuronal death,and com-promise the blood-brain barrier,allowing immune cells to infiltrate the brain parenchyma and interact with glial cells and neurons;all of these factors contribute to disease progression.2 Activated T cells can be found in postmortem brains of different neurological disorders,in AD,particularly in areas with neuronal loss and Tau pathology,i.e.,the hippocampus and limbic structures.

Magnetic resonance spectroscopy to study hepatic metabolism in diffuse liver diseases, diabetes and cancer

This review provides an overview of the current state of the art of magnetic resonance spectroscopy (MRS) in in vivo investigations of diffuse liver disease. So far, MRS of the human liver in vivo has mainly been used as a research tool rather than a clinical tool. The liver is particularly suitable for static and dynamic metabolic studies due to its high metabolic activity. Furthermore, its relatively superfi cial position allows excellent MRS localization, while its large volume allows detection of signals with relatively low intensity. This review describes the application of MRS to study the metabolic consequences of different conditions including diffuse and chronic liver diseases, congenital diseases, diabetes, and the presence of a distant malignancy on hepatic metabolism. In addition, future prospects of MRS are discussed. It is anticipated that future technical developments such as clinical MRS magnets with higher fi eld strength (3 T) and improved delineation of multicomponent signals such as phosphomonoester and phosphodiester using proton decoupling, especially if combined with price reductions for stable isotope tracers, will lead to intensifi ed research into metabolic syndrome, cardiovascular disease, hepato-biliary diseases, as well as non-metastatic liver metabolism in patients with a distant malignant tumor.

Transcriptional inhibition in Schwann cell development and nerve regeneration

Schwann cells, the myelinating glial cells of the peripheral nervous system are remarkably plastic after nerve trauma. Their transdifferentiation into specialized repair cells after injury shares some features with their development from the neural crest. Both processes are governed by a tightly regulated balance between activators and inhibitors to ensure timely lineage progression and allow re-maturation after nerve injury. Functional recovery after injury is very successful in rodents, however, in humans, lack of regeneration after nerve trauma and loss of function as the result of peripheral neuropathies represents a significant problem. Our understanding of the basic molecular machinery underlying Schwann cell maturation and plasticity has made significant progress in recent years and novel players have been discovered. While the transcriptional activators of Schwann cell development and nerve repair have been well defined, the mechanisms counteracting negative regulation of（re-）myelination are less well understood. Recently, transcriptional inhibition has emerged as a new regulatory mechanism in Schwann cell development and nerve repair. This mini-review summarizes some of the regulatory mechanisms controlling both processes and the novel concept of ＂inhibiting the inhibitors＂ in the context of Schwann cell plasticity.

H pylori and gastric cancer: Shifting the global burden

Infection with H pylori leads to a persistent chronic inflammation of the gastric mucosa, thereby increasing the risk of distal gastric adenocarcinoma. Numerous studies have determined a clear correlation between Hpylori infection and the risk of gastric cancer; however, general eradication is not recommended as cancer prophylaxis and time points for treatment remain controversial in different areas of the world. Prevalence rates in Western countries are decreasing, especially in younger people （〈 10%）; and a decline in distal gastric adenocarcinoma has been observed. Risk groups in Western countries still show considerably higher risk of developing cancer, especially in patients infected with cagA^＋ strains and in persons harboring genetic polymorphism of the IL-1B promoter （-511T/T） and the corresponding IL-1 receptor antagonist （IL-1RN^＊2）. Thus, general eradication of all infected persons in Western countries not recommended and is limited to risk groups in order to achieve a risk reduction. In contrast, infection rates and cancer prevalence are still high in East Asian countries. A prevention strategy to treat infected persons may avoid the development of gastric cancer to a large extent and with enormous clinical importance. However, studies in China and Japan indicate that prevention of gastric cancer is effective only in those patients that do not display severe histological changes such as atrophy and intestinal metaplasia. Thus, prophylactic strategies to prevent gastric cancer in high risk populations such as China should therefore especially aim at individuals now at younger age when the histological alterations caused by the bacterial infection was still reversible. In countries with a low prevalence of gastric cancer, risk groups carrying cagA^＋ strains and IL-1 genetic polymorphisms should be identified and treated.
