Comparison of outcomes with stapler versus hand-sewn closure of the pancreatic stump following minimally invasive distal pancreatectomy: a retrospective cohort study

Background: Pancreatic fistula after distal pancreatectomy is a common and potentially lethal complication. The optimal closure method for the pancreatic remnant during minimally invasive distal pancreatectomy (MDP) remains ***: Data of consecutive patients who underwent MDP in our institution between July 2018 and June 2021 were collected. The outcomes of MDP with stapler and hand-sewn closure were compared. The primary outcome was clinically relevant postoperative pancreatic fistula (CR-POPF) per the International Study Group of Pancreatic Surgery ***: Of the 384 patients (stapler closure, 339; hand-sewn closure, 45) enrolled, 249 developed CR-POPF (grades B and C: 242 and 7 patients, respectively). The rates of grade B and grade C POPF in the stapler group were similar to the corresponding rates in the hand-sewn group (64.6% and 1.5% vs 51.1% and 4.4%,P = .078 andP = .223, respectively). No differences between the stapler and hand-sewn groups were observed regarding the median operation time (207 vs 222 minutes,P = .139), incidence of major complications (16.5% vs 20.0%,P = .559), and mortality (0.2% vs 0%,P = 1.000). The independent risk factors of CR-POPF were abdominal abscess, prolonged operation time, and transection site (P = .004, .006, and .001, respectively).Conclusion: The incidence and severity of CR-POPF by stapler closure of the pancreatic stump were comparable to those associated with hand-sewn closure in MDP in this retrospective cohort. Randomized controlled trials are needed to verify this finding.

铁改性煤矸石去除废水中镉的研究

废水中的镉会严重威胁环境和人体健康。采用水热法制备铁改性煤矸石,并将其应用于含镉废水的处理。利用红外光谱(FT-IR)、X射线衍射(XRD)、扫描电镜(SEM)、比表面积(BET)和Zeta电位等方法对材料的结构和表面性质进行分析,研究铁改性煤矸石对Cd^(2+)的吸附行为,考察了pH和共存离子等因素对其吸附性能的影响,探讨了其吸附动力学、吸附等温线。结果表明:铁改性煤矸石经改性后其比表面积提高了2.85倍,最佳反应pH为8,共存离子CO_(3)^(2-)、SO_(4)^(-)、F-促进吸附反应;据伪二级动力学和Freundlich吸附等温式,表明对Cd^(2+)的吸附是单分子层化学吸附,吸附过程是自发吸热过程;30℃和50℃时铁改性煤矸石对Cd^(2+)最大拟合吸附量分别为32mg/g和34.18mg/g。铁改性煤矸石对废水中的Cd^(2+)具有良好的吸附性,有望用于重金属离子废水的处理。

Emitter discharge characteristics of vertical tube irrigation affected by various factors

To examine the working principle of vertical tube irrigation, variations in vertical tube emitter discharge and their causes were analyzed in the laboratory experiment. The effects of the pressure head, initial soil water content, and tube diameter on the emitter discharge of the vertical tube were studied. The results show that quantitative relationship between the time and cumulative infiltration and emitter discharge of the vertical tube is obtained, and R 2 is more than 0.98. Emitter discharge exhibits a positive and negative correlation with the pressure head and soil water content, respectively. Tube dia- meter has a nonsignificant effect on the emitter discharge. Changes of the soil water content around the emitter water outlet are the main causes of emitter discharge variations. In the experiments, the range of vertical tube emitter discharge is 0.056-1.102 L/h. The emitter of vertical tube irrigation automatically adjusts the soil water content and maintains the root zone soil water content within an appropriate range, which achieves continuous irrigation, and further achieves the effect of water-saving.

MULTIPLE LEVELS OF PD-L1 REGULATION IN CANCER IMMUNOTHERAPY To date,the PD-1/PD-L1 blockade strategy has been tested in a variety of cancer ***,only bladder cancer,melanoma,mismatch repair-deficient colorectal cancer,and certain hematopoietic malignancies have been highly responsive in terms of clinical outcomes.1,2 It is presumed that the expression level of the PD-L1 protein largely determines the therapeutic efficacy of PD-1/PD-L1-targeted cancer immunotherapy.3,4 Previous studies have shown that several transcriptional factors are involved in the upregulation of PD-L1 by directly binding to its promoter region.5 For instance,individual activation of STAT1,NF-κB,HIF-1α,c-Myc,or MAPK increased PD-L1 transcription and immune evasion of tumor *** the other hand,oncogenic RAS signaling reportedly stabilizes PD-L1 mRNA to promote tumor immunoresistance.
