Liver abscess caused by burkholderia pseudomallei in a young man: A case report and review of literature

Pyogenic liver abscess is a common entity in Indian subcontinent and is mostly caused by gram negative bacteria. Melioidosis is not commonly seen in India and only a few cases are reported. It can give rise to multiple abscesses at different sites including liver. We report a case of isolated liver abscess caused by burkholderia pseudomallei(b. pseudomallei) in a 29-yearold recently diagnosed diabetic, immunocompetent male. Diagnosis was made by imaging and culture of pus aspirated from the abscess and he was treated with percutaneous pigtail catheter drainage followed by antibiotics(meropenem and trimethoprim-sulphmethoxazole). Melioidosis is an emerging infection in India and has high mortality rate, so early diagnosis and promptmanagement is warranted which requires clinical vigilance and an intensive microbiological workup. Clinicians should be aware of isolated liver abscess caused by b. pseudomallei in appropriate clinical settings.

We present theoretical and experimental studies of Schottky diodes that use aligned arrays of single-walled carbon nanotubes. A simple physical model, taking into account the basic physics of current rectification, can adequately describe the single-tube and array devices. We show that for as-grown array diodes, the rectification ratio, defined by the maximum-to-minimum-current-ratio, is low due to the presence of metallic-single-walled nanotube (SWNT) shunts. These tubes can be eliminated in a single voltage sweep resulting in a high rectification array device. Further analysis also shows that the channel resistance, and not the intrinsic nanotube diode properties, limits the rectification in devices with channel length up to 10 μm.

Strainburst is the most common type of *** research of strainburst damage mechanisms is helpful to improve and optimize the rock support design in the burst-prone *** this study,an improved global-local modeling approach was first adopted to study strainburst damage *** extracted stresses induced by multiple excavations from a three-dimensional(3D)global model established by fast Lagrangian analysis of continua in 3 dimensions(FLAC3D)are used as boundary conditions for a two-dimensional(2D)local model of a deep roadway built by universal distinct element code(UDEC)to simulate realistic stress loading paths and conduct a detailed analysis of rockburst damage from both micro and macro *** results suggest that the deformation and damage level of the roadway gradually increase with the growth of surrounding rock stress caused by the superposition of mining-or excavation-induced stresses of the panel and nearby *** significant increase of surrounding rock stresses will result in more accumulated strain energy in two sidewalls,providing a necessary condition for the strainburst occurrence in the dynamic *** strainburst damage mechanism for the study site combines three types of damage:rock ejection,rock bulking,and *** the strainburst,initiation,propagation,and development of tensile cracks play a crucial role in controlling macroscopic failure of surrounding rock masses,although the shear crack always accounts for the main proportion of damage *** deformation and damage level of the roadway during a strainburst positively correlate with the increasing peak particle velocities(PPVs).The yielding steel arch might not dissipate kinetic energy and mitigate strainburst damage effectively due to the limited energy absorption *** principles to control and mitigate strainburst damage are proposed in this *** study presents a systematic framework to investigate strainburst damage mechanisms

Natural History, Outcomes and Antibiotic Treatment for Ventilator-Associated Tracheobronchitis in Critical Ill Patients

We assessed incidence and outcomes of patients with ventilator-associated respiratory infections (VARI) due to tracheobronchitis (VAT) and pneumonia (VAP), including length of intensive care unit (ICU) stay and ventilator days. We also examined pathogens, rate of progression from VAT to VAP, and impact of antibiotic therapy for VAT. Data analysis included 234 patients, 100 patients (43%) had at least moderate (+++) bacterial growth in their semi-quantitative endotracheal aspirate (SQ-ETA) cultures. VAT and VAP were each diagnosed in 34 (15%) patients. Staphylococcus aureus was the most common pathogen isolated and had the highest rate of progression from VAT to VAP. Seven (21%) of the 34 patients were diagnosed with VAT that later progressed to VAP in averaged 3 days. Patients diagnosed with VAT had significantly more ventilator days (9 vs 6, p p bserved in the clinical outcomes of the 25 VAT patients with timely, appropriate antibiotics compared to the 9 VAT patients who did not receive timely appropriate antibiotics. VAT was a risk factor for increased ventilator days, longer length of ICU and hospital stay. The time window from VAT to VAP allowed physicians to identify the pathogens and sensitivity profile needed to treat VAT with appropriate antibiotics. Data from well-designed studies were needed to assess the impact of early, appropriate antibiotic therapy for VAT, the choice of antibiotics, as well as the duration and route of administration.

We present the results obtained from detailed timing and spectral studies of a black hole candidate MAXI J1813-095 using Swift,NICER,and NuSTAR observations during its 2018 *** timing behavior of the source is mainly studied by examining NICER light curves in the 0.5−10 keV *** did not find any signature of quasi-periodic oscillations in the power density spectra of the *** carry out spectral analysis with a combined disk blackbody&power law model,and physical two-component advective flow(TCAF)*** the combined disk blackbody&power-law model,we extracted thermal and non-thermal fluxes,photon index and inner disk *** also find evidence for weak reflection in the *** have tested the physical TCAF model on a broadband spectrum from NuSTAR and Swift/*** parameters like mass accretion rates,the size of Compton clouds and the shock strength are *** result affirms that the source remained in the hard state during the entire outburst which indicates a‘failed’*** estimate the mass of the black hole as 7.4±1.5M⊙from the spectral study with the TCAF *** apply the LAOR model for the Fe K line *** this,the spin parameter of the black hole is ascertained as a^(∗)>*** inclination angle of the system is estimated to be in the range of 28°−45°from the reflection *** find the source distance to be∼6 kpc.

Diminishing surface defect states in perovskite nancx^rystals is a highly challenging subject for enhancing optoelectronic device *** synthesized organic/inorganic lead-halide perovskite MAPbbr3(MA=methylammonium)clusters comprising nanocrystals with diameters ranging between 20 and 30 nm and characterized an enhanced photoluminescence(PL)quantum yield(as much as^7 times)by encapsulating the MAPbbr3 with graphene(Gr).The optical properties of MAPbbr3 and Gr/MAPbbr3 were investigated by temperature-dependent micro-PL and time-resolved PL *** functional theory calculations show that the surface defect states in MAPbbr3 are removed and the optical band gap is reduced by a 0.15 eV by encapsulation with graphene due to partial restoration of lattice distortions.

Insufficient dietary intake of micronutHents, known as ＂hidden hunger＂, is a devastating global burden, affecting two billion people. Deficiency of folates （vitamin b9）, which are known to play a central role in Cl metabolism, causes birth defects in at least a quarter million people annually. biofortification to enhance the level of naturally occurring folates in crop plants, proves to be an efficient and cost-effective tool in fighting folate deficiency. Previously, introduction of folate biosynthesis genes GTPCHI andADCS, proven to be a successful biofortification strategy in rice and tomato, turned out to be insufficient to adequately increase folate levels in potato tubers. Here, we provide a proof of concept that additional introduction of HPPK/DHPS and/or FPGS, downstream genes in mitochonddal folate biosynthesis, enables augmenta- tion of folates to satisfactory levels （12-fold） and ensures folate stability upon long-term storage of tubers. In conclusion, this engineering strategy can serve as a model in the creation of folate-accumulating potato cultivars, readily applicable in potato-consuming populations suffedng from folate deficiency.
