Hemorrhagic pericardial effusion following treatment with infliximab:A case report and literature review

BACKGROUND Infliximab(IFX)is an anti-tumor necrosis factor alpha(TNF-α)agent that is widely used for the management of a variety of autoimmune and inflammatory diseases,including Crohn's disease(CD).As a result of its increasing administration,new complications have emerged.Hemorrhagic pericardial effusion,secondary to IFX therapy,is a rare but life-threatening complication.CASE SUMMARY A 27-year-old man was diagnosed with CD(Montreal A2L3B1)6 years prior.After failing to respond to mesalazine and methylprednisolone,he took the first dose of IFX 300 mg based on his weight(60 kg,dose 5 mg/kg)on December 3,2018.He responded well to this therapy.However,on January 21,2019,1 wk after the third injection,he suddenly developed dyspnea,fever,and worsening weakness and was admitted to our hospital.On admission,computed tomography scan of the chest revealed a large pericardial effusion and a small rightside pleural effusion.An echocardiogram showed a large pericardial effusion and normal left ventricular function.Then successful ultrasound-guided pericardiocentesis was performed and 600 mL hemorrhagic fluid was drained.There was no evidence of infection and the concentrations of TNF-α,IFX,and anti-IFX antibody were 7.09 pg/mL(reference range1.0μg/mL),and 373 ng/mL(<30 ng/mL),respectively.As the IFX instruction manual for injection does mention pericardial effusion as a rare adverse reaction(≥1/10000,<1/1000),so we discontinued the IFX.Monitoring of the patient’s echocardiogram for 2 mo without IFX therapy showed no recurrence of hemorrhagic pericardial effusion.Follow-up visits and examinations every 3 to 6 mo until April 2021 showed no recurrence of CD or pericardial effusion.CONCLUSION This is a case of hemorrhagic pericardial effusion following treatment with IFX.It is a rare but life-threatening complication of IFX.Early recognition helps prevent the occurrence of hemorrhagic pericardial effusion and minimize the impact on the natural evolution of the disease.

Existing research has shown that political crisis events can directly impact the tourism industry. However, the current methods suffer from potential changes of unobserved variables, which poses challenges for a reliable evaluation of the political crisis impacts. This paper proposes a panel counterfactual approach with Internet search index, which can quantitatively capture the change of crisis impacts across time and disentangle the effect of the event of interest from the rest. It also provides a tool to examine potential channels through which the crisis may affect tourist outflows.This research empirically applies the framework to analyze the THAAD event on tourist flows from the Chinese Mainland to South Korea. Findings highlight the strong and negative short-term impact of the political crisis on the tourists' intentions to visit a place. This paper provides essential evidence to help decision-makers improve the management of the tourism crisis.

Microphysical Insights into a Tornadic Supercell from Dual-Polarization Radar Observations in Jiangsu, China on 14 May 2021

Tornadoes are incredibly powerful and destructive natural events,yet the microphysical characteristics of the parent storm and its effects on tornadogenesis remain unclear.This study analyzed polarization radar data of a tornadic supercell that occurred in Jiangsu Province of China on 14 May 2021,in comparison with another tornadic supercell and two non-tornadic supercells that occurred in the same region in 2023.The two tornadic supercells exhibited lower differential reflectivity(ZDR)in the hook echo region compared with the non-tornadic supercells,indicating smaller median drop sizes.A distinct increase in ZDR from the melting of frozen hydrometeors,observed between2.5-and 4.0-km altitude in the non-tornadic storms,was absent in the tornadic cases.The non-tornadic supercells also displayed substantially higher specific differential phase(KDP)below the melting level,likely aroused from enhanced melting and cooling.These findings suggest fundamental microphysical contrasts between tornadic and nontornadic supercells.Specifically,tornadic supercells have smaller droplets and may reduce melting in hook echoes.Moreover,greater separation between the ZDR arc and the KDP foot was observed during tornadogenesis.The vertical gradient of KDP related to the cooling pool strength of the hook echo,regulating rear-flank downdraft thermodynamics.Despite the limited number of cases investigated,the findings of this study indicate that monitoring ZDR,KDP,and drop size distribution trends could assist with tornado prediction and warnings.

In this article,by studying the Bernstein degrees and Goldie rank polynomials,we es-tablish a comparison between the irreducible representations of G=GL_(n)(C)possessing the minimal Gelfand-Kirillov dimension and those induced from finite-dimensional representations of the maximal parabolic subgroup of G of type(n-1,1).We give the transition matrix between the two bases for the corresponding coherent families.

Bone screws are devices used to fix implants or bones to bones.However,conventional screws are mechanically fixed with thread and often face long-term failure due to poor osseointegration.To improve osseointegration,screws are evolving from solid and smooth to porous and rough.Additive manufacturing(AM)offers a high degree of manufacturing freedom,enabling the preparation of predesigned screws that are porous and rough.This paper provides an overview of the problems currently faced by bone screws:long-term loosening and screw breakage.Next,advances in osseointegrated screws are summarized hierarchically(sub-micro,micro,and macro).At the sub-microscale level,we describe surface-modification techniques for enhancing osseointegration.At the micro level,we summarize the micro-design parameters that affect the mechanical and biological properties of porous osseointegrated screws,including porosity,pore size,and pore shape.In addition,we highlight three promising pore shapes:triply periodic minimal surface,auxetic structure with negative Poisson ratio,and the Voronoi structure.At the macro level,we outline the strategies of graded design,gradient design,and topology optimization design to improve the mechanical strength of porous osseointegrated screws.Simultaneously,this paper outlines advances in AM technology for enhancing the mechanical properties of porous osseointegrated screws.AM osseointegrated screws with hierarchical design are expected to provide excellent long-term fixation and the required mechanical strength.

