Beta-blocker therapy in elderly patients with renal dysfunction and heart failure

OBJEcTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(cKD)aged≥75 *** AND RESULTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFR)≤60 m L/min per 1.73 m^*** analyzed the relationship between treatment with BB and mortality or cardiovascular *** mean age of our population was 82.6±4.1 *** ejection fraction was 27.9%±6.5%.GFR was 60-45 m L/min per 1.73 m^2 in 50.3%of patients,45-30 m L/min per 1.73 m^2 in 37.4%,andconclusion of follow-up,67.4%of patients were receiving *** median follow-up was28.04(IR:19.41-36.67)*** the study period,211 patients(54.1%)died and 257(65.9%)had a major cardiovascular event(death or hospitalization for heart failure).BB use was significantly associated with a reduced risk of death(HR=0.51,95%cI:0.35-0.74;Pceiving BB consistently showed a reduced risk of death across the different stages of cKD:stage IIIa(GFR=30-45 m L/min per 1.73 m^2;HR=0.47,95%cI:0.26-0.86,PcI:0.26-1.06,P=0.007),and stages IV and V(GFRcI:0.11-0.76;P=0.047).cONcLUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better *** of BB should be encouraged when possible.

Design and Evaluation of a Vision-Based UI for People with Large cognitive-Motor Disabilities

Recovering from multiple traumatic brain injury (TBI) is a very difficult task, depending on the severity of the lesions, the affected parts of the brain and the level of damage (locomotor, cognitive or sensory). Although there are some software platforms to help these patients to recover part of the lost capacity, the variety of existing lesions and the different degree to which they affect the patient, do not allow the generalization of the appropriate treatments and tools in each case. The aim of this work is to design and evaluate a machine vision-based UI (User Interface) allowing patients with a high level of injury to interact with a computer. This UI will be a tool for the therapy they follow and a way to communicate with their environment. The interface provides a set of specific activities, developed in collaboration with the multidisciplinary team that is currently evaluating each patient, to be used as a part of the therapy they receive. The system has been successfully tested with two patients whose degree of disability prevents them from using other types of platforms.

colorectal peritoneal metastases:Optimal management review

The peritoneum is a common site of dissemination for colorrectal cancer, with a poorer prognosis than other sites of metastases. In the last two decades, it has been considered as a locoregional disease progression and treated as such with curative intention treatments. cytoreductive surgery(cRS) and hyperthermic intraperitoneal chemotherapy(HIPEc) is the actual reference treatment for these patients as better survival results have been reached as compared to systemic chemotherapy alone, but its therapeutic efficacy is still under debate. Actual guidelines recommend that the management of colorectal cancer with peritoneal metastases should be led by a multidisciplinary team carried out in experienced centers and consider cRS + HIPEc for selected patients. Accumulative evidence in the last three years suggests that this is a curative treatment that may improve patients disease-free survival, decrease the risk of recurrence, and does not increase the risk of treatment-related mortality. In this review we aim to gather the latest results from referral centers and opinions from experts about the effectiveness and feasibility of cRS + HIPEc for treating peritoneal disease from colorectal malignancies.

Surgical Excision of Ischiorectal Fossa Tumors

Background: Ischiorectal Fossa Tumors are rare and their management has not been protocolized. Patients: We retrospectively review four consecutive cases treated in our department, from January 2015 to July 2015. All of them were discussed in a multidisciplinary team meeting. None of them were secondary to an inflammatory process. Results: A Magnetic Resonance Imaging was performed in all the four patients, and as it was not a malignant diagnosis made in any of them, we proceeded to resecate the lesions. None biopsies were performed and the benign diagnoses were confirmed by the pathologist. All the patients underwent a local posterior or perineal approach, because all the lesions (epidermoid cyst, hamartoma, lipoma and an aggressive angiomyxoma) were localized purely in the ischiorectal fossa, under the levator ani muscle and not invading any adjacent structures. In all of them, an R0 resection was performed. conclusions: Ischiorectal fossa tumors are rare and there are few cases already published. Non-inflammatory lesions located in the ischiorectal fossa, with none invasion of rectum or levator ani muscle, are mainly benign lesions prone to their complete excision by a posterior approach. Biopsy is not always necessary unless there’s a suspicion of a malignancy or invasion of adjacent structures, and only in that case a biopsy should be made, because in some of them, a neo-adjuvant treatment can be useful to reduce the tumor and to perform an R0 resection.

Temperature Field on Structural carbon Steel under Fire conditions

The behaviour of structural steels components subjected to a high temperature field depends not only on the absolute temperature reached, but also on the temperature versus time function and heating rate. The research objective of the present work was the determination of the heating rate impact on selected strength parameters of structural steels. Tests were carried out on structural carbon steel in order to find experimental parameters required for the limit state analysis of steel structures under the thermal conditions as occur in fires. An electrical furnace has been designed and constructed to simulate the effect of the fire in buildings, which allowed treat a sample thermally following the temperature profile for the different standard curves of fire. For this purpose, an electrical controller has been used to heat the furnace and to control the experimental temperatures achieves into the furnace when a specific fire curve was applied. The furnace temperature time fields for the internal and external faces of the sample as well as the furnace atmosphere were registered. A spreadsheet method was used to predict the temperature of the steel samples and compare them with temperatures obtained experimentally, following the European convention for constructional Steelwork.

Risk Groups and Mortality in Patients with SARS-coV-2 from Family Medicine Unit No. 43 in Southeastern Mexico

SARS-coV-2 infection is a viral disease that causes respiratory infections that can cause multiple complications. Patients with adjacent morbidities have a higher chance of complications and mortality. The National Health and Nutrition Survey (ENSANUT) in 2018 reported that in Mexico, 75.2% of adults aged 20 years and over are overweight and obese, both situations are closely related to the cases reported as severe cOVID and even with a higher risk of mortality. The main objective was to evaluate the risk and mortality groups in patients with SARS-coV-2 from family medicine in the south of México. The study was carried out during the 2020 year, in the Family Medicine Unit No. 43 of Villahermosa, Tabasco, a cross-sectional, retrospective investigation was carried out with an analytical approach, and the sample was obtained using the formula of n for infinite populations that correspond to 215, patients were included who met the selection criteria;3 groups were integrated: DM2, HAS and obesity. Descriptive statistics andχ2 formula were used, using the SPSS version 25 Windows program. The female gender presented the highest number of infections 122 (56.7%) and mortality: 5 (2.32%), obesity comorbidity presented the highest number of infections: 194 (90.2%), HAS: 153 (71.2%), and MD: 44 (20.46%). complications were observed in 23 (10.69%), distributed in: SIRA: 7 (3.3%), multiple organ failure: 6 (2.8%), heart failure: 6 (2.8%) and pneumonia 4 (1.9%). The observed mortality was 8 (3.72%). The comorbidities of patients with the highest mortality were: SAH: 6 (75%) (χ2 = 9.169;df = 1;p = 0.010) and obesity: 6 (75%) (χ2 = 9.169;df = 1;p = 0.010). The group with the highest risk for SARS-coV-2 was patients with overweight/obesity comorbidities. Mortality was 3.7%. The patients who died had at least one or more comorbidities. The main complications were ARIS, heart failure, and multiple organ failure.
