Patient Feedback for Acupuncture Practice Improvement:A survey from Mayo Clinic

Objective： To understand patient satisfaction with acupuncture appointment duration for the first and subsequent appointments in the plan of care, time spent with the acupuncture provider, willingness to pay for longer duration appointments, and lack of insurance coverage. Methods： Patients who were referred for an acupuncture consult to the Center for Integrative Medicine at Mayo Clinic were asked to participate in the survey in August 2014. Patients were given paper survey at time of appointment check-in and entered into Research Electronic Data Capture survey database tool to reveal patient-reported outcomes. Results： In total, 104 of 329 patients receiving acupuncture treatment responded to the survey with a 32% response rate. Insight and recommended changes to the duration of the initial and subsequent appointments were obtained. Most patients reported that time spent with the provider（76 cases, 74%） and appointment length（74 cases, 68%） were ＂just right＂. seventy cases（70%） of respondents indicated that they wanted longer treatment. Patients reported return appointments with duration of 30 min was not enough. Fifty-three patients（62%） were willing to pay for additional costs not covered by insurance. Conclusions： We used patient feedback to assess the acupuncture practice in a complementary and integrative medicine program. Most patients referred for the acupuncture consult appeared to be satisfied with the current level of practice. These findings were used to establish a quality improvement plan that may be implemented to improve patient satisfaction with the acupuncture practice.

Grinding is one of the most widely used material removal methods at the end of many process *** force is related to almost all grinding parameters,which has a great influence on material removal rate,dimensional and shape accuracy,surface and subsurface integrity,thermodynamics,dynamics,wheel durability,and machining system *** that grinding force is related to almost all grinding parameters,grinding force can be used to detect grinding wheel wear,energy calculation,chatter suppression,force control and grinding process *** prediction of grinding forces is important for optimizing grinding parameters and the structure of grinding machines and *** there are substantial research papers on grinding mechanics,a comprehensive review on the modeling of grinding mechanics is still absent from the *** fill this gap,this work reviews and introduces theoretical methods and applications of mechanics in grinding from the aspects of modeling principles,limitations and possible future trendencies.

Fuel desulfurization is an appealing topic for the chemical industry since severe environmental regulations regarding sO_2 emissions have been legislated in many countries. In order to reduce the amount of sulfur-containing compounds in fuels,responsible for high sO_x emission levels,a green chemistry approach is compulsory. In this paper,vanadium salen and salophen complexes were used in the oxidation of a model aromatic sulfide,such as dibenzothiophene( DBT),in the presence of H_2O_2 as green oxidant. The oxidative process was successfully coupled with the extraction of the oxidized compounds by ionic liquids. The system resulted highly selective for sulfide oxidation,showing poor reactivity toward the oxidation of alkenes and allowing a significant reduction of s content in a model benzine. To note,the use of microwave in place of standard heating allowed to obtain 98% of DBT oxidation and almost complete sulfur extraction in the model fuel in 1000 s. For these reasons,this system was considered an easy,rapid and clean process to achieve fuel desulfurization.

AIM： To describe a quantitative analysis method for liver biopsy sections with a machine that we have named ＂Dioguardi Histological Metriser＂ which automatically measures the residual hepatocyte mass （including hepatocytes vacuolization）, inflammation, fibrosis and the loss of liver tissue ***： We analysed digitised images of liver biopsy sections taken from 398 patients, The analysis with Dioguardi Histological Metriser was validated by comparison with semi-quantitative scoring ***： The method provides：（1） the metrical extension in two-dimensions （the plane） of the residual hepatocellular set, including the area of vacuoles pertinent to abnormal lipid accumulation; （2） the geo- metric measure of the inflammation basin, which distinguishes intra-basin space and extra-basin dispersed parenchymal leukoo/tes; （3） the magnitude of collagen islets, （which were considered truncated fractals and classified into three degrees of magnitude）; and （4） the tectonic index that quantifies alterations （disorders） in the organization of liver tissue. Dioguardi Histological Metriser machine allows to work at a speed of 0.1 mm^2/s, scanning a whole section in 6-8 ***： The results are the first standardized metrical evaluation of the geometric properties of the parenchyma, inflammation, fibrosis, and alterations in liver tissue tectonics of the biopsy sections. The present study confirms that biopsies are still valuable, not only for diagnosing chronic hepatitis, but also for quantifying changes in the organization and order of liver tissue structure.

