bACKGROUND Chronic hepatitis b(CHb)virus infection is a major cause of liver-associated morbidity and mortality,particularly in low-income countries.A better understanding of the epidemiological,clinical,and virological characteristics of CHb will guide appropriate treatment strategies and improve the control and management of CHb in *** To investigate the characteristics of CHb in Eastern Ethiopia and assess the efficacy and safety of antiviral *** This cohort study included 193 adults who were human immunodeficiency virus-negative with CHb between June 2016 and December *** assessments included chemistry,serologic,and viral markers.χ^(2) tests,Mann-Whitney U tests,and logistic regression analyses were used to identify the determinants of *** disoproxil fumarate(TDF)was initiated using treatment criteria from the Ethiopian CHb pilot *** A total of 132 patients(68.4%)were men,with a median age of 30 years[interquartile range(IQR):24-38].At enrollment,60(31.1%)patients had cirrhosis,of whom 35(58.3%)had decompensated *** use,hepatitis b envelope antigen positivity,and a high viral load were independently associated with ***,66 patients(33.4%)fulfilled the treatment criteria and 59(30.6%)started *** 29 patients who completed 24 months of treatment,the median aspartate aminotransferase to platelet ratio index declined from 1.54(IQR:0.66-2.91)to 1.10(IQR:0.75-2.53)(P=0.002),and viral suppression was achieved in 80.9%and 100%of patients after 12 months and 24 months of treatment,*** the treated patients,12(20.3%)died within the first 6 months of treatment,of whom 8 had decompensated *** This study highlights the high prevalence of cirrhosis,initial mortality,and the efficacy of TDF *** up measures to prevent and control CHb infections in Ethiopia is crucial.

Exploring Variability in Sea Level at a Tide Gauge Station through Control Charts

Monitoring temporal changes in sea level is important in assessing coastal *** level anomalies at a tide gauge station,if kinematically conceived,include systematic variations such as trend,acceleration,periodic oscillations,and random *** them,the non-stationary nature of the random sea level variations of known or unknown origin at coastal regions has been long recognized by the sea level *** study proposes the analyses of subgroups of random residual statistics of a rigorously formulated kinematic model solution of tide gauge variations using X-bar and S control *** approach is demonstrated using Key West,Florida tide gauge *** mean and standard errors of 5-year-long subgroups of the residuals revealed that sea level changes at this location have been progressively intensifying from 1913 to the *** oscillations in sea level at this locality may be attributed partly to the thermal expansion of seawater with increasing temperatures causing larger buoyancy-related sea level fluctuations as well as the intensification of atmospheric events including wind patterns and the impact of changes in inverted barometer effects that will alter coastal risk assessments for the future.

The Making of a Health Profession:How Chinese Medicine became a Nationally Registered Allied Health Profession in Australia

In 2012, traditional Chinese medicine(TCM) practitioners in Australia became nationally-registered allied health professionals in three categories: acupuncturist, Chinese herbal medicine practitioner, and Chinese herbal medicine dispenser. Australia was the first Western country to introduce national registration for Chinese medicine, followed by Portugal and, recently, New Zealand. The practice of TCM in Australia can be traced back to the beginning of Chinese immigration to Australia during the Victorian Gold Rush which began in the 1850s. The process which led to national registration commenced in the early 1970s with the establishment of the first acupuncture courses. Decades of gradual development of courses from unaccredited part-time diplomas to accredited bachelor degrees, and the development, by the profession, of national consensus on educational standards were essential elements in the process which led to registration. Professional associations, such as the Australian Acupuncture and Chinese Medicine Association Ltd.(AACMA), also developed a framework of professional selfregulation, including Codes of Ethics, Codes of Conduct, ongoing professional development requirements and Infection Control Guidelines, and provided leadership in the development of the profession. After decades of tribalism and division within the TCM profession, the National Academic Standards Committee brought almost all stakeholders together to reach a consensus on the Australian Guidelines for Traditional Chinese Medicine Education which were published by AACMA in 2001. Professional associations also collaborated on joint submissions to the government in support of registration, which was introduced first in the state of Victoria in 2000, and subsequently became national in 2012. Stacking is the process of overlaying inferred species potential distributions for multiple species based on outputs of bioclimatic envelope models(bEMs).The approach can be used to investigate patterns and processes of species *** data limitations on individual species distributions are inevitable,but how do they affect inferences of patterns and processes of species richness?We investigate the influence of different data sources on estimated species richness gradients in *** fitted bEMs using species distributions data for 334 bird species obtained from(1)global range maps,(2)regional checklists,(3)museum records and surveys,and(4)citizen science data using presence-only(Mahalanobis distance),presence-background(MAXENT),and presence–absence(GAM and bRT)*** species predictions were stacked to generate species richness ***,we show that different data sources and bEMs can generate spatially varying gradients of species *** environmental predictors that best explained species distributions also differed between data *** using citizen-based data had the highest accuracy,whereas those using range data had the lowest *** richness patterns estimated by GAM and bRT models were robust to data *** multiple data sets exist for the same region and taxa,we advise that explicit treatments of uncertainty,such as sensitivity analyses of the input data,should be conducted during the process of modeling.

