To study whether paramedical workers from rural primary health centers in India are able to administer oral misoprostol and actively manage the third stage of labor to prevent postpartum hemorrhage (PPH). Method: Cluster randomization was used to enroll 1200 women at 30 peripheral health centers from 5 states in India, 600 forming the study’s intervention group (active management of the third stage of labor with 600 μ g of oral misoprostol) and 600 forming the comparison group (in which the current government guidelines for the prevention of PPH were followed). The primary outcome was blood loss after delivery, which was measured using a calibrated blood collection drape. Results: Age, literacy level, occupation, and gravidity were similar in the 2 groups. More than 70% of women in both groups had moderate anemia (hemoglobin level s followed instructions in almost all deliveries in the intervention group (99% ). There was a significant reduction in duration of the third stage of labor (7.9 ± 4.2 min vs. 10.9 ± 4.3 min; p ss after delivery (100 mL vs. 200 mL; p s observed ( s. A greater number of women had moderate to severe shivering (12.7% vs. 0.5% ) and a temperature higher than 38° C (9.7% vs. 4.3% ) in the intervention group, which was statistically significant. Conclusion: simple interventions can be easily implemented in rural health care settings to reduce the blood loss during labor. This finding has significant implications for developing countries, in which the prevalence of anemia is high.

Early Detection of Lung Cancer Using CT scan and Bronchoscopy in a High Risk Population

Background: Computed tomography (CT) and bronchoscopy have been shown to improve the detection rates of peripheral and central lung cancers (LC), respectively. However, the performance of the combination of CT and bronchoscopy in detecting LC, in high-risk patients, is not clear. Patients & Methods: This prospective study included 205 high-risk patients with a history of at least 2 of the following risk factors: (1) heavy smoking;(2) aero-digestive cancer;(3) pulmonary asbestosis or;(4) chronic obstructive pulmonary disease. Patients were offered chest X-ray, sputum cytology, conventional white-light followed by autofluorescence beonchoscopy (WL/AFB) and low-dose spiral CT both at baseline and follow-up visits. Results: seven patients (3.4%) were diagnosed with LC or carcinoma in-situ (CIs) at baseline: CT evaluation detected 5 LC/CIs, while WL/AFB evaluation also identified 5 LC/CIs, 2 of which were not detected on CT. six (85%) of these baseline lesions were early stage (0/IA). The relative-sensitivity of CT with WL/ AFB was 40% better than CT alone. On four year follow-up, 20 patients (9.8%) were diagnosed with an LC/CIs. CT with WL/AFB detected 19 cases (95%), whereas CT alone detected 15 cases (75%). The exploitation of ultrafast electron dynamics in quantum cascade lasers(QCLs)holds enormous potential for intense,compact mode-locked terahertz(THz)sources,squeezed THz light,frequency mixers,and comb-based metrology *** the important sub-cycle dynamics have been notoriously difficult to access in operational THz ***,we employ high-field THz pulses to perform the first ultrafast two-dimensional spectroscopy of a free-running THz *** incoherent and coherent nonlinearities up to eight-wave mixing are detected below and above the laser *** data not only reveal extremely short gain recovery times of 2 ps at the laser threshold,they also reflect the nonlinear polarization dynamics of the QCL laser transition for the first time,where we quantify the corresponding dephasing times between 0.9 and 1.5 ps with increasing bias currents.A density-matrix approach reproducing the emergence of all nonlinearities and their ultrafast evolution,simultaneously,allows us to map the coherently induced trajectory of the Bloch *** observed high-order multi-wave mixing nonlinearities benefit from resonant enhancement in the absence of absorption losses and bear potential for a number of future applications,ranging from efficient intracavity frequency conversion,mode proliferation to passive mode locking.

