bAcKGROUND chronic hepatitis b(cHb)virus infection is a major cause of liver-associated morbidity and mortality,particularly in low-income countries.A better understanding of the epidemiological,clinical,and virological characteristics of cHb will guide appropriate treatment strategies and improve the control and management of cHb in *** To investigate the characteristics of cHb in Eastern Ethiopia and assess the efficacy and safety of antiviral *** This cohort study included 193 adults who were human immunodeficiency virus-negative with cHb between June 2016 and December *** assessments included chemistry,serologic,and viral markers.χ^(2) tests,Mann-Whitney U tests,and logistic regression analyses were used to identify the determinants of *** disoproxil fumarate(TDF)was initiated using treatment criteria from the Ethiopian cHb pilot *** A total of 132 patients(68.4%)were men,with a median age of 30 years[interquartile range(IQR):24-38].At enrollment,60(31.1%)patients had cirrhosis,of whom 35(58.3%)had decompensated *** use,hepatitis b envelope antigen positivity,and a high viral load were independently associated with ***,66 patients(33.4%)fulfilled the treatment criteria and 59(30.6%)started *** 29 patients who completed 24 months of treatment,the median aspartate aminotransferase to platelet ratio index declined from 1.54(IQR:0.66-2.91)to 1.10(IQR:0.75-2.53)(P=0.002),and viral suppression was achieved in 80.9%and 100%of patients after 12 months and 24 months of treatment,*** the treated patients,12(20.3%)died within the first 6 months of treatment,of whom 8 had decompensated *** This study highlights the high prevalence of cirrhosis,initial mortality,and the efficacy of TDF *** up measures to prevent and control cHb infections in Ethiopia is crucial.

Rhombohedral phase Hf_(x)Zr_(1.x)O_(2)(HZO,x from 0 to 1)films are promising for achieving robust ferroelectric polarization without the need for an initial wake-up pre-cycling,as is normally the case for the more commonly studied orthorhombic ***,a large spontaneous polarization observed in rhombohedral films is not fully understood,and there are also large discrepancies between experimental and theoretical *** this work,in rhombohedral ZrO_(2)thin films,we show that oxygen vacancies are not only a key factor for stabilizing the phase,but they are also a source of ferroelectric polarization in the *** is shown experimentally through the investigation of the structural properties,chemical composition and the ferroelectric properties of the films before and after an annealing at moderate temperature(400℃)in an oxygen environment to reduce the V_(o)concentration *** experimental work is supported by density functional theory(DFT)calculations which show that the rhombohedral phase is the most stable one in highly oxygen defective ZrO_(2)*** DFT calculations also show that V_(o)contribute to the ferroelectric *** findings reveal the importance of V_(o)for stabilizing rhombohedral ZrO_(2)thin films with superior ferroelectric properties.

Exploring Variability in Sea Level at a Tide Gauge Station through control charts

Monitoring temporal changes in sea level is important in assessing coastal *** level anomalies at a tide gauge station,if kinematically conceived,include systematic variations such as trend,acceleration,periodic oscillations,and random *** them,the non-stationary nature of the random sea level variations of known or unknown origin at coastal regions has been long recognized by the sea level *** study proposes the analyses of subgroups of random residual statistics of a rigorously formulated kinematic model solution of tide gauge variations using X-bar and S control *** approach is demonstrated using Key West,Florida tide gauge *** mean and standard errors of 5-year-long subgroups of the residuals revealed that sea level changes at this location have been progressively intensifying from 1913 to the *** oscillations in sea level at this locality may be attributed partly to the thermal expansion of seawater with increasing temperatures causing larger buoyancy-related sea level fluctuations as well as the intensification of atmospheric events including wind patterns and the impact of changes in inverted barometer effects that will alter coastal risk assessments for the future.

The mountains of Southwest china comprise a significant large mountain range and biodiversity hotspot imperiled by global climate *** high species diversity in this mountain system has long been attributed to a complex set of factors,and recent large-scale macroevolutionary investigations have placed a broad timeline on plant diversification that stretches from 10 million years ago(Mya)to the *** our increasing understanding of the temporal mode of speciation,finer-scale populationlevel investigations are lacking to better refine these temporal trends and illuminate the abiotic and biotic influences of cryptic *** is largely due to the dearth of organismal sampling among closely related species and populations,spanning the incredible size and topological heterogeneity of this *** study dives into these evolutionary dynamics of speciation using genomic and ecomorphological data of Stellera chamaejasme *** identified four previously unrecognized cryptic species having indistinct morphological traits and large metapopulation of evolving lineages,suggesting a more recent diversification(~2.67-0.90 Mya),largely influenced by Pleistocene glaciation and biotic *** factors likely influenced allopatric speciation and advocated cyclical warming–cooling episodes along elevational gradients during the *** study refines the evolutionary timeline to be much younger than previously implicated and raises the concern that projected future warming may influence the alpine species diversity,necessitating increased conservation efforts.

