Performance of Non-invasive blood Parameters for Ruling Out Significant Liver Fibrosis in Patients with Chronic Hepatitis b

background and Aims:Evaluation of significant liver fibro-sis is important for treatment decision and treatment re-sponse evaluation in patients with chronic hepatitis b. Since liver biopsy is invasive and transient elastography (TE) has limited availability, various non-invasive blood parameters need evaluation for their capabilities for detection of signifi-cant fibrosis. Methods:In this retrospective study, records of patients who had undergone liver biopsy for treatment-na?ve chronic hepatitis b were evaluated to obtain various non-in-vasive blood parameters (aspartate aminotransferase-to-pla-telet ratio index [referred to as APRI], Fibrosis-4 score [referred to as FIb-4], gamma-glutamyl transpeptidase-to-platelet ratio [referred to as GPR], and gamma-glutamyl transpeptidase-to-albumin ratio [referred to as GAR]), in ad-dition to TE, to assess significant liver fibrosis and compare these to fibrosis stage in liver biopsy. Results:A total of 113 patients were included in the study (median age 33 [inter-quartile range: 11-82 years], 74% males). Most (75%) pa-tients were HbeAg-negative. The liver biopsy revealed significant fibrosis (Ishak ≥3) in 13% of the patients and nil or mild fibrosis (Ishakble for 85 patients, APRI and FIb-4 for 95 patients, GPR for 79 patients, and GAR for 78 patients. The median values of all the parameters were significantly higher in pa-tients with significant fibrosis, as compared to patients with non-significant fibrosis, and all the blood parameters as well as TE were able to identify patients with significant fibrosis significantly well (pbut negative predictive value above 92%. Compared to TE, all the non-invasive blood pa-rameters had similar area under the curve for detecting sig-nificant fibrosis, with excellent negative predictive value (≥93%). Several chemical compounds can restore pigmentation in vitiligo through mechanisms that vary according to disease *** the present study,we investigated the melanogenic activity of six structurally distinct compounds,namely,scopoletin,kaempferol,chrysin,vitamin D_3,piperine,and *** determined their effectiveness,toxicity,and mechanism of action for stimulating pigmentation in b16F10 melanoma cells and in a zebrafish *** melanogenic activity of 6-benzylaminopurine,the compound identified as the most potent,was further verified by measuring green fluorescent protein concentration in tyrp1 a:eGFP(tyrosinase-related protein 1)zebrafish and mitfa:eGFP(microphthalmia associated transcription factor)zebrafish and antioxidative *** the tested compounds were found to enhance melanogenesis responses both in vivo and in vitro at their respective optimal concentration by increasing melanin content and expression of TYR and MITF.6-benzyamino-purine showed the strongest re-pigmentation action at a concentration of 20μmol·L^(-1 )in vivo and 100μmol·L^(-1) in vitro,and up-regulated the strong fluorescence expression of green fluorescent protein in tyrp1 a:e GFP and mitfa:e GFP zebrafish in ***,its relative anti-oxidative activity was found to be very ***,our results indicated that 6-benzylaminopurine stimulated pigmentation through a direct mechanism,by increasing melanin content via positive regulation of tyrosinase activity in vitro,as well as up-regulating the expression of the green fluorescent protein in transgenic zebrafish in vivo.

Docosanol is the only US Food and Drug Administration(FDA)approved over-the-counter topical product for treating recurrent oral-facial herpes simplex *** analytical methods for docosanol are required to demonstrate the bioequivalence of docosanol topical products.A gas chromatography/selected ion monitoring mode mass spectrometry(GC/SIM-MS)method was developed and validated for docosanol determination in biological *** and isopropyl palmitate(internal standard)were separated on a high-polarity GC capillary column with(88%cyanopropy)aryl-polysiloxane employed as the stationary *** ions of m/z 83 and 256 were selected to monitor docosanol and isopropyl palmitate,respectively;the total run time was 20 *** GC/SIM-MS method was validated in accordance with US FDA guidelines,and the results met the US FDA acceptance *** docosanol calibration standards were linear in the 100-10000 ng/mL concentration range(R^(2)>0.994).The recoveries for docosanol from the receptor fluid and skin homogenates were>93.2%and>95.8%,*** validated method was successfully applied to analyze ex vivo human cadaver skin permeation *** applying Abreva®cream tube and Abreva®cream pump,the amount of docosanol that penetrated human cadaver skin at 48 h was 21.5±7.01 and 24.0±6.95 ng/mg,***,we concluded that the validated GC/SIM-MS was sensitive,specific,and suitable for quantifying docosanol as a quality control *** method can be used for routine analysis as a costeffective alternative to other techniques.