Blockchain technology has witnessed a burgeoning integration into diverse realms of economic and societal development.Nevertheless,scalability challenges,characterized by diminished broadcast efficiency,heightened communication overhead,and escalated storage costs,have significantly constrained the broad-scale application of blockchain.This paper introduces a novel Encode-and CRT-based Scalability Scheme(ECSS),meticulously refined to enhance both block broadcasting and storage.Primarily,ECSS categorizes nodes into distinct domains,thereby reducing the network diameter and augmenting transmission efficiency.Secondly,ECSS streamlines block transmission through a compact block protocol and robust RS coding,which not only reduces the size of broadcasted blocks but also ensures transmission reliability.Finally,ECSS utilizes the Chinese remainder theorem,designating the block body as the compression target and mapping it to multiple modules to achieve efficient storage,thereby alleviating the storage burdens on nodes.To evaluate ECSS’s performance,we established an experimental platformand conducted comprehensive assessments.Empirical results demonstrate that ECSS attains superior network scalability and stability,reducing communication overhead by an impressive 72% and total storage costs by a substantial 63.6%.

The mechanical behavior of polymer networks is intrinsically correlated with the local chain topology and chain connectivity.In this study,we delve into this relationship through the lens of coarse-grained molecular dynamics(CG-MD)simulations.Our aim is to illuminate the intricate interplay between local topology and stress distribution within polymer monomers,cross-linkers,and various components with distinct cross-link connections,thereby elucidating their collective impact on the mechanical properties of polymer networks.We mainly focus on how specific local structures contribute to the overall mechanical response of the network.In particular,we employ local stress analysis to unravel the mechanics of these structures.Our findings reveal the diverse responses of individual components,such as junctions,strands,cross-linkers between junctions,and dangling chain ends,when subjected to stretching.Notably,we observe that these components exhibit varying degrees of deformation tolerance,underscoring the significance of their roles in determining the mechanical characteristics of the network.Our investigations highlight junctions as primary contributors to stress accumulation,and particles with higher local stress showing a stronger correlation between stress and Voronoi volume.Moreover,our results indicate that both strands and cross-linkers between junctions exhibit heightened stress levels as strand lengths decrease.This study enhances our understanding of the multifaceted factors governing the mechanical attributes of cross-linked polymer systems at the microstructural level.

Non-porous magnetic polymer microspheres with a core-shell structure were prepared by a novel micro-suspension polymerization technique. A stable iron oxide ferrofluid was used to supply the magnetic core, and the polymeric shell was made of glycidyl methacrylate （GMA monomer） and ethylene dimethacrylate （cross-linker）. In the preparation, polyvinyl alcohol was used as the stabilizer, and a lauryl alcohol mixture as the dispersant. The influence of various conditions such as aqueous phase volume, GMA and initiator amounts, reaction time and stirring speed on the character of the microspheres was investigated. The magnetic microspheres were then characterized briefly. The results indicate that the microspheres with active epoxy groups had a narrow size distribution range from 1 to 10 μm with a volume-weighted mean diameter of 4.5 μm. The saturation magnetization reached 19.9 emu/g with little coercivity and remanence.

The Jiama deposit,a significant porphyry-skarn-type copper polymetallic deposit located within the Gangdese metallogenic belt in Tibet,China,exemplifies a typical porphyry metallogenic system.However,the mineral chemistry of its accessory minerals remains under-examined,posing challenges for resource assessment and ore prospecting.Utilizing electron microprobe analysis and LA-ICP-MS analysis,this study investigated the geochemical characteristics of apatite in ore-bearing granite and monzogranite porphyries,as well as granodiorite,quartz diorite,and dark diorite porphyries in the deposit.It also delved into the diagenetic and metallogenic information from these geochemical signatures.Key findings include:(1)The SiO_(2) content,rare earth element(REE)contents,and REE partition coefficients of apatite indicate that the dark diorite porphyry possibly does not share a cogenetic magma source with the other four types of porphyries;(2)the volatile F and Cl contents in apatite,along with their ratio,indicate the Jiama deposit,formed in a collisional setting,demonstrates lower Cl/F ratios in apatite than the same type of deposits formed in a subduction environment;(3)compared to non-ore-bearing rock bodies in other deposits formed in a collisional setting,apatite in the Jiama deposit exhibits lower Ce and Ga contents.This might indicate that rock bodies in the Jiama deposit have higher oxygen fugacity.Nevertheless,the marginal variation in oxygen fugacity between ore-bearing and non-ore-bearing rock bodies within the deposit suggests oxygen fugacity may not serve as the decisive factor in the ore-hosting potential of rock bodies in the Jiama deposit.