The Making of a Health Profession:How chinese Medicine became a Nationally Registered Allied Health Profession in Australia

In 2012, traditional chinese medicine(TcM) practitioners in Australia became nationally-registered allied health professionals in three categories: acupuncturist, chinese herbal medicine practitioner, and chinese herbal medicine dispenser. Australia was the first Western country to introduce national registration for chinese medicine, followed by Portugal and, recently, New Zealand. The practice of TcM in Australia can be traced back to the beginning of chinese immigration to Australia during the Victorian Gold Rush which began in the 1850s. The process which led to national registration commenced in the early 1970s with the establishment of the first acupuncture courses. Decades of gradual development of courses from unaccredited part-time diplomas to accredited bachelor degrees, and the development, by the profession, of national consensus on educational standards were essential elements in the process which led to registration. Professional associations, such as the Australian Acupuncture and chinese Medicine Association Ltd.(AAcMA), also developed a framework of professional selfregulation, including codes of Ethics, codes of conduct, ongoing professional development requirements and Infection control Guidelines, and provided leadership in the development of the profession. After decades of tribalism and division within the TcM profession, the National Academic Standards committee brought almost all stakeholders together to reach a consensus on the Australian Guidelines for Traditional chinese Medicine Education which were published by AAcMA in 2001. Professional associations also collaborated on joint submissions to the government in support of registration, which was introduced first in the state of Victoria in 2000, and subsequently became national in 2012. Stacking is the process of overlaying inferred species potential distributions for multiple species based on outputs of bioclimatic envelope models(bEMs).The approach can be used to investigate patterns and processes of species *** data limitations on individual species distributions are inevitable,but how do they affect inferences of patterns and processes of species richness?We investigate the influence of different data sources on estimated species richness gradients in *** fitted bEMs using species distributions data for 334 bird species obtained from(1)global range maps,(2)regional checklists,(3)museum records and surveys,and(4)citizen science data using presence-only(Mahalanobis distance),presence-background(MAXENT),and presence–absence(GAM and bRT)*** species predictions were stacked to generate species richness ***,we show that different data sources and bEMs can generate spatially varying gradients of species *** environmental predictors that best explained species distributions also differed between data *** using citizen-based data had the highest accuracy,whereas those using range data had the lowest *** richness patterns estimated by GAM and bRT models were robust to data *** multiple data sets exist for the same region and taxa,we advise that explicit treatments of uncertainty,such as sensitivity analyses of the input data,should be conducted during the process of modeling.

characterization of Small-Scale Farmers and Assessment of Their Access to crop Production Information in Selected counties of Kenya

Small-scale farming accounts for 78% of total agricultural production in Kenya and contributes to 23.5% of the country’s GDP. Their crop production activities are mostly rainfed subsistence with any surplus being sold to bring in some income. Timely decisions on farm practices such as farm preparation and planting are critical determinants of the seasonal outcomes. In Kenya, most small-scale farmers have no reliable source of information that would help them make timely and accurate decisions. county governments have extension officers who are mandated with giving farmers advisory services to farmers but they are not able to reach most farmers due to facilitation constraints. The mode and format of sharing information is also critical since it’s important to ensure that it’s timely, well-understood and usable. This study sought to assess access to geospatial derived and other crop production information by farmers in four selected counties of Kenya. Specific objectives were to determine the profile of small-scale farmers in terms of age, education and farm size;to determine the type of information that is made available to them by county and Sub-county extension officers including the format and mode of provision;and to determine if the information provided was useful in terms of accuracy, timeliness and adequacy. The results indicated that over 80% of the farmers were over 35 years of age and over 56% were male. Majority had attained primary education (34%) or secondary education (29%) and most farmers in all the counties grew maize (71%). Notably, fellow farmers were a source of information (71%) with the frequency of sharing information being mostly seasonal (37%) and when information was available (43%). Over 66% of interviewed farmers indicating that they faced challenges while using provided information. The results from the study are insightful and helpful in determining effective ways of providing farmers with useful information to ensure maximum benefits.