The novel coronavirus-related coronavirus disease 2019(COVID-19)pandemic has been relentless in disrupting and overwhelming healthcare the world *** outcomes of COVID-19 in patients with chronic comorbidities,especially in those with metabolic syndrome,are well *** liver disease and cirrhosis patients are a special subgroup,among whom the management of COVID-19 is *** the pathophysiology of COVID-19 in patients with cirrhosis and portal hypertension improves our identification of at-risk patients for disease progression that will further help compartmentalize generalized and specialized treatment options in this special patient *** this exhaustive review,we critically review the impact of COVID-19 on the liver and in chronic liver disease and cirrhosis *** further discuss common features associated with the pathophysiology of COVID-19 and cirrhosis,based on the renin-angiotensin system and deliberate current literature on guidelines for the treatment of COVID-19 and extrapolate the same to the cirrhosis population to provide a concise and stepwise,evidence-based management for cirrhosis patients with severe and critical *** are no specific management guidelines for cirrhosis patients with COVID-19 and current recommendations for treatment are as per guidelines for general ***,specific issues like avoiding corticosteroids in decompensated patients with variceal bleeding,suspected sepsis,high grade hepatic encephalopathy and acute kidney injury,use of early mechanical ventilation strategies in those with severe ascites and hepatopulmonary syndrome,avoidance of remdesivir in advanced liver disease,and application of liver-specific severity scores for prognostication and identification of futility need to be highlighted.

Influence of addition of Gr_p/Al_2O_(3p )with SiC_p on wear properties of aluminum alloy 6061-T6 hybrid composites via friction stir processing

Aluminum alloy base surface hybrid composites were fabricated by incorporating with mixture of （SiC＋Gr） and （SiC＋Al2O3） particles of 20 μm in average size on an aluminum alloy 6061-T6 plate using friction stir processing （FSP）. Microstructures of both the surface hybrid composites revealed that SiC, Gr and Al2O3 are uniformly dispersed in the nugget zone （NZ）. It was observed that the addition of Gr particles rather than Al2O3 particles with SiC particles, decreases the microhardness but immensely increases the dry sliding wear resistance of aluminum alloy 6061-T6 surface hybrid composite. The observed microhardness and wear properties are correlated with microstructures and worn micrographs.

Correlation of transient elastography with hepatic venous pressure gradient in patients with cirrhotic portal hypertension: A study of 326 patients from India

AIMTo study the diagnostic accuracy of transient elastography (TE) for detecting clinically significant portal hypertension (CSPH) in Indian patients with cirrhotic portal *** retrospective study was conducted at the Institute of Liver, Gastroenterology, and Pancreatico-biliary Sciences, Sir Ganga Ram Hospital, New Delhi, on consecutive patients with cirrhosis greater than 15 years of age who underwent hepatic venous pressure gradient (HVPG) and TE from July 2011 to May 2016. Correlation between HVPG and TE was analyzed using the Spearman’s correlation test. Receiver operating characteristic (ROC) curves were prepared for determining the utility of TE in predicting various stages of portal hypertension. The best cut-off value of TE for the diagnosis of CSPH was obtained using the Youden *** study included 326 patients [median age 52 (range 16-90) years; 81% males]. The most common etiology of cirrhosis was cryptogenic (45%) followed by alcohol (34%). The median HVPG was 16.0 (range 1.5 to 30.5) mmHg. Eighty-five percent of patients had CSPH. A significant positive correlation was noted between TE and HVPG (rho 0.361, P best predicted CSPH with a positive predictive value (PPV) of 93%.CONCLUSIONTE has a fair positive correlation with HVPG; thus, TE can be used as a non-invasive modality to assess the degree of portal hypertension. A cut-off TE value of 21.6 kPa identifies CSPH with a PPV of 93%.

Tribology of WC reinforced SiC ceramics: Influence of counterbody

Hot pressed silicon carbide(SiC) composites prepared with 0, 10, 30 or 50 wt% tungsten carbide(WC) are subjected to dry sliding wear against WC-Co and steel ball. In particular an attempt has been made to answer the following important questions:(i) How does the load(from 5 to 20 N) effect sliding wear behaviour of SiC-ceramics against WC-Co and steel counterbodies?(ii) Is there any effect of WC content on friction and wear characteristics of SiC ceramics?(iii) Does the dominant material removal mechanism of SiC ceramics change with the addition of WC or counterbody?(iv) What is the influence of mechanical properties on the sliding wear? Experimental results indicated that coefficient of friction(COF) for the SiC ceramics varied between 0.66 and 0.33 with change in load and counterbodies. Wear volume for SiC ceramics found approximately 6-10 times more against WC-Co as compared against steel. Wear volume changes from 2.0 × 10^(–3)mm^3 to 1.2 × 10^(–2)mm^3 with change in counterbodies for SiC -(10, 30 or 50 wt%) WC composite at 20 N. SiC ceramics indicated abrasion and composites reveal tribochemical wear as major material removal mechanisms. Wear is influenced by the hardness of counterbody and fracture toughness of SiC -WC composites.

Remote sensing and GIS for land use/cover mapping and integrated land management： case from the middle Ganga plain

In India, land resources have reached a critical stage due to the rapidly growing population. This challenge requires an integrated approach toward harnes- sing land resources, while taking into account the vulnerable environmental conditions. Remote sensing and Geographical Information System （GIS） based technologies may be applied to an area in order to generate a sustainable development plan that is optimally suited to the terrain and to the productive potential of the local resources. The present study area is a part of the middle Ganga plain, known as Son-Karamnasa interfluve, in India. Alternative land use systems and the integration of livestock enterprises with the agricultural system have been suggested for land resources management. The objective of this paper is to prepare a land resource development plan in order to increase the productivity of land for sustainable development. The present study will contribute necessary input for policy makers to improve the socio-eeonomic and environmental conditions of the region.