Understanding pediatric surgical needs in North Korea: a modeling analysis

background Sixty-five percent of children worldwide lack access to surgical care,the majority of whom live in low-income and middle-income countries(LMiCs).Developing surgical infrastructure requires information on surgical need;however,this information is often limited in *** Korea(Democratic People's Republic of Korea,DPRK)has a low amount of publicly available ***,we analyzed available modeled data to understand the causes of pediatric deaths due to conditions treatable with surgery in *** We used World bank data and models from the Institute for Health Metrics and Evaluation to identify causes of pediatric deaths affecting pediatric patients(agebetween DPRK and countries with similar economic *** between 1990 and 2019,the number of overall pediatric deaths has *** 2019,32.2%of allpediatric deaths in DPRK were caused by surgical *** leading categories of surgical conditions were injuries(53.9%),congenital conditions(34.2%),tumors(8.8%),and abdominal conditions(3.2%).DPRK has a lower relative rate of pediatric deaths compared with other LMICs with similar gross domestic product per ***,it has a higher relative rate of pediatric deaths due to conditions requiring treatment with *** injuries contribute significantly to the high rate of pediatric deaths in *** Although DPRK may be allocating overall resources toward pediatric healthcare more efficiently than economic peers,DPRK may benefit from improvement in pediatric surgical *** availability of data and close international collaboration could be potential solutions to bridge this gap.

This study investigates the efficacy of sodium alginate(SA),xanthan gum(XG),guar gum(GG)and chitosan(CS)d each applied at five different solid biopolymer-to-water mass ratios(or dosages)and cured for 7 d and 28 d d on the unconfined compressive strength(UCS)performance of a high plasticity clayey ***,on identifying the optimum biopolymer-treatment scenarios,their performance was compared against conventional stabilization using hydrated *** a given curing time,the UCS for all biopolymers followed a riseefall trend with increasing biopolymer dosage,peaking at an optimum dosage and then subsequently decreasing,such that all biopolymer-stabilized samples mobilized higher UCS values compared to the unamended *** optimum dosage was found to be 1.5%for SA,XG and CS,while a notably lower dosage of 0.5%was deemed optimum for ***,for a given biopolymer type and dosage,increasing the curing time from 7 d to 28 d further enhanced the UCS,with the achieved improvements being generally more pronounced for XG-and CS-treated *** of the investigated biopolymers was able to produce UCS improvements equivalent to those obtained by the 28-d soilelime samples;however,the optimum XG,GG and CS dosages,particularly after 28 d of curing,were easily able to replicate 7-d lime stabilization outcomes achieved with as high as twice the soil’s lime ***,the fundamental principles of clay chemistry,in conjunction with the soil mechanics framework,were employed to identify and discuss the clayebiopolymer stabilization mechanisms.

Smart,low-cost and portable gas sensors are highly desired due to the importance of air quality monitoring for environmental and defense-related ***,electrochemical and nondispersive infrared(IR)gas sensors are designed to detect a single specific *** IR spectroscopy-based sensors provide superior performance,their deployment is limited due to their large size and high *** this study,a smart,low-cost,multigas sensing system is demonstrated consisting of a mid-infrared microspectrometer and a machine learning *** microspectrometer is a metasurface filter array integrated with a commercial IR camera that is consumable-free,compact(~1 cm^(3))and lightweight(~1 g).The machine learning algorithm is trained to analyze the data from the microspectrometer and predict the gases *** system detects the greenhouse gases carbon dioxide and methane at concentrations ranging from 10 to 100%with 100%*** also detects hazardous gases at low concentrations with an accuracy of 98.4%.Ammonia can be detected at a concentration of 100 ***,methyl-ethyl-ketone can be detected at its permissible exposure limit(200 ppm);this concentration is considered low and *** study demonstrates the viability of using machine learning with IR spectroscopy to provide a smart and low-cost multigas sensing platform.