结合轴线偏移情况开展回转窑滚圈多轴疲劳寿命研究,采用了Wang和Brown提出的多轴疲劳准则和雨流计数法及Miner准则进行累积损伤评估.回转窑运行轴线偏移会导致滚圈承受的支承载荷分布严重不均,因此引入载荷比来描述轴线偏移程度以进一步定量分析滚圈应力.采用有限元软件ANsYs进行了各载荷比下滚圈的接触应力分析,并根据滚动接触区域情况计算得到了切向摩擦应力.以某回转窑滚圈为分析对象,计算得到了随载荷比变化的疲劳寿命曲线,揭示了轴线偏移对滚圈疲劳寿命的影响规律.研究对减少滚圈疲劳失效有重要意义,同时也可作为轴线调整和优化调窑的理论依据.

This study examines the construct validity and reliability of the Malay language questionnaire for urinary incontinence diagnosis (QUID) in women. study Design: Random sampling design was used in this cross-sectional survey. Materials and Methods: The Americanized English language questionnaire was translated to the Malay language and distributed to community-dwelling Malaysian women living in various locations in selangor. The construct validity was tested using exploratory factor analysis (EFA) followed by confirmatory factor analysis (CFA). The reliability was determined using Cronbach’s α. Results: A total of 111 women completed the Malay language QUID in this pilot study. The Keiser-Meyer-Olkin (KMO) measure of sampling adequacy of 0.675 and Bartlett’s test of sphericity (χ2 = 284.633, df = 15, p = 0.001) indicated that the EFA was possible. The total variance and the scree plot identified two factors above the initial eigenvalue of 1 while a third factor was just below it (0.758). The CFA output showed a recursive model with the solution being not admissible because two unobserved and exogenous variables had negative variance estimates. The following values of absolute fit indices showed an acceptable level of fit: 1) Chi-square test with χ2 = 4.997, df = 5, p = 0.416, indicated a smaller difference between the expected and observed covariance matrices;2) GFI = 0.986, AGFI = 0.939, RMR = 0.021 and CMIN/DF = 1.0 indicated acceptable level of fit;3) The baseline comparison values of NFI = 0.983 and CFI = 1.0 also indicated a good fit to the data;4) RMsEA = 0.000 was considered a perfect fit indicating that the hypothesized model was a good fit to the observed data. Under the hypothesis of “close fit”, the probability of getting a sample RMsEA as large as 0.000 was 0.567. The Cronbach’s α coefficient of 0.823 indicated good reliability. Purpose:To evaluate whether transpupillary thermotherapy(TTT)reduces the risk of moderate visual loss in patients with occult choroidal neovascularization(CNV)secondary to agerelated macular degeneration(AMD).Methods:A group of 25 patients were recruited and randomized in to TTT or placebo *** were included if they had a subfoveal purely or predominantly(> 50%)occult CNV secondary to AMD with best corrected visual acuity(BCVA)of 6/60 or better and the lesion was not larger than 4.5 *** was carried out using an 810-nm Oculight diode laser with a fixed spot size covering the whole lesion according to the standard *** same procedure was used for the control group,except that the power was set at *** patients were followed up at 6 weeks,3 months and then every 6 months for up to 2 years.A maximum of three treatments were administered in both groups if there was evidence of persistent leakage from ***:At the 12-month follow-up,there was no significant difference in the mean values for BCVA distance and near or contrast sensitivity between the treatment and control *** Mann-Whitney test was used to assess the differences in BCVA and contrast sensitivity between the groups,both at baseline and at the 12-month *** statistically significant difference was found;both groups lost on average two lines of ***:Transpupillary thermotherapy appeared to have been of no benefit in preventing further visual loss in patients with occult CNV in this pilot study.

仔猪的细菌性尤其大肠杆菌性腹泻是十分常见的,并由于会导致生长抑制和严重死亡而造成重大经济损失。频频滥用抗腹泻药剂,会使病菌不断产生抗药性和改变抗原结构,从而导致不能有效地预防和控制腹泻。因而需要研制新的抗菌药物。本研究报告了在印度应用硫酸普乐霉素(Apramycin)作为饲料添加剂对仔猪腹泻的预防效果。本研究中使用的是英国Elanco有限公司生产的商品,名为Apralan100预混料。但尚未在农场管理条件下进行试验。