Objective： Uncontrol ed diabetes has become a major cause of mortality and morbidity by reason of vascular angiopathy. The aim of this study was to evaluate the concentrations of soluble forms of vascular adhesion molecule-1 （sVCAM-1）, intercel ular adhesion molecule-1 （sICAM-1）, E-selectin, and thrombomodulin in patients with wel-control ed and uncontrol ed diabetes type 2. Methods： The study was conducted on 62 patients with diabetes. Group I consisted of 35 patients with wel-control ed diabetes. The second group included 27 patients with uncontrol ed diabetes with micro-albuminuria. A control group was made up of 25 healthy volunteers. The concentrations of sVCAM-1, sICAM-1, sE-selectin, and soluble thrombomodulin were assayed in plasma. serum concentration of cre-atinine was measured and the plasma concentrations of fasting glucose and glycated hemoglobin （HbA1c） determined. Results： Lower concentrations of ICAM-1 were found in the group of uncontrolled diabetes patients compared with those with wel-control ed disease. In patients with uncontrol ed diabetes, VCAM-1 levels were significantly higher compared with the group with wel-control ed diabetes. In patients with uncontrol ed diabetes a positive correlation was obtained between glomerular filtration rate and sE-selectin and a negative correlation between the levels of creatinine and ICAM-1, although there was a positive correlation between （HbA1c） and ICAM-1. Conclusions： The study con-firmed the participation of the inflammatory process associated with impaired vascular endothelial function in the pathogenesis of type 2 diabetes. The opposite effect of uncontrolled hyperglycemia on adhesion molecules suggests different functions of VCAM-1 and ICAM-1 in complications of diabetes.

Cuba has been leading marine protected area(MPA)designation in the Caribbean region to ensure conservation of its valuable marine ***,an efficient monitoring program for MPAs is still to be implemented and will benefit from pre-existing *** highly regulated MPA of Punta Francés National Park is one of the first Cuban MPAs and was established mainly to protect species and ecosystems for tourism *** of protection effects on commercial fish species was lacking in this *** study aimed at increasing local scientific knowledge by providing a baseline study about the most commercially fished families(Haemulidae,Lutjanidae and serranidae)around Punta Francés *** collected represent only a limited period but can be used as a base point to support future *** abundance in number and biomass,and size were collected to test for differences between inside and adjacent areas outside the Punta Francés MPA,in different coral reef *** main result of this study was the significantly larger size and biomass of snappers and groupers outside the MPA where intense fisheries *** observation consistent with a large spawning aggregation was also recorded outside the *** with a limited set of data,these results suggest that,at least temporarily,the most targeted species[and sizes]are still highly vulnerable to *** habitats in the adjacent area,that are apparently missing within the MPA,may support some of the results *** the limits of the Punta Francés MPA to include those habitats outside may be critical to ensure the effectiveness of this MPA in contributing to protect the most commercial species of the *** should be done together with efficient fisheries management measures in the region,such as the significantly increase of minimum legal sizes and temporal closures during spawning migrations.

Cytosolic Ca^2＋ is an important second messenger in virtually every type of cell. Moreover, Ca^2＋ generally regulates multiple activities within individual cells. This article reviews the cellular machinery that is responsible for Ca^2＋ signaling in cholangiocytes. In addition, two Ca^2＋-mediated events in cholangiocytes are discussed： bicarbonate secretion and apoptosis. Finally, emerging evidence is reviewed that Ca^2＋ signaling is involved in the pathogenesis of diseases affecting the biliary tree and that Ca^2＋ signaling pathways can be manipulated to therapeutic advantage in the treatment of cholestatic disorders.
